Age- and sex-specific values of regional bone mineral density from opportunistic CT of the lower lumbar spine, pelvis, sacrum, and proximal femur.

OBJECTIVE: Standard osteoporosis assessment using dual-energy X-ray absorptiometry (DXA) provides limited 2D measures (BMD, T-score), potentially missing localized bone density differences relevant to fracture risk. Opportunistic CT allows comprehensive 3D volumetric bone density analysis. This study presents a detailed 3D statistical model of CT-derived apparent volumetric bone mineral density (CT-vBMD) covering the L4 and L5 vertebrae, pelvic bones, sacrum, and proximal femur and examines spatial distributions and associations with age and sex. MATERIALS AND METHODS: Retrospective analysis

Bone-Targeting Antibodies for Skeletal Precision Therapy.

Therapeutic antibodies have transformed the treatment of cancer, inflammation, infection, and disorders of bone biology by recognizing disease-associated molecular targets with high specificity. However, achieving therapeutically effective antibody concentrations at the site of disease remains a major challenge, as systemic administration does not inherently result in efficient tissue-specific delivery. Bone is a particularly demanding destination because mineralized matrix, limited vascular access, marrow complexity, and active remodeling can restrict local antibody exposure. Recent advances

High genetic predisposition and vertebral fracture burden in pregnancy and lactation-associated osteoporosis: clinical insights from a Taiwanese cohort.

BACKGROUND: Pregnancy and lactation-associated osteoporosis (PLO) is a rare disorder causing rapid bone loss and severe fragility fractures. While familial clusterings suggest genetic susceptibility, the specific clinical features, genetic landscape, and skeletal recovery trajectory of PLO remain poorly characterized in non-Caucasian populations. This study aimed to evaluate a Taiwanese PLO cohort to optimize diagnostic awareness. METHODS: Eleven postpartum women with PLO were evaluated using bone densitometry and spinal imaging. Whole-exome sequencing was performed utilizing a targeted 398-g

Time-matched Secondary Fracture Prevention in Older Adults: A Narrative Review of Imminent Risk, Time-to-Benefit, and Treatment Sequencing.

PURPOSE: After a fragility fracture, subsequent fracture risk is highest during the first 6 to 12 months and remains elevated for 1 to 2 years. Anti-osteoporosis therapies differ in onset, persistence, discontinuation kinetics, and safety constraints, yet guidelines rarely operationalize treatment timing within this imminent-risk period. This narrative review examines temporal mismatch between post-fracture risk and treatment effect and proposes a time-matched secondary fracture prevention framework. METHODS: We conducted an argument-driven narrative review using a structured, non-systematic

Recommendations for the Diagnosis and Treatment of Genitourinary Syndrome of Menopause from the Italian Society of Menopause (SIM) and Italian Society of Gynecology of the Third Age (SIGiTE).

Genitourinary syndrome of menopause (GSM) is a chronic condition associated with estrogen and androgen deficiency that affects the vulva, vagina, urethra, and bladder, significantly impairing quality of life, sexual wellbeing, and urinary function. Despite its high prevalence, GSM remains frequently underdiagnosed and undertreated. These recommendations, developed by the Italian Society of Menopause (SIM) and the Italian Society of Gynecology of the Third Age (SIGiTE), provide updated evidence-based guidance for the diagnosis and management of GSM. The recommendations were formulated through a

Diffuse Idiopathic Skeletal Hyperostosis (DISH): Potential Links Between Gut Microbiota and Spinal Degenerative Diseases.

Diffuse Idiopathic Skeletal Hyperostosis (DISH) is a systemic, non-inflammatory skeletal disorder characterized by progressive calcification and ossification of spinal ligaments and entheses. Its strong associations with aging, obesity, type 2 diabetes, and metabolic syndrome raise the possibility that metabolic and inflammatory pathways may contribute to disease development. The gut microbiota participates in nutrient metabolism, short-chain fatty acid production, bile-acid transformation, intestinal-barrier maintenance, and immune regulation; these functions provide a plausible framework for

An integrated deep learning and large language model for automated knee osteoarthritis grading and reporting from the Vietnam Osteoporosis Study.

OBJECTIVES: Knee osteoarthritis (KOA) is a prevalent joint condition affecting multiple structures of the knee. Clinicians need reliable tools for early detection and accurate grading to support timely management, but radiographic grading remains subjective and time-consuming. We aimed to develop a deep learning and large language model (LLM) hybrid system for KOA severity classification and automated clinical reporting. METHODS: We analysed bilateral knee radiographs from 2435 adults (≥40 years) in the Vietnam Osteoporosis Study (VOS), using the Kellgren-Lawrence (KL) scale. Data were split

Gut microbiota-mediated Th17/Treg regulation in osteoporotic fracture: from postmenopausal dysbiosis to callus repair.

Osteoporotic fractures are traditionally regarded as severe clinical consequences of low bone mass and impaired bone strength, but fracture risk and repair outcomes are also shaped by systemic inflammation, intestinal barrier integrity, microbial metabolism, and osteoimmune regulation. Increasing evidence indicates that gut dysbiosis associated with postmenopausal osteoporosis is accompanied by increased lipopolysaccharide exposure, altered microbial metabolites, enhanced pro-inflammatory cytokine signaling, and dysregulation of T helper 17 and regulatory T cells. These changes may promote bon

Jiawei Duhuo Jisheng Mixture Attenuates H2O2-induced Oxidative Injury and Osteogenic Dysfunction in Primary Osteoblasts: An Integrated Target Analysis and Experimental Validation.

INTRODUCTION: Oxidative stress-induced osteoblast dysfunction is an important event in osteoporosis (OP). Jiawei Duhuo Jisheng Mixture (JDJM) is clinically used for bone-related disorders, but its molecular basis in OP remains unclear. This study explored OP-related candidate targets of JDJM and evaluated its cytoprotective effects in a hydrogen peroxide (H2O2)-induced primary osteoblast injury model. METHODS: Candidate JDJM targets were predicted from its identified constituents and intersected with OP-related genes. The study integrated network-based and enrichment analyses with single-cell

Research progress on the mechanisms by which gut microbiota dysbiosis remodels the bone marrow neuroimmune microenvironment to drive age-related osteoporosis.

Age-related osteoporosis arises from the interplay between impaired bone remodeling, immune remodeling, and chronic low-grade inflammation during aging. Beyond local degeneration in bone tissue, alterations in the gut microbiota, mucosal barrier, and autonomic nervous regulation may reshape the bone marrow immune environment and amplify osteoclastogenic responses. This review integrates evidence linking the gut, neural regulation, and the bone marrow immune microenvironment. It summarizes their potential contributions to age-related bone loss and provides a conceptual framework for understandi

Micro-CT-derived nonlinear micro-finite element estimation of compressive fracture load in murine vertebrae: a pilot feasibility study in sclerostin antibody-treated and control mice.

Fragility fractures caused by osteoporosis remain a growing health burden, highlighting the need for reliable, non-destructive methods to assess bone strength. We aimed to develop and assess the feasibility of a micro-computed tomography (µCT)-based nonlinear micro-finite element analysis (µFEA) workflow for estimating compressive fracture load in murine vertebrae. Fourth lumbar vertebral bodies from untreated and sclerostin antibody-treated (Scl-Ab) female C57BL/6 mice were imaged ex vivo using µCT and subjected to destructive axial compression testing. µFEA models incorporated heterogeneous

[Osteoporosis in rheumatology-Diagnostics, risk assessment and modern treatment strategies].

Inflammatory rheumatic diseases such as rheumatoid arthritis (RA) lead to local periarticular demineralization through localized arthritis in one or more joints; however, the autoimmune-mediated systemic inflammatory process is also a driver of systemic bone loss, osteoporosis, which leads to an increased risk of fractures. The Umbrella Organization Osteology (DVO) guidelines 2023 recommend a risk-adapted approach involving systematic fracture risk assessment, early baseline diagnostics and consistent specific osteoporosis treatment, including antiresorptive and osteoanabolic strategies. Dexamethasone-mediated modulation of the renin-angiotensin system: current insights.

Dexamethasone (DEX) exerts pleiotropic effects that extend beyond classical glucocorticoid receptor signaling and include interactions with the renin-angiotensin system (RAS). Growing evidence indicates that DEX regulates both systemic and tissue-specific RAS activity through genomic and non-genomic mechanisms, affecting the expression and activity of key components, including angiotensin-converting enzyme (ACE), angiotensin-converting enzyme 2 (ACE2), and angiotensin receptors AT1 and AT2. This mini-review summarizes current evidence on DEX-induced RAS modulation and its physiological and pat

Osteoporosis risk in patients with primary open-angle glaucoma using muscarinic and prostaglandin agonists: A retrospective cohort study.

BackgroundGlaucoma is the leading cause of irreversible blindness worldwide, with primary open-angle glaucoma (POAG) arising as the most common subtype. Recent studies have shown that muscarinic and prostaglandin pathways can modulate bone remodeling under systemic, context-dependent conditions. M3 muscarinic receptor activation has been linked to increased bone formation and decreased resorption, whereas continuous prostaglandin E2 (PGE2) administration inhibits bone resorption, with intermittent use inducing bone formation. Given the minimal systemic findings of topical ophthalmic medication

Immune-metabolic convergence in skeletal remodeling disorders: shared osteoimmune, senescent and lipid mechanisms across aging-related and immune-mediated bone disease.

Bone and joint disorders of the aging musculoskeletal system are conventionally treated as separate clinical entities. This disease-centered view aids diagnosis but obscures the immune, inflammatory, senescent and metabolic programs they share. This Review compares five of them, grouped by etiology rather than by age: two aging-driven remodeling disorders, osteoporosis (OP) and osteoarthritis (OA); two immune-mediated arthritides in which aging modifies rather than causes the skeletal outcome, rheumatoid arthritis (RA) and psoriatic arthritis (PsA); and one focal, largely monogenic disorder in

Peptide‑calcium chelates from microalgae enhance calcium bioavailability and regulate osteogenic and osteoclastogenic signaling in ovariectomized mice.

Estrogen deficiency-induced postmenopausal osteoporosis (PMOP) poses a global health burden requiring high-bioavailability nutritional therapies, while conventional calcium supplements exhibit poor absorption and gastrointestinal irritation; peptide‑calcium chelates stand out for superior uptake efficiency. In this study, we investigated the osteoprotective effects and molecular mechanisms of two microalgae-derived peptide-calcium chelates, specifically Chlorella pyrenoidosa peptide-calcium chelate (CP-Ca) and Arthrospira platensis peptide-calcium chelate (AP-Ca), using an ovariectomized (OVX)

Modulation of Arachidonic Acid Metabolism by Pinoresinol Diglucoside Ameliorates Osteogenic Dysfunction via Coordinated NF-κB Suppression and GSK3β/β-Catenin Activation in Diabetic Osteoporosis.

Diabetic osteoporosis (DOP) is characterized by chronic low-grade inflammation and impaired bone formation. Pinoresinol diglucoside (PDG), a bioactive compound derived from Eucommia ulmoides Oliv., has shown therapeutic potential in metabolic bone disorders; however, its direct molecular targets and integrated mechanisms remain unclear. This study aimed to elucidate the metabolic basis and molecular targets through which PDG protects against high glucose (HG)-induced osteoblast dysfunction and diabetic bone loss. Osteogenic effects of PDG were evaluated in HG-exposed osteoblasts, with untarget

Bone marrow adipose tissue in cardiometabolic disease: marrow niche remodeling, inflammatory hematopoiesis, and vascular pathology.

Bone marrow adipose tissue (BMAT) is increasingly recognized as a metabolically active, endocrine, and immunomodulatory component of the marrow niche rather than a passive fat depot. This narrative review synthesizes biological, imaging, epidemiological, and therapeutic evidence linking BMAT and marrow metabolic activation with cardiometabolic disease. BMAT arises from marrow stromal progenitors and exhibits depot-specific heterogeneity, endocrine activity, insulin responsiveness, and context-dependent expansion during aging, diabetes, obesity, menopause, osteoporosis, and chronic kidney disea
