Application and optimization of bioengineering strategies in facilitating tendon-bone healing.

Tendon-bone insertion trauma is prevalent in both rotator cuff and anterior cruciate ligament injuries, which are frequently encountered conditions in the field of sports medicine. The main treatment for such injuries is reconstructive surgery. The primary determinant impacting this process is the graft's capacity to integrate with the bone tunnel. In recent years, researchers have attempted to use a variety of methods to facilitate tendon-bone healing after reconstructive surgery. Such as the implantation of biological materials, cytokines and the local application of permanently differentiat

Distal interphalangeal joint capsule enthesopathy of the middle phalanx and articular cartilage loss.

BACKGROUND: Significant distal interphalangeal joint (DIPJ) pathology, particularly affecting soft tissue and articular cartilage, is often not identifiable on radiographs but can be accompanied by joint capsule enthesopathy on the middle phalanx (P2), which can be readily identified. OBJECTIVES: To explore frequency and correlations between DIPJ capsule enthesopathy identified on magnetic resonance imaging (MRI) and radiographs and DIPJ articular cartilage abnormalities and other concurrent soft tissue or osseous pathology within the foot found on MRI. STUDY DESIGN: Retrospective descriptiv

A Case Report on Umbilical Cord Connective Tissue Allograft Application in Combination with Other Modalities for Defects of the Achilles Tendon.

Damaged connective tissues between the bone and tendons or ligaments are common among adults regardless of activity level. Achilles tendinosis is one of the most common tissue defects and enthesopathies. This case report presents the novel application of Wharton's jelly to supplement tissue defects in the Achilles tendon and its insertion. The patient in this study is a 54-year-old female with slow-onset chronic Achilles tendinosis from chronic enthesopathy at the Achilles tendon insertion with a retrocalcaneal exostosis progressively worsening for 3 years, who failed standard-of-care practice

Ultrasound assessment of patellar enthesopathy in patients with Ehlers-Danlos syndrome with and without concomitant axial spondyloarthropathy.

AIM: Ehlers-Danlos syndromes (EDS) are connective tissue disorders characterized by joint hypermobility, skin hyperextensibility, and tissue fragility. When spondyloarthritis (SpA), an inflammatory arthritis, co-occurs with EDS, overlapping symptoms of joint pain may arise, making diagnosis and management challenging. The aim of this study was to investigate enthesopathic differences in the patellar tendon between patients with EDS and those with co-occurring SpA. MATERIAL AND METHODS: Twenty-three participants with EDS and co-occurring SpA (SpA + EDS) and 14 participants with EDS without SpA

Fibromyalgia with concomitant immune-mediated rheumatic diseases: an evaluation of clinical characteristics, diagnostic criteria and multimodal treatment outcomes.

OBJECTIVES: Fibromyalgia (FM) is a relevant comorbidity in immune-mediated rheumatic diseases (IMRD). Immune mechanisms have been postulated to trigger FM, potentially leading to distinct clinical features compared to FM occurring without IMRDs. This study aims to provide a comprehensive comparison of FM characteristics in patients with and without concomitant IMRD and to evaluate the differences in diagnostic FM criteria between the two groups. METHODS: A comprehensive dataset of clinical, psychosocial, and sleep variables, along with validated questionnaires, was prospectively collected fro

The Diagnosis and Therapy of XLH.

X-linked hypophosphatemia is a rare genetic disease caused by pathogenic variants in the PHEX (phosphate-regulating endopeptidase homolog X-linked) gene with X-linked dominant inheritance that causes metabolic bone disease and other severe complications. PHEX dysfunction results in increased production and secretion of the phosphaturic hormone fibroblast growth factor 23 (FGF23) from bone. The consequences of FGF23 excess are renal phosphate wasting and decreased calcitriol synthesis, leading to hypophosphatemia and subsequently rickets and osteomalacia. Children with XLH usually become sympto

Clinical practice recommendations for the diagnosis and management of X-linked hypophosphataemia.

X-linked hypophosphataemia (XLH) is a rare metabolic bone disorder caused by pathogenic variants in the PHEX gene, which is predominantly expressed in osteoblasts, osteocytes and odontoblasts. XLH is characterized by increased synthesis of the bone-derived phosphaturic hormone fibroblast growth factor 23 (FGF23), which results in renal phosphate wasting with consecutive hypophosphataemia, rickets, osteomalacia, disproportionate short stature, oral manifestations, pseudofractures, craniosynostosis, enthesopathies and osteoarthritis. Patients with XLH should be provided with multidisciplinary ca

X-Linked Hypophosphataemia and Burosumab: A Systemic Disease With a New Treatment.

X linked hypophosphataemia (XLH) is a systemic, chronic condition that significantly impairs quality of life. In XLH, a phosphate regulating endopeptidase homologue X-linked (PHEX) gene mutation leads to excess fibroblast growth factor 23 (FGF23), causing hypophosphataemia and subsequent rickets, lower limb deformity, pain and other sequelae, however there are likely other non-FGF23 mediated mechanisms contributing to disease. Burosumab is an FGF23 inhibiting monoclonal antibody that has been shown to be significantly more effective in treating X linked hypophosphataemia than previously availa

Factors associated with impaired physical functionality in X-linked hypophosphatemia.

X-linked hypophosphatemia (XLH) is a rare inherited disorder characterized by elevated levels of FGF23, chronic hypophosphatemia, impaired bone mineralization, and chronic long-term manifestations. Treatment for XLH has been mainly focused on normalizing its biochemical abnormalities. Despite treatment, patients with XLH often present impaired physical function and decreased quality of life (QoL). We hypothesize that physical functionality and QoL are more strongly associated with chronic pain and decreased muscle mass than persistent biochemical abnormalities or exposure to conventional treat

Spectrum of ultrasonography findings in nail unit disorders: a pictorial review.

Nail unit is affected by a variety of disorders. Diagnosis of nail disorders relies largely on clinical examination, supplemented by biopsy/scrapings of the nail. Imaging such as ultrasound and MRI can aid in better lesion characterization, accurate diagnosis, guiding biopsies and in follow up. Ultrasound is a lucrative modality for nail unit assessment as it is non-invasive, low cost, readily available, can demonstrate all components of the nail unit in high resolution and is real time. MRI on the other hand is expensive, has limited availability and cannot assess multiple nails. This pictori

Macroscopic and histological analysis of the medial and lateral collateral ligaments of the distal phalanges of the foot.

BACKGROUND: Given the conflicting reports in the literature regarding the presence of ligaments in the distal phalanx of the foot, this study aimed to verify the existence of ligamentous structures in this region. Confirmation of these structures is clinically important because numerous pathological lesions have been reported in the lateral regions of the distal phalanges, such as exostoses and osteophytes, which may cause discomfort to patients. MATERIALS AND METHODS: The study was conducted on 10 feet (5 from male and 5 from female donors) obtained from fresh-frozen human cadavers. The dono

Epidemiology and management of Fusarium wilt of Eucalyptus camaldulensis through systemic acquired resistance.

OBJECTIVES: The evolution of psoriasis (PsO) to psoriatic arthritis (PsA) has been proposed recently. There are three phases that occur in sequence prior to classifiable PsA: PsO patients, PsO patients with a positive imaging, and PsO patients with arthralgia not explained by other diagnosis. The purpose of this study was to compare the differences among preclinical phases using ultrasound and clinical assessment. METHODS: Patients with psoriasis were recruited. Patients who had been previously diagnosed with psoriatic arthritis or who had used biologics were excluded. A 52-joint ultrasound (
