Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by excess fibroblast growth factor 23 (FGF23), leading to hypophosphatemia and osteomalacia. It typically manifests in adulthood, with pediatric cases being exceedingly rare. Early diagnosis is critical to prevent irreversible skeletal deformities. We report the case of a 19-year-old man with severe lower limb deformities and loss of ambulation since childhood. Initially misdiagnosed with X-linked hypophosphatemia (XLH) and treated with burosumab, genetic testing ruled out hereditary hypophosphatemic disorders. Further e

Hereditary hypophosphatemia (HH) are rare diseases, characterized by excessive renal phosphate wasting, rickets and osteomalacia in growing children, and osteomalacia in adults. There are several types of HH caused by pathogenic variants in a number of different genes. We present a large Danish family consisting of 19 family members with clinically and biochemically hypophosphatamia caused by a previously unreported deep intronic splice variant in PHEX: c.1080-687 A > G (p.?). The affected family members displayed a milder HH phenotype compared with a larger group of individuals with other dis

Tumour-induced osteomalacia (TIO) is a rare cause of acquired hypophosphataemia most often due to paraneoplastic overproduction of fibroblast growth factor-23 (FGF-23). A woman in her 60s presented with an 18 month history of insidious back, knee and ankle pain, and progressive weakness. Biochemistry revealed low serum phosphate and a bone scan showed diffuse bony lesions, initially suspected to be metastatic sarcoma given her history of uterine sarcoma. However, a positron emission tomography scan identified a solitary destructive lytic lesion in the right posterior second rib. Endocrine work

Excess fibroblast growth factor 23 (FGF23), a mature osteocyte-derived phosphaturic hormone, causes chronic hypophosphatemic osteomalacia in adults. This rare condition was recently reported in 2 alcoholic patients, with marked improvement upon cessation of alcohol consumption, suggesting a link between alcohol and FGF23-related hypophosphatemia within the highly limited cases. This study aimed to investigate whether the source of excess FGF23 in alcohol-induced FGF23-related hypophosphatemic osteomalacia is the bone or the other organs. To achieve this goal, an immunohistochemical approach fo

PURPOSE: X-linked hypophosphatemia (XLH) is a rare, genetic, progressive, lifelong disorder caused by pathogenic variants in the PHEX gene, leading to excess fibroblast growth factor 23 (FGF23) and renal phosphate wasting. Advancing Patient Evidence in XLH (APEX) is a 10-year global data unification project that combines 3 regional observational studies (XLH Disease Monitoring Program [DMP], International XLH Registry [IXLHR], and SUNFLOWER). APEX aims to describe the burden and lifelong progression of XLH, to collect real-world data on treatment effectiveness and safety, and to investigate re

OBJECTIVE: Although guidelines recommend targeted vitamin D testing for high-risk populations, testing has increased globally. Limited studies have examined real-world testing patterns and their relationship with deficiency outcomes. This study investigates trends, demographic determinants and deficiency outcomes associated with voluntary vitamin D testing among Taiwanese adults.
DESIGN: A retrospective cohort study analysing electronic medical records to assess vitamin D testing trends, demographic predictors of deficiency and status changes following consecutive tests within 2 years. Vitami

PURPOSE: Hypophosphatasia (HPP) is a rare, dento-osseous disorder caused by impaired activity of tissue non-specific alkaline phosphatase (TNSALP), a key enzyme in tissue mineralization. This review provides a clinical perspective on the current medical treatment of both children and adults with HPP.
RECENT FINDINGS: Dental problems, rickets in children, and osteomalacia in adults are common in HPP. However, disease manifestations in individual patients are exceptionally variable. Recent studies broadened our understanding of HPP symptoms. For example, data showed behavioral health challenges

PURPOSE OF REVIEW: In this review, we will examine the pathophysiology, anatomy, biochemistry, and genotype-phenotype correlation of femoral fractures in adult hypophosphatasia.
RECENT FINDINGS: Hypophosphatasia (HPP) is a rare genetic disease characterized by low activity of tissue-nonspecific alkaline phosphatase (TNAP). The disease presents a broad spectrum of clinical manifestations primarily determined by the degree of residual TNAP activity. Adults with HPP of moderate clinical severity may present with spontaneous femoral fractures that are like the atypical femoral fractures (AFF) of

Rare diseases, defined by the 2002 Rare Disease Act, affect fewer than 5 in 10,000 individuals. Rare metabolic bone diseases (MBDs), such as osteogenesis imperfecta, hypophosphatasia, osteopetrosis, and other unclassified disorders, can disrupt bone development and remodeling, posing diagnostic and management challenges. This study analyzed data from the rarembd.in registry (2010-2024), a 15-year database documenting only rare MBDs. Clinical presentation and demographic data of patients with rare MBDs were collated. Common MBDs (osteoporosis, primary hyperparathyroidism) were excluded. Genetic

Burosumab, a human immunoglobulin G1 monoclonal antibody that targets fibroblast growth factor 23 (FGF23), was developed to treat X-linked hypophosphatemic rickets/osteomalacia (XLH). By binding to FGF23, burosumab promotes the reabsorption of phosphorus from the kidney, increases the production of active vitamin D, and promotes the intestinal absorption of phosphorus and calcium. Herein, we report a one-year evaluation of physical function in adult patients with XLH who were treated using burosumab. A 40-year-old male patient with a history of lower limb varus deformity was diagnosed with vit

Tumor-induced osteomalacia (TIO) is a rare acquired paraneoplastic syndrome caused by a mesenchymal tumor secreting a phosphaturic hormone called FGF23. Patients present with bone pain, fragility fractures and muscle weakness. Biochemical results show hypophosphatemia, raised serum alkaline phosphatase and reduced calcitriol. We report the case of a 44-year-old man who presented to the Emergency Departement with acute low back pain revealing extensive subchondral fractures between D2 and L5. Investigations showed partial Fanconi syndrome; nevertheless, he had profound hypophosphatemia, low 1,2

BACKGROUND: Due to its occult position, complex anatomical structure, and spatial relationships, the causative tumor of Tumor-Induced Osteomalacia (TIO) in the hip region is quite difficult to detect and qualitatively diagnose in clinical practice. In this regard, clinicians often lack sufficient knowledge about such tumors, leading to frequent missed diagnoses, misdiagnoses, and unreasonable treatment.
OBJECTIVE: This study aimed to investigate the clinical characteristics of TIO patients with culprit soft tissue tumors in the hip region and evaluate the effect of surgical treatment on these

We report two bone-forming tumors driven by a newly recognized fusion. The first is a lesion in the distal radius of a 59-year-old female subject, which had been present for 10 years. Imaging showed an intramedullary tumor with an exophytic dorsal component and marked sclerosis, histologically best classified as low-grade osteosarcoma. The second was a destructive lesion in the left ulna of a 13-year-old female with a short history of elbow pain. Imaging showed a lytic lesion with subtle tumor ossification, histologically best classified as an atypical osteoblastoma-like tumor. A NIPBL::BEND2

OBJECTIVES: Rare metabolic bone diseases can present with symptoms mimicking more common rheumatological conditions including spondyloarthritis, osteoarthritis, and fibromyalgia. Increasing awareness of these rare diseases within the rheumatology community is vital to ensure that affected patients are diagnosed and appropriately treated. The literature includes several reports of tumour-induced osteomalacia initially diagnosed as rheumatic disease, but other rare diseases such as X-linked hypophosphatemia (XLH) and hypophosphatasia (HPP) also deserve attention. Here, we describe two cases of a

BACKGROUND Phosphaturic mesenchymal tumor (PMT) is an extremely rare mesenchymal neoplasm that is commonly seen in bone and soft tissue. It is associated with a paraneoplastic syndrome, oncogenic osteomalacia, due to tumor-induced urinary phosphate wasting. It is demonstrated to be predominantly mediated by fibroblast growth factor 23 (FGF23)/fibroblast growth factor receptor 1 (FGFR1) axis. Clinically, PMT usually presents as a solitary lesion in the bone. The diagnosis of PMT is challenging due to its non-specific clinical manifestation, radiologic findings, and morphological features. Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome characterized by hypophosphatemia, bone mineralization disorders with increased risk of fragility fractures, muscle pain, and progressive weakness. TIO has been associated with increased production of the phosphaturic hormone Fibroblast Growth Factor 23 (FGF23) usually by mesenchymal tumors of soft tissue or bone (Phosphaturic Mesenchymal Tumors-PMTs). In rare cases TIO may be observed in association with other malignancies. We report the case of a 66-year-old woman with an occasional diagnosis of both a PMT and an ovarian canc

PURPOSE: Skeletal fluorosis (SF) results from chronic exposure to fluoride (F-) causing excessive aberrantly mineralized brittle bone tissue, fractures, and exostoses. There is no established treatment other than avoiding the source of F-. Still, excess F- can persist in bone for decades after exposure ceases.
CASE PRESENTATION: A 50-year-old woman presented with multiple, recurrent, low AQ2 trauma fractures yet high radiologic bone mineral density. Serum F- was elevated, and osteomalacia was documented by non-decalcified transiliac biopsy. She reported intermittently "huffing" a keyboard cle
