A challenging diagnosis of syphilis in a patient with a testicular lump: a case report.

BACKGROUND: Syphilis is a re-emerging sexually transmitted infection known for its heterogeneous clinical manifestations and ability to mimic neoplastic, inflammatory, and infectious diseases. Testicular involvement is rare and may radiologically resemble testicular cancer, frequently leading to unnecessary orchiectomy. We report a case of syphilitic orchitis initially suspected to be a testicular malignancy, highlighting the importance of including syphilis in the differential diagnosis of atypical testicular lesions. CASE PRESENTATION: A previously healthy 36-year-old man presented with a t

Abnormalities in retinal vascularity and blood perfusion associated with early syphilis revealed by optical coherence tomography angiography.

AIM: To explore the feasibility of optical coherence tomography angiography (OCTA) in the early detection of syphilis and syphilitic chorioretinopathy. METHODS: Patients with newly diagnosed syphilis were enrolled from September 2022 to November 2023. All the participants underwent serological examinations including rapid plasma reagin (RPR) test and treponema pallidum particle agglutination (TPPA) test, as well as macular OCTA scanning. Quantitative indicators including macular vessel flow density (VFD), vessel length density (VLD), and foveal avascular zone (FAZ) were measured and analyzed.

Phosphodiesterase-5 Inhibitor Use Masking a Necrotizing Syphilitic Infection: A Rare Differential Diagnosis of Priapism.

BACKGROUND: Acute penile enlargement is often presumed to indicate priapism, particularly after Phosphodiesterase-5 inhibitor use. However, alternative diagnoses, including infectious and necrotizing processes, should be considered-especially in patients with a history of unprotected sex-to avoid delayed or inappropriate management. CASE PRESENTATION: A 43-year-old male with a history of intravenous drug use presented with acute penile swelling and leukocytosis (20,500/μL) following unprotected sexual intercourse and tadalafil consumption. Although priapism was suspected, corporal blood gas a

A rare case of syphilitic hepatic inflammatory pseudotumor misdiagnosed as liver metastasis: Case report and mini-review.

RATIONALE: Syphilitic hepatic inflammatory pseudotumor is an exceedingly rare benign hepatic manifestation of syphilis. Due to its clinical and radiological resemblance to malignant liver tumors, it frequently poses a diagnostic challenge, often leading to potential misdiagnosis as metastatic disease. PATIENT CONCERNS: A 48-year-old male truck driver presented with a 1-month history of fatigue, decreased appetite, and persistent right upper abdominal discomfort, accompanied by a 5-kg weight loss. DIAGNOSES: Physical examination revealed tenderness in the right upper abdomen. Laboratory tests

Unknown Duration or Late Syphilis Cases with High Titers-North Carolina, 2015 and 2023.

BACKGROUND: While recent national increases in unknown duration or late syphilis (UDLS) have outpaced diagnoses of earlier stages of syphilis, the causes and contributors remain unknown. We evaluated changes in the proportion of UDLS cases with high nontreponemal test titers in one jurisdiction to identify if UDLS cases, which often have lower titers compared with early syphilis cases, might increasingly be early syphilitic infections. METHODS: We identified UDLS cases with high titers (defined as rapid plasma reagin titers ≥1:32) reported to North Carolina during the years 2015 and 2023. We

Large vessel vasculitis: syphilitic aortitis as a reversible mimicker.

Large vessel vasculitis (LVV) in the elderly often prompts consideration of giant cell arteritis; however, mimickers must be carefully excluded. We report the case of a 70-year-old woman with constitutional symptoms, elevated inflammatory markers, and FDG-PET findings suggestive of LVV. Spontaneous clinical and laboratory improvement in the absence of immunosuppression raised suspicion for an alternative diagnosis. Further evaluation revealed positive treponemal serology, and syphilitic aortitis was confirmed. Treatment with penicillin led to complete resolution of vascular inflammation. This

Rectal syphilis: a great imitator of rectal malignancy.

A 48-year-old transgender woman presented with a sudden onset of bloody stool. Colonoscopy revealed an ulcerated, circumscribed mass in the lower rectum, closely resembling a Borrmann type 2 malignancy. The patient lacked classic signs of syphilis, such as anal pain, primary genital chancres, or localized lymphadenopathy. However, serological assays were highly reactive, and immunohistochemical analysis of the biopsy specimen confirmed the presence of Treponema pallidum, establishing the diagnosis of syphilitic proctitis. A four-week oral amoxicillin regimen (1,500 mg/day) led to complete clin

Malignant syphilis with presumed multifocal osteomyelitis in a patient with human immunodeficiency virus: A case report.

Malignant syphilis is a rare and aggressive form of secondary syphilis that occurs predominantly in individuals living with human immunodeficiency virus. Owing to its rarity, the prevalence of systemic involvement and the optimal therapeutic approach remain incompletely defined. A 37-year-old man with human immunodeficiency virus presented with fever, weight loss, and a generalized nonpruritic rash characterized by ulcerative lesions covered with rupioid crusts. The initial rapid plasma reagin test was negative; however, repeat testing revealed seroconversion of the rapid plasma reagin test, w

Treponema pallidum TprD and TprK Are Adhesins and Promote Spirochetes Opsonophagocytosis.

UNLABELLED: Treponema pallidum subspecies pallidum ( T. pallidum ) causes systemic syphilis, exclusively infects humans in nature and can persist for decades in the absence of treatment despite generating robust adaptive immune responses. The T . p allidum r epeat (Tpr) family of outer membrane proteins are immunogenic but have been implicated in immune evasion due to antigenic variation, indicating that Tprs are virulence factors displayed on the spirochetes surface. Long-term survival of T. pallidum is largely attributed to the sparse surface-exposed outer membrane proteins and antigenic var

"Ocular syphilis with reactive arthritis mimicking rheumatoid arthritis" - A diagnostic dilemma, case report, and review of literature.

Syphilis is a great mimicker of diseases and has a broad presentation ranging from painless chancre, constitutional symptoms, adenopathy, rash, synovitis, neurological, and ocular findings. Syphilis-related bone and joint involvement is usually found in congenital form, but it can also be seen in adults with acquired syphilis as a rare sequela of infectious syphilis. We report a case of a patient diagnosed as rheumatoid arthritis, who presented to the ophthalmology outpatient department with an acute onset of black spots in his visual field, whose clinical examination was a diagnostic dilemma,

Congenital syphilis in an infant at a Nigerian Hospital.

A 2-month-old male infant was diagnosed as having congenital syphilis at the Outpatient children's Clinic at LAUTECH, Teaching Hospital, Ogbomoso. He was delivered at a primary health center with an abnormally shaped nose and oval flesh-like swelling adjacent to the lateral wall of the left nostril. Over the first 2 months of life, he developed a slowly progressive swelling around the wrist. Despite several courses of antibiotics prescribed by several doctors at both tertiary and private hospitals, for an unknown illness, he did not get better. The mother of the infant referred her infant clos

Placenta Non Grata: A Rare Presentation of a Reemerging Disease.

BACKGROUND: Maternal syphilis rates in the United States have risen dramatically, from 87.2 per 100,000 births in 2016 to 357.9 per 100,000 births in 2024, according to Centers for Disease Control and Prevention (CDC) reports. Congenital syphilis rates have also been on the upswing, increasing from 60.0 to 109.6 per 100,000 births between 2020 and 2024, with cases on the rise for the twelfth consecutive year. This trend poses a pressing public health concern. Diagnosing syphilis, particularly in pregnancy, can be challenging due to its variable and often nonspecific presentation. Delayed recog

A Case of Adult-onset Presumed Congenital Syphilitic Keratitis: A Multidisciplinary Management Approach.

BACKGROUND: Interstitial keratitis is a classic manifestation of late congenital syphilis, typically presenting in childhood or adolescence. Adult-onset presentation is rare and poses a diagnostic and therapeutic challenge, particularly in patients without a known history of congenital infection. CASE REPORT: We report a 28-year-old man with an 11-year history of progressive bilateral ocular redness, tearing, and visual decline who was found to have bilateral interstitial keratitis with positive treponemal and negative nontreponemal serologies, consistent with late congenital syphilis. Despit

First-year dynamics of the plasma virome and cytokine profile in infants born to mothers with syphilis.

BACKGROUND: The early-life development of the human plasma virome and its immunological implications remain poorly understood. We aimed to explore the dynamic interplay between viral colonization and immune maturation in infancy. METHODS: We conducted a retrospective longitudinal study of the plasma virome and cytokine profile in a cohort of 77 pregnant women with syphilis and their 89 infants. Plasma samples were collected from mothers at delivery and infants at multiple time points (the first day, and at 3, 6, 9 and 12 months of age). Virome composition was characterized via metagenomic sequ

Clinical and trichoscopic features of early congenital syphilis: a single-center cross-sectional study.

INTRODUCTION: Early congenital syphilis (ECS) remains an emerging public health problem in Latin America. Syphilitic alopecia (SA) is a rare and underreported manifestation in neonates, and its trichoscopic characteristics have not yet been systematically described. METHODS: We have conducted a 1-year cross-sectional analytical study involving 30 neonates diagnosed with ECS, classified into clinical-serological scenarios 1 and 2 according to the Centers for Disease Control and Prevention (CDC) criteria, who were hospitalized at a tertiary care center in Guadalajara, Mexico. Sociodemographic,

Onychoscopic Alterations in Newborns with Early Congenital Syphilis: A Single-Center Cross-Sectional Study of 10 Cases.

INTRODUCTION: Early congenital syphilis (ECS) is a vertically transmitted infection that remains difficult to diagnose in the neonatal period, as many newborns are asymptomatic at birth. Although mucocutaneous manifestations are well recognized, nail involvement has been rarely described. METHODS: We conducted a prospective, single-center study including neonates with ECS fulfilling scenario 1 or 2 criteria established by the Centers for Disease Control and Prevention and the American Academy of Pediatrics. Ten neonates admitted to the neonatal intensive care unit between April and July 2025

Eponymous Osseous Signs in Congenital Syphilis: A Narrative Historical Review.

The early origin of syphilis remains debated; bone involvement was recognized in early clinical accounts and described with increasing precision as pediatric pathology and radiography developed in the late nineteenth and early twentieth centuries. Many such observations entered medical usage through eponymous labels, yet these have not been examined as a coherent group. We reviewed the origins and early clinical descriptions of eponymous osseous signs of congenital syphilis between 1871 and 1928 and to outline their present-day relevance. A documentary review was conducted using PubMed/MEDLINE

Syphilitic Neuroretinitis with Positive Cytomegalovirus Serology in an Immunocompetent Adolescent: A Case Report.

INTRODUCTION: Neuroretinitis is an inflammatory optic neuropathy characterized by optic disc edema followed by macular star formation. It is most commonly caused by Bartonella henselae, but other infectious etiologies including syphilis and cytomegalovirus (CMV) should be considered, particularly due to their potential for severe visual morbidity if untreated. CASE PRESENTATION: A 17-year-old immunocompetent female presented with unilateral visual blurring and headache. Ophthalmologic examination revealed optic disc edema, macular star exudates, and peripapillary hemorrhages in the right eye.

Imaging evaluation and management of liver infections: a multimodality review.

Liver infections comprise a large category of hepatic diseases which vary in etiology and geographic distribution. They remain a significant cause of morbidity and mortality globally and require prompt diagnosis and management. Because clinical and laboratory findings may not be definitive, imaging often plays an important role in the diagnosis, characterization, and follow-up of hepatic infections. Advanced imaging techniques increase accuracy of diagnosis of specific infectious pathogens and in some cases provide the means of differentiation of various infections from other hepatic pathologi
