Preventing Overuse Cycling Injuries.

In brief: Concurrent with the renaissance in bicycling has come an increase in associated injuries. The most common problem the author sees in his practice is knee pain-usually chondromalacia patellae, patellar tendinitis, or bursitis. The patellofemoral joint is under particular stress in bicycling, and many new recreational riders make two errors: setting the saddle too low and the gears too high. These mistakes cause excessive pressure on the patellofemoral joint. Obtaining the best-fitting bicycle and saddle, adjusting the saddle properly, and using lower gears will help prevent such probl

Iliotibial band syndrome in distance runners.

Iliotibial band syndrome was diagnosed in 48 of 1030 runners treated for lower extremity musculoskeletal complaints. Most athletes had been running 20 to 40 miles a week for one year or longer and had significantly changed their distance, speed, terrain, surface, and/or shoes before the onset of symptoms. Symptoms often persisted for 2 to 6 months. All runners were treated conservatively with rest, stretching, reduced distance, anti-inflammatory medications, local cortisone injections and/or orthoses.

Meniscal lesions of the knee joint: CT diagnosis.

Computed tomography (CT) resulted in a diagnostic accuracy of 89.2% and 96.1% for medial and lateral meniscal lesions, respectively, in 109 patients who underwent surgery after a direct CT study of the knee joint for a clinically suspected meniscal lesion. The meniscal lesions were the only pathologic condition found in 59 patients, while in 35 patients they were associated with various lesions of the cruciate ligaments (31 cases) and collateral ligaments (15 cases) and with cystic bursitis (6 cases). In the remaining 15 patients, the menisci were normal, but in eight of these cases, lesions o

Achilles tendinitis and peritendinitis: etiology and treatment.

One hundred nine runners were treated conservatively without immobilization for overuse injury to the Achilles tendon. Treatment strategies were directed toward rehabilitation of the gastrocnemius/soleus muscle-tendon unit, control of inflammation and pain, and control of biomechanical parameters. One fair, 12 good, and 73 excellent results were reported, with a mean recovery time of 5 weeks. Followup was incomplete in 23 cases. The three most prevalent etiological factors were overtraining (82 cases), functional overpronation (61 cases), and gastrocnemius/soleus insufficiency (41 cases). The

The effect of heel pads on the treatment of Achilles tendinitis: a double blind trial.

Thirty-three subjects entered a blind-observer, random, prospective study of three forms of conservative treatment of sports-induced Achilles tendinitis, results being assessed by clinical and biomechanical parameters. Two patient groups received heel pads, ultrasound, and exercises, while the third received only ultrasound and exercises. All three groups showed some improvement at both 10 day and 2 month assessment, but the claimed benefit of viscoelastic pads widely used by athletes was not substantiated. The more striking benefit from ultrasound and exercises alone occurred in patients with

The nature and response to therapy of 196 consecutive injuries seen at a runners' clinic.

We studied a series of 196 running injuries to determine the nature of the common injuries, the type of runners with the different injuries, specific factors causing the most common injuries, and the response of these injuries to correction of the biomechanical abnormalities believed to cause them. The four commonest injuries were 'runner's knee' (peripatellar pain syndrome) (22%), 'shin splints' (posterior tibial stress syndrome) (18%), the iliotibial band friction syndrome (12%), and chronic muscle injuries (11%). Within 8 weeks of following the biomechanically based treatment regimen, betwe

Containing costs in the treatment of congenital heart disease.

In the Congenital Heart Program at San Diego Children's Hospital, alterations in medical practice have reduced costs without impairing quality or access. Pediatric cardiac catheterization was done in 483 consecutive elective patients without overnight hospital stay. Hospital readmission was required in one patient for psoas tendinitis. Avoiding overnight hospital stay minimized attendant risks of hospital care, lessened psychosocial trauma and reduced the average hospital bill by $493 (29%). Hospital stay was also reduced for elective surgical correction of congenital heart disease on a case-b

High-performance activity with below-knee cast treatment, part I: mechanics and demonstration.

Much of the disability associated with wearing a below-knee cast is a result of the treatment rather than the disease. A simple practice of placing thecasted limb in a shoe with a cushioned heel to tilt the cast so that the knee is ahead of the ankle at mid-stance reduces this disability. High-performance activity, including participation in sports, is possible in most (including bilateral) cases. Despite increased activity, this method reduces the incidence of cast failure. Healing does not appear to suffer and in some instances may be superior. Mechanics are discussed in Part I, including il

Pathologic hindfoot conditions in the athlete.

Millions of people run in the United States, both for competitive reasons and for basic conditioning. Unfortunately, many runners develop cumulative (overload) stress syndromes, a number of which occur in the hindfoot. Among the most common are Achilles tendinitis and its associated conditions, plantar fasciitis, and tendinitis of the posterior tibial and flexor hallucis longus tendons. Most of these conditions respond well to conservative treatment, but in some instances surgery is needed. Surgery frequently can help patients who have not responded to conservative therapy to return to active

Ballet injuries: the Australian experience.

There is a distinct difference between ballet injuries and sports injuries in general, and the sports medicine physician needs to study the technique of dance and the specific injuries that it may produce in order to treat dancers effectively. In Australia, which is typical of other countries where ballet is performed, ballet injuries include strained lumbar muscles, sprained ankle, Achilles tendinitis, clicking hip, jumper's knee, chondromalacia, stress fractures, patellar subluxation, and other knee and tendon problems.

Mechanical and biological properties of the GORE-TEX expanded polytetrafluoroethylene (PTFE) prosthetic ligament.

Data from these studies have resulted in a modified implant technique which has taken into consideration the salient points from this and previous studies. Ongoing animal studies will provide a better understanding of the long-term performance of the material in the anterior cruciate ligament and medial collateral ligament positions. Additional studies will be implemented as new techniques and applications evolve. The key conclusions of this study are as follows: 1. This prosthetic ligament system is not intended to match the mechanical properties of the normal anterior cruciate ligament in do

Patellar tendinitis: pathology and results of treatment.

Patellar tendinitis can be considered as an overload lesion in athletes. Anatomical pathology reveals focal degeneration and microtearing at predisposed areas near the insertion of the quadriceps or patellar tendon. Thie prognosis and treatment are dependent on the stage of the affection. A program of conservative treatment is effective in the early stages of the disease but it fails in a high proportion (16/38) of cases in the later stages. Surgery directed towards the tendon, rather than a bony procedure, yielded favourable results in 27 out of 29 patients.

Pediatric cardiac catheterization with same-day discharge.

A system was developed for cardiac catheterization in children without overnight hospital stay (called same-day discharge). Over a 4 year period, 233 children (aged 5 weeks to 20 years) had catheterization with same-day discharge staying an average of 11.8 hours in the hospital. In all but 1, no problems occurred after hospital discharge; 1 child required readmission for psoas tendinitis after retrograde aortography. Same-day discharge was safely applied regardless of the patient's age, diagnosis, and use of systemic heparinization, large-bore sheaths, retrograde arterial catheterization, or c

Orthotics in the Prevention of Runners' Overuse Injuries.

In brief: Three hundred runners were surveyed about the effectiveness of orthotics in controlling the symptoms and recurrence of overuse and impact shock injuries. The most common overuse injuries in the 146 respondents were knee pain, shinsplints, plantar fasciitis and arch strain, and Achilles tendinitis. The most commonly encountered problems with the orthotics themselves were blister formation, edge irritation, heel slippage from the extra bulk in the shoe, and discomfort from the stiffness of the orthotic. The author feels that orthotics can be of significant therapeutic value in treating

Dynamic patellar brace: a new orthosis in the management of patellofemoral disorders. A preliminary report.

The diagnosis and treatment of patellofemoral dysfunction, especially subluxation of the patella, remains controversial. Many authors feel conservative treatment will give better long-term results than operative treatment. To aid in nonoperative treatment of patellofemoral conditions a dynamic patellar stabilizing brace has been developed. The brace applies an active, medially displacing force to the lateral border of the patella and maintains constant pressure during flexion, extension, and rotation of the knee. This brace consists of an elastic sleeve with a patellar cutout and two circumfer

Achilles tendinitis associated with chondrocalcinosis.

Achilles tendinitis was observed in 3 patients with idiopathic articular chondrocalcinosis. It was associated with linear radiological calcifications which in this condition are not unusual but generally asymptomatic. Crystals presumed to be calcium pyrophosphate dihydrate (CPPD) were found in the 2 cases that were biopsied. In 2 patients the acute inflammation of the tendon subsided after several days, but in the 3rd it lasted 4 1/2 months, and resulted in a cicatricial thickening of the tendon with a progressive disappearence of the calcifications. Therefore, the development of Achilles tend
