Radiofrequency Echographic Multi Spectrometry (REMS) is a portable and radiation-free technology that can evaluate and monitor osteoporosis. In particular, REMS has been shown to measure bone mineral density (BMD) at axial skeletal bones with a precision, repeatability and accuracy not inferior to those of dual-energy X-ray absorptiometry (DXA). Moreover, REMS may be useful in the assessment of impaired bone quality and to predict fragility fracture risk. Due to these characteristics, REMS could be usefully used in the diagnosis and follow up of osteoporosis in rare bone diseases. The clinical

Patients with primary hyperparathyroidism (PHPT) ineligible for surgery require medical management for hypercalcemia and osteoporosis. The aim of this retrospective study was to determine the long-term effects of evocalcet on serum corrected calcium (cCa) levels and bone mineral density (BMD) in PHPT. The study included 26 patients with PHPT and hypercalcemia treated with evocalcet (7 switched from cinacalcet) for at least 24 months. Their mean age was 75.5 years. At baseline, cCa, phosphorus, and median intact parathyroid hormone levels were 10.76 mg/dL, 2.91 mg/dL, and 99.0 pg/mL, respective

PURPOSE: The primary objective of this study was to retrospectively evaluate the demographic, biochemical, and clinical characteristics of patients with asymptomatic primary hyperparathyroidism (aPHPT), analyze their long-term outcomes, and discuss the effectiveness of current therapeutic strategies in light of the existing literature. We anticipate that our study will provide clinicians with guidance regarding surgical decision-making beyond the standard criteria for aPHPT.
METHODS: This was a nationwide, multicenter, observational, retrospective cohort study. All tertiary care endocrinology

Parathyroid hormone (PTH), produced by the parathyroid glands, plays a critical role in the regulation of calcium and phosphate homeostasis, acting primarily on bone and kidney to maintain serum calcium levels within a narrow range. Parathyroid hormone also plays important roles in bone remodeling by directly stimulating osteoblasts and osteocytes, integrating its calcemic response with stimulation of bone formation. Through the RANK/RANK-ligand system, these cells activate osteoclasts, promoting a balanced process of bone formation and resorption that maintains bone density and strength. Dysr

Lithium is a key treatment for bipolar disorder but is associated with significant endocrine and metabolic side effects, particularly lithium-induced hyperparathyroidism (LIH). Characterized by hypercalcemia and inappropriately normal or elevated parathyroid hormone (PTH) levels, LIH can lead to skeletal complications such as osteoporosis. Managing this condition is particularly challenging in psychiatric patients due to adherence issues with follow-up care. We describe the case of a 56-year-old Caucasian female patient with bipolar disorder on long-term lithium therapy (16 years), presenting

A 73-year-old woman with a remote history of successfully treated primary hyperparathyroidism was referred to our office because of multiple skeletal fractures and proximal muscle weakness. Prior laboratory evaluation demonstrated hypocalcemia, vitamin D deficiency, elevated PTH, and elevated alkaline phosphatase. Updated evaluation in our clinic additionally showed hypophosphatemia prompting measurement of serum fibroblast growth factor 23 (FGF23). FGF23 proved to be markedly elevated prompting consideration of tumor-induced osteomalacia. Positron emission tomography with Cu-64 oxodotreotide

BACKGROUND: Dysregulation of microRNAs contributes to bone diseases. However, the microRNAs involved in primary hyperparathyroidism (PHPT)-induced osteoporosis remain unknown.
METHODS: The parathyroid tissue samples were obtained from PHPT patients with or without osteoporosis (n = 5/group) during parathyroid resection and subjected to high throughput microRNA sequencing. The differentially expressed microRNAs were identified and further verified using qRT-PCR. Alizarin Red Staining was performed to detected the osteogenic differentiation. Gain- and loss-of-function assays were performed to i

Multiple endocrine neoplasia type 1 (MEN-1; OMIM 131100) is a rare, autosomal dominant syndrome caused by heterozygous inactivating mutations in the MEN1 tumor suppressor gene (11q13; OMIM 613733). MEN-1 is characterized by polyglandular pathology, which typically involves the parathyroid glands (90%), pancreas (30-80%) and anterior pituitary (15-50%). To date, over 1,600 pathogenic MEN1 variants have been documented, including nonsense, frameshift, and splice-site mutations, as well as rare large deletions. While germline mutation detection rates reach 70-90% in clinically diagnosed probands,

BACKGROUND: Early postoperative kinesiophobia represents a significant psychological barrier in the rehabilitation trajectory following arthroscopic rotator cuff repair (ARCR). Existing research predominantly focuses on patients in the chronic phase, paying insufficient attention to the heterogeneity within this population. This study aimed to identify potential subtypes of kinesiophobia in patients after rotator cuff injury (RCI), analyze the causes affecting different subtypes of kinesiophobia, and provide a basis for formulating accurate early intervention plans. METHODS: This study was con

OBJECTIVE: Primary hyperparathyroidism significantly impacts bone health leading to decreased bone mineral density (BMD). We assessed changes in BMD following parathyroidectomy in patients with biochemically mild primary hyperparathyroidism.
METHODS: A retrospective chart review of 93 patients with primary hyperparathyroidism who underwent parathyroidectomy between 2000 and 2022 was conducted. Dual-energy x-ray absorptiometry (DXA) scans were done at the same site pre- and postparathyroidectomy at varying time points. Statistical analyses were performed using ANOVA as the homogeneity of varia

UNLABELLED: Gaucher disease (GD), the most common lysosomal disorder, is caused by a deficiency of the enzyme glucocerebrosidase (GCase). Accumulation of the substrate, glycosylceramide (Gb-1), and its lysosomal derivative, glucosylsphingosine, Lyso-Gb1, are associated with immune-mediated inflammation. Patients with GD experience progressive bone disease, including early-onset osteoporosis (OSR). Bone marrow infiltration with Gaucher cells and reduced bone mineral density (BMD) suggest that glycosphingolipids affect hematopoiesis, osteoclast differentiation and activity. Unlike the general po

Tributyltin (TBT), a recognized endocrine disruptor, is associated with metabolic diseases, including obesity, type 2 diabetes, non-alcoholic steatohepatitis, and osteoporosis. Bile acids (BAs) play pivotal roles in lipid digestion and absorption. However, there are no studies to illustrate the effects of TBT on BA pool composition in circulation. Here, rats were treated with TBT (50 μg/kg) or a vehicle control once every three days for sixty days to analyze serum BA levels using ultra-high-performance liquid chromatography-tandem mass spectrometry (UHPLC-MS/MS). Patients with dialysis have a high risk of osteoporosis and fractures. However, the optimal treatment strategies for osteoporosis in dialysis patients remain unclear due to the complexity of chronic kidney disease (CKD)-mineral and bone disorder. This case report describes severe symptomatic hypocalcemia and prolonged heart failure following denosumab treatment in a peritoneal dialysis (PD) patient with secondary hyperparathyroidism and osteoporosis. A 70-year-old woman on PD for dialysis caused by diabetic nephropathy was diagnosed with osteoporosis (T score -2.5) during a routine checkup. De

BACKGROUND: Hungry bone syndrome (HBS) is a complication after parathyroidectomy (PTX) for primary hyperparathyroidism (PHPT), with variable prevalence worldwide. Data on predictors and outcomes are limited. This study assessed the prevalence, risk factors, and outcomes of HBS after PTX for PHPT. METHODS: A retrospective review of patients undergoing PTX for PHPT (2003–2023) at Inkosi Albert Luthuli Central Hospital, KwaZulu-Natal, South Africa. HBS was defined as hypocalcaemia (corrected serum calcium < 2.1 mmol/l) lasting > 4 days post-PTX with normal or elevated intact parathyroid hormone (

Background: Magnesium (Mg2+) plays a fundamental role in various physiological processes, including neuromuscular function, glucose metabolism, cardiovascular regulation, and bone health. Despite its significance, the influence of sex on magnesium metabolism, requirements, and health outcomes remains unexplored. The aim of this review is to analyze sex-based differences in magnesium homeostasis, with a particular focus on hormonal regulation, body composition, and disease susceptibility. Methods: This narrative review, based on a non-systematic MEDLINE search conducted in January 2025, priorit

BACKGROUND: Endocrine science remains underrepresented in European Union research programmes despite the fundamental role of hormone health in human well-being. Analysis of the CORDIS database reveals a persistent gap between the societal impact of endocrine disorders and their research prioritization. At national funding level, endocrine societies report limited or little attention of national research funding towards endocrinology. The EndoCompass project - a joint initiative between the European Society of Endocrinology and the European Society of Paediatric Endocrinology - aimed to identif

Parathyroid glands typically exist in pairs on the posterior surface of the thyroid, and primary hyperparathyroidism (PHPT) is often caused by adenomas in these locations. However, ectopic parathyroid glands may occasionally develop in unusual sites. We present a case of a 71-year-old female patient who exhibited gradual increases in both serum calcium and parathyroid hormone (PTH) levels, along with the development of osteoporosis, suggesting the presence of PHPT. Routine examination, including ultrasonography, sometimes fails to identify an ectopic parathyroid adenoma. In this case, further
