Determining Risk Factors and Rate of Surgery After Collagenase Injections for Dupuytren Contracture.

PURPOSE: Collagenase injections are a nonsurgical treatment for Dupuytren contracture, but their long-term effectiveness in preventing surgery remains unclear. This study aimed to evaluate the rate of surgical intervention following collagenase treatment and identify whether specific risk factors increase the likelihood of future surgical correction. METHODS: This retrospective cohort study utilized the TriNetX US Collaborative network to identify patients diagnosed with Dupuytren contracture between January 2007 and September 2024. Patients initially treated with collagenase injections were

Objective Improvements, Subjective Uncertainty: Surgical Outcomes and the Role of PROMIS in Severe PIP Contractures.

BACKGROUND: Severe proximal interphalangeal (PIP) contractures in Dupuytren disease significantly impair hand function and quality of life. Surgical correction is common, but the relationship between improved joint mobility and patient-reported outcomes remains unclear. This study evaluated surgical outcomes for severe PIP contractures and assessed patient-reported function using Patient-Reported Outcomes Measurement Information System (PROMIS). METHODS: A retrospective chart review included patients with severe PIP contractures treated surgically. Contractures were categorized by severity: g

Incidence of Dupuytren's disease following hand trauma: a systematic review.

Trauma, particularly surgical trauma, has been suggested as a potential trigger for Dupuytren's disease (DD). This systematic review examined the prevalence of DD after surgical and non-surgical hand trauma by conducting thorough searches of the PubMed, Embase, Cochrane and Scopus databases. Qualitative methods were used to synthesise the data and summarize the findings that were unsuitable for meta-analysis. The findings revealed an increased risk of DD associated with exposure to hand-transmitted vibration, sports-related trauma and distal radial fractures. Surgical procedures such as trigge

Comparing Treatment Approaches for Dupuytren Contracture: Percutaneous Needle Fasciotomy With and Without Corticosteroids.

INTRODUCTION: Percutaneous needle fasciotomy (PNF) is a standard treatment for Dupuytren contracture. In some instances, clinicians administer intraoperative corticosteroids (ICS) and/or postoperative corticosteroids (PCS) alongside PNF to improve outcomes. However, few studies have systematically investigated the efficacy of ICS and/or PCS as adjuvant therapies to PNF. The purpose of this study was to review clinical outcomes between PNF alone and PNF with corticosteroid use in the treatment of Dupuytren contracture. METHODS: A systematic review was done in February 2024 using SCOPUS and OVI

The Cost-Effectiveness of Collagenase Injection Versus Limited Fasciectomy for Moderate Dupuytren's Contracture: An Economic Evaluation of the Dupuytren's Interventions Surgery Versus Collagenase Trial and a Decision Analytical Model.

OBJECTIVES: To compare the cost-effectiveness of collagenase injection (collagenase) and limited fasciectomy (LF) surgery in treating moderate Dupuytren's contracture (DC) in the United Kingdom over different time horizons. METHODS: An incremental cost-effectiveness analysis was conducted alongside a multicenter, pragmatic, parallel randomized controlled trial (Dupuytren's Interventions: Surgery versus Collagenase trial), to determine the short-term cost-effectiveness of collagenase compared with LF. A Markov decision analytic model was developed to assess long-term cost-effectiveness. RESUL

Management of Dupuytren disease of the little finger.

BACKGROUND: The surgical management of Dupuytren disease (DD) is associated with a high rate of complications. Recurrences are relatively common and may result in permanent disability, particularly when the little finger (LF) is involved. This study aims to provide both objective and subjective information, along with professionals' experiences. METHODS: A questionnaire survey, comprising both open and closed questions, was distributed to hand surgeons, physiotherapists, and occupational therapists engaged in the management of DD across five continents. The involvement and role of the LF in D

Comparison of optimized magnetic resonance sequences for patient-specific treatment planning in surface brachytherapy.

BACKGROUND: The clinical standard practice of surface brachytherapy (SB) planning has long been to use computed tomography (CT) imaging to visualize applicators for catheter reconstruction in the treatment planning process. Recent work in SB has suggested that magnetic resonance (MR)-guidance can be used in place of CT-guidance in SB planning to utilize the increased soft tissue contrast for visualization of diseased tissue. This soft tissue visualization can be used to verify the target depth for enhanced coverage of the clinical target volume. Two optimized MR sequences (pointwise encoded ti

Telemedicine-based measurement of finger joint range of motion in patients: A reliability and concurrent validity study.

BACKGROUND: Although the use of telemedicine has persisted in hand surgery and therapy practices beyond the COVID-19 pandemic, there remains a need for simple, validated means of remotely measuring finger joint range of motion for integration in fast-paced virtual clinics. We propose on-screen measurement, a technique previously validated in the elbow, which involves holding a goniometer up to the telemedicine appointment screen. PURPOSE: This study aimed to determine the reliability and concurrent validity of on-screen measurements relative to the gold standard, in-person goniometry. Congrue

Radiation Therapy for Nonmalignant Diseases: An ASTRO-ESTRO-DEGRO-RANZCR Framework.

The field of radiation medicine has an opportunity to explore emerging indications, including nonmalignant disease, where radiation therapy may be beneficial. Because radiation therapy has anti-inflammatory and antiproliferative effects, there is rationale to deploy it in the management of a range of nonmalignant conditions including plantar fasciitis, Dupuytren disease, and osteoarthritis. Given the prevalence of many potentially relevant pathologies, a body of evidence (albeit commonly lower level), and an often absence of a widely accepted standard treatment for a particular patient cohort,

Limitations of Artificial Intelligence Generated Images for Hand Surgery Patient Education.

PURPOSE: The role of artificial intelligence (AI) in medicine is rapidly evolving, with potential to improve both the clinician and patient experience. We sought to evaluate whether popular AI text-to-image generators could create anatomically accurate images of common hand surgery procedures. We hypothesized that the AI-generated images would not be adequate as patient education materials. METHODS: We queried five AI text-to-image generators: Craiyon, DALL-E, DeepSeek, Gemini, Midjourney, and Stable Diffusion. They were given the prompt, "Create an anatomically accurate image with labels of

Management of early stages of Dupuytren's disease: a narrative review.

<b>Introduction:</b> Dupuytren's disease is a common condition of the palmar aponeurosis, leding to contractures of the fingers. The disease onset is the formation of nodules in the palmar aponeurosis, which can be painful in some patients. Resection of the affected part of the palmar aponeurosis and release of finger contracture is a method of treating the advanced form of the disease, while in the early phase, awaiting progression has so far been recommended. Research presented over the past few years indicates the possibility of active treatment of early Dupuytren's di

Collagenase treatment does not impair fiber contractile function in muscle biopsies from children with cerebral palsy.

Cerebral palsy (CP) often presents with increased passive stiffness of the skeletal muscles, primarily due to increased collagen in the extracellular matrix. Collagenase from Clostridium histolyticum (CCH), an enzyme that degrades collagen, is used clinically to treat fibrotic conditions such as Dupuytren's contracture and Peyronie's disease. Although prior work demonstrated reduced passive stiffness in muscle bundles from children with CP following CCH incubation, its effects on active contractile properties remain unknown. Thus, this study was aimed at investigating the maximal active force,
