Is primary metatarsalgia associated with a longer 2nd metatarsal? A comparison of metatarsal lengths in individuals with and without metatarsalgia.

BACKGROUND: Metatarsalgia is a generic term used to describe localised forefoot pain around the metatarsals. Primary metatarsalgia is thought to be caused by excessive loading over one or multiple metatarsal heads resulting in localised inflammation and painful callosities. An ideal foot morphotype was described by Maestro et al., which is currently used to plan for corrective osteotomies. The aim of this study is to investigate whether there is a true difference in metatarsal lengths between symptomatic and asymptomatic individuals. METHODS: A retrospective single centre study was carried ou

Endoscopic Plantar Partial Sesamoidectomy for the Management of Metatarsalgia of the First Metatarsal Head After Arthrodesis of the First Metatarsophalangeal Joint.

First metatarsophalangeal arthrodesis has been used to treat various pathologies of the great toe with good clinical outcome. First metatarsal head metatarsalgia can occur after first metatarsophalangeal arthrodesis. Surgery is indicated if conservative measures cannot alleviate the symptoms. If there is isolated excessive plantar pressure of the first metatarsophalangeal joint without significant sagittal deformity of the first metatarsophalangeal joint, plantar partial ostectomy of the sesamoid bone(s) is indicated. The purpose of this technical note is to describe the technical details of e

Association of TP53 Mutational Status and Survival in Anaplastic Thyroid Cancer Treated with Dabrafenib Plus Trametinib.

OBJECTIVE: Anaplastic thyroid cancer (ATC) is an extremely aggressive tumour, with high mortality rates. Dabrafenib plus Trametinib (DT) has significantly impacted the survival of patients with BRAF p.V600E ATC, with noticeable variations in survival between patients. TP53 is frequently mutated in ATC, but its impact on DT response is still unclear. We aimed to investigate the association between TP53 mutational status and clinical outcomes in ATC patients treated with DT. METHODS: BRAF positive ATC patients (n=25) treated with DT between 2018 and 2026 were included. Response was assessed by

Custom foot orthoses for chronic metatarsalgia: Study protocol for a participant- and assessor-blinded superiority randomized controlled trial.

BACKGROUND: Chronic metatarsalgia (CM) causes significant pain and disability, affecting quality of life. Foot orthoses (FOs) including medially wedged designs with a metatarsal pad decrease excessive plantar pressure under the metatarsal heads, which is a suggested risk factor for developing CM. This FOs model may be effective in diminishing pain and improving function in these individuals. Thus, the objective of this trial will be to compare the effects of medially wedged FOs with a metatarsal pad and sham FOs on pain and foot function in individuals with CM. METHODS/DESIGN: This participan

Direct plantar plate repair for second metatarsal phalangeal joint instability: Outcomes from 196 cases with long-term follow-up.

BACKGROUND: Lesser metatarsophalangeal (MTPJ) joint instability, often presenting as second crossover toe deformity, commonly results from plantar plate attenuation. Indirect surgical approaches, such as metatarsal osteotomy or tendon transfer, can correct alignment but often lead to complications like floating toe, transfer metatarsalgia, and stiffness. The direct plantar approach allows for anatomic repair but is underutilized due to concerns about scar-related pain. PURPOSE: To evaluate outcomes of second plantar plate repair via a direct plantar approach, focusing on pain, alignment, sati

Gastrocnemius recession in the treatment of isolated metatarsalgia: A systematic review of surgical outcomes and complications.

Background: Metatarsalgia is a frequent cause of forefoot pain, often linked to isolated gastrocnemius tightness, which increases forefoot pressure during gait. Gastrocnemius recession has been proposed as a surgical treatment to correct this biomechanical dysfunction. This systematic review aims to evaluate the clinical outcomes, complication rates, and quality of evidence regarding gastrocnemius recession performed exclusively for the treatment of isolated metatarsalgia. Materials and Methods: A systematic search of PubMed, MEDLINE, and the Cochrane Library was conducted in February 2025 fol

Clinical and Structural Associations of Disability and Gait Performance in Patients With Rheumatoid Arthritis in Remission and Metatarsal Pain.

BACKGROUND: Patients with rheumatoid arthritis (RA) may continue to experience foot-related disability despite clinical remission. Foot involvement is associated with self-reported disability and objective gait alterations. Foot involvement is heterogeneous; studying patients with localised forefoot pain may help clarify which structural and inflammatory factors are associated with functional impairment. OBJECTIVES: To identify clinical, structural and imaging factors associated with disability and gait performance in patients with RA in clinical remission with metatarsal-region forefoot pain

Recurrence and Function After Minimally Invasive Hallux Valgus Correction: A Dual-Centre Retrospective Comparative Cohort Study of MICA Alone vs MICA+DMMO.

BACKGROUND: Hallux valgus (HV) is a common forefoot deformity associated with pain, gait impairment, and reduced quality of life. Minimally invasive chevron-Akin (MICA) osteotomy provides reliable correction with low reported recurrence. However, when combined with distal minimally invasive metatarsal osteotomies (DMMOs) for coexistent metatarsalgia, altered forefoot biomechanics may increase recurrence risk. No prior study has directly compared outcomes between isolated MICA and combined MICA+DMMO. The primary objective was to compare radiographic recurrence at 12 months, with secondary objec

The Eye Cannot See What the Brain Does Not Know: A Report of Two Cases of Morton's Neuroma.

It is rare for patients to present to the plastic surgery department with foot pain. If this occurs, there is a high probability that these are patients who would have visited numerous hospitals without finding a solution to their complaints and have not recovered despite receiving various treatments. In such cases, Morton's neuroma is a diagnosis that remains unseen by the eyes unless it is kept in mind. Following its correct diagnosis, surgical treatment is definitely within the scope of plastic surgery. In this report, we present two male patients, aged 35 and 40 years, treated in our clini

Geometric Assessment and Tissue Damage Control in Anatomically, Ultrasonographically, and Fluoroscopically Guided Intracapsular DICMO Osteotomies Conducted on Cadaveric Specimens.

Introduction: Distal intracapsular minimally invasive osteotomies (DICMOs) for central metatarsals are described as intracapsular procedures; however, neither their intracapsular location throughout the entire cut nor the optimal anatomical position for their execution have been fully validated. The aim of this study was to assess the geometric position of the DICMO osteotomy in the central metatarsals (third and fourth) and quantify associated anatomical damage when performed under three different guidance modalities: anatomical palpation, fluoroscopic control, and ultrasound guidance. Materi

Optimizing the correlation between first metatarsal head position and osteotomy plane angle in modified scarf osteotomy: a three-dimensional digital analysis.

BACKGROUND: A modified Scarf osteotomy can not only correct hallux valgus deformity but also relieve lateral metatarsalgia by lowering the head of the first metatarsal. This effect is achieved by inclining the longitudinal osteotomy plane from dorsomedial to plantar-lateral. However, with the increase in the inclined angle, the correction ability for the intermetatarsal angle will weaken, which is critical for hallux valgus correction. The balance between intermetatarsal angle correction(IMAC) and the lowering of the first metatarsal head is crucial for good clinical benefits, and the inclined

Trephine-based in situ arthrodesis of the central tarsometatarsal joints: Clinical outcomes.

BACKGROUND: Osteoarthritis of the second and third tarsometatarsal (TMT) joints is a common cause of midfoot pain. Arthrodesis is a reliable treatment, although optimal joint preparation and fixation remain debated. METHODS: A retrospective cohort study included 31 feet in 30 patients undergoing isolated or combined second and third TMT arthrodesis for idiopathic osteoarthritis (2016-2022). The technique used trephine-based joint preparation, calcaneal autograft, and nitinol staple fixation. Outcomes included MOXFQ, AOFAS scores, union, and complications. RESULTS: Radiographic union was achi

Comparison of Radiographic Outcomes and Risk of First Metatarsal Shortening Between different Hallux Valgus Surgeries: An Asian Perspective.

BACKGROUND: The prevalence of hallux valgus is up to 22% in Asia, making it the second-highest in the world. Iatrogenic transfer metatarsalgia is a known complication of hallux valgus correction from excessive shortening of the first metatarsal. Most studies show that first metatarsal shortening should be less than 4.0 mm to avoid such complications. This study aims to compare the risk of first metatarsal shortening, radiologic, and clinical outcomes among surgical procedures used to treat symptomatic hallux valgus. METHODS: A retrospective study was conducted at a single tertiary hospital us

Imaging Considerations in Differentiating Plantar Plate Pathology and Webspace Neuroma.

Myriad pathologies affect the lesser toes. In this article, the focus is on the challenging radiological differential diagnosis of plantar plate (PP) degeneration and tear versus webspace neuroma. It is now understood that PP tear and even degeneration without tear is most accompanied by reactive pericapsular soft tissue thickening (pseudoneuroma), which contributes to neuritic symptoms that are often indistinguishable from webspace neuroma. In this article, the authors will review the differing clinical presentations and radiographic, sonographic, and MRI findings of these entities and the di

Metatarsophalangeal Joint Instability: Anatomy and Physiopathology.

Metatarsophalangeal (MTP) instability is a common cause of metatarsalgia of the lesser toes. Instability often presents as pain in the plantar forefoot, progressing to coronal and transverse plane malalignment. MTP joint dislocation can develop in some cases. The plantar plate plays an integral role in MTP joint stabilization, and injury to the plantar plate is the main contributor to the instability noted at the MTP joint.

Lesser Metatarsophalangeal Joint Instability: Nonsurgical Treatment Alternatives.

Lesser metatarsophalangeal (MTP) joint instability can cause pain in the MTP joints with ambulation. The goal of nonsurgical treatment is to offload the metatarsal heads and stabilize the MTP joints. Achilles stretching can be effective by decreasing pressure in the forefoot with ambulation. Metatarsal offloading does not stabilize the lesser MTP joints but can alleviate pain in the plantar surface of the metatarsal heads. Crossover taping and strapping can stabilize the MTP joint and resist MTP hyperdorsiflexion. Steroid injections can alleviate inflammation but is also associated with furthe
