Major adverse cardiovascular events, morbidity, and mortality, among people living with and without HIV in two northern Uganda hospitals.

BACKGROUND: Cardiovascular disease (CVD) morbidity and mortality are increasing globally, including among patients living with human immunodeficiency virus (HIV). We aimed to determine the risk of all-cause mortality and morbidity in patients with CVD, comparing those with and without HIV infection, and whether mortality differed by HIV status during hospitalization for Major Adverse Cardiovascular Events (MACE) components (stroke, acute myocardial infarction (AMI), and heart failure) in northern Uganda. METHODS: We conducted a retrospective cohort study at two hospitals in northern Uganda, co

Brain structural and MRI phenotypes in intracranial cryptococcosis: a comparative study based on HIV status.

BACKGROUND: The impact of cryptococcosis on cerebral structure and MRI characteristics in people living with HIV (PWH) who have cryptococcal meningoencephalitis remains underexplored. Existing evidence mainly comes from small-scale studies. This study aimed to clarify the effects of cryptococcal meningoencephalitis on brain structural changes and MRI features in PWH. METHODS: A total of 190 patients with intracranial cryptococcosis were enrolled and categorized, based on HIV serostatus, into the PWH group (n = 127) and the people linving without HIV(PWOH) group (n = 63). Laboratory parameters,

Region-Specific NRF2 Signaling in HIV-Associated Neurocognitive Disorders: A Transcriptomic and Computational Histology Study.

BACKGROUND/OBJECTIVES: Oxidative stress is a key contributor to HIV-associated neurocognitive disorders (HANDs), yet the regional organization and functional engagement of the NRF2 antioxidant pathway in the human brain remain incompletely defined. This study aimed to characterize NRF2 pathway architecture, baseline brain expression, and disease-associated transcriptional and coexpression remodeling across HAND stages. METHODS: The NRF2 signaling network was reconstructed using curated pathway data and protein-protein interaction analysis to identify central hub genes. Baseline expression in

Common factors and unique pathways for linkages between HIV/STI prevention and syndemic behaviours in high-risk youth: protocol for a secondary analysis of harmonised data from six clinical trials.

INTRODUCTION: HIV/sexually transmitted infection (STI) prevention interventions are only modestly successful among youth, particularly for young people of colour and sexual and gender minority (SGM) youth. Even among disparate intervention modalities delivered with high fidelity, differences between intervention types have been minimal. One consistent theme has emerged: the role of the youth:provider relationship in predicting intervention response. In line with calls for examination of relational factors, the next essential step is a harmonised analysis to evaluate connections between the you

[A case of cerebellar-limited HIV-associated progressive multifocal leukoencephalopathy diagnosed with the aid of the shrimp sign].

A 43-year-old man presented with dizziness and slurred speech. A brain MRI with diffusion-weighted imaging and fluid-attenuated inversion recovery imaging revealed an abnormally high signal in the white matter of the left cerebellar hemisphere sparing the dentate nucleus, consistent with the characteristic "shrimp sign". Pathological findings from brain biopsy were non-specific, however, JC virus-DNA was detected in the cerebrospinal fluid and anti-human immunodeficiency virus (HIV) antibodies were positive in the serum, leading to a diagnosis of HIV-associated progressive multifocal leukoence

Herpes simplex virus type 2 encephalitis in critically ill adults: A retrospective analysis of EURECA and Herpetics cohort studies.

BACKGROUND: Herpes simplex virus type 1 (HSV-1) is the leading cause of sporadic infectious encephalitis. Although typically associated with benign meningitis, HSV-2 can also cause encephalitis in adults, a condition that remains poorly characterized in intensive care unit (ICU). We aimed to compare the characteristics, management, and outcomes of critically ill adult with HSV-2 and HSV-1 encephalitis. METHODS: We performed a retrospective analysis of adults with herpes simplex encephalitis (HSE) admitted to the ICU in two large multicenter cohorts. Patients were classified according to cerebr

New onset seizure etiologies and outcomes among adults with HIV in the era of broadly available antiretroviral therapies.

OBJECTIVES: In this era of widely available antiretroviral therapy (ART), information regarding new onset seizure, seizure recurrence and mortality is needed to guide clinical decision-making. METHODS: A rural, prospective, cohort study was conducted April 2016 to June 2019. Inclusion criteria were HIV positivity, age ≥ 18 years, and new onset seizure. Two-year seizure recurrence was the outcome. Risk factors for seizure recurrence were evaluated using a Fine-Gray model to account for the competing outcome of death. RESULTS: 258 people were screened, 95 were eligible and 89 were enrolled. 47

Microglial-Targeted GCPII Inhibition Reverses Neurocognitive Impairment and Synaptic Loss After EcoHIV Infection.

HIV-associated neurocognitive impairment persists despite combination antiretroviral therapy, largely driven by chronic microglial activation that sustains neuroinflammation and neuronal injury. Activated microglia contribute to HIV-associated brain pathology by releasing proinflammatory mediators that disrupt synaptic integrity and impair cognition. N-acetylaspartylglutamate (NAAG), an abundant neuropeptide that maintains glutamatergic homeostasis, is hydrolyzed by glutamate carboxypeptidase II (GCPII) to glutamate. We previously demonstrated that reduced brain and cerebrospinal fluid NAAG le

Brain Structure of South African Children Born to Mothers on Dolutegravir Versus Efavirenz-Based Antiretroviral Therapy.

BACKGROUND: The impact of in utero exposure to specific antiretroviral therapy (ART), particularly integrase strand transfer inhibitors, on early brain development, remains poorly understood. We used magnetic resonance imaging (MRI) to compare brain structure in children who are HIV-exposed and uninfected (CHEU) with prenatal dolutegravir (DTG) vs efavirenz (EFV) exposure. METHODS: DolPHIN-2 was a randomized trial of pregnant women initiating DTG- vs EFV-based ART in the third trimester. At 3-4 years of age, a subgroup of their children from the South African cohort, along with HIV-unexposed

Risk factors for HIV-associated neurocognitive impairment and frailty in aging people with HIV in Latin America: a systematic review.

BACKGROUND: As people living with HIV (PWH) in Latin America and the Caribbean (LAC) have longer life expectancy due to improved access to antiretroviral therapy (ART), complications such as HIV-associated neurocognitive impairment (NCI; including HIV-associated neurocognitive disorder [HAND]) and frailty are increasingly more prevalent. We aimed to identify the risk factors for neurocognitive impairment and frailty among aging PWH in LAC. METHODS: We conducted a systematic review following PRISMA guidelines. We searched five databases that reported on NCI, HAND or frailty in PWH in LAC. Eligi

The influence of frailty on co-medication network structures and anticholinergic burden in people living with HIV.

BACKGROUND: Advances in antiretroviral therapy have extended the lifespan of people living with HIV, increasing age-related comorbidities and polypharmacy. This study aimed to examine how frailty is associated with co-medication network structure and anticholinergic burden in adults living with HIV. METHODS: This cross-sectional study analysed co-medication networks across robust, prefrail, and frail states using data from the Positive Brain Health Now cohort of adults aged ≥35 years. Frailty was assessed using a modified Fried Frailty Phenotype. Network metrics such as density (proportion of

HIV Promoters Isolated from Brain and Peripheral Tissue of Virally Suppressed PWH Are Phylogenetically and Functionally Similar.

Despite viral suppression with antiretroviral therapy (ART), reservoirs of Human Immunodeficiency Virus (HIV) persist in anatomical compartments throughout the body, including the brain. We have previously demonstrated that the HIV long terminal repeats (LTRs) isolated from the brains of non-virally suppressed people with HIV (PWH) are phylogenetically and functionally distinct from those isolated from matched peripheral tissue. While intact, transcriptionally competent HIV genomes persist within the brains of virally suppressed PWH, whether HIV LTRs are intact, functional, and compartmentaliz

Longitudinal changes in plasma biomarkers of immune activation, neuronal inflammation and injury in persons with HIV initiating ART.

BACKGROUND: Data on changes in biomarkers of brain health, and their associations with cognitive function in adults commencing either dual- or triple-antiretroviral therapy (ART) are sparse. METHODS: Plasma biomarkers (neurofilament light [NfL], glial fibrillary acidic protein [GFAP], sCD14, CXCL10, neopterin and IL-6) were measured at baseline and after 96 weeks on ART in individuals randomized to darunavir/ritonavir and either tenofovir-DF/emtricitabine (triple-ART, n = 119) or raltegravir (dual-ART, n = 119) in NEAT-001/ANRS143. Regression models examined associations of baseline and week-

Effects of HIV and azidothymidine on Alzheimer's-like pathology and amyloid beta transporters in hu-PBL-NSG mice, brain endothelial amyloid beta uptake and endothelial barrier integrity.

BACKGROUND: Despite increased life expectancy with antiretroviral therapy, people with HIV (PWH) have significantly higher prevalence of comorbid diseases, including neurocognitive impairment. PWH and neurocognitive impairment often show features of Alzheimer’s-like pathologies, including increased brain amyloid-beta (Aβ) and phospho-Tau; it is not known if antiretroviral drugs contribute to the development of these pathologies or potentiate HIV effects. We aimed to investigate whether azidothymidine (AZT), a drug still used by PWH in many resource-limited countries, alters or potentiates HIV-

Detection of JC virus DNA in CSF of pediatric patients with non-PML neurological disorders.

BACKGROUND: JC virus (JCV) is a human polyomavirus classically associated with progressive multifocal leukoencephalopathy (PML). Its role in other central nervous system (CNS) disorders - and the contribution of related polyomaviruses such as BK virus (BKV) and simian virus 40 (SV40) - remains poorly defined in pediatric populations, especially among immunocompromised patients. METHODS: We analysed 64 cerebrospinal fluid (CSF) specimens collected from children evaluated for suspected viral encephalitis or meningoencephalitis between March and October 2024. Broad-range and virus-specific PCR a

Deep mining of the human antibody repertoire identifies frequent and genetically diverse CDRH3 topologies targetable by vaccination.

Germline targeting vaccination strategies against highly variable pathogens such as HIV aim to elicit broadly neutralizing antibodies (bnAbs) with particular immunogenetic or structural features. The V2 apex of the HIV Env protein is a promising target for a class of bnAbs that contain conserved structural motifs in the heavy chain complementarity determining region 3 (CDRH3). Here, we show that these structural motifs are targetable by vaccination by characterizing V2 apex "axe-like" CDRH3s in the human repertoire and developing immunogens capable of engaging them. We determined the frequency
