Lesser Metatarsophalangeal Joint Instability: Nonsurgical Treatment Alternatives.

Lesser metatarsophalangeal (MTP) joint instability can cause pain in the MTP joints with ambulation. The goal of nonsurgical treatment is to offload the metatarsal heads and stabilize the MTP joints. Achilles stretching can be effective by decreasing pressure in the forefoot with ambulation. Metatarsal offloading does not stabilize the lesser MTP joints but can alleviate pain in the plantar surface of the metatarsal heads. Crossover taping and strapping can stabilize the MTP joint and resist MTP hyperdorsiflexion. Steroid injections can alleviate inflammation but is also associated with furthe

Lesser Metatarsophalangeal Joint Instability: Arthroscopic Treatment Alternatives.

Instability of the lesser toes metatarsophalangeal (MTP) joints is a common forefoot problem and can present as metatarsalgia, synovitis, and lesser toe deformities. The plantar plate contributes to MTP joint stability through its interaction with the plantar aponeurosis, and plantar plate insufficiency is the primary cause of MTP joint instability. Conservative treatment options, including the use of nonsteroidal anti-inflammatory medications, intra-articular steroid injections, orthosis, and accommodative shoe modifications, generally provide only temporary relief and do not effectively halt

Is It Necessary to Repair the Plantar Plate in the Management of Metatarsophalangeal Joint Instability?

Lesser metatarsophalangeal joint (MTPJ) instability is a common cause of metatarsalgia and is associated with plantar plate pathology. The instability is often due to chronic attenuation and attritional changes of the plantar plate and collateral ligaments, leading to eventual rupture. The plantar plate and collateral ligaments serve as the primary stabilizers of the MTPJ, and rupture or attenuation of these ligaments leads to instability. Surgical intervention is often necessary for plantar plate tears, with various approaches and techniques available.

First metatarsal length change and clinical outcomes after osteotomy for mild-to-moderate incongruent hallux valgus: A retrospective cohort study.

BACKGROUND: Radiographic correction after hallux valgus surgery may not fully explain clinical outcomes, and first metatarsal length change may be an additional structural factor associated with function. PURPOSE: To evaluate the association between postoperative first metatarsal length change and clinical outcomes after distal Turan (Lindgren-Turan) and proximal medial open-wedge osteotomies for mild-to-moderate incongruent hallux valgus. STUDY DESIGN: Retrospective cohort study (Level 3). METHODS: Fifty patients who underwent isolated distal Turan (Lindgren-Turan) or proximal medial open-

Effect of foot orthoses on the biomechanics of stair ascent and descent in chronic metatarsalgia.

In individuals with musculoskeletal disorders such as chronic metatarsalgia, foot orthoses (FOs) are frequently prescribed to reduce plantar pressure, improve foot function and alleviate pain. During walking, medially wedged FOs (MWFO + MPs) with a metatarsal pad are more effective than standard FOs (SFOs) in reducing plantar pressure under the metatarsal heads. As stair ambulation involves higher physical demands than walking and modifies plantar loading, we questioned whether FOs remain effective during more challenging tasks. This study aimed to determine whether MWFO + MPs reduce peak plan

Surgical management and postoperative imaging assessment of hallux valgus.

Hallux valgus (HV) is a common and complex three-dimensional deformity of the first ray, resulting from multifactorial interactions involving anatomical, biomechanical, ligamentous, muscular, and extrinsic factors. The absence of a single etiological mechanism explains the wide variety of proposed surgical techniques and highlights the need for individualized management. The therapeutic strategy is based on a structured radioclinical weight-bearing assessment integrating angular parameters, first-ray stability, sesamoid alignment, and joint status. Depending on the severity of the deformity an

A Rare Morphological Variant of Postaxial Foot Polydactyly With an Accessory Articulating Osseous Structure of the Fifth Ray.

Postaxial polydactyly of the foot is a common form of digital duplication that often causes difficulty with footwear, metatarsalgia, and cosmetic concerns. Although it may present as an isolated anomaly, it can also be associated with genetic conditions. Surgical excision is the primary intervention, with generally excellent outcomes. A 5-year-old male presented with postaxial polydactyly of the left foot, characterized by an accessory articulating osseous structure associated with the fifth ray. The main complaints were difficulty with footwear and cosmetic appearance. Radiographic evaluation

Dynamic ultrasound (DUS) versus magnetic resonance imaging (MRI) for the diagnosis of Morton's neuroma: A prospective comparative study.

INTRODUCTION: Morton's neuroma (MN) is primarily diagnosed clinically, with imaging commonly used for confirmation and differential diagnosis, particularly from intermetatarsal bursitis (IMB). Dynamic ultrasonography (DUS) and magnetic resonance imaging (MRI) performance in detecting MN remains debated. The aim of this study was to compare DUS and MRI using concordant imaging findings, histopathologic confirmation, and clinical follow-up. METHODS: Prospective single-centre comparative study, patients with suspect MN underwent DUS and MRI examinations, interpreted independently by blinded radi

Metatarsophalangeal arthrodesis of the hallux: A clinical comparison of fixation methods and a review of the literature.

BACKGROUND: First metatarsophalangeal joint arthrodesis (FMTPA) reliably treats severe hallux deformities. While fixation techniques differ biomechanically, there is no evidence that this influences clinical outcomes. This study compares clinical and radiologic results across three fixation methods. METHODS: One hundred and ten FMTPA were performed, followed for a mean of 41 months (range, 12-156). Plate-and-screws were used in 74 feet, staples in 28 and crossed-screws in 8. Diagnoses included advanced hallux rigidus (72%), hallux valgus with severe deformity (19%) and rheumatoid foot with ad

A scientometric review of blue hydrogen production.

BACKGROUND: Batracylin is a heterocyclic arylamine topoisomerase inhibitor with preclinical anticancer activity. Marked species differences in sensitivity to the toxicity of batracylin were observed and attributed to differential formation of N-acetylbatracylin by N-acetyltransferase. A Phase I trial of batracylin in cancer patients with slow acetylator genotypes identified a dose-limiting toxicity of hemorrhagic cystitis. To further explore the metabolism of batracylin and N-acetylbatracylin across species, detailed studies using human, rat, and dog liver microsomal and hepatocyte preparation
