Achilles tendinitis is particularly frequent in athletes. In fact, this entity is made up of various anatomical lesions located in Achilles tendon or at the point where it is attached to the calcaneum. The diagnosis is basically clinical, and it is confirmed by carefully selected paraclinical examinations. Management essentially relies on rest associated with adequate physiotherapy and, when required, with other adjuvant treatments. Management also rests on the prevention of recurrences and on the suppression of the frequently found sport-related causal agent. Surgical treatment is seldom indi

Especially in sports disciplines involving high uniform stress by running, such as all running competitions in athletics, the condition known as achillodynia is a much-dreaded lesion. We evaluated an own series of 74 consecutive patients who reported pain at the achilles tendon, with regard to etiological aspects. It was a remarkable fact that besides the well-known classical reasons (degenerative changes in the center of the achilles tendon with spindle-shaped distension, aseptic irritated condition of the paratenon, bursitis subachillea mostly associated with Haglund's exostosis) numerous ot

During a period of 8 months 121 male and 99 female joggers were interviewed by a standardised questionnaire in respect of their running behaviour and problems caused by jogging. Only such runners were included who had jogged regularly on at least 2 days, regarding women on at least one day per week for at least 30 minutes during the last 2 years. Additionally a detailed orthopaedic examination was carried out in the ambulance of the Orthopaedic University clinic. The overwhelming majority of male and female joggers had a secondary education and practised easy physical activities. Only 4 per ce

This prospective study describes the ultrasound, computerized tomography and magnetic resonance imaging findings in 16 cases of patellar tendinitis. In all cases tendon enlargement and reduced echogenicity were visible on ultrasound. Computerized tomography demonstrated enlargement of the tendon with reduced attenuation of the central portion. Magnetic resonance imaging showed focal tendon enlargement in all patients with high signal lesions in 88% of cases. This study has shown that patellar tendinitis may be identified with all three modalities. Ultrasound is recommended as the initial inves

A retrospective review of 60 acute fractures of the tibia treated with reamed intramedullary nailing was undertaken to document the spectrum of complications associated with this procedure. Forty-five tibial fractures were followed to radiographic union; follow-up averaged 25 months (range, 10-63 months). Complications were categorized into intraoperative, early postoperative, and late postoperative groups. Intraoperative complications occurred in 6 of the 60 (10%) fractures and included propagation of the tibial fracture into the insertion site of the nail in four cases. In each of two other

Five hundred questionnaires were distributed to long-distance runners who had used, or who were using orthotic shoe inserts for symptomatic relief of lower extremity complaints. Three hundred forty-seven (69.4%) responded (males, 71%; females, 29%). The mean age of the respondents was 36 years (range, 15 to 61). The average distance run per week was 39.6 miles (range, 5 to 98). The mean duration for use of the orthotic inserts was 23 months (range, 1 to 96). The predominant (63%) type of orthotic device used was flexible. The presumed diagnoses in the population studied were excessive pronatio

The bursa of the tibial collateral ligament (TCL) may be visualized at magnetic resonance (MR) imaging when it becomes distended with fluid. In the authors' experience, this finding signifies a pathologic condition either in the medial capsuloligamentous complex or in the bursa itself. Such a finding may indicate TCL bursitis. TCL bursitis can be suspected in the setting of isolated pain in the medial joint line in the absence of mechanical symptoms. Prolonged relief of symptoms after injection of steroid into the bursa is supportive of the diagnosis. Seven cases are presented in which a fluid

The case of a patient with a 15-year history of isolated HLA-B27-associated bilateral Achilles tendinitis without other features of seronegative spondyloarthropathies is reported. This case suggests that Achilles tendinitis may be, for a long time, the only clinical manifestation of an HLA-B27-associated disease process. The theory of a separate B27-associated disease entity cannot be excluded yet.

Though badminton is one of the most widely played sports in the world, it has received little sports medical interest. Based on the few existing studies on injuries in badminton, compared to other sports it is of relatively low risk and dominated by overuse injuries. The injury duration is relatively long, but only a few working days are lost. Anatomically, most injuries are localised to the foot and ankle. The single most frequent injuries are Achilles tendinitis and tennis elbow. Rupture of the Achilles tendon is a rare injury, which is typically seen in older recreational players. When the

Overuse injuries of tendons are known to occur in persons whose activities submit the tendon to excessive stress. Classical ballet dancers performing en pointe, demie point, or plié exert forces that, although normal in magnitude, are increased in frequency, thus overusing the Achilles tendon. In the present study all cases of Achilles tendinopathy seen in a period of three years in three ballet companies were reviewed by a special orthopedic clinic. The cause, whether by abnormal tension or incorrect use, development, and progression to chronic tendinopathy, as well as measures to prevent it,

Posterior heel pain can be caused by inflammation or pathology of the achilles tendon, the tendon sheath, the retrocalcaneal and subcutaneous tendoachilles bursa, can be caused by osteophytes or stress fracture of the os calcis and by rheumatoid or metabolic diseases. The majority of patients can be treated successful nonoperatively; however, there is a small group of patients who are refractory to nonoperative managements. In the years 1984 to 422 patients with posterior heel pain were treated conservatively, 26 of these patients were refractory to that management. There were 9 cases of achil

"Jumper's knee" or patellar tendinitis was first described in an article by Blazina in 1973. He noted an increasing incidence of peripatellar tendinitis, affecting either the quadriceps tendon or the patellar tendon insertion onto the patella, most commonly in jumping athletes. Since that time, numerous articles have further defined the etiology of this condition and the actual pathologic lesion, along with classifying the stages of its development and outlining appropriate methods of treatment and prevention. Although jumper's knee is quite common, its treatment remains somewhat controversial

In brief: Recreational runners are prone to a variety of lower extremity injuries; one of the most common is pain in the heel and sole of the foot. Among the most frequent problems are plantar fasciitis, Achilles tendinitis, and calcaneal stress fractures. Management may be challenging, since an underlying biomechanical problem often must be corrected in addition to treatment of symptoms. The authors describe the signs, symptoms, and management of heel pain and include several exercises to prevent it.

One of the principal drawbacks to clinical implementation of magnetic resonance imaging is comparatively long imaging times involved. A case is reported here where the most useful image sequence for the diagnosis of Achilles tendinitis was also the shortest--a sagittal, low flip, angle-scan.

Achilles tendinitis is an injury that commonly affects athletes in the running and jumping sports. It results from repetitive eccentric load-induced microtrauma that stresses the peritendinous structures causing inflammation. Achilles tendinitis may be classified histologically as peritendinitis, tendinosis, or partial tendon rupture. Training errors are frequently responsible for the onset of Achilles tendinitis. These include excessive running mileage and training intensity, hill running, running on hard or uneven surfaces, and wearing poorly designed running shoes. Biomechanical abnormaliti

In brief: Overuse injuries of the lower extremity represent a frequent, significant problem. This article discusses management of the most common overuse disorders: anterior tibialis strain, medial tibial stress syndrome, compartment syndrome, stress fractures, and Achilles tendinitis. These injuries often are the culmination of a process (repetitive stress) rather than the result of a single event. Grading injuries on a scale of 1 to 4 according to severity can help determine the correct treatment. The primary treatment goal is to reduce inflammation through icing, rest, and appropriate use o

The authors report on the clinical observation of 23 patients (5 women, 18 men) who suffered from seronegative spondylarthritis following chlamydia infection diagnosed clinically and serologically. Nine patients (2 women, 7 men) carried HLA B27 histocompatibility antigen. Sacroileitis confirmed by radiology was found in 16 cases. Genicular synovitis was the most frequent peripheral articular syndrome as well as Achilles tendinitis, "sausage-like" swelling of the finger and toes were observed and the ankle-, wrist-, elbow- and in a few cases the sternoclavicular and temporomandibular joint show
