Specification and Causal Pathway Diagramming of Implementation Strategies to Support Development of Access-Enhancing Injectable HIV Treatment Programmes at Eight Diverse Health Clinics Across the United States.

INTRODUCTION: Long-acting injectable antiretroviral therapy (LAI ART) represents a transformative development in HIV care because it may mitigate longstanding barriers that contribute to gaps in health outcomes. Providing LAI ART to people not consistently on HIV treatment or lacking treatment options requires intentional programme planning to overcome implementation barriers, without which only well-resourced patients benefit, worsening health outcome gaps. The ALAI UP Project delivered clinic-level implementation strategies to support eight clinics across Ending the HIV Epidemic jurisdiction

Factors perceived as relevant to starting antimicrobial treatment in patients with non-tuberculous mycobacterial lung disease in the Italian scenario: a repeated cross-sectional survey after three years of educational activities.

BACKGROUND: Non-tuberculous mycobacterial pulmonary disease (NTM-PD) is increasingly recognised, yet initiating antimycobacterial therapy remains challenging. Following a three-year period of national educational activity in Italy, we re-assessed which factors Italian clinicians perceive as relevant to the decision to start treatment. METHODS: We conducted a repeated cross-sectional web-based survey. The second wave was administered between November 2023 and February 2024 to members of the Italian SITA and IRENE networks, using the instrument administered in 2020. Participants rated the relev

LGBTQ+ inclusion in local authority public health: a review of needs assessments in England.

AIMS: A Joint Strategic Needs Assessment (JSNA) is the process through which health and wellbeing boards in England assess the needs of their local populations to identify inequalities in health and its wider determinants and inform both commissioning priorities and a Joint Local Health and Wellbeing Strategy. We sought to determine how and to what extent JSNAs address the needs of LGBTQ+ people in England. METHODS: The local authorities in receipt of a Public Health Grant in 2024 were identified and their websites searched for current (i.e. published in 2020 or later) JSNAs. Those which refe

Shifting neuropsychiatric diagnoses across the antiretroviral therapy era at an urban Ugandan HIV centre: a descriptive analysis of routinely collected electronic health records, 2017 to April 2026.

BACKGROUND: As antiretroviral therapy (ART) coverage has matured in sub-Saharan Africa, the clinical profile of people living with HIV has shifted from acute opportunistic disease toward chronic neuropsychiatric morbidity. Few high-volume African HIV programmes have systematically described the full neuropsychiatric diagnostic architecture captured in their routine electronic records. We characterised the neuropsychiatric diagnoses recorded at Mildmay Uganda, an urban HIV centre serving approximately 14,500 active patients. METHODS: We conducted a descriptive analysis of routinely collected,

A multi-phase project to develop a patient-reported measure of barriers to antiretroviral therapy adherence for use in HIV care: The 7-Item I-Score.

Adherence to antiretroviral therapy (ART) is a priority in HIV care. Our goal was to develop and validate a new, short patient-reported outcome measure to screen for barriers to daily oral ART adherence in HIV care in Canada and France. The overarching design was that of a multi-phase, multi-site instrument development project. It involved a previously published qualitative research synthesis (to develop a conceptual framework from which to generate items). Here, we present the results of cognitive interviews (for content validity testing of the preliminary items) and of a longitudinal survey-

Point-of-care early infant HIV diagnosis at birth in a pragmatic cluster-randomized trial in Mozambique and Tanzania: A comparative cost and cost-effectiveness study.

BACKGROUND: Timely access to early infant diagnosis (EID) is crucial for newborns with HIV, as late diagnosis can delay lifesaving antiretroviral treatment (ART). We assessed the comparative cost and cost-effectiveness of integrating point-of-care EID at birth into routine care in primary healthcare settings. METHODS AND FINDINGS: This pre-specified secondary analysis was nested in the cluster-randomized LIFE study conducted at 28 primary healthcare facilities in Mozambique and Tanzania from October 2019 to September 2021. We estimated the health system cost of point-of-care birth plus 4-8-we

Difficulties with clinical practice guidelines for congenital syphilis prevention in Colombia: a qualitative study.

BACKGROUND: Congenital syphilis (CS) is considered a preventable disease. However, it remains a major public-health concern in Colombia, where healthcare authorities have suggested that the main issue is the incorrect implementation of the clinical practice guidelines (CPG). This study aimed to understand why this preventable disease cannot be prevented in Chocó and Caldas, two regions of Colombia. METHODS: A qualitative online study, using a multi-method approach following interpretative and ethnographic online research principles, was conducted in two regions of Colombia (Chocó and Caldas) d

Towards ending tuberculosis in South Africa: an uncertainty analysis of the programmes and factors most critical to future declines.

BACKGROUND: The WHO End Tuberculosis (TB) strategy targets 80% and 90% reductions in TB incidence and mortality, respectively, between 2015 and 2030. OBJECTIVE: To assess which policy-changeable and external factors are most critical to reducing future TB in South Africa. METHODS: We adapted an existing mathematical model of TB and HIV in South Africa. Prior distributions were specified to represent uncertainty ranges for 27 model parameters that are highly uncertain and potentially important in driving future TB dynamics. Latin Hypercube Sampling was used to sample 1000 parameter combinatio

%diag_test: a generic SAS macro for evaluating diagnostic accuracy measures for multiple diagnostic tests.

BACKGROUND: Measures of diagnostic test accuracy provide evidence of how well a test correctly identifies or rules-out disease. Commonly used diagnostic accuracy measures (DAMs) include sensitivity and specificity, predictive values, likelihood ratios, area under the receiver operator characteristic curve (AUROC), area under precision-recall curves (AUPRC), diagnostic effectiveness (accuracy), disease prevalence, and diagnostic odds ratio (DOR) etc. Most available analysis tools perform accuracy testing for a single diagnostic test using summarized data. We developed a SAS macro for evaluating

Determinants of underweight among HIV-Infected children in pastoral communities of Southern Ethiopia: a facility-based cross-sectional study.

BACKGROUND: The effect of HIV infection on the nutritional status of patients has been widely known for several years, as well as the synergy between the infection and the conditions of poverty in developing countries. However, particular communities like pastoralists in underdeveloped countries live in a unique crossroad where challenges related to HIV prevention and control, and vulnerability to the disease exacerbate each other. Thus information from such range of contexts is valuable in addressing the complicated medical condition resulting from the combination of HIV infection and malnutr

Self-Efficacy and One-Year HIV Outcomes for Hospitalized People with HIV in Tanzania: A Mediation Analysis of the Daraja Clinical Trial.

Examining the causal pathways through which the Daraja intervention led to sustained improvement in HIV outcomes, including retention in HIV care, ART adherence, and viral load suppression, could highlight critical mechanisms for improving post-hospital outcomes in people with HIV. This pre-specified analysis of the Daraja trial assessed five mediators identified from the Gelberg-Andersen model (self-efficacy, social support, perceived need for HIV services, perceived stigma, and traditional HIV beliefs) and two additional potential mediators, depression and alcohol use. Mediators were quantif

Integrase versus protease inhibitor therapy in advanced HIV disease (LAPTOP): a multicountry, randomised, open-label, non-inferiority trial.

BACKGROUND: To date, clinical trials have been underpowered to assess which antiretrovirals perform best in people with advanced HIV disease. We aimed to investigate the efficacy and safety of an integrase inhibitor-containing versus a boosted protease inhibitor-containing regimen for this population. METHODS: In this open-label, multicentre, non-inferiority trial in seven European countries (Spain, France, Italy, Germany, Belgium, Ireland, and the UK), therapy-naive adults with advanced HIV disease were randomly allocated (1:1) to receive either bictegravir, emtricitabine, and tenofovir alaf

Does antibiotic therapy for anal abscess reduce the risk of fistula surgery? A retrospective study.

BACKGROUND: Management of first-time perianal abscesses remains controversial. While the French school of proctology systematically investigates fistulas, other approaches advocate simple incision, given that over 60 % of patients do not develop recurrence. The role of post-incision antibiotic therapy is also debated. This study aimed at evaluating our department's approach to managing first perianal abscesses and identifying predictive factors for recurrence. METHODS: We retrospectively included all patients presenting in 2019 with a first perianal abscess treated by incision under local ane
