Fournier's gangrene (FG) is a rapidly progressive necrotizing fasciitis of the perineal and genital regions associated with high mortality. Prompt intervention is crucial, particularly in patients with predisposing risk factors. A 75-year-old man with type 2 diabetes mellitus receiving long-term corticosteroid therapy presented with progressive scrotal pain and left thigh swelling. Physical examination revealed scrotal necrosis, and contrast-enhanced computed tomography demonstrated a perianal abscess with extensive subcutaneous emphysema. Emergency surgical debridement and bilateral orchiecto

BACKGROUND: Necrotizing soft tissue infection represents a spectrum of bacterial infections that result in rapidly progressive, life-threatening tissue necrosis. Early diagnosis remains a challenge due to the rarity of the condition and variable presentation. Herein, we present a case of Meleney's gangrene managed at a tertiary healthcare facility in Nigeria.
CASE PRESENTATION: The patient presented to the emergency department with a 13-hour history of inability to pass urine associated with pain. Physical examination revealed a tender urinary bladder consistent with a diagnosis of acute urin

Necrotizing soft tissue infections (NSTIs) are rapidly progressive, life- and limb-threatening emergencies requiring immediate surgical source control. Group B Streptococcus (GBS; Streptococcus agalactiae) is a rare cause of NSTIs in adults. We present a 44-year-old woman with poorly controlled diabetes who developed progressive right foot pain and swelling after minor ankle trauma. Examination showed spreading erythema, crepitus, dusky discoloration, malodor, and a deep plantar wound, accompanied by marked leukocytosis and diabetic ketoacidosis. Computed tomography demonstrated infection exte

Necrotizing soft tissue infections are life-threatening infections of soft tissue that are treated with rapid surgical debridement when the diagnosis is suspected. Necrotic tissue is removed until vital tissue is identified, guided by the surgeon's vision and hands. Fluorescence-guided surgery is increasingly popular for assessing tissue perfusion. This surgery may have the potential to improve the diagnosis and treatment of necrotizing soft tissue infections. A systematic review on fluorescence-guided surgery for necrotizing soft tissue infections was conducted according to the PRISMA guideli

Background: Bacteria-mediated cancer therapy is emerging as a promising approach due to its low cost, scalability, and sustained efficacy. While bacterial immunomodulation has been extensively studied, the role of bacterial metabolic competition, particularly for glucose, in tumor suppression remains unclear. We aimed to investigate how engineered Salmonella strains exploit metabolic competition to trigger antitumor immunity and improve therapeutic outcomes in bladder cancer models. Methods: Using genetic screening, we generated a gluconeogenesis-deficient Salmonella strain, the AISIH strain w

UNLABELLED: The study investigated the effects of a one-year literary program on children's intergroup attitudes toward peers with a migration background and on their perceptions of inclusive classroom norms. Fourth- and fifth-grade students and their teachers discussed four literary fiction books in small groups, with children taking the lead by asking their own questions, engaging in dialogue, and challenging one another. The books addressed topics such as intergroup exclusion, inclusive group norms, and civil courage. A total of 744 children participated in pre- and posttest assessments. Th

OBJECTIVE: We evaluated the early diagnostic value of interleukin-6 (IL-6) for type I and type II necrotizing fasciitis (NF).
METHODS: A retrospective analysis was conducted of patients with NF between September 2020 and December 2024. They were divided into type I and type II NF. General clinical data, cytokines, and inflammatory markers were compared between the two groups. Threshold cytokine concentrations predictive of type I and type II NF were determined using receiver-operating characteristic (ROC) curve analysis.
RESULTS: A total of 106 patients with NF were included in the study, 74

Necrotizing soft tissue infections (NSTI) are serious infections that are typically treated with aggressive debridements. Coverage of those extensive full-thickness integumentary defects requires complex reconstructions of both the dermal and epidermal layers with the goal of achieving close to normal pliability. A key determinant factor in the development of scar tissue is the time to create a graftable surface and curtail inflammatory reactions. A second limiting factor is the extent of donor skin needed for coverage. Kerecis, a fish skin-derived regenerative tissue matrix, is a novel produc

BACKGROUND Necrotizing fasciitis, a subtype of necrotizing soft-tissue infection, is a rare but life-threatening condition that requires a prompt and decisive approach. Early surgical debridement remains a key treatment; however, after debridement, the management of complex wounds, particularly in high-risk patients, remains a major clinical challenge and often requires prolonged hospitalization and multiple surgical interventions. These situations may warrant alternative techniques, such as negative pressure wound therapy, to support effective wound healing. This case demonstrates that a beds

INTRODUCTION: In the Caribbean, centipede bites are well known for their painful symptoms. Although these bites are common, they rarely result in medical consultations. This study aimed to describe the frequency, clinical presentation, and management of centipede bites presenting to the emergency department.
METHODS: A retrospective, descriptive study was conducted over 7 years (2015-2021) of all patients presenting to the emergency department for centipede bites. Patients were categorized into two groups: those with severe envenomation requiring hospitalization (hospitalized group) and those

BACKGROUND: Necrotizing fasciitis (NF), a fulminant soft tissue infection requiring urgent surgical intervention, presents substantial diagnostic challenges due to its non-specific clinical manifestations during initial stages. This study investigates the prognostic value of two novel inflammatory biomarkers-the systemic inflammatory response index (SIRI) and C-reactive protein-to-albumin ratio (CAR)-in predicting disease progression and clinical outcomes in NF patients.
METHODS: This retrospective cohort study analyzed clinical data from 192 patients with confirmed soft tissue infections, co

Background Necrotizing fasciitis (NF) is a rapidly progressive, life-threatening infection with significant morbidity and mortality. Rural and remote regions face unique challenges in the timely diagnosis and management of NF. Methodology This retrospective study included all patients diagnosed with NF in the Kimberley region, Western Australia, between June 2020 and June 2025. Data were extracted from medical and surgical records, microbiology databases, and theater logs. Demographic, clinical, microbiological, and outcome variables were analyzed descriptively. Results In total, 21 patients w

BACKGROUND: Necrotizing fasciitis (NF) is a fulminant soft-tissue infection with high in-hospital mortality. Although NF is traditionally classified into types I-IV by microbiology, emerging data - particularly from East Asia - suggest that monomicrobial Gram-negative (mono-GNB) NF is epidemiologically and prognostically distinct. This systematic review and meta-analysis aimed to quantify global subtype distributions and in-hospital mortality, with a focus on mono-GNB NF.
METHODS: We searched PubMed/MEDLINE, Embase, Web of Science, and Cochrane Library from inception to 1 September 2023, for

Necrotizing soft tissue infections (NSTIs), particularly necrotizing fasciitis (NF), are life-threatening conditions with high mortality, where early diagnosis is essential to improve outcomes. However, the clinical presentation often mimics less severe infections, contributing to diagnostic delays. This case series explores the utility of point-of-care ultrasound (POCUS) in the early recognition of NF through the presentation of 3 adult patients evaluated in the Emergency Department of a tertiary care center. All patients underwent bedside ultrasound at the time of admission, with findings co

INTRODUCTION: Subcutaneous emphysema (SE) of the upper extremity is a rare condition, typically associated with penetrating trauma, infections, or chest injuries. Its occurrence following blunt trauma with an isolated scapular fracture is exceedingly uncommon and may mimic more serious conditions such as necrotizing fasciitis. We present a rare case of upper-limb SE associated with an open scapular fracture and propose a diagnostic approach to distinguish benign SE from limb-threatening infections.
CASE REPORT: A 65-year-old male presented with pain and swelling in the right upper limb follow

A 67-year-old man with no history of diabetes presented with generalized body pain and impaired mobility. He was admitted with severe hyperglycemia, diabetic ketoacidosis, and septic shock. Although the patient experienced minimal cutaneous symptoms, he experienced progressive swelling and pain in the right upper and left lower extremities. Whole-body imaging revealed an extensive fascial plane abscess. He underwent three surgical debridements and improved with prolonged antibiotic therapy. Necrotizing soft tissue infections are rapidly progressing and life-threatening. This case highlights a
