Soluble Indicators of Endothelial Injury and Activation Emerge During Acute HIV Infection and Persist Despite Rapid Antiretroviral Therapy Initiation.

BACKGROUND: Vascular activation occurs during human immunodeficiency virus (HIV) infection and is a risk factor for cardiovascular disease (CVD). This study investigates whether vascular endothelial and hemostatic perturbations emerge during acute HIV infection (AHI) and persist despite immediate antiretroviral therapy (ART) initiation. METHODS: Thirteen plasma biomarkers indicative of endothelial and hemostatic perturbations were measured in 83 RV254 AHI cohort participants during pre-ART AHI and at week 288 post-ART, compared with cross-sectional measures from 32 people without HIV (PWoH).

Carotid inflammation on FDG-PET is associated with lower cognitive performance in people with treated HIV infection.

OBJECTIVES: Carotid atherosclerosis is an established risk factor for cognitive impairment. FDG-PET detects subtle inflammatory changes in the arterial wall that may precede plaque formation and rupture. We investigated whether carotid inflammation is associated with cognitive function and its relative importance as a predictor of concurrent cognitive function compared with other factors. DESIGN/METHODS: : In this cross-sectional study, we recruited individuals ≥40 years with undetectable viral load at moderate-to-high cardiovascular risk. Participants underwent FDG-PET, stress and cognitive

Adaptation and Validation of Brief Tablet-Based Cognitive Assessment Tool in Uganda.

BACKGROUND: Sub-Saharan Africa (sSA) faces accelerated growth in Alzheimer's disease and related dementias (ADRDs), including in older people living with HIV (PLWH). However, there are limited contextually relevant diagnostic tools to assess cognitive impairment in the region. The Tablet-based Cognitive Assessment Tool (TabCAT) is a digital platform hosting cognitive tests with automated scoring that can be administered by non-specialists and are designed for use across diverse cultures, languages, and education levels. We translated, adapted, and assessed the validity of a brief battery of Ta

Occludin modulates HIV and ischaemic stroke response via the mitochondrial antiviral signalling pathway.

Specific cell types of the blood-brain barrier (BBB), such as pericytes, can be infected by HIV-1. Importantly, alterations of the expression of tight junction protein occludin (OCLN) have been linked to regulation of HIV-1 infection. In the present study, we hypothesized that OCLN can influence HIV-1 infection via modulating innate immunity responses. Unbiased transcriptome analysis was conducted on wild-type and OCLN-silenced primary human BBB pericytes. The role of differentially expressed innate-immunity pathways was then evaluated in the context of mitochondrial dysfunction and HIV-1 infe

Suicidal thoughts and behaviours in Cape Town: a cross-sectional study of prevalence, social, contextual, and clinical correlates.

BACKGROUND: Suicidal thoughts and behaviours (STBs) are traditionally attributed to mental disorders; however, increasing evidence indicates that social determinants contribute to suicide risk. We examined STBs and their socioeconomic, psychosocial and clinical correlates in two communities in Cape Town. METHODS: We conducted psychiatric diagnostic interviews (MINI) at three public-sector facilities (2023-2024). Adults aged ≥18 years, with and without HIV, were recruited in a 2:1 ratio. We examined associations between suicidal ideation and sociodemographic factors, violence exposure, perceiv

Early insights from a multi-centre national stroke surveillance initiative in Tanzania.

BACKGROUND: Stroke is a leading cause of death and disability globally, with sub-Saharan Africa, bearing the greatest burden. Tanzania has only one active stroke registry, limiting evidence-based care and policy development. We aimed to expand the registry into a multi-centre study across eight major tertiary hospitals to generate baseline data for a national stroke registry. METHODS: From January to August 2024, we analysed de-identified data from adults (≥18 years) admitted with a World Health Organisation defined stroke. Data collected included demographics, risk factors, imaging, and in-h

COVID-19 and Influenza Booster Vaccination Elicits Robust Antibody Responses in Patients with Primary Brain Tumors Comparable to Healthy Adults.

Background: Patients with primary brain tumors face profound disease- and treatment-related immunosuppression, placing them at high risk for severe infections. Their capacity to mount protective immune responses to vaccination, particularly to repeated antigen exposures such as COVID-19 boosters, remains critically under-defined, leading to uncertainty in clinical practice. Methods: In this prospective cohort study, we analyzed humoral responses to seasonal COVID-19 (mRNA-based) and influenza vaccination in 17 patients with primary brain tumors (recruited from an initial cohort of 37) who rece

Distinct Antibody Fc-profiles in Lymph During Homeostasis and Chronic HIV Infection.

BACKGROUND: Antibodies play a critical role in the control of pathogens and tumors through their ability to recognize non-self and then direct immune-mediated destruction. Antibodies are generated by plasma cells or plasmablasts, located throughout the tissues, and are transported between blood, lymph, mucosal secretions, and tissues to survey all sites for pathogens or malignant cells. However, mounting evidence suggests antibodies that transit across compartments (from the blood to the brain, mucosal tissues, or placenta) differ from those in systemic circulation. Whether antibodies also dif

Reframing disease burden: validation of DALY-per-case as a per-diagnosis severity metric.

BACKGROUND: Population-weighted metrics (incidence, mortality, disability-adjusted life years (DALYs), mortality to incidence ratio (MIR) can obscure per-case severity for less prevalent but high-impact conditions. This paper introduces DALY per case, total DALYs divided by incident cases, as a standardized estimate of healthy life-years lost per new diagnosis, integrating years of life lost (YLL) and years lived with disability (YLD). Validated using cancers and applied across diverse diseases, the metric enables prevalence-independent severity comparisons. METHODS: Using GBD 2021, we comput

From Neuroadaptation to Neuroprogression: Rethinking Chronic Cocaine Exposure Through a Model of Cocaine-Related Cerebropathy.

Background: Chronic cocaine exposure is increasingly associated with persistent brain alterations, yet it remains unclear whether these changes reflect reversible neuroadaptation, accelerated brain ageing, or a degeneration-like trajectory in a vulnerable subgroup. This Perspective proposes a neuroprogressive vulnerability framework-referred to as cocaine-specific encephalopathy/cerebropathy only in a heuristic sense-to organise heterogeneous evidence without implying a distinct neurodegenerative disease entity. Methods: We conducted a structured, critical synthesis of peer-reviewed human and

Endothelial KLF4 depletion drives age-related neurovascular dysfunction and neuropsychiatric impairment.

Deterioration of the blood-brain barrier (BBB), including impaired neurovascular uncoupling, contributes to cognitive decline in aging. The BBB is formed principally by brain microvascular endothelial cells (ECs), and ECs throughout the body are enriched for the transcription factor Krüppel-like factor 4 (KLF4). Because KLF4 levels in ECs decrease with age, we tested whether that decline contributes to aging-related BBB deterioration, neurovascular dysfunction, and cognitive impairment. Using EC-specific Klf4 knockout mice (EC-K4KO), we show that loss of EC KLF4 accelerates multiple age-relate
