Autologous Millifat Grafting as a Reconstructive Strategy for Complex Lower Limb Defects in a Diabetic Patient After Necrotizing Fasciitis.

Lower limb ulcers, especially in patients with diabetes mellitus, represent a major therapeutic challenge, particularly when complicated by severe infections such as necrotizing fasciitis. These conditions often require extensive surgical interventions, including amputation. We present the case of a 68-year-old female patient with poorly controlled type 2 diabetes mellitus who developed extensive necrotizing fasciitis in the left lower limb following trauma with plant material, resulting in a large soft tissue defect. The patient was not a candidate for regional or distant flaps due to her non

Economic Burden of Inpatient Odontogenic Infections in the United States.

BACKGROUND: Odontogenic infections represent a significant yet preventable cause of health care expenditure in the United States. However, their economic burden is not well defined. PURPOSE: The purpose of this study was to estimate the per-admission hospital costs of managing odontogenic infections and identify factors associated with increased cost. STUDY DESIGN, SETTING, AND SAMPLE: This was a retrospective cohort study using the National Inpatient Sample database from years 2017 to 2019. Adults admitted for management of an odontogenic infection based on ICD-10 coding were included. Pati

Pyogenic flexor tenosynovitis complicated by necrotizing soft tissue infection in postpartum: A case report.

INTRODUCTION: Pregnancy and the immediate postpartum are periods of immune reconstitution inflammatory syndrome (IRIS), exposing patients to an increased risk of severe infections. Among these, necrotizing soft tissue infection (NSTI) is a rare but serious, life- and function-threatening condition. CASE REPORT: We report the case of a 31-year-old patient three weeks postpartum, admitted for acute right hand pain with septic signs. The initial clinical picture, typical of a pyogenic flexor tenosynovitis of the ring finger, rapidly evolved into NSTI of the palm. Despite urgent medical and surgi

Abdominal Wall Abscess after Prophylactic Percutaneous Endoscopic Gastrostomy Placement in a Patient Undergoing Chemoradiotherapy for Laryngeal Cancer.

Prophylactic percutaneous endoscopic gastrostomy (PEG) placement for nutritional management prior to chemoradiotherapy is a common procedure in patients with head and neck cancer, for which serious complications are rare. Herein, we present a case of abdominal wall abscess that developed 12 days following prophylactic PEG placement in a patient with laryngeal cancer. This rare complication was initially difficult to diagnose due to subtle abdominal symptoms. In this case, antibiotic treatment alone was inadequate, necessitating drainage tube insertion. This case highlights that delayed diagnos

Case Report: Negative pressure wound therapy with instillation and dwell time as adjuvant therapy for limb salvage in a complicated necrotizing fasciitis on ischemic diabetic foot.

BACKGROUND: Necrotizing fasciitis superimposed on ischemic diabetic foot ulcers represents a complex and limb-threatening condition, especially in patients with multiple comorbidities such as end-stage renal disease, peripheral arterial occlusive disease, and poorly controlled diabetes mellitus. Optimal wound management strategies that minimize surgical burden while enhancing infection control and tissue regeneration are essential. This case highlights the use of multimodal adjuvant therapies and multidisciplinary management for limb salvage in a high-risk patient. CASE PRESENTATION: A 60-yea

Case Report: Total atherosclerotic occlusion of perforators in anterolateral thigh flap.

Atherosclerosis and calcification of vessels, commonly encountered in diabetic patients, is a well-known complicating factor in microsurgical reconstruction. However most studies on this problem have focused on axial vessels and their major branches. We present a case of total atherosclerotic occlusion of perforators rising from intact lateral circumflex femoral artery branches in bilateral anterolateral thigh (ALT) flap elevation. A 51-year-old male with a history of diabetes mellitus, ischemic heart disease and kidney transplantation presented with necrotizing fasciitis of the left foot. Ser

Necrotizing Fasciitis in a Child in a Third-World Country: A Case Report.

Necrotizing fasciitis is a soft tissue infection that represents an emergency due to its alarmingly fast progression. It requires surgical debridement and antibiotics, but more advanced interventions can be needed, such as reconstructive procedures, including an Ollier-Thiersch dermograft, as in the case we are presenting. We introduce the history and clinical evolution of a patient with necrotizing fasciitis, who underwent dermoplasty of the right temporal region. This case report was conducted in the Plastic Surgery Service at Hospital Militar Escuela Dr. Alejandro Dávila Bolaños, during 202

Declining Diagnostic Accuracy of the Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) Score in Necrotizing Fasciitis: A Case Report With Contemporary Evidence Review.

Necrotizing fasciitis is a rapidly advancing soft-tissue infection with high morbidity and mortality, and markedly worse prognosis when complicated by delayed diagnosis and intervention. Distinguishing necrotizing fasciitis from less severe soft-tissue infections remains a significant clinical challenge, prompting the development of the Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score to aid early recognition. Early enthusiasm for the score led to widespread clinical use; however, subsequent experience has raised concerns about its utility as a sensitive rule-out tool. We pre

Individualized multidisciplinary management of severe odontogenic deep neck infections with and without descending necrotizing mediastinitis: a report of two cases.

Severe odontogenic deep neck infections (DNIs) can rapidly extend along cervical fascial planes and progress to descending necrotizing mediastinitis (DNM) with high mortality, requiring early diagnosis and multidisciplinary management, especially in elderly patients with comorbidities. We report two elderly patients with rapidly progressive odontogenic infections: one with DNM fulfilling Estrera's criteria (Endo type IIA) and another with extensive DNI without mediastinal spread. Clinical features, serial contrast-enhanced computed tomography (CECT) findings, laboratory risk indicator for necr

Fatal Necrotizing Fasciitis Caused by Pasteurella multocida Without Cutaneous Findings: When Pain Is the Clue.

We report a case of fatal necrotizing fasciitis in an elderly woman presenting with severe right upper-arm pain and elevated inflammatory markers in the absence of apparent skin changes or diagnostic imaging findings. Despite broad-spectrum antimicrobial therapy and supportive care, the patient deteriorated rapidly and died within 24 hours of hospital admission. Post-mortem blood cultures identified Pasteurella multocida, a zoonotic pathogen commonly found in cats and dogs. Retrospective review of the initial CT scan revealed fascial edema and soft tissue stranding, compatible with necrotizing

Overdiagnosis of necrotizing fasciitis: clinical-radiological suspicion versus intraoperative findings in 62 fasciotomy patients.

INTRODUCTION: Necrotising fasciitis (NF) is a rare but life-threatening soft tissue infection, whose diagnosis remains challenging due to non-specific early clinical signs. This retrospective single-center study aimed to compare the clinical, radiological, and microbiological characteristics, as well as outcomes, of patients undergoing fasciotomy for suspected NF, distinguishing between those with surgically confirmed necrosis and those without. METHODS: Data from 62 adults who underwent fasciotomy for suspected NF between 2013 and 2023 were retrospectively analyzed. Patients were stratified

Life-threatening cervical necrotizing fasciitis caused by congenital pyriform sinus fistula: A case report.

We report the case of a 2-year-old boy who presented with cough for one week and fever for 4 days. On admission, physical examination revealed restricted neck movement and a firm, tender mass on the left side of the neck with a well-defined border and limited mobility. Within 24 h, the mass enlarged rapidly, and blue-red discoloration of the overlying skin appeared and progressed quickly. The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score was 9. The child underwent urgent surgical debridement. Intraoperative findings confirmed the presence of a congenital pyriform sinus fis

Prediction of mortality in necrotizing fasciitis: comparative evaluation of established prognostic scores and a novel scoring system in a retrospective cohort.

BACKGROUND: Necrotizing fasciitis (NF) is a rare but serious surgical emergency that progresses rapidly, requires urgent operative intervention, and carries a high mortality rate. Current prognostic scoring systems may have limited predictive power for mortality across different patient groups. The aim of this study was to identify clinical, laboratory, and radiological factors associated with mortality in patients with necrotizing fasciitis. In addition, the study aimed to provide a foundation for the development of a practical prognostic scoring system that could support early risk stratific

A Silent Killer in the Waters: A Report of Two Cases of Vibrio vulnificus Bacteremia.

In this case series, we report two unusual cases of Vibrio bacteremia due to exposure to floodwater after Hurricane Helene. This presentation is particularly alarming in light of growing evidence that warming ocean temperatures are contributing to stronger hurricanes, which, in turn, lead to more extensive flooding and storm surges, a trend that creates ideal conditions for the spread of waterborne pathogens such as Vibrio vulnificus. Previously, within the humid subtropical region, only a handful of cases have been documented over the past 20 years. With the increasing number of cases, it is
