Successful cryoablation for a phosphaturic mesenchymal tumor in the maxilla causing tumor-induced osteomalacia: a case report.

BACKGROUND: A 35-year-old man was diagnosed with tumor-induced osteomalacia (TIO) caused by a phosphaturic mesenchymal tumor (PMT) in the right maxilla. The patient underwent CT-guided cryoablation at our institution, which successfully eradicated the tumor while preserving the pulpal vitality of adjacent teeth. We describe the clinical history, physical examination findings, laboratory tests, functional imaging, and follow-up protocol for evaluating treatment outcomes. Cryoablation demonstrated therapeutic efficacy through symptomatic relief, normalization of serum phosphorus levels, and main

Phosphaturic mesenchymal tumor of the skull base presenting with tumor-induced osteomalacia and multiple fractures: a case report.

BACKGROUND: This case reports a rare disease in which the clinical symptoms are completely inconsistent with the primary site. As the occurrence of this disease is partly rare, the challenges in the diagnosis of phosphaturic mesenchymal tumors are discussed. It also provides a new clinical treatment option, that is, the combined treatment with anlotinib capsules after surgery and the long-term follow-up to observe the effect of the combined treatment. CASE PRESENTATION: A 27-year-old Chinese woman presented with generalized pain and multiple fractures for 2 years. The present case was initial

Bone microarchitecture using HR-pQCT in adult patients with renal tubular acidosis presenting with rickets/osteomalacia.

UNLABELLED: Adults with renal tubular acidosis (RTA) show high bone turnover, reduced trabecular volumetric bone mineral density (vBMD), and compromised bone microarchitecture, especially in trabecular bone. HR-pQCT analysis reveals lower total vBMD, reduced trabecular number, increased separation, and cortical thinning, highlighting bone deterioration in RTA patients compared to controls. BACKGROUND: Renal tubular acidosis (RTA) impairs urinary acidification, leading to a normal anion gap metabolic acidosis and often hypokalemia. RTA can result in poor bone mineralization and reduced bone de

Bone Scintigraphy and Laboratory Clues to Osteomalacia in a Middle-Aged Man With Hip Fractures.

A 43-year-old man with a 1-year history of bilateral hip pain, worsened by standing and walking, presented with a limping gait. x-rays showed diffuse osteoporosis (osteoblastic-lytic), and a bone scan revealed symmetric uptake in the costochondral junctions (rosary beads), femoral neck, proximal tibia, and feet, with no visualization of the kidneys, suggesting a metabolic disorder or neoplasm. Computed tomography revealed osteolytic lesions in multiple bones. Suspecting metabolic bone disease or metastasis, laboratory tests were conducted. Magnetic resonance imaging confirmed bilateral femoral

Tumour-induced osteomalacia due to a rare intrathoracic phosphaturic mesenchymal tumour.

Phosphaturic mesenchymal tumours (PMTs) are rare neoplasms that secrete fibroblast growth factor 23 (FGF23), a hormone that disrupts phosphate homeostasis and induces tumour-induced osteomalacia (TIO). This condition causes hypophosphataemia, bone pain, fractures and musculoskeletal weakness. Due to the non-specific nature of these symptoms, TIO is often misdiagnosed with treatment delays. Surgical resection of PMTs is curative with rapid normalisation of serum phosphorus and full symptom resolution. PMTs are often located in the extremities and axial skeleton but can also occur in less common

Bouquet of flower calcification in renal tubular acidosis.

Nephrocalcinosis, marked by calcium phosphate deposition in the kidneys, is often linked to distal renal tubular acidosis (dRTA). While typically driven by hypercalciuria, hyperphosphaturia can also promote crystal formation in alkaline urine. A 24-year-old female with recurrent fractures and hip pain presented with hypokalemia, metabolic acidosis, and hyperphosphaturia but without hypercalciuria. Imaging revealed medullary nephrocalcinosis and osteomalacia, secondary to dRTA. Potassium citrate improved her condition. This case emphasizes early recognition of dRTA to prevent renal and skeletal

Joint position on vitamin D prescription in the adult Mexican population by AMMOM, AMEC, AMG, CMIM, CMO, CMR, CONAMEGER, FEMECOG, and FEMECOT.

BACKGROUND: Vitamin D deficiency remains a critical health concern linked to skeletal disorders such as osteoporosis, osteomalacia, and fractures. Recent evidence highlights the broader role of vitamin D in preventing chronic conditions, including autoimmune diseases, diabetes, and cardiovascular events. However, inconsistencies in clinical practice across Mexico and limited population-specific data necessitate standardized guidelines to address diagnostic and therapeutic challenges. OBJECTIVE: To establish evidence-based recommendations for diagnosing and prescribing vitamin D supplements ta

3D-DXA reveals significant effects of burosumab on trabecular and cortical skeletal envelopes in symptomatic adults with X-linked hypophosphatemia.

Fractures and pseudofractures cause considerable morbidity in adults with X-linked hypophosphatemia (XLH). They frequently occur in cortically-enriched bones of the lower extremities. Burosumab, a neutralizing antibody to FGF23, heals fractures in adults with XLH, presumably by healing osteomalacia. Histomorphometry has documented healing of osteomalacia in trabecular bone. The effects of burosumab on cortical bone have not been reported. Therefore, 3D-DXA measurements of the proximal femur were used to examine the impact of 1 yr of burosumab therapy on cortical and trabecular bone in symptoma

Failure of femoral internal fixation device secondary to phosphaturic mesenchymal tumor: a case report and literature review.

BACKGROUND: Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by mesenchymal tumors that secrete fibroblast growth factor 23 (FGF23). Phosphaturic mesenchymal tumor (PMT) is a novel histopathologic entity that has been identified as a separate cause of TIO. Clinically, PMT is typically diagnosed late due to its rarity. CASE PRESENTATION: The paper reports on a patient with a sustained left-sided sub-trochanteric femur fracture in 2018 who subsequently underwent fracture reduction and internal fixation placement. Six years postoperatively, the patient developed a recurr

A nomogram for predicting surgery outcome of tumor-induced osteomalacia: a case-control study.

UNLABELLED: This study presents a preoperative prediction model for tumor-induced osteomalacia (TIO) surgery outcomes on the basis of patient characteristics. The model, which was validated in 309 patients, identifies key risk factors and aids in clinical decision-making to optimize treatment strategies, reduce the number of unnecessary surgeries, and improve patient care. PURPOSE: Tumor-induced osteomalacia (TIO) should be curable by complete removal of the causative tumor. Knowledge of the prognosis of surgery is lacking. This study aimed to establish a prediction model that uses the preope

Biochemical osteomalacia in adults undergoing vitamin D testing in the North-East of Scotland.

BackgroundInternational guidelines give greatly varying definitions of 25-hydroxyvitamin D (25OHD) insufficiency and deficiency. Vitamin D testing is increasing despite 2016 UK guidance for adults advising routine vitamin D supplementation October-March and year-round for high risk groups. A service evaluation of vitamin D testing and biochemical osteomalacia in the North-East of Scotland (57-58°N) could inform definitions and testing guidance.MethodsWe identified adult 25OHD requests 8/7/2008-29/2/2020 and albumin-adjusted serum calcium (aCa), parathyroid hormone (PTH) and alkaline phosphatas

Daily or intermittent vitamin D supplementation in patients with or at risk of osteoporosis: Position statement from the GRIO.

Advantages and disadvantages of intermittent versus daily vitamin D supplementation especially in adults with or at risk of osteoporosis are discussed by the Osteoporosis Research and Information Group (GRIO). The analysis of the literature suggests that intermittent long-term high doses vitamin D supplementation (such as 60,000IU/month or more), may increase the risk of falls, fracture and premature death in certain populations, while daily doses of 800-1000IU with calcium decrease falls and non-vertebral fractures in the elderly with vitamin D deficiency. In patients with or at risk of osteo

Oncogenic rickets diagnosed at age 8 and the risk of persistent rickets: a rare case of pediatric-onset tumor-induced osteomalacia.

Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by excess fibroblast growth factor 23 (FGF23), leading to hypophosphatemia and osteomalacia. It typically manifests in adulthood, with pediatric cases being exceedingly rare. Early diagnosis is critical to prevent irreversible skeletal deformities. We report the case of a 19-year-old man with severe lower limb deformities and loss of ambulation since childhood. Initially misdiagnosed with X-linked hypophosphatemia (XLH) and treated with burosumab, genetic testing ruled out hereditary hypophosphatemic disorders. Further e

A deep intronic PHEX variant in a large Danish family with hereditary hypophosphatemia and a milder skeletal, but more severe dental phenotype.

Hereditary hypophosphatemia (HH) are rare diseases, characterized by excessive renal phosphate wasting, rickets and osteomalacia in growing children, and osteomalacia in adults. There are several types of HH caused by pathogenic variants in a number of different genes. We present a large Danish family consisting of 19 family members with clinically and biochemically hypophosphatamia caused by a previously unreported deep intronic splice variant in PHEX: c.1080-687 A > G (p.?). The affected family members displayed a milder HH phenotype compared with a larger group of individuals with other dis

Biochemical osteomalacia reaffirmed by signs and symptoms and perinatal outcome. A prospective cohort study of women in Sweden.

BACKGROUND: Vitamin D deficiency-induced osteomalacia remains underexplored, despite the substantial migration to northern sun-deprived latitudes. In women, osteomalacia may impair smooth and striated muscle function and disrupt the birth canal. This study aimed to investigate the associations and effect estimates of biochemical osteomalacia on perinatal outcomes. METHODS: A prospective cohort study was conducted to examine 71 Swedish and 52 Somali women during pregnancy and breastfeeding, addressing the heightened risk of severe vitamin D deficiency among Somali women. The baseline data comp

Burosumab treatment of tumour-induced osteomalacia from a rib lesion.

Tumour-induced osteomalacia (TIO) is a rare cause of acquired hypophosphataemia most often due to paraneoplastic overproduction of fibroblast growth factor-23 (FGF-23). A woman in her 60s presented with an 18 month history of insidious back, knee and ankle pain, and progressive weakness. Biochemistry revealed low serum phosphate and a bone scan showed diffuse bony lesions, initially suspected to be metastatic sarcoma given her history of uterine sarcoma. However, a positron emission tomography scan identified a solitary destructive lytic lesion in the right posterior second rib. Endocrine work

PHEX Protein in the Parathyroid Gland Contributes to Phosphate Sensing.

CONTEXT: Loss-of-function variants in the PHEX gene cause X-linked hypophosphatemia (XLH) with inappropriate secretion of fibroblast growth factor 23. The PHEX protein is therefore predicted to be involved in the phosphate (Pi)-sensing mechanism in mature osteocytes. The parathyroid glands sense short-term fluctuations in serum Pi levels and secrete PTH accordingly. However, the precise mechanisms for Pi sensing in the parathyroid gland have not been elucidated. OBJECTIVE: To clarify the involvement of PHEX in phosphate sensing in the parathyroid glands, PTH reactions after Pi loading were re
