Fournier gangrene (FG) is a form of necrotizing fasciitis that affects the genital and perineal tissues. This life-threatening condition is most often caused by a polymicrobial bacterial infection of the tissues. Predisposing factors include obesity, advanced age, chronic alcohol abuse, and immunosuppression. Although rare, FG can be life-threatening and can significantly impact the patient's quality of life. Adept nursing care can improve patient outcomes. This article discusses the pathophysiology and common complications of FG and outlines expected nursing considerations for patients with t

Fournier's gangrene tracking cephalad along spermatic cords complicates source control and soft tissue reconstruction. We describe a 61-year-old male with poorly controlled diabetes who presented with extensive scrotal necrotizing fasciitis tracking superiorly to the suprapubic region. Following wide excisional debridement, bilateral Penrose drains were passed through the spermatic cord tunnels and brought out through dependent perineal counter incisions. This technique provided effective through-and-through drainage of the cephalad tracts while limiting abdominal tissue loss. Deep wound cultu

BACKGROUND: The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score is a widely used adjunctive diagnostic tool for necrotizing soft tissue infection (NSTI). However, patients with high LRINEC scores do not uniformly have NSTI and often represent a diagnostic gray zone requiring urgent differentiation from other severe soft tissue infections. This study aimed to clarify the clinical characteristics of NSTI among patients with high LRINEC scores.
METHODS: We retrospectively reviewed patients hospitalized for severe soft tissue infections with sepsis between January 2014 and Dece

Vibrio vulnificus is a highly virulent marine Gram-negative pathogen that rapidly induces soft tissue necrosis and fatal septic shock, with infections generally linked to raw seafood ingestion or open wound contact with seawater. We report an atypical severe case of V. vulnificus septic shock in a 60-year-old male patient without recalled seafood or seawater exposure and without classic underlying high-risk diseases such as liver cirrhosis and diabetes mellitus. The patient presented with rapidly progressive swelling and subcutaneous ecchymosis of the right lower limb, which quickly deteriorat

INTRODUCTION: The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score was developed to estimate the likelihood of necrotizing fasciitis (NF); however, its diagnostic performance varies across populations, and evidence from Latin America remains limited.
OBJECTIVE: To assess the diagnostic performance of the LRINEC score in a Colombian population with skin and soft tissue infections.
MATERIALS AND METHODS: A retrospective cohort study was conducted among patients hospitalized at two tertiary care institutions in Colombia between 2016 and 2021. Patients with signs of systemic in

BACKGROUND: Scrotal abscesses requiring hospitalization are uncommon, with progression to sepsis being even rarer. A feared complication of a scrotal abscess is Fournier gangrene, an uncommon type of necrotizing fasciitis that starts in the scrotum or perineum. Fournier gangrene is associated with a mortality rate of about 40% and accounts for less than 0.02% of all hospital admissions. The patient in this report was hospitalized for a complex scrotal abscess and, during repeat incision and drainage (I&D), was found to have Fournier gangrene.
CASE DESCRIPTION: The patient was a 49-year-old ma

INTRODUCTION AND IMPORTANCE: Necrotizing fasciitis is a rapidly progressive, life-threatening soft-tissue infection associated with high mortality, particularly when diagnosis is delayed. While most cases involve the extremities or perineum, retroperitoneal necrotizing fasciitis is exceedingly rare. Its deep anatomical location often results in vague clinical presentations, allowing extensive infection before overt signs develop, which poses significant diagnostic and therapeutic challenges. Early recognition and timely intervention are critical to improving outcomes.

Fournier's gangrene is a rare, life-threatening form of necrotizing fasciitis affecting the perineum and genital region. In Ayurveda, it correlates with Kotharoga, as both share symptoms such as severe tissue necrosis, pus discharge, and a foul odor. This case report discusses a 50-year-old male who presented with fever, severe scrotal pain, blackish discoloration, and purulent discharge two days after undergoing incision and drainage (I&D) of a perianal abscess. The case was diagnosed based on clinical examination and imaging studies. Broad-spectrum antibiotics, along with herbo-mineral medic

BACKGROUND: Operating in acute surgical settings is significantly different from elective surgery. One aspect is the complexity of acutely ill or injured patients with abnormal physiology, unoptimized comorbidities, and unplanned interventions. Traditional visual and tactile assessments of the surgical field anatomy and tissue viability have documented limitations. The need for integrated technology to augment such assessments is described. Indocyanine green fluorescence-guided surgery (ICG-FGS) is a promising tool, given its history, safety, low cost, and accumulated experience in elective se

BACKGROUND: Odontogenic infections may progress to life-threatening complications if not managed promptly, and socioeconomic circumstances have been linked to their presentation, particularly where access to dental care is limited. Because every patient studied was already hospitalised, this analysis addresses the clinical type of infection at presentation, not the risk of developing one.
METHODOLOGY: A retrospective cross-sectional review of hospital records was undertaken at the Allied Ward of Sunyani Teaching Hospital (January 2021 to January 2025). Of 811 consecutive admissions, 525 were

Fournier's gangrene (FG) is a fulminant and life-threatening polymicrobial necrotizing fasciitis of the perineal, perianal, and genital regions. It typically affects patients with significant comorbidities, particularly poorly controlled diabetes mellitus, and is associated with high mortality rates. We describe the case of a 57-year-old male patient who presented with a one-month history of severe perineal pain, initially misdiagnosed as uncomplicated hemorrhoidal disease. Upon admission, the patient exhibited systemic inflammatory response syndrome and a large, purulent perianal mass. The pa

Wohlfahrtiimonas chitiniclastica is a rare Gram-negative bacillus associated with fly larvae, myiasis, chronic wounds, and severe polymicrobial infection. We report a 71-year-old woman with diabetes, peripheral neuropathy, severe peripheral vascular disease, and previous contralateral above-knee amputation who presented with septic shock secondary to an infected right hallux ulcer. Radiographs demonstrated soft-tissue gas extending into the distal calf. Emergency exploration revealed extensive necrosis and dishwater pus tracking proximally along the flexor hallucis longus tendon, consistent wi

Bevacizumab-associated intestinal perforation and Fournier's gangrene have been reported separately; however, the potential contribution of residual anti-VEGF exposure prior to subsequent chemotherapy in previously irradiated pelvic tissue remains underrecognized. Radiotherapy causes fibrosis and microvascular injury, bevacizumab impairs healing, and irinotecan-induced diarrhea may exacerbate perforation risk. Deferring DRE, imaging, and surgical consultation in patients with unexplained perineal or buttock pain increases the risk. A 61-year-old man with rectal adenocarcinoma received neoadjuv

BACKGROUND: Early differentiation of necrotizing fasciitis (NF) from non-necrotizing soft tissue infections (non-NSTI) remains a clinical challenge because delayed diagnosis is associated with substantial morbidity and mortality. Existing diagnostic scoring systems have shown inconsistent performance across different populations and often rely on multiple laboratory variables, limiting their routine clinical applicability. Therefore, this study aimed to develop and internally validate a simple diagnostic score based on routinely available laboratory parameters for the early identification of N

Deep chest wall abscesses may be difficult to distinguish from primary pyomyositis and necrotizing soft-tissue infection because of overlapping clinical and radiographic findings. Although soft-tissue gas raises concern for necrotizing infection, it is not independently diagnostic and may reflect anaerobic bacterial activity, communication with the skin, or prior instrumentation. Infections of the pectoral region involving uncommon polymicrobial organisms are rarely reported. A 61-year-old man with prediabetes, hypertension, chronic kidney disease, and a prior cerebrovascular accident presente

Survival after necrotizing fasciitis has improved, producing a growing population of survivors whose function and appearance are determined by reconstruction. Guidance on reconstructive timing and technique, however, remains limited. This structured narrative review synthesizes evidence from PubMed/MEDLINE on reconstruction after necrotizing fasciitis from the perspective of the plastic surgeon and proposes a practical, timing-based algorithm. Its central thesis is that reconstruction is planned from the first debridement: wound-bed readiness and patient physiology, rather than a preferred tec

INTRODUCTION: Posterior medial meniscal root tears (PMMRTs) are biomechanically similar to meniscectomy and often occur in middle-aged women with low-energy trauma. The literature reports consistently high failure rates, meniscal extrusion, and early progression of osteoarthritis following isolated arthroscopic repair in varus-malaligned knees, while isolated arthroscopic repair is reported to have good outcomes in well-aligned knees. The need for concomitant high tibial osteotomy (HTO) to protect root repair in varus knees is an ongoing controversial topic with emerging high-quality evidence.
