Mind the gap: Adherence to denosumab dosing and cessation guidelines in Australian residential aged care.

AIMS: Denosumab and bisphosphonates are the main treatments for osteoporosis in residential aged care (RAC). Strict adherence to 6-monthly denosumab dosing is critical as delayed doses can result in bone resorption and vertebral fractures. If denosumab is ceased, then a bisphosphonate is recommended to avoid vertebral fractures. We aimed to examine adherence to denosumab dosing guidelines, practices of ceasing or switching to bisphosphonate treatment and how these vary in Australian RAC. METHODS: A retrospective cohort study using routinely collected electronic medication administration data.

Role of Bone Turnover Markers and Bone Mineral Density in Monitoring Bone Health at One- and Three-Year Post-antiresorptive Therapy Initiation in Patients With Osteoporosis: A Retrospective Cohort Study.

Background The gold-standard assessment of bone health is bone mineral density (BMD) measurement using dual-energy X-ray absorptiometry (DEXA); however, this modality is limited in its ability to detect short-term changes. Bone turnover markers (BTMs), specifically serum C-terminal telopeptide of type I collagen (CTX) and procollagen type I N-terminal propeptide (P1NP), offer the advantage of more frequent monitoring, yet their reliability as surrogate markers of bone health remains uncertain. This study investigates the relationship between BTMs and BMD during osteoporosis treatment. Methods

Therapeutic Effects of Fermented Soy Extract on Osteoporotic Changes in Ovariectomized Mice as a Model of Menopause.

The objective of this study was to evaluate the effects of fermented soy extract (FSE) on bone metabolism and neuroendocrine regulation in an ovariectomized (OVX) mouse model of postmenopausal conditions. FSE administration improved bone microarchitecture and modulated key markers associated with bone turnover, including reduced CTX-1 and RANKL levels and increased OPG expression, indicating suppression of osteoclastogenesis. In addition, FSE enhanced estrogen receptor-related activity and upregulated genes involved in bone metabolism. Notably, FSE also increased estradiol, serotonin, and nore

Precision and age-related changes of 3D-DXA reconstructions and FEA of the proximal femur in comparison to DXA.

The growing interest in 3D-DXA reconstructions for possible uses in fracture risk prediction or patient follow-up, e.g., in conjunction with finite element analyses, asks for precision data to increase the interpretability of results making use of the 3D-DXA technique. Using two datasets and a total of 2427 DXA scans of the proximal femur we evaluated the short- and long-term precision of standard DXA measurements. All the available scans were reconstructed using 3D-Shaper software and processed through a nonlinear finite element pipeline for femoral strength estimation. Short- and long-term p

Sea cucumber-inspired dual-responsive black phosphorus/amorphous calcium-based bone-targeting nanoplatform for synergistic therapy of osteoporosis.

The core pathological mechanism of osteoporosis (OP) resides in the imbalance between osteoblast-mediated bone formation and osteoclast-mediated bone resorption. Furthermore, the accumulation of reactive oxygen species (ROS) and pro-inflammatory factors in bone tissue induces disorders in the immune microenvironment, which substantially elevates the risk of fracture. Inspired by the biological behaviors of sea cucumbers, we designed and constructed a bone-targeting platform (BA@PDA-PA) based on black phosphorus (BP) and amorphous calcium carbonate (ACC). This platform enables precise targeted

Bone health in young adults with inflammatory arthritis: assessment of associated factors of low bone mineral density.

RATIONALE: To evaluate bone mineral density (BMD) in young adults with inflammatory arthritis. MAIN RESULT: Low BMD was detected in 42% of patients, associated with male sex, lower BMI, slower gait speed, higher ESR, and prolonged glucocorticoids. SIGNIFICANCE: This population requires early, comprehensive bone health screening to prevent long-term complications. INTRODUCTION: Bone loss and consequent fractures significantly reduce quality of life, causing disability and premature mortality. These issues, common in elderly patients, are also linked to inflammatory arthritis (IA). The impact

Connexin 43 subcellular localization: From trafficking dynamics to pathophysiological outcomes and therapeutic opportunities.

Connexin 43 (Cx43) exhibits remarkable functional diversity that is precisely dictated by its dynamic subcellular localization. Beyond its canonical role at the plasma membrane, where it assembles into gap junctions (GJs) and hemichannels (HCs) to mediate intercellular communication, Cx43 translocates to the nucleus and mitochondria, where it exerts non-channel functions including transcriptional regulation and metabolic adaptation. At the plasma membrane, dysregulation of Cx43 trafficking, anchoring, or turnover leads to excessive HC opening and impaired GJ communication, contributing to card

The Anabolic-First Strategy in Osteoporosis: A Systematic Review and Meta-Analysis of Fracture Outcomes in Patients at Very High Fracture Risk.

Background and Objectives: Individuals classified as having very high fracture risk remain vulnerable to imminent fractures even when treated with antiresorptive therapies. This meta-analysis evaluated whether initiating treatment with anabolic agents, including teriparatide, abaloparatide, and romosozumab, provides superior fracture protection in this high-risk population. Materials and Methods: A systematic review and meta-analysis of randomized controlled trials was conducted following PRISMA standards. Eligible studies included adults at very high fracture risk, defined by recent or multip

Randomised controlled trial comparing antibiotic cement bead pouch versus negative pressure wound therapy for the management of severe open tibia fracture wounds: Beads versus VAC (BvV) protocol.

INTRODUCTION: Early open fracture management aims to minimise the risk of complications. For the most severe open fracture wounds, multiple irrigation and debridement surgeries are required to overcome severe wound contamination, to reassess the evolving tissue injury or to temporise and plan further surgery. When multiple irrigation and debridement surgeries are needed, uncertainty remains about how the open fracture wound should be managed to best minimise complications. The primary aim of this trial is to compare the antibiotic cement bead pouch vs negative pressure wound therapy in the man

The Effects of Boron Mineral on Performance, Bone and Mineral Metabolism in Purebred Arabian Foals.

Boron plays significant roles in various biological systems, including mineral, lipid, and energy metabolism, immune and endocrine systems, and brain function. It has been suggested to enhance performance and may prevent conditions such as osteoporosis, osteoarthritis, and arthritis. Despite these known benefits, its effects on growth performance and mineral metabolism in horses remain understudied, particularly in young animals like Purebred Arabian foals. The primary objective of this study was to investigate the effects of different doses of boron supplementation (0, 5, 10, and 15 mg/day pe

An intelligent auxiliary diagnostic system for early osteoporosis screening using stacking ensemble learning.

BACKGROUND: Osteoporosis is a common chronic condition in older adults, characterized by reduced bone mineral density and increased fracture risk. Dual-energy X-ray absorptiometry is the diagnostic gold standard, but its cost and limited availability restrict large-scale screening in primary and community care. There is a need for auxiliary diagnostic systems that use routinely collected clinical data to identify individuals at high risk of osteoporosis. METHODS: Data from the 2017–2020 National Health and Nutrition Examination Survey were analysed. Adults aged 50 years or older with valid dua

GenoBERT: A Language Model for Accurate Genotype Imputation.

Genotype imputation enables dense variant coverage for genome-wide association and risk-prediction studies, yet conventional reference-panel methods remain limited by ancestry bias and reduced rare-variant accuracy. We present Genotype Bidirectional Encoder Representations from Transformers model (GenoBERT), a transformer-based, reference-free framework that tokenizes phased genotypes and uses self-attention mechanism to capture both short- and long-range linkage disequilibrium (LD) dependencies. Benchmarking on two independent datasets-the Louisiana Osteoporosis Study (LOS) and the 1000 Genom

Targeted IL-4 nanoparticles for osteal macrophages modulation in osteoporosis.

Osteoporosis (OP) is a chronic disease that compromises bone health. It is characterized by a disruption of bone tissue homeostasis, resulting in reduced bone mineral density and an increased risk of fractures. This enhanced fracture risk is associated with an elevated osteoclastic-to-osteoblastic activity ratio, reflecting excessive bone resorption relative to formation. Interleukin-4 (IL-4) is an immunomodulatory cytokine known to suppress osteoclast differentiation and influence the fate of osteal macrophages, cells that play a supportive role in bone resorption. Consequently, targeted IL-4

Different Lrp5-HBM mutations induce magnitude- and surface-selective effects that are not explained by receptor trafficking.

The Wnt co-receptor LRP5 plays a crucial role in skeletal physiology. Homozygous loss-of-function mutations in the Lrp5 gene produce severe osteopenia, whereas certain heterozygous point mutations produce extreme high bone mass (HBM). Different HBM-causing missense mutations are associated with variable effect magnitudes and sites of action in the skeleton. It is currently unknown whether the inclusion of 2 different HBM-causing mutations (eg, p.G171V and p.A214V), moved into the same genome, would potentiate bone mass beyond either mutation alone by manifesting their individual and divergent

Tri-Functional MgTA@MnO2 Nanozymes Orchestrate Redox Defense, Angiogenesis, and Immunometabolism for Osteoporotic Bone Regeneration.

PURPOSE: In osteoporotic conditions, bone regeneration is hindered by a pathological triad: persistent oxidative stress, compromised vascularization, and a dysregulated osteoimmune microenvironment. The purpose of this study was to engineer a multifunctional nanotherapeutic platform capable of orchestrating redox defense, angiogenesis, and immunometabolic reprogramming to accelerate bone repair. METHODS: We synthesized a tri-functional nanozyme (MgTA@MnO2) featuring a metal-phenolic network (MPN) coating, established through the one-step coordination of tannic acid (TA) and magnesium ions (Mg

Orchestrated Engineered Polyphenol-Peptide Condensates Coupled With β-GP Reverses Diabetic Osteoporosis by Remodeling Mitophagy.

Diabetic osteoporosis (DOP) is a chronic complication of diabetes mellitus characterized by reduced bone mass, disrupted microarchitecture, and an elevated fracture risk. Persistent oxidative stress and inflammation further inhibit osteogenesis and angiogenesis, accelerating bone degeneration. In this study, we used a peptide-polyphenol conjugation strategy to develop a multifunctional colloidal nanoplatform (β-GP@EGCG-E7). The bone-targeting peptide E7 was covalently conjugated with epigallocatechin-3- gallate (EGCG) and subsequently loaded with β-glycerophosphate (β-GP). Sequentially, this n

Denosumab safety in men with osteoporosis: a disproportionality analysis of the FAERS database.

This study aims to assess the spectrum of adverse events (AEs) associated with denosumab using in male patients with osteoporosis, leveraging the FDA Adverse Event Reporting System (FAERS) database from the Q3 2010 to Q4 2024. Four disproportionality analysis methods-reporting odds ratio (ROR), proportional reporting ratio (PRR), Bayesian confidence propagation neural network (BCPNN), and empirical Bayesian geometric mean (EBGM) were applied to detect denosumab-associated AEs. Clinical priority was assigned to signals by scoring five features using a rating scale. Among 3370 AEs associated wit

The benefit of cement-augmented pedicle screws for revision surgery in osteoporotic bone - a cadaveric study.

PURPOSE: Increasing numbers of spine surgery cases and an aging society led to challenges during revision surgery with osteoporotic bone. Safe and stable operative techniques are required. METHODS: In this ex vivo study, human lumbar vertebrae were prepared and then screened for assessing bone density. Eighteen donors (65 vertebrae) with osteoporotic conditions were included. Primary dorsal instrumentation was performed using either standard pedicle screws (6.35 mm diameter; Group A, n = 50) or vertebroplasty screws (VT, 6.35 mm; Group B, n = 15). This was followed by a standardized pull-out

11β-Hydroxysteroid dehydrogenase type 1 deficiency causes sexual dimorphism in body composition and bone mass in response to caloric restriction.

Caloric restriction (CR) alters energy metabolism and increases systemic glucocorticoid production, contributing to bone loss and bone marrow adipose tissue (BMAT) expansion. The enzyme 11β-hydroxysteroid dehydrogenase type 1 (11β-HSD1) mediates the local amplification of glucocorticoids within these tissues, but its contribution to CR-induced skeletal alterations remains unclear. Eight-week-old male and female WT and global 11β-HSD1 KO mice were assigned to ad libitum feeding or 30% CR for 8 wk. Body composition was assessed by DXA, bone microarchitecture by micro-CT (μCT), biomechanics by 3-
