Diffuse idiopathic skeletal hyperostosis (DISH) is an ossifying systemic enthesopathy which involves not only the spine but which may also appear in other sites. Degenerative, inflammatory and metabolic factors have been reported for a possible pathogenic role in the new bone growth that characterises DISH. In the present study peripheral bone mineral density (BMD) has been measured in patients affected by DISH and the results compared to those of a control group. Forty-two patients (33 females and 9 males) affected by DISH and 84 controls (66 females and 18 males) were examined. All subjects

UNLABELLED: To evaluate clinical, radiologic, and laboratory features in Lebanese spondylarthropathy patients according to HLA-B27 status.
METHODS: We retrospectively compared demographic, clinical, radiologic, and severity data in 40 HLA-B27-positive and 58 HLA-B27-negative patients. All 98 patients met Amor's or European Spondylarthropathy Study Group criteria for spondylarthropathy, and 51.7% met New York modified criteria for ankylosing spondylitis.
RESULTS: Onset before 16 years of age, hip involvement, and an elevated mean erythrocyte sedimentation rate were significantly associated wi

OBJECTIVE: To investigate the prevalence of diffuse idiopathic skeletal hyperostosis (DISH) in ancient populations.
MATERIAL AND METHODS: We studied 38 women and 61 men aged at least 30 years at death, whose remains were recovered from four cemeteries of the High Middle Ages (VIth to VIIIth century) located in Lorraine (eastern France). Criteria previously developed by Crubézy and Crubézy-Ibanez for paleopathological studies were used to determine whether the skeletal remains exhibited definite, possible, probable, or no DISH.
RESULTS: Definite DISH with flowing ossification along the thorac

The present communication brings a contribution to the confrontation of two treatment methods in enthesopathies (painful overload syndromes). In group A, 26 patients were treated by a combination of systemic enzyme therapy (SET) and non-invasive laser therapy. In group B, 13 persons were treated only by therapeutic laser. The average age in group A and B was of 39.3 and 41.8 years, respectively. The representation of men and women in the two groups was almost identical. The intensity of the pain prior to the treatment and after the treatment, specified by each patient with the help of a ten-gr

Foot involvement is very frequent in patients affected by psoriatic arthritis (PsA). However, evaluation of the painful foot can be problematic, because it is often difficult to distinguish between arthritis, tenosynovitis, and enthesopathy. Plain radiographs can show bone erosion or other features of joint involvement, but give little information about the soft tissues. We therefore studied foot involvement in 31 PsA patients using high resolution sonography, and compared the results with the findings on x-ray and clinical examination. Ultrasound revealed pathological findings in a large prop

Adult hip pathologies are mainly represented by the degenerative disease, so called "osteoarthrosis, or more precisely coxarthrosis". The means of imaging are exposed, according to their specific value: X Rays (measurement of the characteristic angles of the adult hip), Arthrography, CT Scanner, Arthro-CT Scanner, MRI, Bone Scintigraphy, Ultrasonography. Clinical findings differentiate a mechanical syndrome and an inflammatory syndrome. The coxarthrosis is the most frequent, under two forms: primary (idiopathic) coxarthrosis and secondary coxarthrosis. Primary (idiopathic) coxarthrosis has a l

OBJECTIVE: To investigate the relationship in patients with spondylarthropathies (SpA) between the clinical features of the disease and quantitative flow cytometric expression of HLA-B27 by CD3 gating.
METHODS: We performed quantitative analysis of HLA-B27 antigen by flow cytometry using CD3 gating in 61 patients with seronegative and HLA-B27 positive SpA. The patients included 29 with ankylosing spondy-litis (AS), 29 with undifferentiated spondylarthropathy (uSpA), and 3 with reactive arthritis (ReA). In addition, we compared the fluorescence intensity of HLA-B27 with the clinical characteri

OBJECTIVE: To define the clinical spectrum and disease manifestations of spondyloarthropathy (SpA) as seen in a community, rather than a referral setting.
METHODS: Eighty percent (83/104) of all individuals identified as having SpA in a community wide epidemiologic study of Alaskan Eskimos and 83 age and sex matched controls from the same regions participated in a 5 year clinical study. The study included baseline and followup interviews, physical, radiographic, and electrocardiographic examinations, record reviews, and functional assessment. OBJECTIVE: To discuss a case of diffuse peripheral enthesopathy in a patient previously treated with long-term isotretinoin (Accutane) for severe acne.
CLINICAL FEATURES: A 47-year old man with 1 month history of moderate neck and right upper extremity pain, with hypoesthesia of the right second and third fingers. Palpable bony prominences around multiple superficial joints were noted on physical examination, raising the initial question of osteochondromatosis. Multiple active acne pustules were noted. A limited skeletal survey demonstrated diffuse peripheral enthesophyte formation and hypero

OBJECTIVE: The disease in the entheses, which are sites of attachment of tendons, ligaments, fascias or joint capsules to bone, may be noted clinically by the onset of tenderness or pain associated to digitopression in certain points. This revision article aims to draw the attention of pediatricians to the enthesopathies or enthesitis (inflammatory disease of these structures) in childhood and adolescence. METHODS: The study reviews concepts about the structure of the enthesis and the clinical, laboratorial and radiological features concerning enthesopathy, as well as differential diagnosis an

Thirty-two female patients with confirmed ankylosing spondylitis (AS) and 33 women of similar age with pure ileitis terminalis Crohn were examined for genitourinary infection. Urethral syndrome was found in 15 out of 32 patients with AS: 11 of them had urethritis and 4 urethritis associated with vaginitis. Five women of the control group suffered from urethritis. In all cases with genitourinary infection, Chlamydia trachomatis was isolated. By comparing the AS-patients (urogenital infection group and the non-infected group) with regard to other present clinical parameters, it was found, as exp

Ankylosing spondylitis (AS) is a systemic inflammatory rheumatic disease involving spinal and sacroiliac joints. This condition is responsible for back pain, stiffness and discomfort. Several drugs are currently available in the management of AS, and may be divided into 3 groups. The first includes nonsteroidal anti-inflammatory drugs (NSAIDs), which are the main drug group used in AS because they reduce pain and stiffness in most patients. Several NSAIDs are available but phenylbutazone is considered the NSAID of choice in AS. However, other NSAIDs give similar beneficial results and the medi

Eighty-eight Thai patients (61 males and 27 females) with spondyloarthropathy (SpA) were studied. Their mean age and mean duration of the disease were 25.97 and 3.34 years respectively. In 16 cases the disease first appeared before the age of 16 or had juvenile onset. Eleven cases were ankylosing spondylitis (AS), 9 were juvenile AS (JAS), 20 were Reiter's syndrome (RS), 4 were juvenile RS, 14 were psoriatic arthritis (PsA), 27 were undifferentiated SpA (uSpA), and 3 were juvenile uSpA. Peripheral arthritis, especially oligoarthritis of the lower extremity joints, was the most common form of a

PURPOSE: As fibromyalgia syndrome (FM) has gained greater acceptance and awareness in both the medical and the lay community, the possibility of overdiagnosis exists. Diffuse body pain in a woman is likely to suggest this diagnosis. We report the diagnosis of FM in 11 female patients whose primary cause for musculoskeletal symptoms was spondyloarthritis rather than only FM.
PATIENTS AND METHODS: Of a total of 321 new rheumatology referrals in a 1-year period, 35 (11%) were diagnosed with FM. A further 11 (3%) were referred with either a previous diagnosis of FM or a presumed diagnosis of FM i

The structure, range of functions, blood supply, nerve supply, biochemical composition and development of tendons and ligaments are reviewed. The importance of their cells is often overlooked because of the obvious role of the extracellular matrix (ECM) in determining the physical properties of tendons and ligaments. However, it is emphasised that tendon and ligament cells have elaborate cell processes that form a three dimensional network extending throughout the extracellular matrix. The cells communicate with each other via gap junctions that could form the basis of an important load sensin

Murine ankylosing enthesopathy (ANKENT) is a spontaneous joint disease with numerous parallels to the human seronegative spondylarthropathies at the level of disease distribution, genetics and pathology. Although several genetic susceptibility factors have been identified previously, there is variation in disease susceptibility among genetically identical individuals, living in a stable environment. Preliminary observations indicate that males housed together with females rarely develop the disease, in contrast to males housed with other males (p < 0.25, p < 0.05, and p < 0.01). Furthermore, m

OBJECTIVE: To evaluate the number of males per cage as a possible risk factor for murine ankylosing enthesopathy (ANKENT)--a spontaneous joint disease with parallels to human seronegative spondylarthropathies--since ANKENT shows incomplete penetrance of genetic susceptibility factors among individuals living in a stable environment.
METHODS: Frequency of ANKENT was compared among males housed with females, with other males, or alone.
RESULTS: In three independent cohorts, a trend was observed that males housed with females rarely develop the disease, in contrast to males housed with other ma

Diagnostic and classification guidelines for osteoarthritis based on clinical and radiographic signs for the most frequently involved joints (knee, hip and hand) have been developed by the American College of Rheumatology. Excellent, new 'high tech' biochemical and imaging methods and guidelines for clinical evaluation of the condition and its therapy have recently been developed and validated. Accordingly, we have consistent and reliable methods for assessing the established condition, but they provide little help in making an early and correct diagnosis for the different forms of osteoarthri

OBJECTIVE: To report multiple cases of recurrent seronegative arthropathy, enthesopathy, or both, occurring in a single family in the absence of the HLA-B27 tissue type, coexistent psoriasis or inflammatory bowel disease.
METHODS: Three generations of one family together with their general practitioners completed a standard questionnaire. All subjects with a positive questionnaire and two randomly chosen subjects with negative questionnaires were then examined by a single observer. HLA tissue typing and standard sacroiliac radiographs were performed.
RESULTS: Seven of 12 family members with
