In a group of 11 men ranging in age from 35 to 50 years with idiopathic osteoporosis, most were mild alcoholics and heavy smokers. Two had absorptive hypercalciuria. Histomorphometry showed that the patients had low trabecular bone volume and mean trabecular thickness when compared with age-matched control subjects. Mean wall thickness was also markedly reduced in patients as compared with control subjects. The quantity of resorbed bone was extrapolated from the calculated mean interstitial bone thickness. The mean wall thickness (MWT) and duration of formation periods (sigma f) of trabecular bone packets have been measured in iliac crest biopsies following double tetracycline labeling from 9 women having primary osteoporosis, with vertebral crush fractures and reduced trabecular bone volume (TBV), and 9 age- and sex-matched controls. We describe four children with idiopathic juvenile osteoporosis. All patients were initially seen between the ages of 10 and 13 years and spontaneously recovered following puberty. We review 27 similar cases reported in the literature. Theories on the cause of idiopathic osteoporosis in children are critically discussed. It may be that milder forms remain undiagnosed because of the self-limited course and the pain being confused with a variety of rheumatic disorders. It would be worth observing these cases to determine if they are otherwise prone to development of osteoporosis during pregnancy

Every form of osteoporosis--except the physiological involution osteoporosis--requires treatment. Osteoporosis is no disease by itself but a result or a concomitant phenomenon of various diseases. Two premenopausal women (aged 40 and 34 years) and multiple undisplaced, often asymptomatic fractures of the femurs, ribs, metatarsals and other bones. The fractures, which appeared on roentgenograms as transverse radiolucent zones with variable callus formation, healed slowly or not at all despite treatment with calcium and vitamin D. Osteoporosis is defined as 'too little normal bone', the disorder being rarer in children than adults. The varied forms in childhood can be classified as those secondary to some other disease and primary forms of the disorder which include the genetically determined osteogenesis imperfecta types and idiopathic forms of osteoporosis. Loss of architecture within the femoral neck due to ageing follows a distinct pattern. This may serve as an index of physiologic ageing of bone. In pathologic osteoporosis (e.g. Cushing's disease, thyreotoxicosis and idiopathic osteoporosis) this systematic reduction of trabecular structure cannot be seen in the femoral neck although the vertebral bodies show all the signs of severe osteoporosis. Radiographic comparison between the proximal end of femur and the spine may, therefore, serve as a useful means of differentiation between pathologic and age-osteoporosis.

Case report of a 18 year old boy with short stature, microceophaly, mental retardation and multiple dysmorphic signs. At the age of 9 years a severe generalised osteoporosis was discovered.
