Improving quality of care in osteoporosis: opportunities and challenges.

Osteoporosis is a chronic disease with a projected escalation in fracture prevalence and costs as the population ages. Osteoporosis care aims to prevent fractures and ultimately improve health-related quality of life. Despite significant evidence-based advances in testing and treatment, a large proportion of patients with fragility fracture or other osteoporosis risk factors are never evaluated with bone mineral density measurement testing and never receive therapy. This suboptimal quality of care can be attributed to barriers at the patient, provider, and health system levels. In addition, si

Osteoporosis: how long should we treat?

PURPOSE OF REVIEW: Bisphosphonates are the most commonly used treatment for osteoporosis. The pharmacology of bisphosphonates suggests the possibility of discontinuing treatment for a period, and patients frequently ask about this. In an attempt to help answer these questions, this review will consider recent data relevant to continuing and discontinuing long-term bisphoshonate therapy and other osteoporosis therapy. RECENT FINDINGS: Morphometric vertebral fracture protection continues for 1 year after discontinuation of risedronate for 3 years and there are now similar bishosphonate data for

Teriparatide: a review of its use in osteoporosis.

Recombinant teriparatide (Forteo; Forsteo) is an anabolic (bone forming) agent. Studies have shown that subcutaneous teriparatide 20 microg/day is effective in women with postmenopausal osteoporosis, men with idiopathic or hypogonadal osteoporosis and patients with glucocorticoid-induced osteoporosis. Teriparatide improves bone mineral density (BMD) and alters the levels of bone formation and resorption markers; histomorphometric studies showed teriparatide-induced effects on bone structure, strength and quality. Subcutaneous teriparatide 20 microg/day administered over a treatment period of 1

Osteoporosis preventive care in white and black women in community family medicine settings.

BACKGROUND: Osteoporosis has been studied predominantly in white postmenopausal women. MATERIALS AND METHODS: We performed a cross-sectional survey of a random sample of 400 women aged 45 years and older enrolled in a family medicine community-based research network. Participants responded to 42 items regarding osteoporosis screening and prevention during primary care visits. Odds ratios were calculated comparing black versus white women, adjusting for age, weight, fracture history, family history of osteoporosis, and practice site. RESULTS: Compared with black women, white women had 5.96 (9

Persistence with weekly alendronate therapy among postmenopausal women.

INTRODUCTION: Although clinical trials indicate that oral bisphosphonates reduce osteoporotic fracture risk, compliance with bisphosphonate therapy in practice is suboptimal, with 1-year discontinuation rates exceeding 50%. METHODS: We conducted a retrospective cohort study among female members of a large integrated health care delivery system (Kaiser Permanente of Northern California), age 45 years and older, to determine their persistence with weekly alendronate (defined as continuous use, allowing for a refill gap of 60 days), predictors of discontinuation, and subsequent osteoporosis ther

Effect of the Women's Health Initiative on osteoporosis therapy and expenditure in Medicaid.

UNLABELLED: Decreasing HRT use among postmenopausal women may have a reciprocal impact on other osteoporosis therapy. Time series analysis of prescribing trends for millions of Medicaid beneficiaries revealed a 57% decline in HRT without augmenting the pace of bisphosphonate use. Prescribing changes dramatically increased Medicaid spending on osteoporosis therapy over the last decade and requires further evaluation of cost effectiveness. INTRODUCTION: Hormone replacement therapy (HRT) has been commonly prescribed to postmenopausal women, but its use is decreasing because adverse cardiac outco

Simple computer model for calculating and reporting 5-year osteoporotic fracture risk in postmenopausal women.

PURPOSE: To devise, validate, and test a software model that improves how clinicians calculate individual risk for osteoporotic fracture and expected treatment benefit. METHODS: We developed a simple model of seven easily ascertained items plus bone mineral density (BMD) that calculates absolute fracture risk and expected absolute risk reduction after treatment. Baseline clinical variables and longitudinal fracture data from two large osteoporosis cohort studies validated the model's accuracy in predicting fracture risk. We then surveyed 298 clinicians to evaluate the likelihood they would pr

Relation between homocysteine and B-vitamin status indicators and bone mineral density in older Americans.

Recent studies have found a connection between hyperhomocysteinemia and hip fracture. If this association is causal, it could be mediated through detrimental effects of low B-vitamin status on bone mineral density (BMD). Studies have linked homocysteine (Hcy) and the established Hcy determinants folate and vitamin B12, to BMD, but results have been inconsistent. Furthermore, only one study considered the specific marker of tissue vitamin B12 status, methylmalonic acid (MMA), and none have considered red blood cell (RBC) folate. To further explore associations between Hcy and B-vitamin status i

Osteoporosis treatment in postmenopausal women after peripheral fractures: impact of information to general practitioners.

UNLABELLED: A low-impact fracture in a postmenopausal woman should prompt investigations for osteoporosis followed, if needed, by appropriate treatment. OBJECTIVES: To evaluate the impact of information alerting general practitioners to the need for osteoporosis treatment in postmenopausal women with a recent history of peripheral fracture. METHODS: We conducted a prospective 7-month follow-up study of 78 postmenopausal women, with a mean age of 81.5 years, admitted to the emergency department for peripheral fractures. Three months after the fracture, we sent a letter to the general practiti

Comparison of risedronate to alendronate and calcitonin for early reduction of nonvertebral fracture risk: results from a managed care administrative claims database.

OBJECTIVE: Recent randomized clinical trials have shown that risedronate reduces the risk of nonvertebral fractures and clinical vertebral fractures within 6 months of initiating treatment. The objective of the current study was to determine whether this early antifracture effect could be demonstrated in nonvertebral fractures for risedronate and other osteoporosis therapies in an observational administrative claims database. METHODS: A proprietary administrative claims database was used to identify managed care members who received a new prescription for risedronate, alendronate, or nasal ca

Review article: Osteoporosis and inflammatory bowel disease.

Studies using dual-energy X-ray absorptiometry have suggested a high prevalence of osteoporosis in inflammatory bowel disease. However, population-based data on fracture incidence suggest only a small increased risk of fracture amongst patients with inflammatory bowel disease compared with the general population. Therefore, it would be helpful to identify patients with inflammatory bowel disease at particularly high risk for fracture so that these risks might be modified or interventions might be undertaken. The data on calcium intake as a predictor of bone mineral density are conflicting. Alt

Health care utilization and expenditures in the United States: a study of osteoporosis-related fractures.

BACKGROUND: More than 1.5 million fractures occur due to osteoporosis each year. This study examines the annual health care utilization and associated expenditures of osteoporotic patients who sustain a new fragility fracture and of those without a new fracture. METHODS AND PROCEDURES: The study sample from commercial claims databases consisted of patients enrolled in US plans between January 1, 1997, and December 31, 2001. Patients with both an osteoporosis diagnosis and a related fracture were classified as "osteoporosis with concurrent fracture"; all other osteoporosis patients were classi

Long-term disuse osteoporosis seems less sensitive to bisphosphonate treatment than other osteoporosis.

UNLABELLED: We sought to determine whether risedronate can preserve cortical bone mass and mechanical properties during long-term disuse in dogs, assessed by histomorphometry and biomechanics on metacarpal diaphyses. Risedronate slowed cortical thinning and partially preserved mechanical properties, but it was unable to suppress bone loss to the degree seen in other osteoporoses. INTRODUCTION: Disuse induces dramatic bone loss resulting from greatly elevated osteoclastic resorption. Targeting osteoclasts with antiresorptive agents, such as bisphosphonates, should be an effective countermeasur

Therapeutic potential of parathyroid hormone.

Teriparatide, recombinant human parathyroid hormone (1-34) (rhPTH [1-34]), is approved for the treatment of osteoporosis in men and postmenopausal women at high risk for fracture. The best candidates are those who have already had vertebral compression fractures (symptomatic or asymptomatic) or other osteoporosis-related fractures, or those who have very low bone mineral density, in the T score range of -3.5 or below. Teriparatide is the first anabolic drug approved by the US Food and Drug Administration for osteoporosis. It not only dramatically improves bone mass, but also restores bone micr

Defining strategies for improving the identification and management of osteoporosis in the primary care setting.

New diagnostic and therapeutic options in postmenopausal osteoporosis have emerged in the last decade. Since all untreated postmenopausal women will exhibit a reduction in bone mineral density (BMD), the clinician's task is to identify those at a degree of risk that justifies intervention, and then supply appropriate therapy and monitoring. Various professional organizations and independent studies have furnished guidelines on when and whom to treat. But uncertainty still exists as to even such basic matters as when to measure BMD. This article considers the findings of the NOF, the USPTF, ACO

The acrophysis: a unifying concept for understanding enchondral bone growth and its disorders. II. Abnormal growth.

In order to discuss and illustrate the effects common to normal and abnormal enchondral bone at the physes and at all other growth plates of the developing child, the term "acrophysis" was proposed. Acrophyses include the growth plates of secondary growth centers including carpals and tarsals and apophyses, and the growth plates at the nonphyseal ends of small tubular bones. Abnormalities at acrophyseal sites are analogous to those at the physeal growth plates and their metaphyses. For example, changes relating to the zone of provisional calcification (ZPC) are often important to the demonstra
