Morpho-histological Analysis of the Ultrastructure of the Dupuytren Cord Subjected to Progressive Distraction Using an External Fixator.

BACKGROUND: This study aimed to provide a detailed morphological and ultrastructural characterization of Dupuytren disease cords in advanced stages, with particular attention to the effects of progressive mechanical distraction on extracellular matrix organization and tissue architecture. METHODS: Six patients with stage IV Dupuytren disease affecting the fifth ray were included. Three underwent treatment with a distractible angular external fixator followed by open fasciectomy, while 3 controls received open fasciectomy alone. Excised cords were analyzed by light microscopy and scanning elec

Evaluation of Vapocoolant Spray Before Local Anesthetic Infiltration During Percutaneous Needle Aponeurotomy: A Randomized Controlled Trial.

PURPOSE: Percutaneous needle aponeurotomy (PNA) offers an effective, minimally invasive option for Dupuytren disease, but local anesthetic infiltration into the richly innervated palm can cause substantial procedural pain. Vapocoolant sprays induce rapid cutaneous cooling, potentially suppressing nociceptor activity and modulating pain transmission. Evidence for their role in hand surgery is sparse. This randomized controlled trial evaluated whether vapocoolant application before infiltration reduces injection pain during PNA. METHODS: In a single-center, single-blind, randomized controlled d

An Assessment of the Effect of Intranodular Steroid Injection on Inhibiting the Progression of Early-Stage Dupuytren's Disease: A Preliminary Report.

ABSTRACT: The rationale for intranodular steroid injections in Dupuytren's disease was based on clinical and experimental studies demonstrating the inhibitory effect of steroids on connective tissue proliferation and on degradation of mature collagen in hypertrophic scars. This prompted clinicians to use steroid injections to treat Dupuytren's nodules. ABSTRACT: OBJECTIVE:  This study aimed to evaluate the effect of steroid injections into Dupuytren's nodules on inhibiting early-stage disease progression. ABSTRACT: MATERIALS AND METHODS:  A total of 28 patients (16 men and 12 women; mean age

Treatment of Dupuytren's Disease of the Proximal Interphalangeal Joint: A Systematic Review and Network Meta-analysis.

BACKGROUND: Postoperative outcomes for Dupuytren's disease (DD) of the proximal interphalangeal joint (PIPJ) are highly variable, and the condition often proves refractory to correction. The aim of this study was to summarize the best available evidence on the treatments for DD of the PIPJ. METHODS: A systematic review and network meta-analysis were conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A Rare Anatomical Variant Dual Radial Digital Nerves in the Right Ring Finger: Consideration in Hand Dissection.

Digital nerve anatomy in the fingers is considered a predictable pattern of a single radial and ulnar neurovascular bundle, allowing for often safe midline dissection. In complicated cases, such as Dupuytren contracture or trauma, knowledge of digital nerve anatomy is paramount for safe dissection. The patient was a 68-year-old man with bilateral ring finger Dupuytren contractures. On the right hand, he had a 40° flexion contracture of the ring finger and underwent a palmar fasciectomy. Two distinct radial digital nerves were identified intraoperatively, with one branch in the traditional radi

Uncoupling TGFβ1 signalling from collagen protein synthesis in Dupuytren's disease.

Dupuytren's disease is a fibroproliferative disorder of the palmer fascia (PF) characterised by flexion contractures in the hand. Dupuytren's disease can be treated surgically, but disease recurrence rates are high, potentially due to continual production of matrisomal proteins. Here, metabolic labelling and proteomics identified differences in the new synthesis and composition of matrisomal proteins between Dupuytren's tissue and normal PF. Dupuytren's tissue actively synthesised type I collagen, fibronectin (FN1), matrix metalloproteinases-2 and -3 (MMP2, MMP3) and tissue inhibitor of metall

Dupuytren's disease: achieving a balance between treatment, overtreatment and non-treatment.

INTRODUCTION: Dupuytren's disease is characterized by a highly variable presentation and progression. Notably, minimal manifestations such as nodules do not invariably evolve into contractures with significant functional impairment. These unrecognized variability raises important concerns regarding patient anxiety and the risk of overtreatment.Current staging systems:Staging systems that focus primarily on angular deformity demonstrate limited correlation with functional impairment. They fail to reliably predict treatment necessity, clinical impact, therapeutic outcomes or risk of recurrence.

Exploring spillover effects following surgical de-implementation: an observational evaluation of primary care, referrals, and time to surgical intervention following reductions in the use of tonsillectomy and Dupuytren's contracture.

BACKGROUND: Reducing the use of low-value surgery is important to maintain effective, safe and financially sustainable health systems. However, following de-implementation there are inevitably wider impacts for health systems beyond the targeted reduction in surgery, which have hitherto not been explored. Here we describe the spillover effects following the reduction in the use of two surgical procedures; tonsillectomy and Dupuytren's contracture release (DCR) identified in 2019 by the Evidence Based Interventions (EBI) programme a de-implementation initiative in England. METHODS: This longit

Dupuytren's in the Public Eye: The Impact of Direct-to-Consumer Advertising on Public Interest and Treatment Patterns.

BACKGROUND: In 2019, the "Facts on Hand" direct-to-consumer advertising (DTCA) campaign, featuring former NFL quarterback John Elway, was launched to raise awareness of Dupuytren's contracture (DC) and its nonsurgical treatment options. Although DTCA has been shown to influence health care behavior in other specialties, its impact on treatment patterns for hand conditions remains unclear. METHODS: Google Trends was used to examine the influence of direct-to-consumer advertising (DTCA) on Internet-based information-seeking behavior related to DC. Changes in the proportion of surgical and nonsu

Improvement in Grip Strength Following Percutaneous Needle Aponeurotomy for Dupuytren's Disease: A Prospective Clinical Study.

BACKGROUND: Dupuytren's disease causes progressive flexion contractures and impaired hand function. While percutaneous needle aponeurotomy (PNA) provides rapid correction with low morbidity, the effect on grip strength remains unclear. Grip strength is a key surrogate of hand function and overall health, yet it has been inconsistently evaluated as an outcome in Dupuytren's disease. METHODS: A prospective study was conducted at a tertiary referral center including 53 patients (80 digits) treated with PNA between February 2024 and March 2025. Standardized assessments included grip strength (Jam

"Selective Fasciotomy with Local Flap Interposition for Dupuytren Contracture: A Retrospective Analysis".

BACKGROUND: Dupuytren contracture is a fibroproliferative disorder that causes progressive flexion deformities of the fingers. Various traditional procedures have distinct benefits and limitations. We developed a novel surgical technique combining selective fasciotomy with local flap interposition to restore extension while preventing pathological reconnection. METHODS: This study adhered to the STROBE guidelines. A retrospective review was conducted on 64 patients (90 fingers; average age, 68.1 years) treated between 2011 and 2024 at two institutions with expertise in hand surgery. Pathologi

Identification of novel plasma proteomic biomarkers of Dupuytren disease.

Dupuytren Disease (DD) is a chronic progressive disease that can cause disabling hand deformities. The most common treatments have either high complication rates or high early recurrence rates. Dupuytren lacks a staging biomarker profile to inform the development of preventive therapeutics to improve long-term outcomes. This multi-omic study aimed to create a DD blood proteomic biomarker profile by comparing DD plasma with that of a healthy control group. We measured circulating collagen metabolism peptides and found normal Collagen I synthesis but impaired Collagen I degradation in DD. We mea

Digital Artery Pseudoaneurysm After Collagenase Injection for Dupuytren Contracture: A Case Report.

Collagenase Clostridium histolyticum (CCH; Xiaflex) is an established nonsurgical therapy for Dupuytren contracture. Although adverse effects such as edema, ecchymosis, and skin tears are common, vascular injury is rare and pseudoaneurysm formation has not been previously described. Clinical evaluation and operative management of a patient who developed a digital artery pseudoaneurysm after CCH injection and manipulation were reviewed. A 66-year-old man developed a pulsatile swelling on the volar-radial aspect of the small finger 1 week after manipulation. Duplex ultrasound demonstrated a 1.8

Primary Proximo-Distal Arthrodesis for Severe Dupuytren's Contracture of the Little Finger: A Prospective Cohort Study.

BACKGROUND: Advanced Dupuytren's contracture of the little finger is challenging due to irreversible soft tissue shortening. Conventional options such as limited fasciectomy or dermofasciectomy often yield limited function and high recurrence. Shortening arthrodesis of the proximal interphalangeal joint is usually a salvage procedure; its role as primary treatment is underexplored. METHODS: This prospective single-center study (2019-2024) included patients with severe (Tubiana stage III-IV) little finger contracture treated with primary proximo-distal interphalangeal arthrodesis (PDIP) plus l

Seroma formation following percutaneous needle aponeurotomy and corticosteroid injection for Dupuytren's contracture: first case report in the literature.

BACKGROUND: Dupuytren's contracture is a progressive fibroproliferative condition of the palmar fascia resulting in finger flexion deformities. Commonly reported complications following percutaneous needle aponeurotomy (PNA) may include skin tears, infections, nerve injuries, tendon lacerations and regional pain syndromes. Seroma formation is not known to develop following PNA, particularly when combined with corticosteroid injections (CSIs). CASE DESCRIPTION: We describe the first documented case of an infected seroma following combined PNA and CSI in a 67-year-old female. A 67-year-old fema

Hand therapy or not following collagenase treatment for Dupuytren's disease? A randomized controlled trial.

BACKGROUND: Collagenase treatment can improve finger extension and hand function in patients with Dupuytren's disease. The role of hand therapy in enhancing or prolonging these effects remains unclear. PURPOSE: The primary aim was to examine whether hand therapy, compared to no hand therapy, improves self-perceived occupational performance after collagenase treatment for Dupuytren's disease. The second aim was to investigate the effect of hand therapy on finger extension, grip strength, pain and overall patient impression. A third aim was to investigate whether outcomes differed between patie

Case Report: Dupuytren's Disease in a 12-Year-Old Child; Unusual Clinical Presentation.

INTRODUCTION: Dupuytren's disease is a progressive fibroproliferative disorder primarily affecting men over 40. Its occurrence in children is extremely rare, with fewer than 25 cases reported since 1955. Due to its rarity, it is often overlooked in pediatric patients presenting with palmar masses. The etiology in children remains unclear, and clinical presentation may differ from the adult form, sometimes mimicking other fibromatoses or even malignancies. CASE REPORT: We report the case of a 12-year-old boy with a painful, progressively enlarging palmar mass. Examination revealed a firm subcu

S100A4-TLR4-TGF-β axis as a therapeutic target for Dupuytren's contracture in diabetic patients.

Dupuytren's contracture is a superficial fibrotic disease of the hands that causes flexion contractures of the affected fingers. Diabetes mellitus (DM) is a risk factor for Dupuytren's contracture. However, the exact underlying mechanisms by which DM is involved in its development and progression remain unknown. This study investigated the involvement of glycometabolic disorders in the pathogenesis of Dupuytren's contracture. RNA sequencing revealed that S100A4 expression was significantly increased in Dupuytren's contracture-derived fibroblasts under high-glucose conditions compared with low-
