Vaccine safety in HIV-infected adults within the Vaccine Safety Datalink Project.

OBJECTIVES: We evaluate safety of routine vaccination among adults infected with human immunodeficiency virus (HIV) in five healthcare organizations in the United States. METHODS: We conducted a retrospective cohort study of HIV-infected adults who received inactivated influenza vaccines, hepatitis B vaccines, pneumococcal vaccines, or tetanus, diphtheria, and acellular pertussis vaccines between 2002 and 2013. We conducted self-controlled case series analysis to estimate the relative risk (RR) for 11 pre-specified adverse events (AEs) requiring medical attention. RESULTS: Among 20,417 HIV-i

Adjunctive vitamin D in tuberculosis treatment: meta-analysis of individual participant data.

BACKGROUND: Randomised controlled trials of adjunctive vitamin D in pulmonary tuberculosis (TB) treatment have yielded conflicting results. Individual participant data meta-analysis could identify factors explaining this variation. METHODS: We meta-analysed individual participant data from randomised controlled trials of vitamin D in patients receiving antimicrobial therapy for pulmonary TB. Primary outcome was time to sputum culture conversion. Secondary outcomes were time to sputum smear conversion, mean 8-week weight and incidence of adverse events. Pre-specified subgroup analyses were don

Underascertainment, underreporting, representativeness and timeliness of the Iranian communicable disease surveillance system for tuberculosis.

OBJECTIVES: A well-functioning disease surveillance system is essential for effective control of diseases. Therefore, conducting evaluation studies on the performance of disease surveillance systems is necessary. This study was conducted to evaluate the performance of the Iranian syndrome-based surveillance system for tuberculosis (TB) in rural areas of Fars, the third largest province located in southern Iran. STUDY DESIGN: This was an evaluation study. METHODS: Two independent sources of information (data from a population-based survey and data from the surveillance system) were used in th

Generalizing the per-protocol treatment effect: The case of ACTG A5095.

BACKGROUND: Intention-to-treat comparisons of randomized trials provide asymptotically consistent estimators of the effect of treatment assignment, without regard to compliance. However, decision makers often wish to know the effect of a per-protocol comparison. Moreover, decision makers may also wish to know the effect of treatment assignment or treatment protocol in a user-specified target population other than the sample in which the trial was fielded. Here, we aimed to generalize results from the ACTG A5095 trial to the US recently HIV-diagnosed target population. METHODS: We first replic

Long-term topical corticosteroid use and risk of skin cancer: a systematic review.

OBJECTIVE: The objective of this systematic review was to synthesize available research evidence to determine the risk of skin cancer in patients with long-term use of topical corticosteroids (TCS). INTRODUCTION: Topical corticosteroids are one of the most commonly prescribed medicines in dermatology and the mainstay of the treatment of atopic dermatitis and other skin conditions such as psoriasis. They are often required for months or years to control the disease and ultimately restore patients' quality of life. In some patients, TCS may have a local immunosuppressive effect and theoreticall

Human Herpesvirus 8 and Lymphoproliferative Disorders.

The spectrum of lymphoproliferative disorders linked to human herpesvirus 8 (HHV-8) infection has constantly been increasing since the discovery of its first etiologic association with primary effusion lymphoma (PEL). PEL is a rapidly progressing non-Hodgkin's B-cell lymphoma that develops in body cavities in an effusional form. With the increase in the overall survival of PEL patients, as well as the introduction of HHV-8 surveillance in immunocompromised patients, the extracavitary, solid counterpart of PEL was later identified. Moreover, virtually all plasmablastic variants of multicentric

Treatment for HIV-associated cryptococcal meningitis.

BACKGROUND: Cryptococcal meningitis is a severe fungal infection that occurs primarily in the setting of advanced immunodeficiency and remains a major cause of HIV-related deaths worldwide. The best induction therapy to reduce mortality from HIV-associated cryptococcal meningitis is unclear, particularly in resource-limited settings where management of drug-related toxicities associated with more potent antifungal drugs is a challenge. OBJECTIVES: To evaluate the best induction therapy to reduce mortality from HIV-associated cryptococcal meningitis; to compare side effect profiles of differen

HIV/AIDS in South Africa: how many people died from the disease between 1997 and 2010?

OBJECTIVES: Empirical estimates of the number of HIV/AIDS deaths are important for planning, budgeting, and calibrating models. However, there is an extensive misattribution of HIV/AIDS as an underlying cause-of-death. This study estimates the true numbers of AIDS deaths from South African vital statistics between 1997 and 2010. METHODS: Individual-level cause-of-death data were grouped according to a local burden of disease list and source causes (i.e. causes under which AIDS deaths are misclassified) that recorded a rapid increase. After adjusting for completeness of registration, the morta

The safety of subcutaneously administered depot medroxyprogesterone acetate (104mg/0.65mL): A systematic review.

CONTEXT: Depot medroxyprogesterone acetate (DMPA), a progestogen-only contraceptive injectable, has traditionally been formulated as a crystalline suspension delivered intramuscularly (IM) at a dose of 150mg/1.0mL. A new, lower dose formulation of DMPA (104mg/0.65mL) has been developed for subcutaneous administration (SC). Given its increasing global availability and public health relevance, DMPA-SC was prioritized for inclusion as a new method referenced in the World Health Organization (WHO) Medical Eligibility Criteria for Contraceptive Use (MEC), 5th Edition. OBJECTIVE: This systematic re

Extended microbiological characterization of Göttingen minipigs: porcine cytomegalovirus and other viruses.

BACKGROUND: To prevent transmission of zoonotic microorganisms from pig transplants to human recipients when performing xenotransplantation using pig cells, tissues, or organs, donor pigs have to be carefully characterized. Göttingen minipigs (GöMP) are often used for various biomedical investigations and are well characterized concerning the presence of numerous bacteria, fungi, viruses, and parasites. Recently, we studied the prevalence and expression of porcine endogenous retroviruses and the prevalence of hepatitis E virus (HEV) in GöMP. Here, we studied the presence of the porcine cytomeg

The per-protocol effect of immediate versus deferred antiretroviral therapy initiation.

OBJECTIVE: The Strategic Timing of AntiRetroviral Treatment (START) trial found a lower risk of a composite clinical outcome in HIV-positive individuals assigned to immediate initiation of antiretroviral therapy (ART) compared with those assigned to deferred initiation. However, 30% of those assigned to deferred initiation started ART earlier than the protocol specified. To supplement the published intention-to-treat (ITT) effect estimates, here we estimate the per-protocol effect of immediate versus deferred ART initiation in START. DESIGN: The START trial randomized 4685 HIV-positive partic

Identifying optimal biomarker combinations for treatment selection through randomized controlled trials.

BACKGROUND/AIMS: Biomarkers associated with treatment-effect heterogeneity can be used to make treatment recommendations that optimize individual clinical outcomes. To accomplish this, statistical methods are needed to generate marker-based treatment-selection rules that can most effectively reduce the population burden due to disease and treatment. Compared to the standard approach of risk modeling to derive treatment-selection rules, a more robust approach is to directly minimize an unbiased estimate of total disease and treatment burden among a pre-specified class of rules. This problem is

Effects of cash transfers on Children's health and social protection in Sub-Saharan Africa: differences in outcomes based on orphan status and household assets.

BACKGROUND: Unconditional and conditional cash transfer programmes (UCT and CCT) show potential to improve the well-being of orphans and other children made vulnerable by HIV/AIDS (OVC). We address the gap in current understanding about the extent to which household-based cash transfers differentially impact individual children's outcomes, according to risk or protective factors such as orphan status and household assets. METHODS: Data were obtained from a cluster-randomised controlled trial in eastern Zimbabwe, with random assignment to three study arms - UCT, CCT or control. The sample incl

Factor Configurations with Governance as Conditions for Low HIV/AIDS Prevalence in HIV/AIDS Recipient Countries: Fuzzy-set Analysis.

This paper aims to investigate whether good governance of a recipient country is a necessary condition and what combinations of factors including governance factor are sufficient for low prevalence of HIV/AIDS in HIV/AIDS aid recipient countries during the period of 2002-2010. For this, Fuzzy-set Qualitative Comparative Analysis (QCA) was used. Nine potential attributes for a causal configuration for low HIV/AIDS prevalence were identified through a review of previous studies. For each factor, full membership, full non-membership, and crossover point were specified using both author's knowledg

Adjudicated morbidity and mortality outcomes by age among individuals with HIV infection on suppressive antiretroviral therapy.

BACKGROUND: Non-AIDS conditions such as cardiovascular disease and non-AIDS defining cancers dominate causes of morbidity and mortality among persons with HIV on suppressive combination antiretroviral therapy. Accurate estimates of disease incidence and of risk factors for these conditions are important in planning preventative efforts. METHODS: With use of medical records, serious non-AIDS events, AIDS events, and causes of death were adjudicated using pre-specified criteria by an Endpoint Review Committee in two large international trials. Rates of serious non-AIDS which include cardiovascu

Estimating HIV incidence from case-report data: method and an application in Colombia.

OBJECTIVE: Quantifying HIV incidence is essential for tracking epidemics but doing this in concentrated epidemic can be a particular challenge because of limited consistent high-quality data about the size, behaviour and prevalence of HIV among key populations. Here, we examine a method for estimating HIV incidence from routinely collected case-reporting data. METHODS: A flexible model of HIV infection, diagnosis and survival is constructed and fit to time-series data on the number of reported cases in a Bayesian framework. The time trend in the hazard of infection is specified by a penalized

Incorporating incidence information within the UNAIDS Estimation and Projection Package framework: a study based on simulated incidence assay data.

OBJECTIVES: The objective of this study is to extend the UNAIDS incidence estimation model, the UNAIDS Estimation and Projection Package (EPP), so that it can incorporate data from incidence assays. METHODS: We propose combining the likelihood of the incidence assay data with the likelihood of other data, in a manner that is consistent with the biomarker-based incidence estimator using incidence assay data. Two calibrating parameters specify the performance of the incidence assays: the false recent rate and the mean duration of recent infection. We then use synthetic data, based on prevalence
