Tarsal tunnel syndrome has only recently been noted to be a cause of foot and ankle pain in runners. The tarsal tunnel is located just posterior to the medial malleolus and may compress the posterior tibial nerve as it passes through it, producing numbness and paraesthesia in the foot. While the aetiology of this condition is frequently multifactorial, abnormal foot and ankle mechanics and excessive training tend to be the most commonly cited aetiological factors. Successful treatment of tarsal tunnel syndrome requires an accurate diagnosis by differentiating it from plantar fasciitis and Achi

In brief Enlargements at the calcaneal brief attachment of the Achilles tendon can cause heel pain. These bumps usually result from rigid, poorly fitting heel counters and are more likely to occur in patients who have a prominent posterosuperior portion of the calcaneus (Haglund's deformity). Extrinsic irritation on this prominence can give rise to bursitis or Achilles tendinitis (Haglund's disease).

The authors determined the incidence of podiatric injuries that occurred during 233,946 recruit days at risk among US Marine Corps recruits undergoing basic training at the Marine Corps Recruit Depot, San Diego, CA, between February 5 and April 25, 1990. Training-related initial injuries to the foot occurred at a rate of 3.0 new injuries per 1,000 recruit days. The highest specific rates of injury occurred with stress fractures to the foot (0.56 per 1,000 recruit days), ankle sprains (0.53 per 1,000 recruit days), and Achilles tendinitis (0.39 per 1,000 recruit days).

The retrocalcaneal bursae, located between the posterior angle of the os calcis and the Achilles tendon, may become inflamed and hypertrophied. There are few objective tests available for the diagnosis of pain syndromes involving the hindfoot and it is often difficult to differentiate Achilles tendinitis and retrocalcaneal bursitis. Retrocalcaneal bursograms can provide further insight into chronic changes in the bursae associated with traumatic and inflammatory conditions. This report describes the anatomy of the retrocalcaneal bursa, the technique of its opacification, and the application of

In brief Patients who have pain from such maladies as plantar fasciitis, bunions, ankle sprains, Achilles tendinitis, shin splints, or patel-lofemoral syndrome may benefit from orthotic devices. Orthoses can help alleviate pain that stems from abnormal foot biomechanics, such as overpronation or rigid pes cavus. This guide highlights when orthoses may be appropriate adjuncts for treating active people.

We describe 7 Achilles tendinitis occurring during fluoroquinolone treatment. Antibiotic agents used were pefloxacin and ofloxacin. In 3 cases the course was complicated by rupture of the tendon, one of which is histologically documented. The toxic effects of quinolones on tendon is discussed.

Achilles tendinitis is a common occurrence in long-distance runners. Although most respond well to conservative therapy, there are some who require operative intervention. The short-term results of surgical treatment are good, with a success rate more than 85%. The authors present nine successful long-term results in runners. However, in two of the runners who continued to compete and train at long distances, symptoms recurred. Reoperations were performed to permit successful running careers for five and nine years. Runners resumed their careers after the second operation by supervised trainin

Overuse injuries of the Achilles tendon are common in patients engaging in recreational athletics. Achilles tendon overuse injuries exist as a spectrum of diseases ranging from inflammation of the paratendinous tissue (paratenonitis), to structural degeneration of the tendon (tendinosis), and finally tendon rupture. Factors known to predispose patients to Achilles tendinitis include inadequate stretching, training errors, mechanical malalignment of the lower extremities, rigid training surfaces, and occasionally systemic disease. Treatment of the patient with paratenonitis and tendinosis is in

Achilles tendinitis is a problem encountered frequently. There are certain anatomical and biomechanical principles that help explain the etiology of this entity. We prefer to separate our thinking into "insertional" and "noninsertional" Achilles tendinitis. This is helpful because it allows nonoperative and operative treatment to be problem specific and systematic.

In brief Iliotibial band syndrome (ITBS) is caused by training errors but may be influenced by anatomic and dynamic factors. The diagnosis involves matching a complete and accurate history with characteristic pain over and around the lateral femoral epicondyle. The authors studied US Marine recruits in basic training and found an incidence that suggests ITBS may be underdiagnosed. Treatment usually involves simple measures such as rest and stretching exercises, but surgical resection is sometimes necessary.

The iliotibial band syndrome is most often diagnosed in runners but can be found in athletes who participate in other sports. From our experience most patients are high mileage runners with shoes either worn out or with insufficient cushioning. A total of 19 athletes with this condition have been seen in the past 3 years. Treatment consisted of rest or decreased distance, shoe changes, modification in exercise technique, anti-inflammatory medication, steroid injections and stretching. Surgery was not required.

We determined the incidence of soft-tissue/musculoskeletal injury occurring during 8,076 recruit-months at risk among recruits undergoing basic training at Marine Corps Recruit Depot, San Diego, California, between January and April 1990. We analyzed International Classification of Disease codes relating to initial visit for injuries, and calculated recruit-months from weekly strength figures. Training-related injuries occurred at a rate of 19.9 injuries per 100 recruit-months. Within the sports medicine clinic, iliotibial band syndrome (22.4%), patellar tendinitis (15.1%), and mechanical low

To identify magnetic resonance (MR) imaging characteristics of normal patellar tendons and those affected by tendinitis, the authors evaluated MR images obtained in 10 healthy volunteers, in 50 patients who underwent MR imaging for evaluation of knee structures other than the patellar tendon, in 11 patients with patellar tendinitis, and in two athletes with patellar tendon injuries. Normal tendons had uniformly low signal intensity on T1-, T2-, and proton-density-weighted images and displayed distinct margins, and the anteroposterior (AP) diameter slightly increased proximally to distally. It

In brief Knee bursitis often mimics other brief Pathologies, making correct diagnosis necessary to initiate appropriate treatment. Most commonly affected are the prepatellar, pes anserine, tibial collateral ligament, and two infrapatellar bursae. Other common bursitic conditions include Baker's cysts and posttraumatic adventitious hemorrhagic bursitis. Most of these can be treated conservatively with aspiration of fluid from the bursa, rest, ice, immobilization, and injection of a corticosteroid and analgesic combination. Some chronic bursitic conditions may require surgical excision of the bu

Piroxicam 0.5% gel, administered as 5 mg q.i.d. was compared to placebo gel in a double-blind study of the treatment of selected acute soft tissue injuries (ankle or acromioclavicular sprains, supraspinatus, or achilles tendinitis). A total of 200 patients (100 per treatment) were evaluated. Six patients (6%) in the piroxicam group discontinued treatment due to inefficacy, compared to 42/100 in the placebo group (p less than 0.001). Significantly greater reduction in pain (spontaneous and on movement), degree of joint restriction, pressure threshold and tenderness of the affected site were obs
