[Changes in the mineral composition of orbital bones in thyroid disease].

UNLABELLED: Thyroid diseases lead to significant systemic changes in bone tissue, but analysis and densitometric assessment in this pathology is usually limited to the lumbar vertebrae and the femoral and radial bones. At the same time, there are grounds to assume presence of significant changes in other bony structures, primarily the orbit. To date, no data are available on specific changes in the mineral composition of orbital bones in thyroid pathology. OBJECTIVE: This study assessed the mineral composition of the bone tissue of the lateral orbital wall in thyroid eye disease (TED) using s

The Association Between Serum Vitamin D and Gait Speed in a Middle-Aged and Older Taiwanese Population: A Community-Based Cross-Sectional Study.

Gait speed is a key indicator for diagnosing sarcopenia, validated by both the European Working Group on Sarcopenia in Older People in 2018 and the Asian Working Group on Sarcopenia in 2019 for assessing physical performance. While global data link vitamin D to physical performance, regional sex-stratified evidence in East Asian populations remains limited. This study aims to validate and confirm the association between gait speed and vitamin D levels in a middle-aged and older population in Taiwan. This cross-sectional investigation focused on community-dwelling residents. Participants were a

Post-dexamethasone cortisol levels in non-functioning adrenal incidentalomas: a pooled individual participant data analysis.

INTRODUCTION: Non-functioning adrenal incidentalomas (NFAI) are defined by 1 mg-dexamethasone suppression test (F-1mgDST) cortisol levels < 1.8 µg/dL. However, many individuals with NFAI may exhibit subtle hypercortisolism associated with increased cardiometabolic and skeletal risk. The aims of the present study are to compare F-1mgDST distributions between individuals with NFAI and population-based controls and to explore whether alternative F-1mgDST thresholds would meaningfully modify patient classification. METHODS: This is a pooled individual participant data analysis of 7 studies evalua

Ferroptosis in osteoporosis: core mechanisms, cell-specific effects in bone remodelling, and therapeutic perspectives.

Osteoporosis is an endocrine and metabolic bone disease driven by insufficient bone formation, enhanced bone resorption, and disrupted coupling of bone remodelling. Ferroptosis, characterised by iron-dependent membrane phospholipid peroxidation and failure of anti-lipid-peroxidation defences, is emerging as a mechanistic link between oestrogen deficiency, glucolipotoxicity, glucocorticoid exposure, ageing, iron overload, and dysfunction of bone-remodelling cells. This structured narrative review summarises iron homeostasis, lipid peroxidation, the System Xc--GSH-GPX4 axis, ferritinophagy, mito

Bone marrow adipose tissue in cardiometabolic disease: marrow niche remodeling, inflammatory hematopoiesis, and vascular pathology.

Bone marrow adipose tissue (BMAT) is increasingly recognized as a metabolically active, endocrine, and immunomodulatory component of the marrow niche rather than a passive fat depot. This narrative review synthesizes biological, imaging, epidemiological, and therapeutic evidence linking BMAT and marrow metabolic activation with cardiometabolic disease. BMAT arises from marrow stromal progenitors and exhibits depot-specific heterogeneity, endocrine activity, insulin responsiveness, and context-dependent expansion during aging, diabetes, obesity, menopause, osteoporosis, and chronic kidney disea

Total Hip Arthroplasty for Neglected Pathological Femoral Neck Fracture in a Young Patient with Autoimmune Polyglandular Syndrome.

INTRODUCTION: Neglected pathological femoral neck fractures in young patients are rare and present significant surgical challenges, particularly in the presence of systemic metabolic disorders such as autoimmune polyglandular syndrome (APS). Poor bone quality, impaired healing potential, and endocrine instability complicate management, often limiting the success of head-preserving procedures. PURPOSE: The purpose of the study was to describe the surgical decision-making, technical considerations, and early outcomes of total hip arthroplasty (THA) in a young patient with a neglected pathologic

Are we at risk of increasing sarcopenia? Implications for osteoarthritis and osteoporosis.

Sarcopenia is a progressive skeletal muscle disorder characterised by reduced muscle strength, impaired muscle quantity or quality, and declining physical performance, with important consequences for falls, fractures, disability, and loss of independence. Whether sarcopenia prevalence itself is rising remains difficult to quantify because diagnostic frameworks and surveillance methods vary, yet a growing burden is plausible. Population ageing, increasing multimorbidity, obesity, hospitalisation, surgery, and incomplete recovery after musculoskeletal events are likely to increase the absolute n

Pediatric osteoporosis: a comprehensive imaging-based review of diagnosis, differential diagnosis, and etiology.

Pediatric osteoporosis is a rare but increasingly recognized entity that differs from adult forms in its etiology, diagnostic criteria, and long-term skeletal implications. Unlike age-related osteoporosis in adults, bone fragility in children is categorized into primary forms, such as osteogenesis imperfecta and the monogenic causes of early-onset osteoporosis, and the more prevalent secondary forms arising from chronic systemic disease, endocrine and nutritional disorders, malignancy and its treatment, prolonged medication use, and immobilization. Early and accurate diagnosis is critical, as

Liver transplantation-related osteoporosis and fragility fractures: pathogenic pathways, risk stratification and novel targeted therapy advances.

Liver transplantation (LT) serves as the definitive treatment for end-stage liver disease. Yet, osteoporosis (OP) and fragility fractures remain frequently overlooked skeletal complications that may impair the long-term prognosis of recipients. This review systematically summarizes recent evidence regarding epidemiological characteristics, multifactorial pathogenic pathways, risk stratification approaches, and advances in targeted interventions for LT-related bone disorders. Skeletal damage distributes across pre-transplant waiting, early post-transplant acute and long-term chronic phases; rap

Sex differences in the clinical presentation and complications of primary hyperparathyroidism: insights from a single-center cohort study.

PURPOSE: Primary hyperparathyroidism (PHPT) predominantly affects women, particularly after menopause. Whether sex-related differences extend beyond epidemiology to influence the clinical expression of the disease remains incompletely understood. We investigated sex-related differences in the clinical phenotype of PHPT. METHODS: We retrospectively analyzed clinical, biochemical, imaging, and histopathological data from patients with PHPT referred to our Endocrinology Unit over a 5-year period. RESULTS: A total of 315 patients were included (64 men and 251 women; female-to-male ratio 3.9:1).

Premenopausal Osteoporosis: An Under-recognized Window for Early Skeletal Intervention.

BACKGROUND: Premenopausal osteoporosis is an uncommon but clinically important cause of skeletal fragility in younger women. Unlike postmenopausal osteoporosis, reduced bone mineral density (BMD) in premenopausal women may reflect low peak bone mass rather than active bone loss, and BMD alone does not reliably predict short-term fracture risk. Fragility fractures in this population should therefore prompt evaluation for potentially reversible secondary causes. AIMS AND OBJECTIVES: To summarize the current evidence regarding the epidemiology, etiological factors, clinical presentation, diagnos

Current Status of Animal Models for Premature Ovarian Insufficiency: Similarities to Clinical Manifestations and Therapeutic Applications.

BACKGROUND: Premature ovarian insufficiency (POI) is a reproductive disorder characterized by the early loss of ovarian function before the age of 40, resulting in infertility and increasing the long-term risk of health issues such as osteoporosis, cardiovascular disease, and neuropsychological disorders. Although many studies have been conducted to understand POI pathophysiology and to develop regenerative therapies, the effectiveness of these therapies in restoring ovarian function remains limited. This review provides a comprehensive evaluation of the establishment of animal models of POI a

GULP1 Fuels Resistance to PD-1 Blockade through Lipid-Mediated Metabolic Reprogramming of Tumor-Associated Macrophages in Gastric Cancer.

Background: Immune checkpoint inhibitors targeting programmed cell death protein 1 (PD-1) have substantially improved the treatment of multiple cancers; however, only a small proportion of gastric cancer (GC) patients achieve durable clinical benefit. The underlying tumor-intrinsic mechanisms driving resistance to anti-PD-1 therapy remain poorly understood. This study aimed to identify key regulators of immunotherapy resistance in GC and to elucidate their functional and mechanistic roles. Methods: Transcriptomic data from anti-PD-1-responsive and anti-PD-1-resistant GC patients were analyzed

Effect of Denosumab on Sperm Concentration in Men With Severe Oligospermia: A Randomized Controlled Trial.

OBJECTIVE: RANKL has been suggested as a novel regulator of male reproduction, and a recent randomized controlled trial (RCT) showed that the RANKL-inhibitor denosumab, normally used to treat osteoporosis, can improve semen quality in infertile men with high anti-Müllerian hormone (AMH) and small testicles. Therefore, this RCT aimed to investigate the effect of denosumab in men with severe oligospermia prospectively selected by serum AMH and testicular size. MATERIALS AND METHODS: A double-blinded, placebo-controlled, single-center RCT, including 42 men with oligospermia with the main endpoin
