PURPOSE: Phosphaturic mesenchymal tumors (PMTs) are exceptionally rare neoplasms that secrete fibroblast growth factor 23 (FGF23), leading to tumor-induced osteomalacia (TIO). Owing to their nonspecific clinical manifestations and deceptively bland morphology, diagnosis is frequently delayed unless there is a high index of clinical suspicion or recognition of characteristic histopathological features. While PMTs predominantly arise in the appendicular skeleton, involvement of the sinonasal tract is uncommon, and pediatric sinonasal PMTs have not been reported previously. Although most PMTs fol

Hypophosphatemic osteomalacia is a rare metabolic bone disease in adults, most commonly caused by acquired conditions. Clinically, it may present with widespread musculoskeletal pain, proximal muscle weakness, and gait disturbance, often mimicking rheumatologic disorders. Tenofovir disoproxil fumarate, a widely used nucleotide reverse transcriptase inhibitor in the treatment of human immunodeficiency virus infection, may rarely cause hypophosphatemia and osteomalacia due to proximal renal tubular dysfunction. We report a 33-year-old male patient who presented with progressively worsening diffu

A better understanding of the benefit of burosumab for treating tumor-induced osteomalacia (TIO) in Chinese patients is needed. The objective of this open-label, multicenter, single-cohort, post-marketing Phase 4 study was to assess the efficacy, pharmacokinetics, pharmacodynamics, and safety of burosumab after repeated administration in Chinese adults with TIO. Burosumab was administered subcutaneously every 4 weeks for up to 48 weeks at an initial 0.3 mg/kg dose. We investigated the change from baseline in mean serum phosphorus level over time, and changes in serum and urinary phosphorus lev

Hypophosphatasia (HPP) is a rare metabolic disorder caused by loss-of-function ALPL variants and characterized by heterogeneous skeletal and extra-skeletal manifestations. Although recent diagnostic criteria include ALPL variants, the diagnosis can be made based solely on clinical features. In adult-onset HPP, however, symptoms are often mild or nonspecific, including musculoskeletal symptoms and fatigue, making it difficult to differentiate from other conditions. This retrospective, cross-sectional study aimed to integrate genetic and biochemical data to enhance the clinical interpretation of

BACKGROUND: Type 2 diabetes is a major and growing public health challenge worldwide, contributing substantially to premature mortality and disability. Increasing evidence suggests that long-term exposure to air pollution, plays an important role in the development and progression of type 2 diabetes. Countries with high levels of ambient air pollution may therefore experience a disproportionate diabetes burden attributable to environmental exposure. In Iran, where both diabetes prevalence and air pollution levels are high, national- and subnational-level evidence on the burden of type 2 diabet

A ⁶⁸Ga-DOTATATE PET/CT scan was performed on a 40-year-old woman with suspected tumor-induced osteomalacia (TIO), who presented with progressive bone pain, multiple fractures, and hypophosphatemia. The scan revealed a solitary focus of intense somatostatin receptor avidity within the right lower abdominal wall musculature. No other suspicious lesions were identified. On the basis of this finding, the tumor was surgically resected, and histopathology confirmed a phosphaturic mesenchymal tumor (PMT). Postoperatively, the patient's serum phosphate levels normalized. This case underscores the para

Hypophosphatasia (HPP) is the rare metabolic disorder caused by variants in the ALPL gene, resulting in deficient activity of tissue-nonspecific alkaline phosphatase (ALP). This leads to accumulation of substrates contributing to impaired bone mineralization. Hypophosphatasia manifests with a broad clinical spectrum; however, an increasing number of individuals with ALPL variants have been identified presenting the hallmark biochemical feature of HPP of low serum ALP activity, with or without elevated serum pyridoxal-5-phosphate (PLP) or urine phosphoethanolamine (PEA), while remaining asympto

Autosomal recessive hypophosphatemic rickets type 1 (ARHR1) is a rare, inherited cause of fibroblast growth factor 23 (FGF23)-mediated renal phosphate-wasting that results from homozygous inactivating pathogenic variants in the dentin matrix acidic phosphoprotein-1 (DMP1) gene. A variable clinical spectrum has been described in this condition, including features of rickets (in children), hyperostotic osteomalacia (in children and adults), bone pain, short stature, lower limb deformities, recurrent dental abscesses, and hearing defects. The purpose of this report is to describe the clinical evo

CONTEXT: Cutaneous skeletal hypophosphatemia syndrome (CSHS) is an ultrarare disorder defined by epidermal and/or melanocytic nevi, mosaic skeletal dysplasia, and FGF23-mediated hypophosphatemia. As in other FGF23-mediated hypophosphatemia conditions, individuals with CSHS have renal phosphate wasting and inappropriately normal or frankly low 1,25-dihydroxyvitamin D levels with resultant hypophosphatemia leading to rickets and osteomalacia. Conventional therapy for FGF23-mediated hypophosphatemia consists of multiple daily doses of oral phosphate and active vitamin D.
OBJECTIVE: Burosumab is

PURPOSE: Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by FGF23-secreting tumors, resulting in renal phosphate wasting and hypophosphatemic osteomalacia. Low bone mineral density (BMD) in TIO frequently leads to misdiagnosis as the more prevalent primary osteoporosis (POP) and subsequent diagnostic delay. This study characterizes the distinct biochemical profile of TIO and proposes a stepwise diagnostic approach to differentiate it from POP for timely clinical identification and appropriate management.
METHODS: We conducted a retrospective comparative analysis of 1

BackgroundInternational guidelines give greatly varying definitions of 25-hydroxyvitamin D (25OHD) insufficiency and deficiency. Vitamin D testing is increasing despite 2016 UK guidance for adults advising routine vitamin D supplementation October-March and year-round for high risk groups. A service evaluation of vitamin D testing and biochemical osteomalacia in the North-East of Scotland (57-58°N) could inform definitions and testing guidance.MethodsWe identified adult 25OHD requests 8/7/2008-29/2/2020 and albumin-adjusted serum calcium (aCa), parathyroid hormone (PTH) and alkaline phosphatas

We report a case of a primary cutaneous spindle cell sarcoma (SCS) with FN1::FGFR1 fusion. The tumor lacked the typical histologic and immunohistochemical features associated with other FN1-rearranged neoplasms, such as phosphaturic mesenchymal tumors (PMT) and calcified chondroid mesenchymal neoplasms (CCMN). Unlike PMTs, which often feature a cartilaginous matrix and are associated with tumor-induced osteomalacia (TIO), the present case lacked these characteristics and did not show FGF23 mRNA expression. Immunohistochemically, the tumor cells showed patchy staining for CD34 but were negative

Advantages and disadvantages of intermittent versus daily vitamin D supplementation especially in adults with or at risk of osteoporosis are discussed by the Osteoporosis Research and Information Group (GRIO). The analysis of the literature suggests that intermittent long-term high doses vitamin D supplementation (such as 60,000IU/month or more), may increase the risk of falls, fracture and premature death in certain populations, while daily doses of 800-1000IU with calcium decrease falls and non-vertebral fractures in the elderly with vitamin D deficiency. In patients with or at risk of osteo

BACKGROUND AND OBJECTIVE: Predicting overall survival (OS) for advanced gastric cancer patients after radiotherapy is critical for developing an individualized treatment plan. However, existing studies have focused on gastric cancer CT images with a large amount of redundant information, neglecting the role of physicians' prior knowledge in guiding gastric cancer CT image information. We propose a multimodal fusion method based on prior knowledge to predict OS after radiotherapy in advanced gastric cancer patients to assist physicians in clinical diagnosis and treatment.
METHODS: A prior know

Background: Phosphaturic mesenchymal tumours secreting fibroblast growth factor 23 (hereinafter referred to as FGF23+ PMT) are rare neoplasms that can cause hypophosphataemic osteomalacia, owing to excessive FGF23 production. Mast cells (MCs) play a key role in tumour biology by modulating proliferative activity of atypical cells, resistance to innate and acquired immunity, angiogenesis, and metastatic behaviour. However, MCs associated with FGF23+ PMT have not previously been investigated. This study, to our knowledge, is the first to characterise features of the tumour microenvironment throu

UNLABELLED: Indolent systemic mastocytosis (ISM) is a rare cause of skeletal fragility. In this study, patients with ISM showed a markedly higher prevalence of biconcave vertebral fractures compared with primary osteoporosis. Assessing fracture morphology may aid in differentiating ISM, reducing diagnostic delay, and guiding appropriate management.
PURPOSE: Indolent systemic mastocytosis (ISM) is a rare disorder frequently associated with bone fragility and vertebral fractures (VFx). While the morphology of VFx has been studied in other metabolic bone diseases (e.g., osteomalacia), its role i

Tubular maximum phosphate reabsorption per glomerular filtration rate (TmP/GFR) is a key diagnostic test for renal phosphate wasting. However, there is a lack of international consensus regarding the reporting of age-related reference intervals. The TmP/GFR Harmonization Working Group, formed by The Australasian Association for Clinical Biochemistry (AACB) and Laboratory Medicine and The Australian and New Zealand Bone and Mineral Society (ANZBMS), aimed to evaluate analytical differences amongst commercial creatinine and phosphate assays, harmonize age-specific cut-offs and compare the ANZBMS

INTRODUCTION: Fanconi syndrome is a disorder characterized by defective proximal renal tubular reabsorption, resulting in the wasting of glucose, phosphate, bicarbonate, amino acids, and uric acid. While most common in childhood and usually secondary to other conditions in adults, idiopathic cases with adult onset are rare. Skeletal complications such as osteomalacia and fractures may result from chronic phosphate loss and can be the initial presentation in adult cases.
CASE REPORT: A 35-year-old previously healthy man presented with chronic back pain and bilateral hip pain, later diagnosed a
