Intraarticular findings in the chronically painful shoulder. A study of 32 posttraumatic cases.

32 consecutive patients suffering from chronic shoulder pain for more than 6 months after a single, nondislocating shoulder trauma were examined clinically and by special radiographs, dynamic sonography, MRI and arthroscopy. Typical complaints were pain during loading, especially during over the head activities. Symptoms of a "dead arm" and instability were also present. Patients with previous dislocations, traumas or radiographic signs of degenerative shoulder lesions were excluded. The patients had a decreased active range of motion and positive signs of apprehension and impingement, but onl

[Normal and pathological aspects of the glenoid labrum in opaque arthro-scanner].

The morphology of the glenoid labrum was studied by CT arthrography in 88 patients. These were divided in 2 groups. In the first group, the patients underwent CT arthrography for a clinical instability of the shoulder. The second group was the reference group to study the morphology of the glenoid labrum, it included patients with rotator cuff tears or other pathology of the shoulder, without clinical instability. We described the normal labrum which presents important morphologic variations. Its study may be difficult because of the proximity of capsular structures of the shoulder, mainly the

Posterior superior impingement of the rotator cuff on the glenoid rim as a cause of shoulder pain in the overhead athlete.

A case of impingement of the deep surface of the supraspinatus tendon on the posterior superior rim of the glenoid and mild anterior laxity has been presented. The partial-thickness tear of the undersurface rotator cuff tendon, degenerative tear of the posterior superior labrum, and osteochondral impression fracture of the humeral head have been documented. The purpose of this case report is to present the pathological findings associated with posterior superior glenoid rim impingement and emphasize its role as a cause of shoulder pain in the overhead athlete. In addition it is important to st

Magnetic resonance imaging of the glenoid labrum.

Shoulder complaints are common in the athletic population. Successful treatment is dependent upon the ability to correctly identify the pathologic changes associated with a specific disorder and arrive at an accurate diagnosis. Labral abnormalities are traditionally associated with glenohumeral instability. Isolated labral tears or degenerative changes may also cause shoulder dysfunction. A reliable clinical tool to assess the status of the labrum would be useful for diagnosis and management of shoulder disorders. The magnetic resonance images of 48 shoulders that had subsequent surgical exami

Shoulder arthroscopy: its role in evaluating shoulder disorders in the athlete.

We retrospectively reviewed 100 initial shoulder arthroscopies from a general sports medicine orthopaedic practice to determine if arthroscopy is helpful diagnostically and to see if specific lesions correlate well with specific diagnoses. Seventy-two percent of the patients in the review had glenoid labral tears. Ninety-two percent of patients who had a diagnosis of either recurrent anterior dislocation or recurrent anterior subluxation demonstrated a tear of the anterior inferior margin of the glenoid labrum. Sixty-eight percent of patients with a diagnosis of impingement demonstrated tears

[Magnetic resonance of the shoulder: technic, anatomy ana clinical results].

MR imaging was employed in 232 patients with traumatic or degenerative lesions of the shoulder. MR diagnosis was compared with arthrotomic findings in 19 cases, and with arthroscopic results in 3 cases. MR technique is here presented and the choice of scanning planes is discussed, together with the sequences of image acquisition and with the features of the surface coil employed. Axial, sagittal, and coronal scans were used in all cases. Both sagittal and coronal planes were performed obliquely on the basis of precise anatomical structures. T1-weighted sequences were used for they are reliable

Computed tomography (CT) arthrography of shoulder instabilities in athletes.

Sixty professional and recreational athletes underwent CT arthrography of the shoulder for evaluation of suspected shoulder joint derangement. These athletes, 46 males and 14 females ranging in age from 15 to 60 years (mean, 32 years), all had persistent pain that interfered with their sports activity and was resistant to conservative treatment. Seventeen patients had shoulder instability based on clinical manifestations and CT arthrographic findings. An additional five patients, also based on clinical manifestations and CT arthrographic findings, were considered to have an unobtrusive degree

Athlete shoulder injuries: CT arthrographic findings.

Forty-three professional and amateur athletes with persistent shoulder pain that interfered with their sports activities were evaluated by computed tomographic (CT) arthrography. In 19 patients, glenohumeral instability (14 anterior, two posterior, three multidirectional) was diagnosed with CT arthrography based on the simultaneous presence of labral and capsular lesions. The findings were crucial in establishing the diagnosis of instability in six patients in whom the condition was not suggested or could not be confirmed clinically. Another significant injury consisted of labral lesions not a
