Hydrogen reshapes the senescent microenvironment of callus to enhance the healing of anti-osteoporotic-drug-induced atypical femoral fracture.

Long-term bisphosphonates (BPs) are widely used to treat osteoporosis, however, they are paradoxically associated with the development of atypical femoral fractures (AFFs), which often characterized by impaired healing. In this study, we induced an AFF model using zoledronate (ZOL) administration in ovariectomized (OVX) osteoporotic rats, following a unilateral femoral fracture. Here we identified that a local pro-senescent microenvironment causes persistent inflammation and impairs effective regeneration in rat AFFs. Molecular hydrogen has demonstrated anti-senescence and anti-inflammatory pr

Discovery of Potential New Drug Targets for Osteoporosis of Various Subtypes With or Without Pathological Fracture Complications: A Drug Target Mendelian Randomization Study.

BACKGROUND: Osteoporosis, a prevalent metabolic bone disease, often leads to pathological fractures. Identifying causative relationships between proteins, genetic loci, and osteoporosis can highlight potential therapeutic targets. METHODS: This study employed Mendelian randomization (MR) and summary-data-based MR (SMR) to investigate the causal relationships between protein quantitative trait loci (pQTLs), expression quantitative trait loci (eQTLs), and various forms of osteoporosis, including drug-induced osteoporosis. Data from large-scale proteomic and genomic studies were used. Sensitivit

Duration Matters: Anticonvulsant Therapy Linked to Bone Loss in Interim Cross-Sectional Study.

BACKGROUND: Anticonvulsants are widely used in treating patients with mental and neurological disorders. Their long-term use increases the risk of a decrease in bone mineral density (BMD) and low-energy fractures. Despite the growing number of studies of drug-induced osteoporosis, the effect of anticonvulsants on bone microarchitecture remains poorly studied. AIM: To study the effect of treatment duration with different generations of anticonvulsants on bone mineral density and fracture risk. METHODS: We examined 100 adult patients with epilepsy who had been on anticonvulsants for more than

Hematological Profile in Co-Inherited β -Thalassemia Trait with IVS 1-5 (G→C) Mutation and α+-Thalassemia.

Alpha-thalassemia reported to be a genetic modifier in β-thalassemia characterized with hematological and clinical heterogeneity. This study was carried out to analyze the effect of α+-thalassemia (-α3.7 and -α4.2) on the hematological profile of β-thalassemia trait cases with IVS1-5 (G→C) mutation. In this cross-sectional study, cases advised to undergo screening hemoglobinopathies were investigated for complete blood count and hemoglobin fractions. Individuals with suspicion of β-thalassemia trait were subjected to analysis for β-thalassemia mutations and α+-thalassemia (-α3.7 and -α4.2). Ca

Bilateral sub-trochanteric femur fragility fractures in a patient on antiretroviral therapy: a case based discussion and review of literature.

BACKGROUND: The burden of HIV/AIDS is ever increasing, affecting populations all over the world. Along with the disease, its effects and consequences on bone health are also on a rise. A commonly used drug in managing HIV, "Tenofovir" has been shown to affect bone health, specifically on prolonged usage. Osteomalacia and osteoporosis secondary to drug induced Fanconi syndrome and the disease itself, could lead to pathological/fragility fractures with trivial trauma. CASE DESCRIPTION: A 45 year old female on antiretroviral drugs (ART) for HIV, presented with right hip pain and inability to bea

Drug-induced osteoporosis/osteomalacia: analysis in the French and Spanish pharmacovigilance databases.

INTRODUCTION: Osteomalacia and osteoporosis are two metabolic bone disorders that increase the risk of fracture due to several causes. In terms of drugs, apart from corticosteroids, which are known to induce bone disorders, several other drugs used in chronic disease management have also been linked with an increased risk of osteoporosis and osteomalacia. PURPOSE: The aim of this study was to describe spontaneous reports of drug-induced osteoporosis and osteomalacia in the French (FPVDB) and Spanish (SPVDB) pharmacovigilance databases. METHODS: Data were provided by the FPVDB and SPVDB. All

[Drug-induced osteonecrosis of the jaw - not just bisphosphonates].

Metastatic bone disease and osteoporosis have a large impact on quality of life and are associated with the development of skeletal-related events (SREs), such as fractures and spinal cord compression. Pharmacologic managing of metastatic bone disease and osteoporosis typically involves antiresorptive agents such as bisphosphonates and RANKL inhibitors. Undesired adverse effects resulting from the use of these drugs include osteonecrosis of the jaw (ONJ). Dentoalveolar surgery, particularly tooth extraction, appears to increase the risk of ONJ. However, spontaneous events were also documented.

Osteonecrosis of the jaw (ONJ) and atypical femoral fracture (AFF) in an osteoporotic patient chronically treated with bisphosphonates.

The aim of the study is to report the rare association of two complications of long-term treatment of osteoporosis with bisphosphonates in the same Caucasian elderly patient. A female patient of Italian descent, age 87 years, consulted in February 2013. She had a history of osteoporosis and had taken alendronate weekly for 7 years (1999-2006). Due to low back pain, an orthopedist had indicated i.v. zoledronic acid, 5 mg/year for 3 years (2006-2008). She received occasional supplements of ergocalciferol. In 2009, she suffered a fall and sustained a subtrochanteric fracture of the left femur. Sh

[Fracture risk associated with drugs other than glucocorticoids].

Many drugs influence fracture risk besides glucocorticoid, a most common cause of drug-induced osteoporosis. It is well established that drugs used for anti-sex steroids therapy for breast and prostate cancers increase fracture risk, including GnRH agonists, aromatase inhibitors, and anti-androgens. Increased fracture risk is also established with anti-diabetic thiazolidinediones by a randomized control study as well as meta analysis of observational studies. Multiple observational studies have indicated increased fracture risks with selective serotonin reuptake inhibitors and proton pump inhi

Disease modification of breast cancer-induced bone remodeling by cannabinoid 2 receptor agonists.

Most commonly originating from breast malignancies, metastatic bone cancer causes bone destruction and severe pain. Although novel chemotherapeutic agents have increased life expectancy, patients are experiencing higher incidences of fracture, pain, and drug-induced side effects; furthermore, recent findings suggest that patients are severely undertreated for their cancer pain. Strong analgesics, namely opiates, are first-line therapy in alleviating cancer-related pain despite the severe side effects, including enhanced bone destruction with sustained administration. Bone resorption is primari

[Drug-induced osteopathies. Drugs, pathogenesis, forms, diagnosis, prevention and therapy].

A number of drugs can have "side effects" on bone metabolism and formation, causing bone atrophy, impaired mineralisation, as well as osteonecrosis. In both clinical and general practitioner settings, these forms of bone damage have been hitherto considered as adverse drug side effects and have received insufficient attention; moreover, they have not been the subject of patient information. Preventive measures are not instigated prior to initiation of medication and even after onset of bone damage, therapeutic strategies are poorly implemented. Even fracture healing with its complex, staged co

A rare case of osteoporotic spine fracture associated with epidural lipomatosis causing paraplegia following long-term cortisone therapy.

Cushing's syndrome is frequently associated with osteoporosis. Therefore, the incidence of osteoporotic spine fractures is significant. They are a rare cause of paraplegic syndromes. Additionally, epidural lipomatosis may occur in those patients. The combination of both fracture and lipomatosis may cause neurological deficit. A case of a young patient suffering from drug-induced Cushing's syndrome is reported. She developed progressive paraplegia. Radiographs demonstrated kyphosis of the thoracic spine from T7 to T9 and pathologic fractures. Urgent operation was planned to stabilize and decomp
