[Enthesopathy in inflammatory spondyloarthropathy. Incidence, clinical, radiological and anatomical descriptions. Current status of the question. Apropos of 37 cases].

Based on a series of 37 personal patients and data from the literature, a number of characteristics of enthesopathies can be observed in the course of inflammatory spondyloarthropathies. Our series is based on 20 cases of ankylosing spondylitis (ASP), 8 cases of Fiessinger-Leroy-Reiter syndrome, 8 cases of psoriatic rheumatism (Pso Rh) and one combined form (psoriasis + Reiter + ASP). As well as the frequent involvement of the calcaneus (29 patients), we found more unusual extra-calcaneal localisations in 23 patients. The clinical symptomatology consisted of pain on activity and on weight-bear

[Radiographic aspects of the hand in diffuse idiopathic skeletal hyperostosis].

Radiographs of the hand of 53 patients with diffuse idiopathic skeletal hyperostosis (DISH) were compared with 20 acromegalic subjects and 100 matched controls. Bone qualitative changes, characteristic of DISH, include: (1) arrow-heading of the distal phalangeal tufts; (2) enthesopathy of the proximal phalanges; (3) enlarged sesamoid bones; (4) hook-like exostoses at the metacarpophalangeal heads; (5) cortical thickness of diaphyseal tubular bones. Therefore, the hand in this disease appears as a "robust" form, particularly in males, resembling acromegalic pattern, but in this disease, the han

[The elbow exposed to vibrating instruments and idiopathic diffuse skeletal hyperostosis: association or pure coincidence?].

A group of 11 patients with chronic enthesopathy of the elbow caused by vibrating instruments was systematically examined for spinal signs of diffuse idiopathic skeletal hyperostosis (DISH). Nine patients revealed spinal involvement, the two exceptions probably relating to the patients' youth. The findings support the hypothesis that DISH is not a disease but rather a tendency to excessive ossification of ligamentous structures in response to stimuli that generally produce only modest new bone formation. Those suffering from such ossifying diathesis should be directed towards work unlikely to

[Hyperostosis of the sterno-clavicular joint - an enthesopathy?].

Erosion of the insertions of tendons and ligaments in bone (i.e. of the enthesis) due to chronic aseptic inflammation causes reparative ossification. With regard to our clinical and radiological findings in ten patients hyperostosis of the sterno-clavicular joint seems to be the late stage of such an enthesopathy. Pain and swelling of the soft tissues and an elevated erythrocyte sedimentation rate are characteristics of this seronegative rheumatic disease. For visualizing the extraarticular changes in the sterno-costo-clavicular region tomography is necessary. Early diagnosis and consequent an

Entheses and enthesopathy. Anatomical, pathological, and radiological correlation.

Entheses are sites of tendon and ligament attachment to bone, and enthesopathy is a disease process occurring at these sites. It may be inflammatory, degenerative, endocrine, metabolic, or traumatic in nature. Common sites of involvement include the pelvis, femoral trochanter, humeral tuberosity, patella, olecranon, and calcaneus as well as portions of the vertebral column. Specific radiographic features, including bone erosion, hyperostosis, fragmentation, and crystal deposition, may allow a precise diagnosis.

The enthesopathy of ankylosing spondylitis.

The enthesopathic lesion of ankylosing spondylitis consists of a nonspecific, nongranulomatous inflammatory lesion which leads to destruction of the attachment of ligament to bone. Resulting erosion leads to reactive bone formation which may form a new enthesis and around the outer fibres of the annulus may lead to syndesmophyte formation. What determines the distribution of the enthesopathy and ankylosis is not known.

Spondyloarthritis and enthesopathy. Current concepts in rheumatology.

Spondyloarthritis (Sp) is newly defined as arthritis that is clinically, pathologically, and genetically related to and predisposed to ankylosing spondylitis (AS) and Reiter's syndrome (RS) rather than to rheumatoid arthritis (RA). A diagnosis of Sp does not necessarily imply arthritis of the spine and does not depend on the demonstration of roentgenographic sacroiliitis that, in this conceptualization, is recognized not as the essential hallmark, but rather merely as a diagnostic "way station" on a continuum of disease, which may (but need not necessarily) begin with RS or be complicated duri

Arthritis in Behcet's syndrome.

The clinical and radiological features of fourteen patients with Behcet's syndrome have been reviewed with particular reference to joint disease. Thirteen patients (93%) suffered recurrent peripheral oligo- or poly-arthritis, this symptom being the presenting complaint in two (14%). There were no radiographic abnormalities in peripheral joints. Ten patients (71%) complained of low back pain and in seven (50%) there was a mild erosive sacro-iliitis. Five patients (36%) had an enthesopathy (calcaneal spurs) one of whom also demonstrated vertebral body squaring. Patients with sacro-iliitis did no

[Enthesopathies in ankylosing spondylitis and seronegative inflammatory rheumatism. 28 cases].

A few demonstrative cases of severe and disabling enthesopathy in patients with ankylosing spondylitis (AS) and related syndromes, psoriatic arthritis (PA) or Reiter's disease (RD) have prompted the authors to investigate the incidence of enthesopathy in such patients. A retrospective clinical and radiological study was conducted in 48 patients (mean age: 34.8 years) 27 of whom had AS, 9 RD and 12 PA. The overall incidence of enthesopathy was 58.3%. Beside the classical calcaneal lesions (50%), extracalcaneal manifestations of the disease involving the knees and shoulders were found in 39% of

Enthesopathic erosive lesions of patella and tibial tuberosity in juvenile ankylosing spondylitis. Anatomico-radiological study of a case with tibial bursitis.

The authors report the case of a 20-year-old joiner suffering from ankylosing spondylitis, with recurrent knee effusions from the age of 11 and radiological evidence of sacro-iliac involvement. The patient presented with pain and bone erosions localized to the sites of tendinous attachments in the upper pole of the left patella and in tibial tuberosities. On both sides an inflammation of deep infrapatellar bursa was observed on anatomical examination. These destructive lesions are considered as rare forms of the erosive enthesopathy which has been described in ankylosing spondylitis, particula

[The hand in psoriatic arthropathy. Radiographic and xeroradiographic comparison].

The hands of 45 cases of psoriasis with arthritis and of 100 cases of rheumatoid arthritis without psoriasis were studied by radiographic and xerographic techniques. The characteristic findings of both methods were recorded separately and compared later. In 44% of cases of psoriatic arthritis, radiographic and xerographic changes are indistinguishable from typical rheumatoid arthritis, but with predominant involvement of distal interphalangeal joints, often bilaterally asymmetric. On the contrary, in 31% of cases, erosive polyarthritis associated with proliferation of subperiosteal new bone (e

Peripheral enthesopathy in diffuse idiopathic skeletal hyperostosis (DISH): a radiologic study.

Radiographs of selected peripheral entheseal regions from 32 patients with axial diffuse idiopathic skeletal hyperostosis (DISH) were examined for the presence of new bone formation. The prevalence of entheseal new bone in the various regions was as follows: 98% of tibial spines, 91% of posterior heel regions and about 80% of superior patella, olecranon and inferior heel regions. The new bone showed characteristic solid well defined cortical margins without features of inflammatory change. We suggest that further attention to these readily accessible and easily interpretable peripheral enthese

A syndrome of seronegative enthesopathy and arthropathy in children.

Thirty-nine children with a syndrome of seronegative enthesopathy and arthropathy were evaluated. The group included 25 patients with no apparent underlying primary disease and 13 with either ankylosing spondylitis, inflammatory bowel disease, reactive arthritis, or Reiter's syndrome. Significant distinguishing characteristics of the group included male predominance, late age at onset, positive family histories of arthritis, oligoarthropathy, axial skeleton involvement, and the presence of the B27 histocompatibility antigen. This syndrome is distinguishable from other childhood rheumatic disor

Rheumatoid and other diseases of the cervical interspinous bursae, and changes in the spinous processes.

Bursal spaces between the cervical interspinous processes were found at necropsy in 14 out of 27 "normal' adult necks, especially when the spines were close together. In this random series they were the seat of crystallopathic disease in 2 instances out of 14 cases. In spines from 9 cases of adult-onset rheumatoid arthritis, rheumatoid bursitis was seen in 2 and banal bursitis in 2. In juvenile-onset chronic arthritis inflammatory bursal changes of rheumatoid nature were found in 2 out of 5 cases, and are compared with the "normal'. A third case showed crystallopathic destruction. In one insta

HLA-B27-associated spondyloarthritis and enthesopathy in childhood: clinical, pathologic, and radiographic observations in 58 patients.

HLA-B27 typing of all arthritic children helped to identify and focus attention on a subset whose disease was pathogenetically related to and demonstrated clinical features of ankylosing spondylitis and Reiter syndrome, but only rarely fulfilled current diagnostic criteria for those disorders (spondyloarthritis). In contrast to other forms of childhood arthritis, enthesopathy (inflammation at the sites of attachment of ligaments and tendons to bone) was a prominent feature in 75%; a family history of similar arthritis was obtained from 60%; boys were more frequently affected (2:1); urethritis,

Radiographic features of the hand in diffuse idiopathic skeletal hyperostosis (DISH): comparison with normal subjects and acromegalic patients.

Radiographs of the hands of 25 men and 9 women with diffuse idiopathic skeletal hyperostosis (DISH) were compared with age- and sex-matched controls and acromegalic patients. Characteristics of DISH included broadening and "arrowheading" of the distal phalangeal tufts, enlarged sesamoid bones, increased cortical width of tubular bones, prominent enthesopathy in the proximal phalanges, exostoses (particularly at the metacarpophalangeal heads), and new bone in the joint capsule. There was no consistent change in soft-tissue measurements or cartilage width between DISH and control patients. Acrom

Articular pathology of ankylosing spondylitis.

The essential articular pathology of AS may be said to reflect the occurrence, severity, and overall bias of (a) synovitis which tends to produce articular erosion, and (b) an inflammatory enthesopathy which results in capsular ossification in diarthrodial joints and syndesmophyte formation in cartilaginous joints, both of which are primarily responsible for bony ankylosis. Nonspecific secondary mechanisms contribute to the final picture. These include enchondral ossification, which produces synostosis, osteoporosis and altered stress distribution which make the axial skeleton susceptible to t
