Variation in Intrinsic Aerobic Capacity Reveals Distinct Differences in the Maintenance of Rat Knee Joint Integrity.

The objective was to test how intrinsic aerobic capacity affects the articular cartilage, infrapatellar fat pad (IFP), subchondral bone, and synovial membrane of the rat knee joint. Thirty-five 24-week-old male N:NIH rats selectively bred for high aerobic capacity (high-capacity-runner [HCR]; n = 17) and low aerobic capacity (low-capacity-runner [LCR]; n = 18) were fed either a high-fat/high-sucrose (HFHS) diet or a chow diet for 12 weeks. Alterations in articular cartilage, subchondral bone microarchitecture, IFP hypertrophy, and synovial membrane cytokine levels were quantified. Body composi

Functional Connectivity Differences Underly Variable Centralized Symptom Relief After Hysterectomy.

Approximately 1 in 4 women with chronic pelvic pain (CPP) report persistent pelvic pain after hysterectomy. Centralized symptoms-fatigue, multisite pain, and sleep disturbance, quantified using the Fibromyalgia Survey Score (FSS)-often co-occur with this risk, but the trajectory of centralized symptom burden after surgery, independent of pelvic pain outcome, is not well understood. Whether presurgical differences in central nervous system pain processing distinguish women whose centralized symptoms improve (improvers) from those whose do not (non-improvers) has not been tested. In this observa

Evaluation of the efficacy of duloxetine and venlafaxine in chemotherapy-induced neuropathy in cancer patients: a double-blind multicenter randomized clinical trial.

BACKGROUND: Chemotherapy-induced peripheral neuropathy is a common non-hematologic complication and a major dose-limiting factor in cancer treatment, potentially reducing patient survival. Duloxetine and venlafaxine, antidepressant drugs, have demonstrated efficacy in managing neuropathic pain associated with diabetes and fibromyalgia in several studies. METHODS: In this randomized, double-blind clinical trial, cancer patients undergoing chemotherapy were assessed for neuropathy and pain using the Patient Neurotoxicity Questionnaire and McGill Pain Questionnaire at baseline, 6 weeks, and 12 w

Cranial Electrotherapy Stimulation for Fibromyalgia in Veterans: A Randomized Sham-Controlled Feasibility Study of Resting-State Functional Connectivity Magnetic Resonance Imaging to Evaluate Pain and Treatment Response.

BACKGROUND: Fibromyalgia is a chronic pain disorder frequently co-occurring with depression, anxiety, post-traumatic stress disorder (PTSD), and insomnia, particularly in veterans, and safer nonpharmacological therapies are needed. Cranial electrotherapy stimulation, also referred to as cranial electrotherapy stimulation (CES), may help with anxiety and sleep, but its effect on pain is unclear. This feasibility study evaluated magnetic resonance imaging (MRI) to identify neural correlates of pain and treatment response for CES effects on fibromyalgia pain in veterans. METHODS: In a randomized

Fibromyalgia Subgroups Identified Using Model-Based Clustering: Results from UK Biobank.

Fibromyalgia (FM) is a heterogeneous chronic condition with wide variability in symptom burden, psychosocial distress, and functional impact. This study aimed to identify latent FM subgroups reflecting multidimensional patient profiles using comprehensive biopsychosocial variables and model-based clustering methods within UK Biobank. Individuals meeting the 2016 American College of Rheumatology FM criteria (n = 4,172) were analysed using Gaussian mixture clustering. Twenty-eight variables across sociodemographic, clinical symptoms, and psychological/behavioural domains were included. Cluster s

Excellent Response to Eculizumab in a Patient With Complement-Mediated Thrombotic Microangiopathy and a Novel Plasminogen Gene Variant.

A middle-aged man presented with myalgia, fever, hematemesis, and melena. He was found to have acute kidney injury, acute coronary syndrome, thrombocytopenia, and Coombs-negative hemolytic anemia, raising concern for thrombotic microangiopathy (TMA). While awaiting confirmatory testing, empirical plasma exchange was initiated. Initially, minimal red blood cell fragmentation was observed, which later progressed to florid fragmentation. Despite supportive care, his clinical status deteriorated requiring hemodialysis. The presence of TMA, along with ADAMTS13 metalloprotease activity above the dia

An Atypical Abdominal Presentation of Pancreatic Panniculitis in a Patient With Suspected Pancreatic Cancer: A Case Report.

Pancreatic panniculitis (PP) is an uncommon manifestation of pancreatic disorders and usually presents as painful erythematous subcutaneous nodules involving the lower extremities. We describe a 79-year-old man with a pancreatic head tumor clinically suspected to represent pancreatic cancer who developed a painful, erythematous, approximately 10-cm abdominal wall lesion. Contrast-enhanced computed tomography showed inflammatory changes involving the subcutaneous and abdominal wall adipose tissue. Despite the extensive lesion, pancreatic enzyme elevation was modest, with a lipase level of 157 I

Cutaneous Toxicities of Cancer Therapy in Hispanic and Latino Adults.

Cutaneous toxicities are common during cancer therapy and can affect treatment adherence and quality of life but often are underrecognized. Hispanic and Latino patients, many with darker skin tones, may face unique challenges in the presentation, recognition, and long-term effects. In our literature review, reported toxicities included dermatitis, vitiligo, rash, maculopapular exanthema, panniculitis, nail toxicity, and other hypersensitivity reactions. Radiotherapy toxicities were most consistently reported, with rates of moderate to severe dermatitis similar to prior research, but patients w

A systematic review of psychological variables associated with juvenile primary fibromyalgia syndrome: Comparison between individuals with and without this condition.

Juvenile primary fibromyalgia syndrome (JPFS) is a pediatric rheumatologic condition characterized by (1) persistent, widespread musculoskeletal pain in multiple locations and (2) other symptoms such as fatigue, headaches, poor sleep or cognitive symptoms, among others. These alterations may be associated with the development of psychological distress, poorer mental health and lower quality of life, although no clear consensus has been reached. Thus, this study aimed to examine the psychological symptoms associated with JPFS, compared with a healthy comparison group of children and adolescents

Combined Effect of Hypnotic-Cognitive Therapy With Therapeutic Exercise on Quality of Life in Individuals With Fibromyalgia: A Randomized Controlled Trial Protocol.

BACKGROUND: A multidisciplinary approach is viewed as the primary treatment strategy for fibromyalgia (FM). Therapeutic exercise is an important component of this approach. Hypnotic cognitive therapy (HYP-CT), a relatively new intervention strategy, has not traditionally been incorporated in FM treatment, although it has shown beneficial effects in other chronic pain conditions. This study aims to investigate the potential benefits of a combined approach that incorporates HYP-CT with therapeutic exercise in the rehabilitation of individuals with FM. MATERIALS AND METHODS: One hundred and four
