Cerasus humilis proanthocyanidins alleviate osteoporosis by regulating renal calcium metabolism.

BACKGROUND: Postmenopausal osteoporosis is associated with disturbed calcium homeostasis and renal calcium handling. Although Cerasus humilis proanthocyanidins (PA) show anti-osteoporotic activity, their effects on renal calcium metabolism remain unclear. We hypothesized that C. humilis PA could alleviate ovariectomy (OVX)-induced bone loss by improving renal calcium reabsorption. RESULTS: Compared with untreated OVX rats, 12 weeks of PA treatment increased bone mineral density from 125.5 to 232.8 mg cm-3 and improved trabecular microarchitecture (P < 0.05). PA reduced β-C-terminal telopeptid

Denosumab Versus Zoledronic Acid for Muscle Strength and Physical Performance in Postmenopausal Osteoporosis: A 3-Year Prospective Study.

BACKGROUND: Osteoporosis and sarcopenia frequently coexist in older adults, yet whether denosumab provides muscle-related benefits beyond bone remains uncertain. We compared denosumab and zoledronic acid across functional, imaging, biologic and study-defined sarcopenia outcomes based on an Asian Working Group for Sarcopenia (AWGS) 2025-informed framework and examined whether early osteoprotegerin (OPG)/soluble receptor activator of nuclear factor-kappa B ligand (sRANKL) responses tracked with later benefit. METHODS: This prospective observational cohort study included 186 postmenopausal women

Managing Perimenopausal Bone Loss: The Role of Bisphosphonate Treatment.

Preventing the rapid loss of bone density and structure that occurs during the perimenopause is a unique opportunity to prevent the development of postmenopausal osteoporosis and high fracture risk. For women who are not candidates for estrogen therapy, low-dose bisphosphonate therapy for 3 to 5 years can prevent perimenopausal bone loss, after which intermittent bisphosphonate therapy can maintain skeletal health indefinitely. Identifying women at high risk for developing osteoporosis with bone density testing as they approach or enter perimenopause and considering prevention therapy should b

Clinical investigation of effects of eldecalcitol on lipid profile and other metabolic parameters in osteoporosis-treatment-naïve postmenopausal women with osteoporosis.

Osteoporosis is characterized by decreased bone mass and disruption of bone architecture. Eldecalcitol is the newest analog of activated vitamin D that can improve osteoporosis. In addition, dietary/activated vitamin D can improve metabolic parameters. Clinical studies have shown that activated vitamin D analogs improve glucose tolerance and hepatic function. However, the effects of activated vitamin D analogs, including eldecalcitol, on lipid parameters are yet to be clinically investigated. We aimed to reveal the effects of eldecalcitol on lipids and other metabolic parameters for 24 weeks i

Prevention and treatment of osteoporosis in postmenopausal women.

Postmenopausal osteoporosis is a major public health concern characterised by rapid loss of bone mineral density due to estrogen deficiency, leading to increased risk of fragility fractures. This article reviews current clinical strategies for the prevention and treatment of the condition in postmenopausal women. Primary prevention discusses lifestyle modification, including adequate dietary intake of calcium and vitamin D, regular weight-bearing exercise and cessation of smoking and excess alcohol consumption. For women at elevated fracture risk, pharmacological interventions are essential. T

GPX4-Mediated Inhibition of Ferroptosis by Sodium Selenite Ameliorates Bone Metabolic Disorders in Ovariectomized Mice.

Postmenopausal osteoporosis (PMOP) and its associated fracture complications have emerged as pressing public health challenges due to rapidly accelerating global population ageing. The functional decline of bone marrow mesenchymal stem cells (BMSCs) under elevated oxidative stress is a recognised pathogenic cornerstone of PMOP, positioning redox imbalance as a critical therapeutic node. Here, we establish that sodium selenite (Na2SeO3) robustly counters oxidative insults in BMSCs, thereby restoring and enhancing their osteogenic lineage commitment. Furthermore, RNA sequencing analysis indicate

Time-matched Secondary Fracture Prevention in Older Adults: A Narrative Review of Imminent Risk, Time-to-Benefit, and Treatment Sequencing.

PURPOSE: After a fragility fracture, subsequent fracture risk is highest during the first 6 to 12 months and remains elevated for 1 to 2 years. Anti-osteoporosis therapies differ in onset, persistence, discontinuation kinetics, and safety constraints, yet guidelines rarely operationalize treatment timing within this imminent-risk period. This narrative review examines temporal mismatch between post-fracture risk and treatment effect and proposes a time-matched secondary fracture prevention framework. METHODS: We conducted an argument-driven narrative review using a structured, non-systematic

Baseline characteristics and bone mineral density changes associated with lower fracture risk in osteoporosis: a retrospective cohort study.

Late diagnosis of osteoporosis with already existing low-energy fractures, often incorrect reassessment of fracture risk and suboptimal therapy is associated with significant unmet needs in high-risk patients. Predictors for fracture risk assessment are the basis of the long-term strategy for monitoring, stratifying patients and treating osteoporosis. To evaluate whether changes in total bone mineral density (BMD) of the lumbar spine and BMD of the femoral neck over 36 months were associated with transition to a lower estimated fracture risk category after adjustment for baseline patient chara

Gut microbiota-mediated Th17/Treg regulation in osteoporotic fracture: from postmenopausal dysbiosis to callus repair.

Osteoporotic fractures are traditionally regarded as severe clinical consequences of low bone mass and impaired bone strength, but fracture risk and repair outcomes are also shaped by systemic inflammation, intestinal barrier integrity, microbial metabolism, and osteoimmune regulation. Increasing evidence indicates that gut dysbiosis associated with postmenopausal osteoporosis is accompanied by increased lipopolysaccharide exposure, altered microbial metabolites, enhanced pro-inflammatory cytokine signaling, and dysregulation of T helper 17 and regulatory T cells. These changes may promote bon

Gut Microbiota-Derived Short-Chain Fatty Acids Mediate the Bone-Protective Effects of Metformin Against Postmenopausal Osteoporosis.

BACKGROUND: Dysbiosis-induced inflammation has been implicated in postmenopausal osteoporosis (PMOP). Although metformin has shown benefits for bone health, the exact role of the gut microbiota in the mechanism underlying this relationship remains unclear. HYPOTHESIS/PURPOSE: We hypothesised that metformin could alleviate systemic inflammation in PMOP by augmenting the synthesis of short-chain fatty acids (SCFAs) by gut microbes. METHODS: Micro-computed tomography (μCT) was used to analyse the femurs. Faecal samples were collected for 16S rRNA sequencing and SCFAs detection. Quantitative rea

A hypoxia-responsive calixarene supramolecular system enables redox-regulated matrix remodeling for postmenopausal osteoporosis.

Postmenopausal osteoporosis (PMOP) is characterized by progressive bone loss driven by excessive osteoclast activity, impaired osteogenesis, and maladaptive remodeling of the bone microenvironment. Although raloxifene (RLX) is clinically approved for PMOP, its therapeutic potential is constrained by limited bone selectivity and systemic adverse effects. Here, we report a hypoxia-responsive, bone-targeted supramolecular system (RLX@PnAC4A) that integrates site-selective drug delivery with redox regulation to enable coordinated matrix remodeling within the osteoporotic niche. PnAC4A was engineer

Predictors of the response to romosozumab in women with postmenopausal osteoporosis previously treated with denosumab.

Romosozumab is an antisclerostin antibody that increases bone formation and decreases bone resorption. This study aimed to identify predictors of the response to romosozumab in women with postmenopausal osteoporosis previously treated with denosumab. The study had an observational pre-post design and included 167 women with primary osteoporosis. Romosozumab 210 mg was administered subcutaneously every 4 weeks for 12 months in 87 patients who had been treated with denosumab (DMAb group) and 80 who had not (naïve group). The safety, incidence of new fractures, and changes in bone mineral density

Hyodeoxycholic acid rejuvenates BMSCs against postmenopausal osteoporosis by attenuating senescence and aberrant adipogenesis via TGR5-ESR1-mediated mitophagy.

INTRODUCTION: Postmenopausal osteoporosis (PMOP) is driven by bone marrow stromal cell (BMSC) senescence and aberrant adipogenesis, with 17β-estradiol (E2) deficiency as a primary trigger. However, the metabolic mediators linking systemic E2 loss to BMSC dysfunction remain undefined. Bile acids are established endogenous "metabolic rejuvenators," but the functional relevance of hyodeoxycholic acid (HDCA) to BMSC dysfunction and the development of PMOP has not been elucidated. OBJECTIVES: This study aimed to determine the role of HDCA in the metabolic regulation of BMSC homeostasis and to char

A Novel Reactive Oxygen Species-Responsive Bone-Targeted Nanoplatform Based on Cloperastine Attenuates Ovariectomy-Induced Bone Loss by Inhibiting Osteoclast Oxidative Phosphorylation.

Postmenopausal osteoporosis is caused by excessive osteoclast-mediated bone resorption. This study identifies mitochondrial oxidative phosphorylation (OXPHOS) as a tractable metabolic target for antiresorptive therapy. A literature-guided drug repurposing screen focused on compounds reported to affect OXPHOS or related energy metabolism identified cloperastine (Clo) as a potent suppressor of receptor activator of nuclear factor κB ligand-induced osteoclastogenesis. Clo reduces mitochondrial respiration, downregulates OXPHOS complex proteins, decreases mitochondrial reactive oxygen species (ROS

Peptide‑calcium chelates from microalgae enhance calcium bioavailability and regulate osteogenic and osteoclastogenic signaling in ovariectomized mice.

Estrogen deficiency-induced postmenopausal osteoporosis (PMOP) poses a global health burden requiring high-bioavailability nutritional therapies, while conventional calcium supplements exhibit poor absorption and gastrointestinal irritation; peptide‑calcium chelates stand out for superior uptake efficiency. In this study, we investigated the osteoprotective effects and molecular mechanisms of two microalgae-derived peptide-calcium chelates, specifically Chlorella pyrenoidosa peptide-calcium chelate (CP-Ca) and Arthrospira platensis peptide-calcium chelate (AP-Ca), using an ovariectomized (OVX)

Hydroxygenkwanin Identified in Rehmanniae Radix Praeparata-Corni Fructus Ameliorates Postmenopausal Osteoporosis via Modulation of the AGE-RAGE Signaling Pathway.

BACKGROUND: Osteoporosis (OP) is a metabolic bone disease marked by reduced bone mass and compromised microarchitecture, increasing fragility fracture risk. The traditional Chinese herbal pair Rehmanniae Radix Praeparata-Corni Fructus (RRP-CF) is frequently used for OP, but its mechanism remains unclear. METHODS: Network pharmacology, UHPLC-HRMS, molecular docking, and molecular dynamics simulations were integrated to identify the potential active components of RRP-CF and to predict the potential targets and related signaling pathways underlying its anti-osteoporotic effects. These prediction
