A primary care-oriented framework for osteoporosis prediction: integrating multi-algorithm feature selection and machine learning model benchmarking.

SUMMARY: Large-scale screening and primary care diagnosis of osteoporosis are challenging due to the limited availability of dual-energy X-ray absorptiometry (DXA). Therefore, predictive models for osteoporosis that do not rely on bone mineral density measurements are essential. Using routine health examination data and applying a consensus feature selection strategy, we constructed machine learning (ML)-based models and compared their performance across multiple dimensions. PURPOSE: To develop a robust, low-cost, and interpretable framework for osteoporosis prediction based on readily availa

Integrated transcriptomic analysis identifies an AKT1-associated metabolic-inflammatory signature in osteoporosis.

Osteoporosis is a significant global health challenge, and low bone mineral density (Low-BMD) is a major clinical feature and risk indicator of osteoporosis. Fatty acid metabolism has been increasingly implicated in bone remodeling and inflammatory regulation, but the transcriptomic links between fatty acid metabolism and immune-inflammatory alterations in Low-BMD remain unclear. We integrated three GEO datasets, including 60 High-BMD controls and 59 Low-BMD subjects. Differential expression analysis, immune infiltration analysis, NMF clustering, WGCNA, pseudotime analysis, and machine learnin

ARG2 drives osteoblast senescence by repressing mitophagy via the IκB-NF-κB-TXN2 signaling axis: implications for age-associated osteoporosis.

RATIONALE: Age-associated osteoporosis (AAOP) is characterized by progressive bone loss that can be driven by impaired osteoblast function and increased cellular senescence. However, the molecular mechanisms driving osteoblast senescence during aging remain incompletely understood. In this study, we aim to investigate the role of arginase 2 (ARG2) and its underlying mechanisms in osteoblast senescence and AAOP. METHODS: AAOP were assessed in D-galactose-induced aging mice, naturally aged mice, and osteoblast-specific ARG2 knockout mice. In vitro, osteoblast senescence was induced by hydrogen

Young plasma-derived exosomes deliver YOD1 to induce M2 polarization and ameliorate osteoporosis via mTOR signaling pathways.

This study aimed to evaluate the therapeutic potential of young plasma-derived exosomes (Y-Exos) in osteoporosis, focusing on their capacity to modulate macrophage polarization and bone homeostasis. Ovariectomized (OVX) mice were administered young plasma, aged plasma, exosome-depleted plasma, or plasma-derived exosomes. Bone mass, macrophage polarization, and osteoblast/osteoclast activity were analyzed by micro-CT, flow cytometry, and histology. Human Y-Exos were isolated to examine their direct effects on BMSC osteogenesis and BMM polarization, as well as indirect effects on osteoclastogene

Autoimmune hepatitis following long-term denosumab therapy in a patient with Hashimoto thyroiditis.

Denosumab, a receptor activator of nuclear factor-κB ligand (RANKL) inhibitor, is widely used to treat osteoporosis. Its role as a trigger for autoimmune hepatitis (AIH) remains uncertain. We report a 68-year-old woman with osteoporosis and Hashimoto thyroiditis who developed jaundice and severe hepatocellular liver injury after 3 years of denosumab therapy. Alternative viral, obstructive, and metabolic causes were excluded. Liver biopsy demonstrated interface hepatitis with a lymphoplasmacytic portal infiltrate consistent with AIH, and autoimmune serologies were positive. Hemophagocytic lymph

Alendronate-Conjugated Citrate-Based Composite Microscaffolds Promote Osteoporotic Bone Regeneration via Activating Osteogenesis and Suppressing Osteoclastogenesis.

Healing of osteoporotic bone defects is profoundly compromised by the dysregulation of bone remodeling homeostasis, characterized by excessive osteoclastic resorption and diminished osteoblastic activity, thus conventional bioinert bone cements fail to modulate these aberrant conditions or restore physiological remodeling. To address this, alendronate-conjugated citrate-based composite microscaffolds, consisting of an alendronate (ALN) conjugated poly (octamethylene citrate) prepolymer mixed with hydroxyapatite (HA) (APOC-HA), was developed followed by thermal post-polymerization. Advancing fracture risk prediction: established assessment tools and emerging HR-pQCT-based approaches.

Osteoporotic fractures, or fragility fractures, pose a substantial global healthcare burden. Individuals who experience them report significant declines in their quality of life. Identifying individuals at risk of fragility fractures is key to reducing the future fracture burden. However, due to demographic shifts, the proportion of older adults is growing globally. As fracture risk rises with age, an increase in the incidence of osteoporotic fractures is also anticipated. However, existing screening initiatives and fracture risk assessment tools remain challenged in identifying individuals at

Methylmalonic Acid in Skeletal Aging and Osteoporosis: Linking Vitamin B12 Metabolism to Mitochondrial Dysfunction.

Methylmalonic acid (MMA) is a functional marker of vitamin B12 insufficiency and an emerging metabolic factor associated with skeletal aging and osteoporosis. This review synthesizes epidemiological, experimental, and mechanistic evidence linking the vitamin B12-MMA axis to impaired bone homeostasis. Population studies associate elevated circulating MMA with lower bone mineral density, higher odds of osteoporosis, and lower knee extensor strength, while disturbed vitamin B12 metabolism has also been linked to altered bone turnover. Experimental MMA exposure and models of methylmalonyl-CoA muta

Mesenchymal stromal cells in polytrauma: from immune dysregulation to therapeutic potential.

Polytrauma, defined as life-threatening injuries to multiple organ systems with an Injury Severity Score greater than 16, presents significant challenges for fracture healing, with nonunion rates substantially higher than in isolated fractures. The systemic inflammatory response syndrome (SIRS) and subsequent immune dysregulation characteristic of polytrauma create a hostile microenvironment that impairs normal bone regeneration. Mesenchymal stromal cells (MSCs; also termed mesenchymal stem/stromal cells) play critical roles in fracture healing through immunomodulation, osteogenic differentiat

Epigenetic regulatory networks in the pathogenesis and treatment of osteoporosis subtypes.

Osteoporosis is a systemic skeletal disorder characterized by reduced bone mineral density and heightened bone fragility, encompassing primary (senile and postmenopausal) and secondary (glucocorticoid-induced and diabetic) subtypes. Maintaining skeletal health relies on the dynamic balance between osteoblast-mediated bone formation and osteoclast-mediated bone resorption. Among the multitude of factors orchestrating this bone remodeling process, epigenetics, which regulates gene expression via reversible chemical modifications without altering the primary DNA sequence, plays an essential role
