Complications of reamed intramedullary nailing of the tibia.

A retrospective review of 60 acute fractures of the tibia treated with reamed intramedullary nailing was undertaken to document the spectrum of complications associated with this procedure. Forty-five tibial fractures were followed to radiographic union; follow-up averaged 25 months (range, 10-63 months). Complications were categorized into intraoperative, early postoperative, and late postoperative groups. Intraoperative complications occurred in 6 of the 60 (10%) fractures and included propagation of the tibial fracture into the insertion site of the nail in four cases. In each of two other

Jumper's knee. Diagnosis and treatment.

"Jumper's knee" or patellar tendinitis was first described in an article by Blazina in 1973. He noted an increasing incidence of peripatellar tendinitis, affecting either the quadriceps tendon or the patellar tendon insertion onto the patella, most commonly in jumping athletes. Since that time, numerous articles have further defined the etiology of this condition and the actual pathologic lesion, along with classifying the stages of its development and outlining appropriate methods of treatment and prevention. Although jumper's knee is quite common, its treatment remains somewhat controversial

Evaluation of eccentric exercise in treatment of patellar tendinitis.

The purpose of this study was to analyze the effects of a quadriceps femoris muscle eccentric training program on strength gain in patients with patellar tendinitis. The effect of an eight-week eccentric exercise program on quadriceps femoris muscle work was evaluated in four groups of subjects--two groups of "normal" (healthy) subjects and two groups of patients with patellar tendinitis. All four groups participated in a home muscle stretching exercise program, but only two groups--one group of normal subjects (N-A) and one group of subjects with tendinitis (T-A)--received additional eccentri

[Design and effect of sports shoe insoles for the runner].

Running injuries of the lower extremities are more frequent with increasing intensity and extent of training. Biomechanical investigations demonstrate that exertion induced injuries of the foot, achillotendinitis, patellar tendinitis and shin splint syndrome are correlated with disorders in footstrike patterns. Therefore running shoe insoles can eliminate peak forces by exactly placing supporting elements. Furthermore peak forces may be distributed and complaints may be reduced. 50 runners with exertion induced injuries of the lower extremity were provided with appropriate running shoe insoles

Ultrasonography in the knee. Clinical aspects.

In at least three types of soft tissue injuries of the knee, meniscal, tears, ligament tears, and patellar tendinitis, ultrasound can be a useful diagnostic adjunct. In some cases, the use of ultrasonography may reduce the morbidity associated with more invasive diagnostic techniques, such as arthrography and arthroscopy. More work needs to be done comparing ultrasonographic findings with surgical findings. As more experience is gathered by the ultrasonographer, this technique may replace arthrography and possibly diagnostic arthroscopy in predicting anatomic injury.

Magnetic resonance images of chronic patellar tendinitis.

Chronic patellar tendinitis can be a frustrating diagnostic and therapeutic problem. This report evaluates seven tendons in five patients with chronic patellar tendinitis. The etiologies included "jumper's knee" and Osgood-Schlatter disease. In all cases magnetic resonance images (MRI) showed thickening of the tendon. Some of the tendons had focal areas of thickening which helped establish the etiology. All cases had intratendinous areas of increased signal which, in four cases, proved to be chronic tendon tears. MRI is useful in evaluating chronic patellar tendinitis because it establishes th

Computed tomography and ultrasound imaging of jumper's knee-patellar tendinitis.

We present nine cases of this relatively unreported condition affecting the patellar ligament in athletes. A comparison between ultrasound and computed tomography of this lesion, shows that whilst neither modality gives false positives, ultrasound is capable of greater distinction between the histological abnormalities confirmed at surgery.

Jumper's knee and ultrasonography.

Twenty-five patients, aged between 15 and 45 years old, who were athletically active, presented with jumper's knee (patellar tendinitis). By use of ultrasonographic examination, new and precise information was obtained that benefited the diagnosis, choice of treatment, and monitoring of the evolution of jumper's knee in our patients. In 18 of the 25 patients, only 1 knee was affected; in 7 of the patients, both knees were affected, thus making a total of 32 painful tendons. These 25 patients were compared with a control group of 15 healthy, athletically active subjects between the ages of 25 a

Preventing Overuse Cycling Injuries.

In brief: Concurrent with the renaissance in bicycling has come an increase in associated injuries. The most common problem the author sees in his practice is knee pain-usually chondromalacia patellae, patellar tendinitis, or bursitis. The patellofemoral joint is under particular stress in bicycling, and many new recreational riders make two errors: setting the saddle too low and the gears too high. These mistakes cause excessive pressure on the patellofemoral joint. Obtaining the best-fitting bicycle and saddle, adjusting the saddle properly, and using lower gears will help prevent such probl

Patellar tendinitis: pathology and results of treatment.

Patellar tendinitis can be considered as an overload lesion in athletes. Anatomical pathology reveals focal degeneration and microtearing at predisposed areas near the insertion of the quadriceps or patellar tendon. Thie prognosis and treatment are dependent on the stage of the affection. A program of conservative treatment is effective in the early stages of the disease but it fails in a high proportion (16/38) of cases in the later stages. Surgery directed towards the tendon, rather than a bony procedure, yielded favourable results in 27 out of 29 patients.

Dynamic patellar brace: a new orthosis in the management of patellofemoral disorders. A preliminary report.

The diagnosis and treatment of patellofemoral dysfunction, especially subluxation of the patella, remains controversial. Many authors feel conservative treatment will give better long-term results than operative treatment. To aid in nonoperative treatment of patellofemoral conditions a dynamic patellar stabilizing brace has been developed. The brace applies an active, medially displacing force to the lateral border of the patella and maintains constant pressure during flexion, extension, and rotation of the knee. This brace consists of an elastic sleeve with a patellar cutout and two circumfer

Patellar tendinitis (jumper's knee).

A jumper's knee is an overload lesion of patellar or quadriceps tendon near its insertion at the lower or upper pole of the patella. If conservative treatment fails, an operation can be performed in Phase 3 where disabling symptoms, are present. The necrotic tissue in the patellar tendon is excised. The early results of this surgery are encouraging.
