Seventeen patients with chronic painful shoulders who showed evidence of isolated bicipital tendinitis involving only the extracapsular, intertubercular portion of the long head of the biceps were chosen for surgical treatment when conservative treatment failed. The patients were thought to have primary bicipital tendinitis. The latter condition is secondary to other shoulder pathologies. Thirteen patients had tenodeses and four patients had transfer of the long head of the biceps to the origin of the conjoined tendon. Overall, excellent and good results were noted in 94% of both groups of pat

Calcific tendinitis is a painful condition related to deposition of hydroxyapatite crystals; it favors large joints. The shoulder, specifically the tendons of the rotator cuff and the insertion of the long head of the biceps on the superior glenoid rim, is a well-recognized location for this abnormality. The purpose of this article is to describe a second site of calcific tendinitis of the biceps, distal to the joint and corresponding to the junction of the tendon and muscle. Radiographs in 119 cases of calcific tendinitis of the shoulder, obtained between 1980 and 1988, were reviewed. Twenty

MR imaging was employed in 232 patients with traumatic or degenerative lesions of the shoulder. MR diagnosis was compared with arthrotomic findings in 19 cases, and with arthroscopic results in 3 cases. MR technique is here presented and the choice of scanning planes is discussed, together with the sequences of image acquisition and with the features of the surface coil employed. Axial, sagittal, and coronal scans were used in all cases. Both sagittal and coronal planes were performed obliquely on the basis of precise anatomical structures. T1-weighted sequences were used for they are reliable

1266 cases of Periarthropathia humeroscapularis (PHS) were analyzed according to their clinical and sonographic findings. We found in 32.8% a PHS simplex (simple tendinitis), in 12.6% a PHS adhäsiva (stiff and painful shoulder with intact cuff), in 17% a PHS calcarea (calcareous tendinitis) and in 33.3% a PHS destructiva (rotator cuff lesions). Isolated tendinitis of long head of biceps was rare (0.7%). Frozen shoulder (adhaesive capsulitis) was differentiated from PHS adhäsiva and accounted of 1.9%. 33.3% of frozen shoulder patients suffered from type I Diabetes mellitus. Women were more affe

This is a report on the results of a clinical and sonographical re-examination of 66 shoulder joints after reconstruction of complete rotator cuff tears. One-third of the patients were asymptomatic, in more than half of them pre-operatively existing complaints had improved, and in 15% no improvement was seen. Clinical findings were without abnormalities in half of the patients. In as many patients indications were found of a still existing rotator cuff syndrome. Sonographically, in 49 out of 66 cases the continuity of the rotator cuff was maintained. In 17 cases indications of tears were found

The epidemic of chronic upper limb pain is the most important and controversial issue in industrial rheumatology in Australia today. Two hundred and twenty-nine consecutive patients referred with chronic upper limb pain which had been labelled "repetitive strain injury" or "overuse syndrome" were assessed according to a protocol designed to give insight into the questions: Is the pain genuine or falsely reported, i.e., malingering? If genuine is it due to a physical injury, a pain syndrome, or a mixture of both? Twenty-nine patients fulfilled criteria for specific rheumatological diagnoses (fi

Sixty professional and recreational athletes underwent CT arthrography of the shoulder for evaluation of suspected shoulder joint derangement. These athletes, 46 males and 14 females ranging in age from 15 to 60 years (mean, 32 years), all had persistent pain that interfered with their sports activity and was resistant to conservative treatment. Seventeen patients had shoulder instability based on clinical manifestations and CT arthrographic findings. An additional five patients, also based on clinical manifestations and CT arthrographic findings, were considered to have an unobtrusive degree

The cause of shoulder pain can usually be diagnosed after a pertinent history has been obtained and a relevant physical examination performed. To carry out such an evaluation, the examiner must understand the salient anatomic relationships of the shoulder. Causes of shoulder pain include supraspinatus tendinitis (the most common), bicipital tendinitis, impingement syndromes, supraspinatus rupture, subacromial bursitis, arthritis, frozen shoulder, and various conditions that refer pain to the shoulder. Treatment in most cases consists of rest and administration of a nonsteroidal antiinflammator

Forty-three professional and amateur athletes with persistent shoulder pain that interfered with their sports activities were evaluated by computed tomographic (CT) arthrography. In 19 patients, glenohumeral instability (14 anterior, two posterior, three multidirectional) was diagnosed with CT arthrography based on the simultaneous presence of labral and capsular lesions. The findings were crucial in establishing the diagnosis of instability in six patients in whom the condition was not suggested or could not be confirmed clinically. Another significant injury consisted of labral lesions not a

Musculoskeletal and connective tissue disorders are commonly encountered in primary care practice. These disorders include varieties of bursitis, tendinitis, myositis, arthritis, and related conditions. A certain group of these disorders lend themselves to diagnosis by needle aspiration or to treatment by injections. The conditions discussed in this article include subacromial bursitis, supraspinous and bicipital tendinitis, tennis elbow, de Quervain's syndrome, trigger finger, inflammation of the knee, ganglion, and muscle trigger points.

Two double-blind studies were carried out to compare the effectiveness and tolerance of a slow-release tablet formulation of 300 mg fentiazac, given once daily, with the standard tablet formulations of 100 mg, given 4-times daily, or 200 mg, given twice daily. A total of 60 patients suffering from acute bicipital tendinitis and/or subdeltoid bursitis was studied, 15 patients on the slow-release and 15 on one of the two standard tablets in each of the two trials. Patients were assessed on entry and at Days 7 and 14 of treatment. The results in both studies showed that there was significant impr

Of all the forms of nonarticular rheumatism, by far the most common are bursitis and tendinitis. Yet, the bursae and neighboring tendon sheaths are the most neglected anatomic structures of the body. Moreover, like the joints, they are lined by synovial membrane, secrete synovial fluid, and are common sites of rheumatic problems. The vast majority of painful shoulder problems are caused by acute subacromial (subdeltoid) bursitis and bicipital tendinitis. In the management of these periarticular disorders, the ultimate goal is to preserve shoulder motion. Although this is accomplished by daily

A case of an anomalous insertion of the pectoralis minor on to the capsule of the gleno-humeral joint is described. It produced signs of deficiency of the rotator cuff. The anomaly had previously been described in anatomy textbooks but does not seem to have been observed previously by clinicians.

The impingement syndrome is an important source of shoulder pain that occurs when the bony and soft-tissue structures of the superior aspect of the shoulder encroach upon the coracoacromial ligamentous arch during abduction of the arm. Subacromial bursitis, bicipital tendinitis, and rotator cuff disruptions are common sequelae of this abnormality. The presence of bony excrescences arising from the anteroinferior aspect of the acromion and of flattening and sclerosis of the greater tuberosity of the humerus is an important plain radiographic sign of the impingement syndrome. Fluoroscopy, should

A systematic surgical approach for dealing with chronic shoulder pain includes a carefully designed arthrectomy of the acromioclavicular joint, removal of the coracoacromial ligament, cuff repair, biceps tendon transfer, and removal of calcific deposits as indicated. Of 38 patients who have been followed for at least 24 months, 29 were rated as excellent on the basis of achieving a full range of pain-free movement and normal strength. Degenerative arthritis of the acromioclavicular joint was found in all cases, whereas bicipital tendinitis was noted in less than half. Inspection or transfer of

A 50-year-old black man was admitted with a diagnosis of a ruptured rotator cuff syndrome two weeks after an injury to the right shoulder and treatment by intra-articular steroid injection. At operation pus and necrotic debris were found in the shoulder joint, but the rotator cuff was anatomically intact. Surgical debridement, irrigation, and intravenous antibiotics resulted in good recovery with satisfactory function. The pyarthrosis apparently resulted from the preoperative intra-articular injection.

Partial or complete excision of the acromion was performed in 25 patients (29 shoulders) with long-standing pain in the shoulder typical of rotator cuff syndrome. There was no verified history of trauma in nine cases (13 shoulders), while injury was the cause of pain in 16 patients (16 shoulders). In nine shoulders there was a small and in two a large rupture of the rotator cuff. Twelve of the 13 non-traumatic shoulders became painless postoperatively and the pain was relieved in one. In the 16 traumatic shoulders the relief of pain was complete in six, partial in nine, and one remained unchan

The painful periarticular conditions about the shoulder joint-calcific tendinitis, bicipital tendinitis, and frozen shoulder syndrome-are seen commonly in the general practice of medicine or in the practice of orthopedic surgery. Their etiologies are unknown. Their treatment is relatively simple and depends upon a knowledge of the anatomic structures involved and the proper use of rest and exercise. Operative intervention is rarely necessary in any of these conditions. The frozen shoulder syndrome, however, in our experience is best treated in the subacute or chronic stages by infiltration bri
