Acute activation of rat dorsal root ganglion neurons and their satellite glial cells by the serum of patients suffering from fibromyalgia.

OBJECTIVES: The dorsal root ganglia (DRG) may play an important role in fibromyalgia pain. The DRG house the nucleus of somatosensory neurons carrying painful stimuli from different body parts. Each neuronal soma interacts with its enveloping immune-competent satellite glial cells (SGCs). Different mediators may activate SGCs, inducing stronger SGCs-neuron coupling and leading to chronic pain. Mice injected with immunoglobulin G of patients suffering from fibromyalgia develop hyperalgesia and neuropathy; in these instances, immunoglobulin G is deposited in DRG SGCs. Our aim was to determine wh

Hippocampal Function and Markers of Citrullinated Proteins and Bacterial and Viral Infection in Fibromyalgia.

INTRODUCTION: Fibromyalgia is associated with dyscognition or "fibro fog", of unknown aetiology. The hippocampus has major roles in memory and cognition. Certain bacterial and viral infections can cause hippocampal changes. Furthermore, citrullinated proteins may accumulate in the hippocampus in Alzheimer's disease. OBJECTIVES: This study aimed to test the hypothesis that hippocampal functioning in fibromyalgia is associated with markers of bacterial and viral infection and serum anti-cyclic citrullinated peptide antibody (anti-CCP) levels. METHODS: Hippocampal functioning was assessed in 26

Disentangling comorbidity: symptom dimensions of internalizing and functional disorders in a large general population sample.

BACKGROUND: Internalizing disorders (IDs) and functional disorders (FDs) are highly comorbid, however the mechanisms underlying their co-occurrence remain unclear. This study investigated the symptom dimensions of major depressive disorder (MDD), generalized anxiety disorder (GAD), myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS), fibromyalgia (FM), and irritable bowel syndrome (IBS) symptoms in the general population to understand comorbidity at the symptom level. METHOD: We analyzed cross-sectional data from 108,418 adult participants in the Dutch Lifelines Cohort Study, including

Brazilian Society of Rheumatology's fibromyalgia treatment guidelines - part I: monitoring and non-pharmacological management.

Fibromyalgia is one of the most prevalent rheumatologic conditions, affecting millions of individuals worldwide. It is characterized by widespread chronic pain accompanied by symptoms such as fatigue, sleep disturbances, and cognitive impairments. In Brazil, fibromyalgia is the second most common rheumatologic disorder, with a reported prevalence ranging from 2.5% to 5.5%, leading to a substantial economic burden due to high direct and indirect healthcare costs. The pathophysiology of fibromyalgia involves alterations in central pain processing, often in conjunction with comorbidities such as

Prevalence of Bodily Distress Syndrome and Prediction of Patient Outcomes: Cohort Study of 3762 Individuals With Persistent Pain.

BACKGROUND: Persistent physical symptoms are common and often result in disability and high healthcare use. To capture how such symptoms co-occur, Bodily Distress Syndrome (BDS, also called Functional Somatic Disorders) was developed as an empirically derived construct, in contrast to consensus-based syndromes such as fibromyalgia and chronic fatigue syndrome/myalgic encephalomyelitis (CFS/ME). BDS is distinct from ICD-11 Bodily Distress Disorder and reflects the multisystem symptom pattern described in the Functional Somatic Disorder framework. However, its prevalence, symptom structure and p

Inflammation and pain as interconnected targets in axial spondyloarthritis.

Axial spondyloarthritis (axSpA) is a chronic inflammatory disease characterized by complex pain mechanisms that extend beyond inflammation. Although inflammatory nociceptive pain - primarily mediated by pro-inflammatory cytokines - represents the classic pathway and therapeutic target, many patients continue to experience pain despite suppression of inflammation. This residual pain often reflects non-inflammatory processes, including nociplastic and neuropathic pain. Central sensitization, a key mechanism of nociplastic pain, contributes to pain amplification and poor response to treatment. Fi

Fibromyalgia is a complex condition characterized by widespread chronic pain, fatigue, and sleep disturbances, which significantly affect quality of life. Its pathophysiology involves central sensitization (CS) and changes in neurotransmitters. Management is challenging and requires a multidisciplinary approach that integrates pharmacological and nonpharmacological treatments. This consensus, which is based on the BASCE system, presents updated guidelines for drug and nondrug treatments. The methodology focused on the evaluation of meta-analyses and systematic reviews of the literature and on
