Quantum Effects in Evolution: Terrestrial Fine-Tuning of Magnetic Parameters.

OBJECTIVE: To compare information on body fat changes from questionnaire and clinical examination and to study lipoatrophy in HIV-1 patients on highly active antiretroviral therapy (HAART). METHODS: The study was cross-sectional within a randomized trial. One hundred and sixty-eight male HIV-1 patients were examined by questionnaire and clinical examination. Clinical lipoatrophy was studied and defined as fat wasting in the face, legs and/or arms. Fasting blood samples reflecting lipid and glucose metabolism were taken and the role of indinavir, ritonavir (RTV) and RTV/saquinavir (SQV) on lip

Unconventional Uses of Human Growth Hormone.

Growth hormone (GH), a 191-amino acid peptide produced by the anterior pituitary gland, plays a central role in linear growth, body composition, metabolism, and homeostasis. Exogenous GH has been used to treat GH deficiency (GHD) in children and adults since the early 1960s. GH used to be purified from human pituitaries obtained at autopsy, with the consequent limited availability and risk of Creutzfeld Jacob disease due to prion contamination. Since the mid 1980s all the GH used to treat humans is synthesized by recombinant DNA technologies (rhGH). rhGH is currently used in several conditions

Revisiting HIV-Associated Wasting: Results from a U.S. Cross-Sectional Survey of Health Care Providers' Knowledge, Attitude, and Current Practices.

Advancements in antiretroviral therapy (ART) have significantly improved morbidity and mortality rates among people with human immunodeficiency virus (HIV) (PWH), leading to prolonged survival, and many are now living into middle and old age. However, this population now faces aging-related comorbidities, including cachexia HIV-associated wasting (HIVAW), which remains inadequately addressed in the context of contemporary ART. Effective management of HIVAW requires a multifaceted approach that targets underlying causes while addressing coexisting health conditions. Currently, recombinant human

eDNA metabarcoding shows highly diverse but distinct shallow, mid-water, and deep-water eukaryotic communities within a marine biodiversity hotspot.

BACKGROUND: This article is a review of the PhD thesis by Elizabeth Kimani-Murage that explores the double burden of malnutrition in rural South Africa. This is in the context of a worryingly rapid increase in obesity and obesity-related diseases in low- and middle-income countries (LMICs) including South Africa, and in the wake of on-going nutrition transition and lifestyle changes in these countries. OBJECTIVE: To understand the profiles of malnutrition among children and adolescents in a poor, high HIV prevalent, transitional society in a middle-income country. METHODS: A cross-sectional

Multimodal interventions for cachexia management.

BACKGROUND: Cachexia (disease-related wasting) is a complex metabolic syndrome which occurs in people with chronic illnesses, including cancer, HIV/AIDS, kidney disease, heart disease, and chronic obstructive pulmonary disease (COPD). People with cachexia experience unintentional weight loss, muscle loss, fatigue, loss of appetite, and reduced quality of life. Multimodal interventions which work synergistically to treat the syndrome could lead to benefits. OBJECTIVES: To assess the benefits and harms of multimodal interventions aimed at alleviating or stabilising cachexia in people with a chr

Magnitude and factors associated with wasting among children on antiretroviral therapy in the East Wollega zone, Western Ethiopia.

BACKGROUND: Insufficient food intake and infectious diseases cause undernutrition. Undernutrition in children is a severe public health issue globally, especially in low-resource areas. Among the most vulnerable to these challenges, children affected by HIV/AIDS in Ethiopia are at risk of undernutrition, including wasting, which links to increased child morbidity and mortality. Despite the importance of the issues, there is a lack data from East Wollega Zone of Ethiopia. Therefore, the present study aims to assess the magnitude and determinants of wasting among children receiving antiretrovira

Chronic diarrhoea in a human immunodeficiency virus/acquired immunodeficiency syndrome patient: A case report.

BACKGROUND: Chronic diarrhoea in people living with human immunodeficiency virus (PLHIV)/acquired immunodeficiency syndrome presents a diagnostic challenge, often resulting from opportunistic infections (OIs), malignancies, or disease progression itself. We present a case of an advanced human immunodeficiency virus (HIV) patient with chronic diarrhoea, significant weight loss, and antiretroviral therapy (ART) non-compliance, highlighting the diagnostic dilemma between HIV wasting syndrome, OIs, and malignancy. CASE SUMMARY: A 36-year-old female, diagnosed with HIV five years ago on family scr

Interlimb and Intralimb Synergy Modeling for Lower Limb Assistive Devices: Modeling Methods and Feature Selection.

CONTEXT: For clinicians managing weight loss in patients with HIV, it would be useful to understand how changes in lean body mass (LBM) effect physical functioning, and whether LBM is more strongly related to physical functioning than total body weight (TBW). OBJECTIVE: To determine the relationship of changes in LBM and changes in total body weight (TBW) to changes in self-reported physical functioning in men and women with HIV infection. METHODS: Study design was longitudinal analysis of 1474 patient-intervals (each interval was approximately 6 months long) in 486 persons. Patients were pa

Weight loss and mortality in people living with HIV: a systematic review and meta-analysis.

BACKGROUND: In the first reported cases of human immunodeficiency virus (HIV) infection, people living with HIV (PLHIV) suffered weight loss, which was an independent predictor of mortality. Highly active antiretroviral therapy (HAART) has changed this scenario for ideal weight, overweight, and even obesity. However, some PLHIV, even on HAART, continue to lose weight. Thus, the guiding question of the study was: do PLHIV hospitalized using HAART with weight loss have higher mortality than hospitalized PLHIV using HAART without weight loss? METHOD: A systematic review and meta-analysis of pros

Re-evaluating the relationship between youth with HIV and BMI in an age of increasing rates of overweight and obese youth.

BACKGROUND: Newer antiretrivirals (ART) have shifted the metabolic experiences of people with HIV (PWH) from those of wasting syndrome to increases in body mass index (BMI). This study sought to examine the relationship between BMI and ART use among youth with HIV (YWH). METHODS: Charts from YWH ages 10-24 with at least two documented BMIs at least 6 months apart between 2017 and 2020 were included (N = 44). Statistical analyses were conducted in SAS 9.4. RESULTS: Clients were predominately African American (66%) males (73%) aged 19-24 years (64%), with men having sex with men (48%) being th

Tenofovir as a cause of acquired fanconi's syndrome.

Fanconi's syndrome is a disorder that results in generalized involvement of the proximal tubule of the kidney. It is characterized by variable degrees of phosphate, glucose, and amino acid wasting in the urine and a hyperchloremic normal anion gap metabolic acidosis - secondary to defective hydrogen ion excretion and bicarbonate ion absorption. There are hereditary variants such as cystinosis (most common), hereditary fructose intolerance, galactosemia, tyrosinemia, Dents disease, and acquired variants of Fanconi's syndrome. Toxins, drug-induced diseases, and systemic diseases (multiple myelom

A systematic review of sarcopenia prevalence and associated factors in people living with human immunodeficiency virus.

People living with human immunodeficiency virus (HIV) (PLWH) appear to be at an increased risk of sarcopenia, which can have a devastating effect on their life due to consequences such as physical disability, poor quality of life, and finally death. This systematic review examined sarcopenia prevalence and its associated factors in PLWH. A systematic search was conducted using the keywords in the online databases including Scopus, PubMed, Web of Science, Embase and Cochrane databases from the dates of inception up to May 2022. The retrieved articles underwent a two-step title/abstract and full

HIV enteropathy and 'Slim disease': Historical and current perspectives.

OBJECTIVES: Chronic diarrhoea and severe wasting associated with HIV infection were first described in East African patients as slim disease (SD) in 1985. The main histological features are flattening of the villi (villous atrophy) and crypt hyperplasia (elongated crypts), i.e., HIV enteropathy (HIVE). Selective loss of mucosal clusters of differentiation 4 (CD4)+ T helper (Th)17+ lymphocytes is the immunological hallmark of HIVE. This review explores (i) the historical background of HIVE and SD, (ii) the relationship between gut mucosal CD4+ Th17+ and intestinal-resident intra-epithelial gamm

Cultural and religious barriers in the Middle East leading to delayed management of acute HIV case resulting in irreversible blindness: a case report.

UNLABELLED: According to the latest report from the WHO, the HIV attacks the body's immune system, mainly the CD4 helper T cells component of the white blood cells. HIV kills CD4 cells, infirm the immunity against opportunistic infections, bacterial infections such as tuberculosis and fungal infections, severe other viral infections such as Cytomegalovirus (CMV), and some malignancies, if not diagnosed early and managed properly, it can lead to a life-threatening debilitating condition of AIDS. It is important to follow-up cases of HIV regularly, as currently there is no radical cure for this
