Musculoskeletal Manifestations of Alkaptonuria: A Case-Based Review of Literature.

Alkaptonuria is a rare metabolic disorder caused by a mutation in the homogentisate 1,2 dioxygenase (HGD) gene, which encodes the HGD enzyme. It is inherited in an autosomal recessive pattern. This leads to the accumulation of homogentisic acid (HGA) and benzoquinone acetic acid (BQA), which is an oxidised product of HGA. Both these compounds form polymerised deposits, which cause bluish black discolouration of the cartilage. They also cause inflammation, degeneration and calcification of ligaments, large joints, tendons and intervertebral discs. A fragmented cartilage forms, which attaches to

Atypical Femoral Fracture in a Patient on Imatinib: A Case Report.

CASE: A 74-year-old woman with chronic myeloid leukemia on long-term imatinib therapy and a history of bisphosphonate use presented with an atypical femoral fracture after low-energy trauma. She underwent staged intramedullary nailing of both femurs. CONCLUSION: This case highlights the potential synergistic effect of bisphosphonates and imatinib on bone metabolism, leading to an increased risk of atypical femoral fractures. Vigilant monitoring of bone health in such patients is crucial for early detection and prevention.

Sympathetic cholinergic nerve fibres promote BMSC differentiation into osteoblasts via NRTN secretion: a potential treatment for osteoporosis.

BACKGROUND: Peripheral nerves within the bone actively participate in bone remodelling by secreting various bioactive molecules, and dysfunction or changes in their abundance greatly contribute to the onset of osteoporosis. While it is well established that sympathetic adrenergic nerve fibres reduce bone mass and contribute to osteoporosis, the role of sympathetic cholinergic signalling in this condition is still poorly understood. We aimed to investigate whether sympathetic cholinergic nerve fibres influence osteoporosis through the secretion of neurturin (NRTN). METHODS: A mouse model of ost

Pathophysiology and evaluation of bone health in adrenal diseases.

Adrenal diseases are associated with an increased risk of osteopenia/osteoporosis and fragility fractures. Although dual-energy X-ray absorptiometry remains the gold standard for assessing bone mineral density, its limitations in accurately predicting vertebral fractures underscore the need for complementary diagnostic tools, particularly in the context of adrenal disorders. Vertebral fractures, often asymptomatic, may go undetected without targeted assessments such as morphometric evaluation. Incorporating advanced diagnostic tools, such as the trabecular bone score, into the evaluation of ad

Revisiting the Incidence of Hypercalcemia in Contemporary Diagnoses of Pediatric Patients With Solid Tumors.

BACKGROUND: Hypercalcemia is rarely observed in pediatric patients with cancer. However, rare cases of renal insufficiency and osteoporosis have been reported as severe consequences. Published literature focuses predominantly on hypercalcemia in pediatric hematologic malignancies, precluding causative associations with solid tumor disease histology or tumor characteristics. METHODS: In this retrospective cohort analysis, we identified 39 patients (aged < 21 years) diagnosed and hospitalized with a solid tumor and prolonged hypercalcemia over the span of 25 years at the Dana-Farber/Boston Chil

Maternal gut microbiota-derived daidzein prevents osteoporosis in female offspring following prenatal prednisone exposure.

Prenatal exposure to glucocorticoids is linked to long-term health risks in offspring, but the role of maternal gut microbiota in mediating these effects remains unclear. Here, we demonstrate that prenatal prednisone therapy (PPT) in humans and prenatal prednisone exposure (PPE) in rats result in sex-specific long bone dysplasia in offspring, including reduced peak bone mass (PBM) and heightened osteoporosis risk in female offspring. Multi-omics profiling and fecal microbiota transplantation show that PPE alters maternal gut microbiota composition and depletes the microbial metabolite daidzein

Epidemiological and Clinical Profiles with Prognostic Implications: A Five-Year Retrospective Study of Head and Neck Cancer in Visakhapatnam, India - A Single Center Study.

BACKGROUND: In 1987, a report from this unit described the changing indications for open adrenalectomy over a 15-year period. The indications for adrenalectomy had switched from it being the principal therapeutic procedure used in advanced breast cancer in the early 1970s, to being predominately performed for Cushing's disease or incidental, asymptomatic, adrenal masses by the early 1980s. The aim of the present study was to evaluate the changes in the presentation and management of adrenal disease in the last 15 years and to compare these findings with our previously published results. Depot medroxyprogesterone acetate (DMPA)-associated early-onset osteoporotic fracture.

OBJECTIVE: Thyroid nodule (TN) is usually managed according to Thyroid Imaging And Reporting Data Systems (TIRADS) with the major aim to reduce as much as possible unnecessary fine-needle aspiration cytologies (UN-FNACs). Since the assessment of autonomously functioning thyroid nodule (AFTN) according to TIRADS is heterogeneous, that virtually benign entity may increase the rate of UN-FNAC. This study retrospectively analyzed the appropriateness of TIRADS-based FNAC indication in AFTNs, also looking at the impact of TSH and nodule size. METHODS: Cases diagnosed with AFTN on scintigraphy were

Effective Therapeutic Strategies to Prevent Frailty and Falls in Community-Dwelling Older Adults.

Frailty and the consequent risk of falls represent significant challenges for community-dwelling older adults, often leading to severe injuries, functional decline, and loss of independence. Falls typically result from multiple interacting risk factors, many of which are modifiable through targeted interventions. This mini-review focuses on evidence from randomized controlled trials evaluating effective therapeutic strategies to prevent frailty and falls. Comprehensive assessment and management of modifiable risk factors have been shown to significantly reduce fall incidence. Key interventions

Amlodipine and frusemide: pharmacological factors contributing to increased fall risk in concurrently treated patients - a retrospective cross-sectional study.

BACKGROUND: Calcium channel blockers, such as amlodipine, are commonly prescribed for hypertension but can cause peripheral edema, often requiring adjunctive frusemide. Concerns exist regarding the potential increase in fall risk, particularly in older populations. However, few studies have assessed the prevalence of amlodipine and frusemide combination (AFC) prescriptions and their association with fall risk factors. OBJECTIVES: The aims of this study are to determine the prevalence of AFC prescriptions and evaluate their association with fall risk factors in an outpatient cardiology clinic

Impact of Medications for Stable Angina Pectoris on Osteoporosis: A Review of Current Evidence.

Stable angina pectoris (SAP) and osteoporosis (OP) are both prevalent conditions among the elderly population. Compared to SAP, the prevention and management of OP are often neglected. Furthermore, certain medications used long-term for SAP may exert significant effects on bone metabolism. This review summarizes the impact of commonly prescribed SAP medications on OP. Extensive research indicates that nitrates not only promote vascular and osteogenic coupling via the nitric oxide (NO)-cyclic guanosine monophosphate (cGMP)-protein kinase G (PKG) pathway, enhancing the osteogenic effects of estr

[Pathological spine fractures].

CLINICAL ISSUE: Pathological vertebral body fractures are nontraumatic fractures caused by metabolic diseases, tumor infiltration, inflammatory processes or other pathological changes. They often represent the first manifestation of an undiagnosed underlying condition. The clinical presentation is variable, most commonly involving acute back pain and, in some cases, neurological deficits up to paraplegia. Tumor-related fractures are often progressive and resistant to conservative treatment. STANDARD RADIOLOGICAL METHODS: Diagnostic evaluation is stepwise, beginning with X‑ray (basic assessmen

Integrating exercise and medication management in geriatric care: a holistic strategy to enhance health outcomes and reduce polypharmacy.

Integrating exercise prescriptions with medication management represents a novel approach for enhancing health and function, optimising medication effectiveness, and reducing adverse drug reactions and polypharmacy in older adults (ie, those aged ≥60 years). This Personal View highlights the need for a comprehensive assessment of lifestyle, diagnoses, geriatric syndromes, and medications with an emphasis on fully incorporating exercise treatment into geriatric care. Exercise is an alternative to less effective or unsafe medications for many conditions, including depression, anxiety, insomnia,

Early-Onset Degenerative Bone Changes as a Manifestation of Alkaptonuria: A Case Report.

Alkaptonuria is a rare inherited metabolic disorder characterized by progressive pigment deposition in connective tissues, leading to early degenerative joint disease and tendon fragility. Although uncommon, its clinical manifestations may closely resemble more prevalent musculoskeletal conditions, often resulting in delayed diagnosis. We present the case of a 53-year-old woman with long-standing, disabling osteoarticular complaints initially attributed to common degenerative disease, but markedly premature for her age. Due to worsening bilateral gonarthrosis, she underwent left total knee art

Different effects of tumor necrosis factor monoclonal antibody and receptor fusion protein on bone metabolism.

BACKGROUND: Tumor Necrosis Factor-alpha (TNF-α) plays a crucial role in bone metabolism. It exists in two forms: soluble TNF-α (sTNF-α) and membrane-bound TNF-α (tmTNF-α). While sTNF-α primarily acts on the TNFRI signaling pathway, tmTNF-α can affect both TNFRI and TNFRII pathways. TNF inhibitors are divided into two categories: TNF-α receptor fusion proteins (such as etanercept) and TNF-α monoclonal antibodies (such as adalimumab). These inhibitors differ in their blocking effects on tmTNF-α and sTNF-α. Etanercept mainly blocks sTNF-α, while adalimumab blocks both tmTNF-α and sTNF-α. Since di
