The coronal and sagittal plane leg movements of 24 experienced male cyclists were assessed using video analysis while cycling on a Kingcycle windload simulator. The cyclists were grouped into those with a history of injury and an asymptomatic group on the basis of self-reported injury status. This study investigated the strain pattern in human patellar tendon in an area of the tendon where changes commonly associated with patellar tendinitis are found. We evaluated the significance of magnetic resonance imaging findings in patients with patellar tendinitis. Midline sagittal magnetic resonance images were taken of 12 knees from 10 patients and of 17 knees from 15 age- and activity-matched subjects who underwent imaging for reasons other than patellar tendinitis. Patellar tendinitis is a well-described clinical entity that usually responds well to conservative treatment. However, a subset of patients continue with symptoms despite exhaustive nonoperative means. The objective of the study was to review the treatment course and operative results of seven patients (eight knees) with chronic patellar tendinitis treated surgically. Five (70%) of these patients were either professional or collegiate athletes. The average age was 30 years (range, 20 to 45 years). Duration of symptoms averaged 1.4 years (range, 3 months to 4 years) before surgical correction.

A total of 33 patients submitted to tibial intramedullary osteosynthesis for fracture (27 cases) and non-union were assessed by ultrasound and x-rays an average of 10.9 months after surgery. Magnetic resonance imaging findings and the results of surgical treatment and histologic evaluation of 11 knees in 9 athletes with recalcitrant patellar tendinitis are reported. The clinical results of 31 knees in 29 patients treated surgically for patellar tendinitis were reviewed. All patients suffered from stage 3 patellar tendinitis according to Roels et al., resistant to conservative treatment.
