Reduced Lumbar Lordosis in Fibromyalgia: Radiographic Evidence of Lumbar Straightening by L1-L5 Cobb Angle.

BACKGROUND: Reduced lumbar lordosis has been observed in patients with low back pain, but lumbar straightening has not been well characterized in fibromyalgia. We hypothesized that fibromyalgia would be associated with reduced lumbar curvature on standing radiographs. OBJECTIVE: To evaluate lumbar lordosis, quantified by the L1-L5 Cobb angle, in patients with fibromyalgia compared with patients with other rheumatic diseases and low back pain. DESIGN: Retrospective, cross-sectional observational study. SETTING: Rheumatology office practice. PARTICIPANTS: A consecutive series of 245 patients

Hibiscus sabdariffa protects against fibromyalgia by modulating the neuroinflammation-oxidative stress-microRNA axis.

Fibromyalgia (FM) is a chronic syndrome characterized by widespread pain, fatigue, cognitive dysfunction and neuropsychiatric disturbances, associated with dysregulated oxidative stress and neuroinflammation. Emerging evidence suggests that microRNAs guiding these pathways, including miR-145-5p and miR-451a, may play a central role in FM pathogenesis. However, currently available pharmacological therapies provide limited efficacy and are frequently associated with adverse effects. Therefore, the present study investigated whether the methanolic extract of Hibiscus sabdariffa (MeOH-Exs), a poly

What the algorithm serves: diet and fitness content on Instagram as independent predictors of orthorexia nervosa in a large Italian female sample.

OBJECTIVE: Instagram use has been associated with orthorexia nervosa (ON), but prior research has generally measured exposure as overall platform use or broad health-related content. We directly compared four content categories (diet and slimming products, physical activity and sport, food in general, and fashion) within one adjusted model and provided an exploratory description of Instagram use and orthorexic symptoms in women with fibromyalgia. METHODS: Cross-sectional web-based survey of 3,655 Italian adult women using Instagram as their primary social media platform. Content exposure was

A novel repurposing opportunity for valsartan: angiotensin II type 1 receptor blockade dampens muscle-to-brain sensitization in fibromyalgia by targeting lncRNA H19/miR-29a-3p, inflammatory mediators, AKT/CREB signaling, neurotrophic factors, and amyloid-Tau pathology.

Fibromyalgia (FM) is a debilitating nociplastic pain disorder increasingly recognized to involve skeletal muscle as an active contributor to peripheral nociception and central sensitization rather than a passive target of disease. Although angiotensin II promotes pain signaling through angiotensin type-1 (AT1) receptor activation, the role of skeletal muscle AT1 receptor signaling in orchestrating the inflammatory, trophic, and proteotoxic disturbances underlying FM remains unexplored. This study investigated, for the first time, whether AT1 receptor blockade by valsartan interrupts maladaptiv

Motor cortex pain compensation is modifiable: allostatic load degrades its analgesic efficiency and rehabilitation restores it.

The primary motor cortex is a critical hub for pain control, yet its few identified regulatory mechanisms - intracortical inhibition and oscillatory markers - predict symptoms inconsistently. The prevailing view holds that pain results from reduced inhibition, a fixed deficit, rather than from resistance to inhibition that rehabilitation can reverse. Here we show that motor cortex pain compensation is instead a modifiable state whose analgesic efficiency depends on allostatic load. In a fibromyalgia trial and a knee osteoarthritis cohort assessed with parallel EEG and transcranial magnetic sti

Sex-specific mechanisms of chronic pain.

For decades, we have known that women, especially younger women, are disproportionately affected by pain disorders, including irritable bowel syndrome, endometriosis, menstrual migraines, fibromyalgia, osteoarthritis, and neuropathic pain, all of which degrade quality of life. The adoption of sex as a biological variable in 2016 has begun to transform the field, revealing distinct sex-specific pathways that initiate, amplify, and resolve pain. In this Review, we synthesize recent advances across visceral and somatic pain, highlighting the impact of sex hormones, immune-neural cross-talk, the g

Transcutaneous Auricular Vagus Nerve Stimulation for Chronic Pain Management: A Narrative Review.

Chronic pain represents a global public health challenge; current therapeutic interventions remain limited and are frequently accompanied by adverse effects. Transcutaneous auricular Vagus Nerve Stimulation (taVNS) has emerged in recent years as a potential non-invasive neuromodulation therapy, providing novel possibilities for chronic pain management. Preliminary research indicates that taVNS demonstrates preliminary therapeutic potential in conditions such as migraine, fibromyalgia, and chronic low back pain, with initial exploratory applications also conducted in trigeminal neuralgia, diabe

High resolution EEG microstate framework in fibromyalgia: An integrated multivariate and source localization approach to pain-related dynamics.

Fibromyalgia (FM) is a nociplastic pain syndrome associated with altered central nervous system processing consistent with central sensitization, leading to enhanced pain perception. Despite its prevalence, the underlying neurophysiological mechanisms remain unclear. In this study, we characterized resting-state EEG (RS-EEG) dynamics in FM using a high resolution microstate (MS) framework combined with multivariate pattern analysis (MVPA) and source localization. 5-min eyes-closed RS-EEG was recorded from 57 female participants (29 FM patients and 28 healthy controls), and an 11-MS template wa
