Clinical, Biochemical and Radiological Features of LRP5 Gene Variants in Children.

Alterations in the low-density lipoprotein receptor-related protein 5 (LRP5) gene have been associated with primary osteoporosis, leading to recurrent low-trauma fractures. Heterozygous carriers typically show a milder phenotype, with reduced bone mass starting in early childhood. In this paper, we described the clinical features and therapeutic outcomes of a cohort of 7 children (5 males) harboring different variants in the LPR5 gene. Eight heterozygous variants of the LRP5 gene were identified (6 missense, 2 nonsense), two of which were likely pathogenic. One male patient was compound hetero

NLRP3 Inflammasome-Mediated Pyroptosis in Osteoporosis: Osteoimmune Mechanisms and Therapeutic Targeting.

Osteoporosis (OP) is a chronic, age-related skeletal disorder characterised by progressive bone loss and microstructural deterioration, which increases bone fragility and fracture risk. The NOD-like receptor family pyrin domain-containing 3 (NLRP3) inflammasome, a multi-subunit protein complex involved in bone homeostasis, mediates inflammatory cascades in response to external stimuli and pathological conditions. This process triggers pyroptosis in various bone-related cells, disrupting bone repair and remodelling. Oestrogen deficiency and aging lead to the overactivation of the NLRP3 inflamma

The use of bone-modifying agents in early breast cancer: AIOM Guidelines update and perspectives.

Breast cancer (BC) is the leading cause of cancer-related mortality among women, with early BC (EBC) comprising most cases. Advancements in neo(adjuvant) therapies have significantly improved outcomes, although they are often associated with cancer treatment-induced bone loss, which increases the risk of fractures and negatively impacts quality of life. Bone-modifying agents (BMAs), such as bisphosphonates and denosumab can mitigate this adverse effect. By reviewing and summarizing the most recent evidence published on BMAs use in EBC, an expert Italian Panel, composed of the authors of the It

Short versus long proximal femoral nail anti-rotation-II (PFNA-II) in the management of unstable intertrochanteric fractures.

OBJECTIVES: Unstable intertrochanteric (IT) fractures, particularly in elderly patients with low bone mineral density, pose significant treatment challenges. Proximal femoral nail anti-rotation-II (PFNA-II) is widely used, but the optimal implant length (short vs. long) remains debated. The objective of this study was to compare the clinical and functional outcomes of short versus long PFNA-II implants in unstable IT fractures. METHODS: A prospective comparative study was conducted at a tertiary hospital from November 2018 to November 2020. Adult patients (age ≥18) with recent (≤3 weeks) unst

Exosome-Based Regulation and Therapy in Osteoporosis: Mechanistic Insights and Translational Advances.

Osteoporosis is a chronic and progressive skeletal disease characterized by decreased bone mass and microarchitectural deterioration, leading to increased fracture susceptibility. Emerging evidence suggests that extracellular vesicles, particularly exosomes, play a pivotal role as mediators of intercellular communication within the bone microenvironment. This review focuses on the functional roles of exosomes derived from bone marrow mesenchymal stem cells (BMSCs) and other bone-resident cells in modulating bone remodeling. We detail the mechanisms by which exosome-encapsulated non-coding RNAs

Treatment of osteoporotic bone defects with carboxymethyl chitosan based biomimetic mineralized hydrogel loaded with icaritin.

Osteoporosis, a systemic bone disease marked by reduced bone mass and deteriorated microstructure, stems primarily from an imbalance between bone formation and resorption. Normally, bone remodeling maintains mass via osteoblast-mediated formation and osteoclast-mediated resorption. In osteoporosis, this balance is disrupted, causing low bone density, increased fragility, higher fracture risk, and pathological bone defects. Current treatments focus on inhibiting resorption (e.g., bisphosphonates, RANKL inhibitors) or promoting formation (e.g., PTH analogues). However, long-term anti-resorptive

Serum iron levels as a risk factor for Osteoporosis-Related pathological fractures.

BACKGROUND: Osteoporosis is a prevalent bone disease characterized by decreased bone mass and deterioration of bone microstructure, leading to an increased risk of fractures. Pathological fractures, particularly in postmenopausal women, are a severe complication of osteoporosis. This study investigates the potential molecular role of micronutrients, particularly iron, in osteoporosis and its association with pathological fractures. METHODS: We performed a Mendelian randomization (MR) analysis using genome-wide association study (GWAS) summary data to explore the relationship between 15 trace

Delayed Neurological Deficits Caused by Epidural Leakage of Intravertebral Contents in a Patient With Osteoporotic Vertebral Fracture.

Delayed neurological deficits (DND) after osteoporotic vertebral fractures (OVF) are a rare but serious complication. In patients with Parkinson's disease (PD), involuntary movements such as dyskinesia may increase biomechanical stress, potentially exacerbating spinal instability. We report the case of a 74-year-old woman with PD and a history of T12 OVF, who presented with progressive bilateral thigh pain and gait disturbance despite conservative treatment and bed rest. Imaging revealed a posterior epidural mass at T12 with an atypically high CT attenuation (300-340 hu), inconsistent with hem

Duration Matters: Anticonvulsant Therapy Linked to Bone Loss in Interim Cross-Sectional Study.

BACKGROUND: Anticonvulsants are widely used in treating patients with mental and neurological disorders. Their long-term use increases the risk of a decrease in bone mineral density (BMD) and low-energy fractures. Despite the growing number of studies of drug-induced osteoporosis, the effect of anticonvulsants on bone microarchitecture remains poorly studied. AIM: To study the effect of treatment duration with different generations of anticonvulsants on bone mineral density and fracture risk. METHODS: We examined 100 adult patients with epilepsy who had been on anticonvulsants for more than

Spontaneous proximal migration of Kuntscher nail: an uncommon complication.

The Kuntscher nail (K-nail) was initially designed with an axial section in the letter 'V' shape. Still, it was later reintroduced in a cloverleaf shape to achieve additional strength and easier use. Migration of the K-nail proximally or distally in the femur is a well-documented complication, with distal migration reported as one of the complications in the 1940s. However, spontaneous extrusion of the K-nail proximally is a rare complication. The common causes of the extrusion include disuse/senile osteoporosis, infection, inappropriate K-nail size, delayed union with shortening and premature

[Pathological spine fractures].

CLINICAL ISSUE: Pathological vertebral body fractures are nontraumatic fractures caused by metabolic diseases, tumor infiltration, inflammatory processes or other pathological changes. They often represent the first manifestation of an undiagnosed underlying condition. The clinical presentation is variable, most commonly involving acute back pain and, in some cases, neurological deficits up to paraplegia. Tumor-related fractures are often progressive and resistant to conservative treatment. STANDARD RADIOLOGICAL METHODS: Diagnostic evaluation is stepwise, beginning with X‑ray (basic assessmen

Due to the shortage of pediatric hospital beds in general and due to the large annual burden of hospital admissions for common respiratory tract infections, such as influenza or RSV [respiratory syncytial virus] in particular, it can be expected that hospitalization of children with suspected or confirmed SARS-CoV‑2 infections will face hospitals with an even more tense situation than usual in the winter 2020/2021. This tenuous shortage may touch various aspects but in the first place it will affect isolation and cohorting. In addition, this shortage will not only apply to acutely ill children

The impact of chronic obstructive pulmonary disease on bone strength.

Chronic obstructive pulmonary disease (COPD) is a lifestyle-related disease that develops in middle-aged and older adults, often due to smoking habits, and has been noted to cause bone fragility. COPD is a risk factor for osteoporosis and fragility fracture, and a high prevalence of osteoporosis and incidence of vertebral fractures have been shown in patients with COPD. Findings of lung tissue analysis in patients with COPD are primarily emphysema with a loss of alveolar septal walls, and the severity of pulmonary emphysema is negatively correlated with thoracic spine bone mineral density (BMD

Detection and evaluation of signals for immune-related adverse events: a nationwide, population-based study.

BACKGROUND: Immune checkpoint inhibitors (ICIs) are one of the main pillars of cancer therapy. Since other studies such as clinical trial and retrospective study have limitations for detecting the immune-related adverse events (irAEs) characterized by unpredictable onset, nonspecific symptoms and wide clinical spectrum, we aimed to identify the incidence of irAEs and to detect and evaluate the signals using real-world data. METHODS: Cancer patients treated with anticancer medications were analyzed using the nationwide health insurance claims database of South Korea from 2017 to 2019, and Clin

High performance injectable Mg doped bioactive glass bone cement for the regulation of osteogenic immune microenvironment.

Although calcium phosphate has been extensively utilized in orthopedic applications such as spine, limbs, dentistry, and maxillofacial surgery, the lack of osteoinductive properties often hinders its effectiveness in treating bone defects resulting from pathological micro-environment such as tumor surgery, osteoporosis, osteomyelitis, and diabetic. Therefore, a novel bone cement based on magnesium-doped bioactive glass was developed in this study. The moderate release of magnesium ions improved the mechanical properties by controlling the crystal size of hydroxyapatite. Through detailed discus

Pathological Thoracic Vertebral Fractures Secondary to Achromobacter denitrificans: A Case Report.

Vertebral fractures remain a diagnostic challenge nowadays. The first and most common diagnosis needed to be ruled out is osteoporosis. Other diagnoses to rule out involve pathological fractures. Pathological fractures are a group of pathologies that result in a spine fracture as part of an underlying disease process that affects the spine. This group includes Paget's disease, tumors, osteomyelitis, and vertebral compression fractures. Fractures secondary to vertebral osteomyelitis are presented as collapsed vertebral bodies secondary to bone destruction and the formation of lytic lesions. Cli
