Necrotizing fasciitis is a severe soft tissue infection characterized by rapid necrosis of the skin, subcutaneous tissue, and fascia. Its onset is sudden, and progress is rapid. Complications such as sepsis, septic shock, and multiple organ dysfunction syndrome (MODS) can appear early, leading to high mortality and disability. The early symptoms lack specificity, posing diagnostic challenges and a high risk of misdiagnosis. This case report describes a 37-year-old male patient who presented with progressive redness, swelling, and pain of the left lower limb. Bedside incision and exploration un

Necrotizing soft tissue infections (NSTIs) represent a group of rapidly progressing, life-threatening infections characterized by widespread tissue necrosis, systemic inflammation, and multiorgan failure. Early diagnosis remains a clinical challenge because of nonspecific initial manifestations and overlapping symptoms with other soft tissue infections. Diagnostic scoring systems such as the Laboratory Risk Indicator for Necrotizing Fasciitis score and its variants have been widely utilized to facilitate early recognition but are limited by variable sensitivity and insufficient predictive valu

Necrotizing fasciitis (NF) rarely spreads from the chest to the lumbosacral region. We report a fatal case in a 61-year-old man, documented by serial imaging and surgical exploration, that provides strong evidence suggesting subcutaneous and fascial spread of infection from a thoracic drainage site. This case highlights a rare but deadly complication and suggests that increased vigilance may be warranted in immunocompromised patients.

Cervicofacial necrotizing fasciitis is a life-threatening surgical emergency associated with significant morbidity and mortality, requiring early diagnosis and aggressive multimodal management. Reconstruction of the resulting soft tissue defects represents an additional challenge, particularly in resource-limited settings where microsurgical infrastructure may not be available. We report the case of a 42-year-old man who developed cervicofacial necrotizing fasciitis secondary to an odontogenic infection. Management consisted of broad-spectrum antibiotic therapy, early surgical debridement, and

PURPOSE: Necrotizing fasciitis (NF) is a severe orthopaedic emergency. While surgical management and critical care have improved survival, long-term quality of life (QoL) for survivors remains poorly understood. This study evaluates patient-reported QoL post-surgery using the EuroQol-5 Dimensions-5 Levels (EQ-5D-5L) and 26-Item Short Form Survey (SF-36), examining their relationship with initial disease severity as measured by the Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) score.
METHODS: This cross-sectional study included 21 patients treated surgically for NF between 2015-

BACKGROUND: Necrotizing fasciitis is a life-threatening soft-tissue infection that still carries an in-hospital mortality of 11-30% and an amputation rate of 7-22% despite modern treatment. Only limited population-based data have been available for Germany to date.
METHODS: We carried out a retrospective, nationwide descriptive analysis of aggregated administrative routine data from the German DRG system. All hospitalizations with a main diagnosis of necrotizing fasciitis (ICD-10-GM: M72.6) from 2019 to 2024 were included. The prevalence, demographic features, and anatomical distribution of n

INTRODUCTION: Limb necrotizing fasciitis (limb NF) is a severe soft tissue infection where early diagnosis is critical but often hindered by non-specific manifestations. This study investigates the diagnostic utility and prognostic assessment ability of the C-reactive protein-to-albumin ratio (CAR) and the neutrophil-to-lymphocyte ratio (NLR) for limb NF diagnosis and prognostic stratification.
METHODS: In this retrospective study, 145 patients with limb soft-tissue infections were categorized into limb NF (n = 61) and non-NF (n = 84) groups. Poor prognosis (n = 28) was defined as in-hospital

Necrotizing fasciitis is a life-threatening soft tissue infection requiring prompt diagnosis and aggressive treatment. Mortality rates have decreased due to improved diagnosis and management in recent years, necessitating a greater focus on preserving quality of life in survivors. Skin-sparing debridement has been championed by plastic surgeons as an alternative to traditional en bloc debridement. Negative-pressure wound therapy with instillation and dwell has also emerged as a treatment for necrotizing fasciitis. This treatment decreases bacterial bioburden in wounds; however, it is typically

Necrotizing fasciitis is a rapidly progressive and life-threatening soft tissue infection in which early diagnosis remains challenging when initial clinical findings are subtle or nonspecific. Hyperglycemic crises are frequently triggered by infection and may obscure the underlying etiology when metabolic abnormalities dominate the initial presentation. We report a case of a 52-year-old female with rheumatoid arthritis receiving chronic corticosteroid therapy, an important risk factor for atypical infectious presentations, who presented with altered mental status, dehydration, and severe hyper

PURPOSE: Necrotising fasciitis (NF) requires rapid diagnosis to prevent morbidity and mortality. Yet clinicians rely heavily on imaging to help guide diagnosis, but type II NF is caused by non-gas producing organisms. Thus, the aim of this study was to assess computer topography (CT) imaging findings of type I compared to type II NF.
METHODS: This was a retrospective study conducted by reviewing medical records with NF from 1/1/2020 to 12/31/2024 at a single medical centre. The presence of air on CT images was evaluated by reviewing radiology reports and manual review of imaging. Comparisons

CASE: A 75-year-old man presented with left ankle septic arthritis on a background of multiple surgeries for bilateral lower limb necrotizing fasciitis involving free flap and skin grafting. We employed a unique technique of ankle arthrodesis using intra-articular pinning with a combination of Schanz pins and Kirschner wires for ankle arthrodesis after joint destruction from septic arthritis following adequate debridement and curettage achieving partial union at 3 months and complete union by 6 months, with the patient returning to full activities of daily living at 1 year.

BACKGROUND: Necrotizing soft tissue infections (NSTIs) are life-threatening and require emergent surgical debridement. Benign subcutaneous emphysema is a rare, noninfectious condition characterized by subcutaneous gas on imaging without systemic symptoms or abnormal laboratory values typical of NSTIs. Patients with subcutaneous emphysema and risk factors for NSTI can pose a diagnostic challenge.
CASE REPORT: We report an 18-year-old female with Ehlers-Danlos syndrome who developed left elbow pain and crepitus following multiple insect bites, freshwater pond exposure, and contact with farm ani

INTRODUCTION: Necrotizing fasciitis (NF) is a rapidly progressive, life-threatening soft tissue infection characterized by necrosis of the fascia and subcutaneous tissues, with high rates of amputation and mortality. Early diagnosis and aggressive surgical intervention are critical determinants of outcome, yet data from eastern India remain scarce. This study aimed to describe the clinicopathological profile of NF and evaluate factors associated with poor outcomes in patients admitted to a tertiary care center in Jharkhand.
METHODS: This hospital-based observational study was conducted in the

Necrotizing fasciitis (NF) is a rapidly progressive, life-threatening soft tissue infection. While the extremities and trunk are most commonly affected, the anterior cervical region represents a rare site and poses additional risks due to its proximity to vital structures such as major vessels and the airway. Prompt diagnosis and aggressive management are critical for patient survival. We report a 44-year-old previously healthy male who presented with a one-week history of worsening anterior neck infection, pain, and tissue necrosis. Laboratory tests revealed elevated acute-phase reactants, an

End-tidal carbon dioxide (ETCO2) monitoring is the standard method for confirming endotracheal tube placement during emergency airway management. However, massive pulmonary embolism can create paradoxical capnography findings that complicate airway confirmation and resuscitation efforts. We present two cases of cardiac arrest where massive pulmonary embolism masked expected ETCO2 changes despite confirmed endotracheal tube placement. In the first case, a 33-year-old male with diabetic ketoacidosis developed cardiac arrest with absent colorimetric ETCO2 change despite direct visualization of co

Necrotizing soft tissue infections (NSTIs) remain associated with high mortality rates. Improvements in patient outcomes have been limited, largely due to persistent challenges in early diagnosis, surgical management, and care organization. NSTI is frequently misdiagnosed at initial presentation, and uncertainty regarding tissue viability often necessitates wide surgical debridement, resulting in large wounds and substantial scarring. In addition, care pathways are frequently fragmented, thereby limiting access to specialized expertise. Nevertheless, recent years have seen promising opportunit

BACKGROUND: Multifocal necrotizing fasciitis (MulNF) is a rare presentation of necrotizing fasciitis (NF) characterized by involvement of more than one non-contiguous anatomical site during the same disease episode. Comparative data between monofocal and multifocal NF remain limited.
METHODS: We performed a retrospective single-center cohort study of patients treated for NF between 2015 and 2026. Patients were classified as MNF (single site) or MulNF (multiple non-contiguous sites) based on chart review. Demographics, comorbidities, microbiology, laboratory values, organ failure, ICU course,

AIMS: Necrotizing fasciitis (NF) is a rapidly progressive surgical emergency. The effects of time from hospital presentation to theatre on short- and long-term outcomes, as well as specific modifiable aspects of the care pathway that would enable early surgery, remain poorly defined.
METHODS: A retrospective cohort study of all suspected cases of limb NF presenting across a large health board region over ten years (January 2015 to April 2025) was undertaken. Early surgery was defined as ≤ six hours from presentation (triage time) to theatre (operation start time). The independent associations
