OBJECTIVES: The aim of this study was to identify the differences in ultrasound bone variables (QUS) and to test the ability to discriminate male patients with and without vertebral fractures.
METHODS: We therefore measured broadband ultrasound attenuation (BUA) and speed of sound (SOS) matched for bone mineral density (BMD) and vertebral deformity in idiopathic male osteoporosis.
RESULTS: One hundred and seventeen men (age 56.6 range 27-78) were divided into three groups (osteoporosis n=25, osteopenia n=58 and age-matched control n=34) according to BMD T-score by WHO criteria. We found 66 p

We report a case of a spontaneous sternal fracture secondary to idiopathic osteoporosis in a man in his late forties. His restrictive lung disorder from thoracic kyphoscoliosis was exacerbated by surgery to repair the sternal fracture. Inpatient cardiopulmonary rehabilitation helped this patient regain independence in functional status and ambulation after repair of the fractured sternum.

Osteoporosis is increasingly being seen in young people. The primitive forms are relatively rare, but the secondary forms--particularly in long-term corticosteroid therapy--are a relevant problem given the much longer survival in chronic diseases such as cystic fibrosis, chronic renal insufficiency, leukaemia, and Duchenne muscular dystrophy. Controlled, prospective studies to evaluate the results of prevention and therapy in children are still lacking. The basis of therapy is the correct daily intake of calcium and the use of vitamin D (or active metabolites). This helps the growing skeleton

BACKGROUND AND PURPOSE: Vertebroplasty is now a widely accepted image-guided intervention in patients with compression fracture. The purpose of this report is to look specifically at the male population undergoing vertebroplasty and identfy the management considerations distinct to men.
METHODS: The hospital medical records and radiographic studies of 24 men treated with vertebroplasty between February 1999 and November 2000 were retrospectively reviewed.
RESULTS: Five patients presented with idiopathic or secondary osteoporosis. In two of these five men, malignancies were discovered by core

Male osteoporosis represents an important public health problem. According to NHANES III (National Health and Nutrition Examination Survey), 7% of white men aged 50 years and over have osteoporosis. One-third of all hip fractures occurred in men and prevalence of death after hip fracture is twice as high as in women. Hypogonadism, alcohol abuse and glucocorticoid excess are the most frequent causes of secondary osteoporosis in men, while in 30-50% of cases osteoporosis is idiopathic. In the article we discuss etiology, epidemiology, diagnostic procedure and treatment of osteoporosis in men.

With the development of improved diagnostic and treatment options, reduced bone mineral density in children is receiving increased attention. The etiology of osteopenia in healthy children is multifactorial and incompletely understood, but poor calcium intake during the adolescent growth spurt may be an important (and potentially reversible) factor. Other clinically relevant causes of reduced bone mineral density in children include osteogenesis imperfecta, rickets, juvenile rheumatoid and other chronic arthritides, osteopenia associated with neuromuscular disorders, and idiopathic osteoporosi

Decreased bone mass is a frequent finding in celiac patients, and subclinical celiac disease (CD) appears to be unusually overrepresented among patients with idiopathic osteoporosis. Since silent CD may be more common than previously believed, it has been suggested that all osteoporotic patients should be checked for occult CD. The aim of this study was to explore the prevalence of CD in a well-defined population of postmenopausal osteoporotic women. We evaluated 127 consecutive postmenopausal patients (mean age: 68 years; range: 50-82 years) with verified osteoporosis. The observed prevalence

The aim of this study is to evaluate the occurrence of idiopathic juvenile osteoporosis by clinical, radiographic and biochemical methods in 7 patients aged 10 years to 18 years. The prevailing clinical symptoms included pain in the lower limbs and spine, gait disturbances and weak muscles; confirmed fractures were seen in four out of seven patients. In all patients, bone mineral density was determined by dual-energy X-ray absorptiometry and calcium-phosphorus metabolism assays were performed. Clinical evaluation of our patients revealed a wide variety of classic clinical symptoms, and also ra

Osteoporosis in men is a severe condition probably more frequent than generally expected. The life-time risk of a low trauma fracture in a 60 year old man is 25%; moreover, approximately one third of hip fractures occurs in men. In contrast to postmenopausal osteoporosis, osteoporosis in men is frequently a secondary condition. In approximately 50% of cases, male osteoporosis is associated with an underlying disease or condition such as hypogonadism or long-term glucocorticoid treatment. Data from our group indicate that low serum estradiol levels and elevated sex hormone binding globulin leve

Osteoporosis in childhood is uncommon, and it may be secondary to a spectrum of diverse conditions. When such causes have been excluded, some patients remain who have a congenital disease (osteoporosis imperfecta) or a disease in which the etiology is obscure, called idiopathic juvenile osteoporosis (IJO). The cause of IJO is unknown, and the diagnosis is based both on the exclusion of other diseases and on its own positive fractures. The basic strategy of treatment is to protect the spine until remission occurs. Sex steroids are contraindicated. Bisphosphonates, calcitriol, fluoride, and calc

Although osteoporosis is generally regarded as a disease of women, up to 30% of hip fractures and 20% of vertebral fractures occur in men. Risk factors for osteoporotic fractures in men include low body mass index, smoking, high alcohol consumption, corticosteroid therapy, physical inactivity, diseases that predispose to low bone mass, and conditions increasing the risk of falls. The key drugs and diseases that definitely produce a decrease in bone mineral density (BMD) and/or an increase in fracture rate in men are long-term corticosteroid use, hypogonadism, alcoholism and transplantation. The prevalence of osteoporosis in men has been underestimated in the past. Vertebral fractures were found in about 10% in men of age 50 and over 20 to 30% of all hip fractures in advanced age occur in men. Lower life expectancy of men but also differences in bone geometry and remodeling contribute to the lower rate of fractures in comparison to the female gender. In men with suspected osteoporosis a thorough history, physical and clinical examination is mandatory to exclude other localized or generalized osteopathies and to differentiate in primary and secondary osteoporosis. Only some small s

DEFINITION: Idiopathic osteoporosis refers to the development of osteopenia and fractures with minimal or no trauma in otherwise young, healthy individuals who are not postmenopausal or have other, identifiable secondary causes of osteoporosis.
EPIDEMIOLOGY: It is a relatively rare disorder, with an incidence of 0.4 cases per 100,000 person-years. It appears to affect both sexes equally and results primarily in the development of trabecular bone fractures such as vertebral compression fractures and Colles' fractures, although hip fractures are also seen.
PATHOPHYSIOLOGY: The disease may be t

BACKGROUND: Osteoporosis in young or middle age men is unusual and requires an extensive diagnostic work-up.
AIM: To report a retrospective review of nine men with osteoporosis.
PATIENTS AND METHODS: The charts of nine men aged 27 to 61 years old (mean 39), that presented with a primary diagnosis of osteoporosis, were reviewed.
RESULTS: Subjects were subjected to a diagnostic work up five years after the onset of symptoms. Their body mass index ranged from 21.7 to 26.3 kg/m2, all had vertebral fractures (crush fractures in 8 and a biconcave deformity in one) between T4 and L4 and all had no

PURPOSE: Fractures, a common complication of cardiac and liver transplantation, have not been reported in association with lung transplantation. However, many patients with end-stage pulmonary disease have multiple risk factors for osteoporosis, and several studies have suggested that osteoporosis before transplantation may increase the risk of fracture after transplantation. Therefore, we evaluated a group of patients with end-stage pulmonary disease who were awaiting lung transplantation to determine the prevalence of osteoporosis.
METHODS: Seventy patients (aged 18-70 years) were evaluated

The features and outcome of 21 children (12 boys, nine girls) with idopathic juvenile osteoporosis (IJO) followed for up to 23 yr are described. The mean age of onset was 7 yr (range 1-13 yr) with no sex difference; the main presenting symptoms were long bone fractures, pain in the back and difficulty in walking. Typically, radiographs demonstrated compression of the vertebrae and metaphyses of the long bones; bone histology sometimes showed an excess of osteocytes associated with woven bone; routine biochemistry was normal for age; and with three exceptions no abnormalities of extracted derma

We report a case of a spontaneous humeral shaft fracture sustained during pitching by a 22-year-old previously healthy female European team handball player. The fracture was treated by closed reposition followed by immobilization in a hanging cast. The fracture healed uneventfully, and normal function and range of motion were reached within 6 months. Between 1 and 2 years after the fracture, bone mineral measurements showed a decreased bone mass (1.43-2.56 SD below mean values of normal women) at all measuring sites; proximal tibia bilaterally (bone mineral content (right) = 2.68 g/cm, bone mi

Juvenile osteoporosis is a disease of childhood and adolescence exhibiting marked clinical variability in bone change. The disease usually begins in the prepubertal period or even younger. A case of a Chinese boy suffering from back pain and generalized osteoporosis since 9 yr of age who spontaneously recovered at 14 yr of age when entering puberty is reported. The affected patient was clinically normal (as shown by X rays), until he fractured his left humerus. Metabolic studies indicated a negative calcium balance, but serum calcium, phosphorus, alkaline phosphatase, parathyroid hormone, calc
