Tricuspid Valve Incompetency Due to the Grenade Chip Embolization to the Heart.

Migration of foreign bodies into the heart, although unusual, has been reported since 1834 when Davis published the first bullet embolus. Our case was a 29-year-old woman who suffered from a grenade explosion wound with a residual burst fragment inside her arm soft tissue. Two months later, she was admitted with progressive fatigue, palpitation, shortness of breath, and swelling in legs and neck. She was taking hormonal therapy for infertility as medical history. Ascultation detected holosystolic respiratory variable murmur maximal over the left lower sternal border. Severe tricuspid regurgita

Cotton bud in external ear canal causing necrotising otitis externa and subdural abscess.

Necrotising otitis externa (NOE) is an infection originating in the soft tissues of the external auditory canal (EAC) spreading to the surrounding bone and rarely causing intracranial complications. It is usually caused by Pseudomonas aeruginosa and has historically occurred in elderly patients with diabetes or immunodeficiency. EAC foreign body is a risk factor for otitis externa but has not been described in NOE. A healthy 31-year-old man presented with new-onset seizures and worsening left-sided otalgia and otorrhoea. Brain imaging revealed left temporal subdural abscesses superior to the p

Removal of Polyacrylamide Gel (Aquamid®) from the Lip as a Solution for Late-Onset Complications: Our 8-Year Experience.

BACKGROUND: The polyacrylamide hydrogel Aquamid® has been used as a permanent filler to enhance facial soft tissue volume and correct wrinkles since 2001. Various long-term studies have proved the safety of the product. Nonetheless, if complications such as migration occur, they can be difficult to treat. METHODS: Eleven patients suffering from late-onset complications after taking Aquamid® injections in the lips underwent product removal and subsequent labial reconstruction between 2009 and 2017. The reconstruction was performed using a modified bikini reduction technique combined, in eight

[Reconstruction of osseous defects using the Masquelet technique].

The Masquelet procedure or induced membrane technique presents a treatment option for relatively large osseous defects, e.g. after trauma, tumour resection or osteomyelitis even in the presence of unfavourable soft tissues. Initially developed at the end of the last century by the French surgeon Masquelet, the technique relies on a bioactive membrane that forms a foreign body reaction around a cement spacer. This spacer is implanted in the residual defect after rigorous debridement of bone and soft tissue during a first-stage procedure. A second-stage intervention is performed 1-2 months later

Histologic findings within peri-implant soft tissue in failed implants secondary to excess cement: report of two cases and review of literature.

A link has been established between peri-implant disease and excess cement extrusion in cement-retained implant restorations. The histologic findings of two patients with failed implants secondary to residual excess cement are reported here. If excess cement is detected early and adequately removed, resolution can occur in the majority of situations. Simple recommendations are proposed, with the intention of preventing further implant failures from residual excess cement.

Residual foreign body in the neck after trauma results in the delayed rupture of the common carotid and internal jugular vein: a case report.

INTRODUCTION: Trauma and foreign body residue occurring in different settings are common in the neck. Some small injuries go unrecognized, and vascular injuries caused by the sharp penetrating trauma of a foreign body are very dangerous. Without early diagnosis and treatment, foreign body residue remains a major cause of mortality. CASE PRESENTATION: A six-cm piece of wooden chopstick was not initially detected in the neck of a 24-year-old Chinese man presenting with a slight bleeding wound after a brawl accident. Three days later, the patient had an expanding neck hematoma and shortness of b

Subungual wooden splinter visualized with bedside sonography.

Bedside ultrasound has become increasingly important as an adjunct to clinical diagnosis and procedures in the emergency department. It is only recently that this modality, which involves no ionizing radiation, has become incorporated into the pediatric emergency department. We report a case of a 10-year-old boy with a suspected subungual wooden foreign body. Bedside ultrasound was used to identify and characterize the foreign body before removal and then to evaluate for any residual foreign body after removal. A brief review of the technique is presented, including the use of a water bath to

Management of a small paracentral corneal perforation using iatrogenic iris incarceration and tissue adhesive.

BACKGROUND: Surgical intervention for corneal perforation is indicated when the anterior chamber does not reform within a short period of time. Herein, we report the successful management of a small paracentral corneal perforation using autologous iris incarceration and tissue adhesive. CASE: A 41-year-old man developed a small paracentral corneal perforation (0.5 mm in size) in the right eye, while the treating physician attempted to remove the residual rust ring after removal of a piece of metallic foreign body. OBSERVATIONS: The eye was initially managed with a bandage soft contact lens t

Intrapulmonary foreign body removal under virtual bronchoscopic navigation.

We report a case of successful removal of a pulmonary foreign body (gauze) using 3-dimensional computed tomography (CT) with virtual bronchoscopy. A 70-year-old woman underwent thoracic surgery for tuberculosis approximately 35 years ago. In 2011, she presented with chronic cough and intermittent fever. A chest CT revealed a soft tissue mass measuring approximately 4.5 cm in diameter in the right residual lower lobe of the lung. On the basis of the chest CT, a foreign body was suspected. The foreign body could not be identified under conventional bronchoscopy because of the presence of multipl

Management outline of oral and maxillofacial missile injuries in Iraq: the value of the intermediate phase.

Oral and maxillofacial surgeons in Iraq are practicing in a paradoxical situation where a high number of extensive injuries are accompanied by limited resources. Steps of management should be followed cautiously in attempts to reduce residual deformities that characterize the end results of severe missile injuries. This study elucidate lessons extirpated from surgical experience in the management of diversity of causalities with special emphasis on the intermediate phase of management as well as different modalities of treatment, trying to get best benefit from available resources.

In vivo detection and imaging of low-density foreign body with microwave-induced thermoacoustic tomography.

Radiography or computed tomography is the most widely available imaging tool for foreign body detection. However, the detectability of low-density substances by x ray is very poor when located in soft tissues. Various dielectric loss factors of foreign bodies contribute great microwave absorption heterogeneity compared with the surrounding tissue. A fast thermoacoustic tomography system at 1.2 and 6 GHz was developed to detect foreign targets in small animals. The 6 GHz system had a much higher signal-to-noise ratio in near-surface imaging but smaller imaging depth than the 1.2 GHz system. The

Histopathological observations of a polylactic acid-based device intended for guided bone/tissue regeneration.

BACKGROUND: Barrier devices have been shown to support alveolar bone and periodontal regeneration, a procedure also known as guided bone/tissue regeneration (GBR/GTR). Popular demand and clinical convenience have raised an interest in bioresorbable barrier devices. Tissue reactions to such bioresorbable devices are, however, generally not well explored. PURPOSE: The objective of this study was to evaluate short- and long-term tissue reactions following implantation of a bioresorbable polylactic acid (PLA)-based barrier device using a rat model. MATERIALS AND METHODS: Twenty-one young adult m

Management of visible granulomas following periorbital injection of poly-L-lactic Acid.

PURPOSE: Injectable poly-L-lactic acid (PLA) is a recently FDA-approved soft-tissue filler. Although approved only for treatment of HIV-associated lipoatrophy, there has been increased interest in its use for cosmetic purposes. Here we describe a case of a granulomatous reaction to injectable PLA in the inferior periorbital region resulting in visible papules and discuss treatment options. METHODS: We review literature pertaining to medical and surgical treatment options for our patient, and we discuss relevant techniques for both the injection of PLA and the management of this complication w

Biodegradable polylactide membranes for bone defect coverage: biocompatibility testing, radiological and histological evaluation in a sheep model.

Large bony defects often show a delayed healing and have an increasing risk of infection. Several materials are used for the coverage of large defects. These materials must be biocompatible, easy to use, and must have an appropriate stability to present a mechanical hindrance. Aim of this study was to investigate two different biodegradable membranes for defect coverage in a sheep model. Round cranial defects (1.5 cm diameter) were created in sheep. Six different treatments were investigated: defects without membrane, defects covered with a poly(D,L-lactide) or with a 70/30 poly(L/D,L-lactide)

[Morphology of bullet wounds when using police ammunition, type QD-P.E.P].

6 years after introduction of the police ammunition QD-P.E.P. (9 mm caliber), 9 bullet wounds are presented from forensic investigations along with the results of experimental shots at pig carcasses. The evaluation reveals the characteristic pattern of such bullet wounds: a skin defect which is almost as large as the caliber and has a sharp-edged border thus looking like it has been punched out; the contusion ring ("abrasion ring") is conspicuously narrow. These findings can be explained by the shape and design of the projectile. In regular police work immediate incapacitation has not always b

Case report: subcutaneous elemental mercury injection--clinical observations and implications for tissue disposal from the histopathology laboratory.

A 31-yr-old man presented to his general physician complaining of pain and swelling in the soft tissue of both hips. He initially denied knowledge of the etiology, but after elemental mercury droplets were expressed from the skin wounds by manual manipulation, he admitted that the source was self-administered sc injections. He did not provide a reason for this or give the time-frame of the injections. The areas of skin discoloration and soft tissue induration were completely excised and the wounds healed uneventfully. Blood levels of mercury declined gradually after excision. Examination of th

Detection of a residual foreign body during root canal treatment.

A 25-yr old female with a 12-month history of orofacial trauma was referred to us with acute pain with positive percussion of the left mandibular first premolar and swelling in the left-posterior mandibular area. The diagnosis was pulpal necrosis with acute apical abscess. Both preoperative and length determination radiographs revealed a semi-opaque area on the root of the first premolar that was later identified as a residual fragment of windshield left in the soft tissues. Thorough medical record, clinical and radiographic examination may be helpful in the detection of foreign bodies in the

Ultrasound for the detection of vegetative foreign body in hand--a case report.

Foreign bodies in soft tissues are commonly encountered in daily orthopaedic practice. While most of the metals and glass foreign bodies can be detected by plain radiograph, organic substances such as wood and vegetative materials are radiolucent. Unfortunately, these radiolucent foreign bodies are usually more prone to cause an inflammatory reaction and infection. The detection can be even more difficult in cases of multiple foreign bodies and in penetrating injuries with small innocuous skin wounds. Ultrasonography is a sensitive and reliable investigation for detection of foreign bodies in

A pilot study to evaluate the effectiveness of small intestinal submucosa used to repair spinal ligaments in the goat.

BACKGROUND CONTEXT: Destabilization of the lumbar spine results from sacrifice of the anterior longitudinal ligament and disc when removed for graft or cage placement. In a similar fashion, transection of the interspinous ligament during surgical approaches to the posterior spine may result in segmental instability. Such instability can cause abnormal motion or implant migration resulting in a higher incidence of pseudarthrosis. Small intestinal submucosa (SIS) is a naturally occurring extracellular collagen-based matrix, which is derived from porcine small intestine. SIS contains cytokines an
