Clinical outcomes of anifrolumab after belimumab failure in systemic lupus erythematosus: A single-center case series.

ObjectivesTo describe clinical outcomes after switching to anifrolumab in patients with systemic lupus erythematosus (SLE) who showed inadequate response to belimumab, and to explore clinical features potentially associated with treatment response in real-world settings.MethodsWe retrospectively reviewed the cases of eight patients with SLE who were switched from belimumab to anifrolumab because of insufficient clinical response. Clinical data, including the SLE Disease Activity Index 2000 (SLEDAI-2K), serological markers, organ involvement, medication use, and attainment of a lupus low diseas

Erythema Induratum of Bazin Associated With a Negative Tuberculin Test: A Case Report.

Bazin's indurated erythema is a chronic lobular panniculitis associated with a hypersensitivity reaction to Mycobacterium tuberculosis. It is characterized by recurrent painful subcutaneous nodules and ulcerative lesions on the lower extremities. We present a 39-year-old male with a one-year history of unintentional weight loss and intermittent evening fever. During this period, the patient developed progressively evolving erythematous cutaneous lesions located on the lower extremities, along with multiple painful, indurated subcutaneous nodules exhibiting a chronic recurrent course with occas

High Prevalence of MRI Features of Mesenteric Panniculitis in Chronic Intestinal Inflammation: A Retrospective 3-T MRI Study.

Background/Objectives: Mesenteric panniculitis (MP) is a chronic inflammatory and sclerosing disorder of the mesenteric fat. This study evaluated the occurrence and MRI appearance of MP in patients with chronic inflammatory bowel disease and chronic inflammatory bowel conditions. Methods: In this retrospective single-center study, 312 individuals underwent standardized 3-T MRI Sellink examinations, including 252 patients with clinically confirmed chronic inflammatory bowel disease or chronic inflammatory bowel conditions and 60 control patients without intestinal inflammation. MP was diagnosed

Subcutaneous Panniculitis-Like T-cell Lymphoma With Scalp Involvement: A Diagnostic Challenge With Lupus Panniculitis.

Subcutaneous panniculitis-like T-cell lymphoma (SPTCL) is a rare primary cutaneous cytotoxic T-cell lymphoma that may show marked clinical and histopathological overlap with lupus panniculitis. We report the case of a 22-year-old woman with a three-year history of slowly progressive depressed hyperpigmented lipoatrophic plaques involving the thighs, back, and left upper limb, associated with an erythematous alopecic scalp plaque. An initial thigh biopsy showed interface dermatitis, dermal mucin deposition, and lobular lymphocytic panniculitis with positive direct immunofluorescence, leading to

[HAVCR2-mutated subcutaneous panniculitis-like T-cell lymphoma presenting with bone marrow infiltration and rapidly fatal hemophagocytic lymphohistiocytosis].

A 37-year-old man presented with recurrent fever. Diagnosis was challenging due to the lack of localized physical findings; however, subcutaneous panniculitis-like T-cell lymphoma (SPTCL) was eventually confirmed through multiple bone marrow biopsies. The patient developed rapidly progressive liver failure and hemophagocytic lymphohistiocytosis (HLH), resulting in a severe bleeding tendency. Despite receiving massive blood transfusions and sequential treatments with corticosteroids, etoposide, and CHP chemotherapy even before the definitive diagnosis, the patient died of cerebral hemorrhage ap

A Cutaneous Clue to Prolonged Fever: Erythema Induratum of Bazin in a Tuberculosis-Endemic Region.

Erythema induratum of Bazin (EIB) is a rare form of cutaneous tuberculosis characterised by recurrent nodular lesions, predominantly affecting the lower extremities. It is considered a hypersensitivity reaction to Mycobacterium tuberculosis rather than a direct infection, hence the term "tuberculid". Due to its nonspecific clinical presentation, it often mimics other dermatological and systemic conditions, leading to diagnostic challenges. We report a case of a 40-year-old woman presenting with prolonged low-grade fever, weight loss, and multiple tender nodular lesions over the lower limbs and

Occurrence and Determinants of Periprosthetic Fractures of the Femur During Intraoperative and Post-operative Period in Patients Undergoing Hip Arthroplasty: A Prospective Cohort Study.

BACKGROUND: Stachybotrys chartarum is a damp building mould and a potent toxin producer that has been related to serious cases of respiratory health problems. However, the direct link between exposure and health symptoms has not been established. OBJECTIVE: To examine the mechanism by which exposure to spores of satratoxin producing and non-producing S. chartarum strains induce inflammatory responses in murine lungs. METHODS: BALB/c mice were intranasally exposed for 3 weeks to spores of a satratoxin-producing and a non-producing S. chartarum strain. Inflammatory cell infiltration was charac

From Cutaneous Necrosis to Septic Shock: An ICU Case of Pancreatico portal Fistula Complicating Necrotizing Acute Pancreatitis.

BACKGROUND: Pancreaticoportal fistula is an exceptionally rare and potentially fatal vascular complication of necrotizing pancreatitis. Its clinical presentation can be atypical, leading to delayed diagnosis and management. CASE SUMMARY: A 58-year-old woman with no previous medical history was admitted for severe necrotizing acute pancreatitis (lipase > 3000 IU/L, CTSI 6) of initially undetermined etiology. After initial improvement, she developed painful erythematous nodules on both legs on day 34, which rapidly evolved into necrotic lesions. On day 48, she was readmitted in septic shock wit

Clinicopathological Concordance in Panniculitis: A Prospective Study of 48 Cases.

BACKGROUND: Retrospective studies suggest that clinicopathologic concordance rates in panniculitis vary significantly, leading to potential misdiagnoses, delayed treatments, and suboptimal patient outcomes. However, prospective data systematically evaluating the concordance between clinical impressions and histopathologic findings in panniculitis is lacking. This study aimed to determine the concordance between the clinical and histopathological diagnoses in cases with clinical suspicion of panniculitis. METHODS: In this prospectively conducted cross-sectional study at a tertiary-care center

[Panniculitis : Patterns and clues].

Inflammatory infiltrates of the subcutis are a particular challenge in routine histopathology as they occur relatively rarely in submitted specimens and often only superficial portions of adipose tissue are captured in biopsies. On the other hand, panniculitides are very heterogeneous in etiology. They can arise as a reaction to physical stimuli, in metabolic disorders, as a drug reaction, in autoimmune diseases or due to pathogens. The correct classification of infiltrate patterns of adipose tissue and knowledge of associated features and diagnostic clues are essential for narrowing down the

Aesthetic Thermotherapy-Induced Panniculitis in a Patient with Psoriasis: A Case Report.

PURPOSE: We describe a rare case of panniculitis following aesthetic thermotherapy in a patient with psoriasis, who responded well to tofacitinib. To the best of our knowledge, this is the first reported case of aesthetic thermotherapy-induced panniculitis in a patient with psoriasis. CASE REPORT: A 39-year-old female with a 4-year history of psoriasis vulgaris developed multiple subcutaneous nodules on the extremities and abdomen one month after completing a one-week course of whole-body aesthetic thermotherapy (approximately 40-50 ° C, approximately one hour per session). Physical examinati

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) in dermatology: cutaneous adverse events and emerging efficacy in inflammatory skin diseases.

INTRODUCTION: Glucagon-like peptide-1 receptor agonists (GLP-1RAs) and next-generation incretin therapies are increasingly used for diabetes and obesity, yet cutaneous adverse events remain incompletely characterized. New-onset dermatological manifestations after GLP-1RA initiation such as rash or alopecia may be misattributed to baseline inflammatory skin disease rather than drug-induced toxicity. AREAS COVERED: Searches of PubMed, Embase and Cochrane from inception to 4 October 2025 yielded 3,006 records; 80 primary studies were included. Small studies suggest improvement in psoriasis and h

Bullous pemphigoid in patients who are frail: a case complicated by cerebral nocardiosis and Pneumocystis jirovecii pneumonia.

Bullous pemphigoid (BP) is the most common autoimmune blistering disorder in older patients and is typically managed with long-term immunosuppressive therapy tailored to the patient's comorbidities and overall health. Patients who are frail with concurrent haematological malignancies represent a particularly vulnerable population due to their impaired immunity and treatment-related immunosuppression. We report the case of an 85-year-old man with mantle cell lymphoma, previously treated with rituximab and ibrutinib, who developed BP confirmed by histology, direct immunofluorescence, indirect im

Impact of Systemic Inflammatory and Autoimmune Diseases on Outcomes in Patients With Myelodysplastic Syndrome and Chronic Myelomonocytic Leukemia.

OBJECTIVE: Systemic inflammatory and autoimmune diseases (SIADs) are frequently reported in association with myeloid neoplasms; however, their prognostic relevance in myelodysplastic syndromes (MDS) and chronic myelomonocytic leukemia (CMML) remains unclear. This study aimed to evaluate the frequency of SIADs and their impact on survival outcomes in patients with MDS and CMML. MATERIALS AND METHODS: We retrospectively analyzed patients diagnosed with MDS, CMML, or acute myeloid leukemia transformed from MDS between 2000 and 2020. Medical records were reviewed for the presence of SIADs, demogr

Eosinophilic granulomatous inflammation and multi-organ involvement probable caused by Paragonimus heterotremus infection in a pediatric patient: a rare case report.

BACKGROUND: Paragonimus heterotremus is a parasitic flatworm endemic to Southeast Asia that causes pulmonary and extrapulmonary infections. While more common in adults, pediatric cases are rare and often present atypically, posing diagnostic challenges. Eosinophilic granulomatous inflammation due to parasitic infection is especially difficult to identify in children. CASE PRESENTATION: A 9-year-old female child initially exhibited subcutaneous swelling and notable peripheral blood eosinophilia, resulting in two hospital stays without a conclusive diagnosis. Upon admission to our center, labora

Rapidly Progressive Cytophagic Histiocytic Panniculitis in a Child Triggered by Trauma and Scrub Typhus: A Dramatic Response to Cyclosporine.

Cytophagic histiocytic panniculitis (CHP) is a rare and potentially life-threatening inflammatory panniculitis characterized by lobular lymphohistiocytic infiltration and cytophagic "bean-bag" histiocytes. Because its early features overlap with infectious panniculitis and hemophagocytic lymphohistiocytosis (HLH), diagnosis is often delayed, contributing to significant morbidity. We describe a rapidly progressive pediatric case of CHP triggered by antecedent trauma and scrub typhus infection. A 10-year-old girl presented with seven days of high-grade fever and painful erythematous nodules over
