Comparative bone histomorphometry effects of combined denosumab and teriparatide vs monotherapy in postmenopausal women with osteoporosis: a randomized controlled trial.

Combined treatment with PTH receptor stimulation (teriparatide 20-μg) and RANKL inhibition (denosumab 60-mg) increases spine and hip BMD and improves estimates of bone strength to a greater extent than either monotherapy. The mechanisms underlying the enhanced efficacy of this combination, however, are not fully defined. In this randomized, 3-arm interventional trial, postmenopausal women with osteoporosis were randomized to receive denosumab 60-mg (n = 9), teriparatide 20-μg (n = 13), or both (n = 12) for 3 mo. Participants received double fluorochrome labeling and underwent a single iliac cr

Gut Microbiota-Decanoic Acid-Interleukin-17A Axis Orchestrates Hyperglycemia-Induced Osteoporosis in Male Mice.

UNLABELLED: Hyperglycemia (HG) is a well-established risk factor for secondary osteoporosis, primarily due to suppressed osteoblast activity. While gut microbiota (GM) dysbiosis has been implicated in various diseases, its role in HG-induced osteoporosis remains poorly understood. Here, we demonstrate that HG mice develop low-turnover osteoporosis accompanied by reduced GM diversity. Fecal microbiota transplantation (FMT) from HG mice (GMHG-FMT) induced osteoporosis in recipient mice, independent of blood glucose levels. A depletion of Bifidobacterium pseudolongum was associated with bone loss

Interim Effects of Zoledronate, Denosumab, or Teriparatide on Bone Microarchitecture in Type 2 Diabetes: A Pilot Trial.

CONTEXT: Type 2 diabetes (T2D) increases fragility fracture risk despite normal/elevated areal bone mineral density (aBMD), attributed to compromised bone microarchitecture. However, evidence guiding pharmacologic management of diabetic bone disease remains limited. OBJECTIVE: We aimed to evaluate interim effects of zoledronate, denosumab, or teriparatide on bone microarchitecture in postmenopausal women with T2D at high fracture risk. METHODS: A 72-week, randomized, open-label, blinded-end point (PROBE) pilot clinical trial (CTRI/2022/02/039978) was conducted at a single tertiary care cente

Anti-Mullerian Hormone and Collagen Type I C-telopeptide Predict Fast, Imminent Bone Loss in Early Perimenopause: SWAN.

CONTEXT: Faster menopause-related bone mineral density (BMD) decline predicts more fractures. OBJECTIVE: Examine anti-Mullerian hormone (AMH) and collagen type I C-telopeptide (CTX) as predictors of fast, imminent BMD loss (BMD decline rate, over the next 1-2 years, ≥mean, annual menopause rate). DESIGN: Repeated measures modified Poisson regression estimated the associations of early perimenopausal levels of (1) AMH or CTX (in separate models), or (2) AMH and CTX (in a single model) with fast, imminent BMD loss. SETTING: Study of Women's Health Across the Nation (community-based cohort).

Safety of low dose inpatient zoledronic acid in acute hip fracture patients with advanced chronic kidney disease.

Zoledronic acid (ZA), cleared renally, carries a higher nephrotoxicity risk at doses ≥4 mg, especially in patients with CKD. While 5 mg is standard for osteoporosis, ZA increases BMD at doses as low as 0.25 mg every 3 mo. Inpatient ZA (IP-ZA) in hip fracture patients reduces mortality and re-fracture risk, but its safety in those with advanced CKD (creatinine clearance rate (CrCl) < 35 mL/min) is unclear. We analyzed real-world data from a large academic healthcare system to assess the safety of reduced-dose IP-ZA (≤3 mg) in hip fracture patients with advanced CKD, compared to propensity-match

Alcohol use disorder is a chronic disease.

Alcohol use disorder (AUD) is a chronic, relapsing brain disease with profound health, societal, and economic consequences. Alcohol misuse not only leads to AUD, but it is also a driver of multimorbidity, exacerbating a wide range of chronic comorbidities, including cancer. Despite being formally recognized over six decades ago as a medical condition, AUD remains one of the most prevalent and costly public health issues in the United States. Alcohol misuse contributes to more than 90,000 deaths annually in the United States, with hundreds of billions of dollars lost annually due to healthcare

Relationship between impaired glucose metabolism and bone mineral density in patients with cystic fibrosis.

UNLABELLED: Cystic fibrosis (CF) is a chronic genetic disorder characterized by pancreatic insufficiency and lung disease. Advancements in highly effective modulator therapies (HEMTs) have improved life expectancy, shifting the focus to endocrine comorbidities, such as CF-related diabetes (CFRD) and bone disease (CFRBD). Therefore, current guidelines recommend routine screening for diabetes and osteoporosis in people with cystic fibrosis (PwCF) starting from age of 10 years. Increased risk of osteoporosis has been shown in type 1 and type 2 diabetes; however, there are limited studies evaluati

Primary Ovarian Insufficiency: Molecular Mechanisms and the Role of MicroRNAs.

Primary ovarian insufficiency (POI) is a multifactorial disorder marked by ovarian function cessation before age 40, leading to infertility and systemic complications, including osteoporosis, cardiovascular disease, and neurocognitive impairment. It results from accelerated ovarian reserve depletion, impaired folliculogenesis, and hormonal dysregulation. The majority of cases are idiopathic, with known causes including genetic mutations, autoimmune disorders, infections, and iatrogenic factors. Recent RNA sequencing advances highlight microRNAs (miRNAs) as critical regulators of ovarian proces

The Relationship Between Bone Health Status of Post-Menopausal Women with Non-Functional Adrenal Tumours/Mild Autonomous Cortisol Secretion and Their Baseline Morning Adrenocorticotropic Level.

Background. Glucocorticoid-induced osteoporosis represents a well-known type of secondary osteoporosis (SOp). While the most prevalent sub-category includes corticotherapy, another important contributor is represented by Cushing's syndrome. In this traditional landscape, adrenal incidentalomas do not involve a standard cause of SOp, since most of them are non-functioning adrenal tumours (NFATs). Yet, 30-40% of them are not entirely "non-functioning", due to mild autonomous cortisol secretion (MACS). Despite not being a guideline-based diagnosis, a lower ACTH might point to various NFATs/MACS c

Underlying Mechanisms of Osteoporosis in the Context of Multimorbidity: Clinical Challenges and Management Strategies.

Osteoporosis and chronic conditions such as type 2 diabetes mellitus, cardiovascular disease, heart failure, and chronic kidney disease share several common biological mechanisms, including chronic inflammation, oxidative stress, hormonal dysregulation, and metabolic alterations. In this context, multimorbidity presents an increasing clinical challenge, particularly in older populations, where osteoporosis remains frequently underdiagnosed and undertreated. This review aims to explore the complex interplay between skeletal fragility and cardiometabolic diseases, emphasizing the role of nutriti

Diverse exercise modalities in alleviating osteoporosis: A narrative review.

Osteoporosis, a systemic skeletal disorder, is typified by diminished bone mineral density, disruption of bone microstructure, and heightened fracture risk. It has emerged as a critical global public health concern. Traditional drug therapies, while capable of alleviating symptoms, come with inherent limitations. These include protracted treatment cycles, adverse effects, and suboptimal patient compliance. In recent years, exercise intervention has emerged as a safe and cost-effective lifestyle intervention, demonstrating remarkable efficacy in the prevention and management of osteoporosis. Th

Dual inhibition of sclerostin and Notum induces synergistic osteoanabolic action in mice.

Expanding the number of clinically approved choices for osteoanabolic therapy represents the next hurdle on the osteoporosis treatment horizon. The WNT pathway is involved in stimulating new bone formation, and the most recent FDA-approved anabolic therapy-sclerostin neutralizing antibody-works by stimulating the WNT pathway in bone. The challenge with all current osteoanabolic therapies is the short treatment window in which they are efficacious. One strategy to dealing with this limited anabolic window is to further maximize bone formation during the window, using combination therapy. For ex

Functional testosterone deficiency in aging men: Clinical impact, diagnostic pathways, and treatment strategies.

BACKGROUND: Testosterone constitutes an indispensable determinant of male corporeal integrity, psychological resilience, and overall vitality across the life course. Testosterone deficiency (male hypogonadism) represents an endocrine disorder capable of engendering a broad spectrum of somatic derangements and psychosocial sequelae. Its origins may lie in testicular insufficiency, hypothalamic-pituitary dysfunction, or, more subtly, in functional hypogonadism arising from comorbid states such as obesity and type 2 diabetes mellitus. METHODS: This review distills contemporary evidence on the pa

An analysis of the factors influencing bone health literacy among middle-aged and elderly breast cancer patients undergoing endocrine therapy: a mixed-methods study based on COM-B theory.

BACKGROUND: Endocrine therapy is a cornerstone in the treatment of breast cancer; however, its prolonged use can exacerbate the deterioration of bone health, particularly in middle-aged and older women, who are already at heightened risk for bone loss due to age-related hormonal changes. Despite the critical role that bone health literacy plays in managing patients′ bone health, there has been a lack of studies examining bone health literacy in middle-aged and older breast cancer patients undergoing endocrine therapy. This study aims to assess the current status of bone health literacy in this
