Medial epicondylitis is a chronic tendinopathic disorder involving the flexor-pronator origin of the medial elbow, characterised by pain and functional limitation related to repetitive loading and microtrauma. The condition exemplifies the broader spectrum of overuse injuries that result from cumulative strain exceeding the capacity for tendon repair. Diagnosis relies primarily on clinical evaluation, with imaging modalities used selectively to define the extent of pathology or to exclude alternative causes of medial elbow pain. Management follows a structured, stepwise approach, centred on co

BACKGROUND: Juvenile psoriatic arthritis and enthesitis-related arthritis are two categories of juvenile idiopathic arthritis (JIA). Despite available treatments including non-steroidal anti-inflammatory drugs, glucocorticoids, conventional synthetic disease-modifying antirheumatic drugs (DMARDs), and biological DMARDs, a substantial proportion of people do not adequately respond to treatment or do not have long-lasting clinical remission. The aim of this trial was to show the efficacy and safety of ixekizumab in children with active enthesitis-related arthritis and juvenile psoriatic arthriti

BACKGROUND: Medial epicondylitis is an overuse syndrome characterized by degeneration of the flexor-pronator tendons in the elbow, resulting from repetitive forced wrist flexion and forearm pronation. Due to its anatomical location, medial epicondylitis patients may also feature ulnar nerve instability, which can exacerbate symptoms and negatively impact treatment outcomes. Although conservative treatments remain the cornerstone of care for managing medial epicondylitis, the optimal treatment method remains an open question.
OBJECTIVE: To evaluate the effects of a combined extracorporeal shoc

BACKGROUND: Ischiofemoral impingement (IFI) syndrome is the narrowing of the space between the lateral aspect of the ischium and the medial aspect of the lesser trochanter. IFI is a frequently unrecognized generator of vague or posterior hip pain that is commonly preceded by trauma or previous hip surgery. IFI can also be a result of atypical pelvic/proximal femur anatomy, adductor/abductor imbalances, or ischial tuberosity enthesopathies. Management typically consists of conservative treatments, including activity modification, anti-inflammatories, physical therapy, and corticosteroid injecti

Background/Objectives: Insertional Achilles tendinopathy (IAT) is frequently associated with posterior calcaneal spurs; however, the prognostic significance of spur morphology for patient-centered treatment outcomes remains unquantified. This study aimed to establish treatment-specific minimal clinically important difference (MCID) and patient acceptable symptom state (PASS) thresholds after extracorporeal shockwave therapy (ESWT) and to determine whether quantitative spur morphology independently predicts achievement of these patient-centered endpoints. Methods: In this retrospective cohort s

BACKGROUND: Medial epicondylitis, a tendinopathy of the flexor-pronator origin, remains less studied than lateral epicondylitis, especially regarding exercise-based interventions. Eccentric exercise has demonstrated benefits in other tendinopathies, but its role in medial epicondylitis has not been systematically evaluated.
OBJECTIVE: To synthesize clinical evidence on the effectiveness of eccentric exercise in reducing pain and improving function in medial epicondylitis.
METHODS: Following PRISMA guidelines, five databases (PubMed, Web of Science, Scopus, SPORTDiscus, Google Scholar) were s

Deep learning achieves high accuracy in medical imaging but requires large datasets. Performance decreases in small datasets. This study proposes a deep feature engineering (DFE) framework for calcaneal spur detection in X-ray images. A curated dataset of 775 X-ray images was analyzed, with 316 labeled as no finding and 459 as calcaneal spur. The framework has five phases: (i) feature extraction from 19 pretrained CNNs, (ii) feature selection with iterative neighborhood component analysis (INCA), (iii) classification with a t-algorithm-based k-nearest neighbors (tkNN) ensemble, (iv) generation

BACKGROUND: Endoscopic plantar fascia release (EPFR) is a common surgical option for recalcitrant plantar fasciitis. Although calcaneal spur excision (CSE) is frequently performed as an adjunct, its added clinical benefit remains unclear. This study compared outcomes of EPFR alone vs EPFR combined with CSE.
METHODS: This retrospective study included 23 patients (30 feet) who underwent endoscopic surgery for plantar fasciitis between 2017 and 2024. Patients were divided into 2 groups according to surgical procedure performed: EPFR (n = 22 feet) and EPFR+CSE (n = 8 feet). Pain and functional ou

BACKGROUND: Medial (ME) and lateral epicondylitis (LE) are common elbow pathologies characterized by flexor-pronator and extensor carpi radialis brevis tendinous degeneration, respectively. While nonoperative treatment is often successful, some patients do require surgical intervention. Little data exists on the long-term postoperative recovery outcomes of patients in terms of reoperation rate, return of symptoms, and return to activity. Even further, little information exists describing risk factors associated with failing nonoperative interventions. The purpose of the study was threefold: 1)

CONTEXT: Rock climbing places significant mechanical demands on the fingers, wrists, elbows, and shoulders. The repetitive use of isometric gripping, extreme joint positions, and dynamic movements contributes to increasing upper extremity overuse injuries as the sport rises in popularity. Many of these injuries stem from inadequate finger and forearm strength, leading to compensation with proximal musculature and secondary stabilizers. Hangboarding has become a well-established regimen for improving finger flexor strength, but its role in injury prevention remains poorly defined.

Baxter's nerve entrapment, also known as Baxter neuropathy, has various causes such as a calcaneal spur or plantar fasciitis. However, no cases of lipoma compression have been reported. We report the case of a 62-year-old woman who presented with a palpable mass on the medial aspect of the plantar surface of left foot. MRI showed a lobulated mass in the medial foot extending into the central plantar space, showing fat signals in all sequences. Additionally, atrophy with fatty change of the abductor digiti minimi muscle was noted. The patient underwent surgical excision, and pathological examin

INTRODUCTION: Calcaneal spurs are bony outgrowths typically originating from the medial calcaneal tuberosity and are commonly associated with chronic plantar heel pain. Radial pressure wave therapy (rPWT) is widely used when symptoms do not respond to conservative treatments; however, optimal treatment intervals and scheduling remain unclear. This study aimed to compare conventional and accelerated rPWT schedules to determine whether a shorter treatment duration could achieve similar clinical outcomes.
MATERIALS AND METHODS: Sixty-eight participants with heel pain lasting at least six months

BACKGROUND: Extra-corporeal shockwave therapy (ESWT) is commonly used for plantar fasciitis (PF). Calcaneal spurs have up to 80% prevalence in PF. The objective was to assess whether calcaneal spurs with associated oedema affect the outcomes of ESWT.
METHODS: Single-centre study of 30 patients over 3-years. Patients with confirmed diagnoses of PF on MRI scan who underwent ESWT were included. Patient reported outcomes scores (EQ5D / MOxFQ) were recorded pre-treatment and at 1-year post-ESWT.
RESULTS: Significant improvement (p < 0.001) in MOxFQ score and subjective improvement with ESWT in al

This study aimed to develop and validate a computed tomography (CT)-based model that combines radiomics features, deep features derived from nnU-Net, and clinical imaging variables for predicting the presence of calcaneal spurs. In this retrospective study, 803 calcaneal CT images were included and randomly divided into a training cohort (n = 642) and a test cohort (n = 161). The calcaneus was segmented with an iterative semiautomatic no new U-Net (nnU-Net) workflow, and conventional radiomics features and nnU-Net deep features were extracted from the reviewed regions of interest. BACKGROUND: First metatarsophalangeal joint arthrodesis (FMTPA) reliably treats severe hallux deformities. While fixation techniques differ biomechanically, there is no evidence that this influences clinical outcomes. This study compares clinical and radiologic results across three fixation methods.
METHODS: One hundred and ten FMTPA were performed, followed for a mean of 41 months (range, 12-156). Plate-and-screws were used in 74 feet, staples in 28 and crossed-screws in 8. Diagnoses included advanced hallux rigidus (72%), hallux valgus with severe deformity (19%) and rheumatoid foot with ad
