Pathological Changes in Extracellular Matrix Composition Orchestrate the Fibrotic Feedback Loop Through Macrophage Activation in Dupuytren's Contracture.

Dupuytren's contracture belongs to a group of fibrotic diseases that have similar mechanisms but lack effective treatment and prevention options. The excessive accumulation of connective tissue in Dupuytren's disease leads to palmar fibrosis that results in contracture deformities. The present study aimed to investigate how the tissue microenvironment in Dupuytren's contracture affects the phenotypic differentiation of macrophages, which leads to an inflammatory response and the development of chronicity in fibrotic disease. We utilized a decellularization-based method combined with proteomic

Active synthesis of type I collagen homotrimer in Dupuytren's fibrosis is unaffected by anti-TNF-α treatment.

Dupuytren's disease is a common fibroproliferative disease of the palmar fascia of the hand, with advanced cases treated surgically. Anti-TNF injection has undergone phase 2 trials and may be effective in slowing early-stage disease progression. Here we sought to determine how new synthesis of type I collagen in Dupuytren's differs from normal palmar fascia samples and to analyze the role of TNF in aberrant collagen synthesis. Model nonfibrotic but fibrous connective tissues were used to analyze active type I collagen protein synthesis in development, aging, and degenerative disease, where it

Epithelioid sarcoma of the hand mimicking Dupuytren's disease: A case report and review of the literature.

UNLABELLED: We report a case of recurrent epithelioid sarcoma in a 30-year-old man whose initial clinical and histological findings were compatible with Dupuytren's contracture. Lymph node spread, distant metastases, and targeted histological examination eventually led to a correct diagnosis. Epithelioid sarcoma should always be considered in the differential diagnosis of Dupuytren's disease, particularly in young patients with recurrent disease. LEVEL OF EVIDENCE: V.

Outcomes of limited fasciectomy, needle fasciotomy and collagenase injection for Dupuytren's disease: a systematic review and meta-analysis of individual patient data.

This systematic review and meta-analysis of individual patient data evaluates the outcomes of treatment for Dupuytren's disease using limited fasciectomy (LF), percutaneous needle fasciotomy (PNF) and collagenase clostridium histolyticum (CCH) injection. A total of 1423 studies were identified, of which 15 met the eligibility criteria for meta-analysis. The postoperative total extension deficit was smaller after LF than after PNF or CCH, but the difference was not clinically relevant. Minor complications were more frequent after CCH than after LF and PNF. The risk of major complications did no

Nationwide rates of limited fasciectomy for Dupuytren's contracture: data from the Finnish National Registry.

Dupuytren's contracture (DC) is often treated with limited fasciectomy (LF), while percutaneous treatment options are gaining popularity. The recent trends in the incidence rates of LF are not well known. Our study aimed to investigate the incidence rates of LF over time, and we collected nationwide data on all LF performed between January 1, 1997 and December 31, 2018. The main outcome variables were the incidence rates of first and subsequent LF for each patient per 100,000 person-years, calculated for each study year, gender, and age group. Data were obtained from the Finnish National Hospi

Celiac Disease and Incident Dupuytren's Contracture: A Matched Nationwide Cohort Analysis.

This study explored the association between celiac disease (CeD) and Dupuytren's contracture (DC) using data from the Swedish ESPRESSO cohort. We analyzed 49,699 CeD patients and 245,267 matched controls, identifying 1420 incident DC cases. CeD patients had a 1.21-fold increased risk of DC compared to controls, with a more pronounced risk in women and older individuals. Further research is needed to understand the underlying mechanisms of this relationship.

Angiotensin Receptor Blockers Are Not Associated With Protective Benefits in Dupuytren's Disease.

BACKGROUND: Dupuytren's disease (DD) is a progressive fibroproliferative disorder which is, in part, mediated by transforming growth factor beta 1 (TGF-β1). Angiotensin II receptor blockers (ARBs) have been shown to lead to upstream inhibition of TGF-β1 in several disease processes and proposed as a potential pharmacologic treatment in DD. The purpose of this study was to assess the association of ARBs with DD procedures and recurrence. METHODS: We queried the PearlDiver national insurance dataset to identify patients with a diagnosis of DD and at least 3 years of follow-up. A 1:1 matched coh

Limited palmar fasciectomy combined with Z-plasty for Dupuytren's disease: A 56 cases review.

INTRODUCTION: Fasciectomy is the gold standard for treating palmodigital cords in Dupuytren's disease. When flexion contracture predominates at the metacarpophalangeal (MCP) joint, a segmental aponeurectomy in the palm combined with a Z-plasty can be performed. The primary objective of this study was to evaluate the improvement in MCP extension achieved by this limited procedure. The secondary objectives were to assess long-term complications and recurrence rates. MATERIALS AND METHODS: A retrospective study was conducted on patients who underwent pure palmar segmental fasciectomy with Z-plas

Discovery and characterization a new collagenase from Hathewaya massiliensis for effective subcutaneous adipolysis.

Collagenases hydrolyze peptide bonds in collagen and have established therapeutic applications in connective tissue disorders and medical aesthetics, including treatment of Peyronie's disease, Dupuytren's contracture and localized adipose accumulation. Here, we describe HmCol, a novel collagenase from Hathewaya massiliensis. A rationally truncated, codon optimized HmCol gene was cloned into pET30a and expressed in Escherichia coli BL21(DE3). The purified enzyme exhibits robust collagenolytic activity, with an optimum at pH 7.5 and 45 °C, underscoring its clinical potential. A single site mutan

"Collagenase injections versus limited fasciectomy in treating patients with Dupuytren's contracture: A systematic review and meta-analysis".

BACKGROUND: Dupuytren's contracture is a progressive disorder characterized by palmar fascia thickening, leading to nodules and cords formation. Limited fasciectomy (LF) is the standard surgical treatment, while Collagenase Clostridium Histolyticum (CCH) injection offers a minimally invasive alternative. Despite multiple studies, the comparative efficacy and safety of these approaches remain debated. PURPOSE: To compare the effectiveness and safety of CCH and LF by analyzing change in extension deficit (ED), recurrence rate, residual contracture, Michigan Hand Questionnaire (MHQ) score, and c

Dupuytren Disease from Past to Present: A Review of the Historical and Evolving Landscape of Its Management.

Dupuytren disease (DD) has been described since the 17th century, but its true aetiology remains a mystery. The treatment of DD largely involves addressing the end stage of a complex fibroproliferative pathway, specifically, the cords. In recent years, there has been a shift to less radical approaches, with an increased adoption of percutaneous needle aponeurotomy (PNA) or collagenase. However, the difficulty in comparing outcomes across various treatments is likely to persist due to limitations in evaluating the effectiveness of those treatments. Patient-reported outcome measures may not accu
