The Musculoskeletal Advanced Transillumination Technique (MATT): A Descriptive Proof-of-Concept Study of a New Method for the Study of the Iliotibial Tract Tested on Fresh Cadaveric Specimens.

Background: The iliotibial band (ITB) is an anatomically complex structure with multiple proximal and distal attachments, making its mechanical behavior difficult to interpret. In the study of iliotibial band syndrome (ITBS), prior research has often considered the underlying lateral femoral epicondyle (LFE) as a fixed reference to describe ITB movement during knee flexion, potentially misrepresenting true tissue dynamics. This proof-of-concept study introduces the musculoskeletal advanced transillumination technique (MATT) to visualize and measure LFE displacement relative to the ITB and the

Iliotibial band strain while running around a curve.

The iliotibial band (ITB) stabilises the hip and knee joints during running, but excessive strain can lead to iliotibial band syndrome (ITBS), a common overuse injury in runners. This study aimed to determine if running around a curve influences ITB strain using a musculoskeletal model with active and passive ITB strands. Sixteen runners completed trials running straight and around a curve, with kinematic and ground reaction force data collected. An ITB model incorporating gluteus maximus and tensor fascia latae muscles, along with a passive strand, was used. Peak strain values during the stan

Effects of Leg-Length Discrepancy Compensation and Wedge Foot-Orthoses on Tensor Fasciae Latae EMG in Runners.

AIMS: Structural lower limb-length discrepancies (LLLD) have been classically associated with the etiology of low back pain. However, their biomechanical effects on lower-limb muscle activity during running remain unclear. This pilot crossover study aimed to evaluate the influence of orthotic interventions-designed to compensate for LLLD and modify foot biomechanics-on the electromyographic (EMG) activity of the contralateral tensor fasciae latae (TFL) in healthy runners. METHODS: A total of 41 recreational male and female runners (mean age 32.27 ± 6.09) with structural LLLD were recruited an

Passive hip adduction flexibility impairment measured with the modified Ober test predicts chronicity of iliotibial band syndrome: a cross-sectional pilot study.

OBJECTIVES: To investigate predictive features of passive hip adduction flexibility impairment measured during modified Ober's test on chronicity of pain in runners with Iliotibial Band Syndrome (ITBS). METHODS: Twenty male runners with ITBS were evaluated for self-reported pain characteristics and passive hip adduction angle using a digital inclinometer during the modified Ober test. The receiver operating characteristics (ROC) analysis identified the cutoff value for hip adduction angle that predicts ITBS chronicity, and linear regression determined the relationship between these two variab

Clinical Outcomes Following Incomplete Arthroscopic Iliotibial Band Release for External Snapping Hip Syndrome: An Observational Study.

OBJECTIVES: Arthroscopic treatment is indicated for external snapping hip (ESH) syndrome in patients refractory to conservative treatment, but snapping does not disappear completely in some case. No previous studies have described the clinical course of ESH syndrome in patients who presented with persistent snapping after an arthroscopic procedure. We demonstrated the clinical outcomes following an incomplete arthroscopic iliotibial band (ITB) release for ESH syndrome. METHODS: This retrospective observational study was performed at two teaching hospitals between October 2015 and December 202

Risk factors associated with a history of iliotibial band syndrome (hITBS) in distance runners: a cross-sectional study in 76 654 race entrants - a SAFER XXXIII study.

BACKGROUND: Despite the numerous health benefits of distance running, it is also associated with the development of 'gradual onset running-related injuries' (GORRIs) one of which is Iliotibial Band Syndrome (ITBS). Novel risk factors associated with a history of ITBS (hITBS) have not been described in a large cohort of distance runners. OBJECTIVE: To identify risk factors associated with hITBS in distance runners. DESIGN: Descriptive cross-sectional study. SETTING: 21.1 km and 56 km Two Oceans Marathon races (2012-2015). PARTICIPANTS: 106 743 race entrants completed the online pre-race med

Neuromuscular and trunk control mediate factors associated with injury in fatigued runners.

This study aimed to determine how fatigue affects factors associated with injury, neuromuscular activity, and control in recreational runners. Previously identified injury risk factors were defined as peak vertical instantaneous loading rates (pVILR) for tibial stress fracture (TSF) and peak hip adduction (pHADD) for patellofemoral pain syndrome and iliotibial band syndrome. Kinematics, kinetics, and electromyography data were collected from 11 recreational runners throughout a fatiguing run. Three trials were collected in the first and final minutes of the run. Coactivation was quantified abo

Iliotibial Band Origin Tendinopathy Is an Underrecognized Cause of Anterolateral Hip Pain: A Narrative Review and Clinical Commentary.

Hip pain is a common concern among athletes. With gluteal tendinopathy, femoroacetabular impingement, and osteoarthritis predominating sports medicine and musculoskeletal practices, less common etiologies may be overlooked. Complex pelvic anatomy and variable pain referral patterns may make identifying an accurate diagnosis challenging. Employing a systematic approach to evaluation and having a thorough understanding of hip region anatomy are essential. A potentially overlooked cause of anterolateral hip pain is iliotibial band origin tendinopathy. Patients often present with pain around the a

Current iliotibial band syndrome alters patterns of running coordination in male and female runners, but not their levels of coordination variability.

The aim of the present study was to compare the coordination patterns and levels of coordination variability of healthy and injured runners with iliotibial band syndrome (ITBS). Sixty runners divided into four groups (15 healthy males, 15 healthy females, 15 males with ITBS and 15 females with ITBS) ran at a steady and freely chosen pace on an over-ground track, and their coordination patterns of the lower limbs were calculated during 10 running stances using the vector coding technique. Both male and female runners with ITBS showed a greater dominance of the pelvis segment and the anti-phase

The Biomechanical Influence of Step Width on Typical Locomotor Activities: A Systematic Review.

BACKGROUND: Step width is a spatial variable in the frontal plane, defined as the mediolateral distance between the heel (forefoot during sprinting) of bilateral feet at initial contact. Variations in step width may impact the lower limb biomechanics. This systematic review aimed to synthesize the published findings to determine the influence of acute changes in step width on locomotion biomechanics and provide implications for injury prevention and enhanced sports performance. METHODS: Literature was identified, selected, and appraised in accordance with the methods of a systematic review. F

Endoscopic Resection of Distal Femoral Exostosis That Causes Distal Iliotibial Band Syndrome.

Distal iliotibial band (ITB) syndrome is the most common cause of lateral knee pain in runners. If the ITB is too tight, the repetitive knee flexion associated with sports may lead to compression of the vascularized fat pad in the sub-ITB space or development of sub-ITB adventitious bursitis as the band crosses over the lateral femoral epicondyle and the ITB itself is normal. Distal ITB syndrome usually responds to conservative treatment. The presence of any underlying causes, for example, exostosis at the sub-ITB space, should be explored in recalcitrant cases of distal ITB syndrome. Although

2D and 3D microstructural analysis of the iliotibial tract.

The fascial system has gained recognition for its integral role in connecting skin, superficial and deep fasciae, and underlying muscles. However, consensus on its microstructure depending on its topography remains elusive as well as its implications in clinical practices, such as reconstructive surgery and physiotherapy techniques. This study focuses on the iliotibial tract (ITT) implicated in the iliotibial band syndrome. The goal is to describe microstructural characteristics using classical 2D histology and cryogenic contrast-enhanced microcomputed tomography (cryo-CECT) such as the total

Effects of conservative treatment strategies for iliotibial band syndrome on pain and function in runners: a systematic review.

INTRODUCTION: This systematic review summarizes the efficacy of conservative treatment strategies on pain and function in runners with iliotibial band syndrome (ITBS), a prevalent running injury constituting about 10% of all running-related injuries. The multifactorial nature of ITBS necessitates diverse treatment approaches; yet, a consensus on an optimal conservative regimen remains unreported. This review seeks to update and expand upon existing literature with recent rehabilitative approaches. METHODS: A systematic search was conducted in Medline, Web of Science, and CINHAL databases, fro

Effectiveness of dry needling on pain & disability in athletes with iliotibial band tightness- a clinical trial.

INTRODUCTION: Dry needling (DN) for Myofascial trigger points (MTrP's) can be done on superficial or deep tissue levels for the management of a range of neuromusculoskeletal pain disorders. There is still much to learn about dry needling, despite the fact that it has been demonstrated to be useful for a variety of illnesses. The aim of the current study was to determine the effectiveness of DN with stretching on pain and disability in the treatment of athletes with iliotibial band (ITB) tightness. DESIGN: A Clinical Trial. METHODS: For this study, 38 athletes both male and female, aged betwe

A new alternative to the Ober test for evaluating the difference between right and left iliotibial band stiffness: A reliability study.

INTRODUCTION: A dysfunctional stiffness of the iliotibial band can be related to pathologies of the pelvis/lumbar spine and the knee. The classic and modified Ober tests are the gold standard for assessing iliotibial band stiffness. However, to the authors' knowledge, this test lacks adequate validation, and its specificity is questionable. A more reliable test is needed to better correlate iliotibial band stiffness to pathologies and to support treatment choices. METHOD: Two examiners assessed the difference in stiffness (right vs. left limb) of the iliotibial band by direct fingertip palpat
