Non-union of Bone Plating in a Mandibular Fracture With Osteomyelitis: A Case Report.

Mandibular fractures are a common occurrence, often associated with complex complications such as non-union and osteomyelitis. Non-union, the failure of a fracture to heal, can be particularly challenging in the mandible due to the limited soft tissue coverage and compromised blood supply in atrophic areas. Additionally, the presence of osteomyelitis, a bone infection, further complicates the healing process and increases the risk of implant failure. This case report aims to provide an overview of the management strategies for non-union of bone plating with mandibular fracture associated with

Mechanically stable but biologically inactive Jones fracture nonunion managed without immediate revision surgery: a case report.

BACKGROUND: Jones fractures are prone to delayed union and nonunion because of the vascular characteristics and mechanical loading environment of the fifth metatarsal metaphyseal-diaphyseal junction. Although nonunion is often attributed to mechanical insufficiency, host-related biological and functional factors may also be clinically relevant in selected cases. We report an illustrative case of Jones fracture nonunion with maintained implant integrity and radiographic stability that united without immediate revision surgery after integrated clinical assessment and management. Early clinical and radiographic outcomes of robot-assisted femoral neck system fixation combined with an additional cannulated compression screw for garden IV femoral neck fractures in young and middle-aged adults: a propensity score-matched retrospective cohort study.

BACKGROUND: Garden IV femoral neck fractures in young and middle-aged adults are highly unstable and prone to shortening, nonunion, and femoral head necrosis. We compared robot-assisted femoral neck system (FNS) fixation plus an additional cannulated compression screw (CCS) in a multiplanar configuration with robot-assisted FNS fixation alone. METHODS: This retrospective cohort included 172 patients aged 18-60 years who were treated from June 2024 to June 2025; 82 underwent robot-assisted FNS plus CCS fixation and 90 underwent robot-assisted FNS fixation alone. After 1:1 nearest-neighbor prop

Distal Ulna Hook Locking Plate Fixation for Unstable Distal Ulna Fractures: A Prospective Clinical Study.

BACKGROUND: Distal ulna fractures, particularly when displaced or unstable, play a critical role in the stability and function of the distal radioulnar joint. Inadequate treatment may result in persistent pain, instability, and impaired wrist function. Recent advances in angular stable fixation have led to the development of anatomically contoured distal ulna hook locking plates; however, clinical data regarding their outcomes remain limited. OBJECTIVE: To evaluate the radiological and functional outcomes of distal ulna fractures treated with open reduction and internal fixation using a dista

External validation of the FRACTING, LEG-NUI, and NURD scores for predicting non-union after tibial intramedullary nailing.

BACKGROUND: Non-union remains a challenging complication of tibial diaphyseal fractures treated with intramedullary nailing. Several clinical prediction scores have been developed to identify patients at increased risk of impaired fracture healing. This study aimed to externally validate the FRACTING, LEG-NUI, and NURD scores for the prediction of non-union in an independent cohort of patients with tibial diaphyseal fractures. METHODS: We retrospectively reviewed consecutive adult patients with tibial diaphyseal fractures treated with reamed intramedullary nailing at a level I trauma center b

Critical-sized bone defects: essential concepts for the higher orthopaedic examination.

PURPOSE: To provide a practical review of the assessment and management of critical-sized bone defects (CSDs) for candidates preparing for higher orthopaedic examinations. METHODS: This narrative review considers definitions and classification, clinical assessment, acute management, reconstructive techniques and salvage options. RESULTS: A critical-sized bone defect (CSD) is unlikely to heal without intervention because of its size and location, although no numerical threshold defines criticality. Assessment should consider skeletal stability, infection, vascularity, soft tissues, host facto

Percutaneous joint preparation for tibiotalocalcaneal arthrodesis: a case series.

PURPOSE: Tibiotalocalcaneal (TTC) arthrodesis is a limb-salvaging procedure for end-stage ankle and hindfoot pathology but carries substantial risks of nonunion and complications. Minimally invasive approaches may reduce surgical morbidity while maintaining fusion outcomes. METHODS: This was a retrospective case series of patients who underwent TTC arthrodesis with percutaneous joint preparation between January 2020 and March 2025 at a single academic Level II trauma center. The primary outcome was radiographic or clinical fusion at final follow-up. Secondary outcomes included alignment, oper

Ballistic femoral neck fractures: Current concepts in evaluation and management.

Ballistic femoral neck fractures are complex, high-energy injuries that require a management approach distinct from conventional blunt trauma. These current concepts review examines the evolving landscape of evaluation and treatment for these rare injuries based on contemporary literature. Initial management emphasizes the Advanced Trauma Life Support (ATLS) protocol, with a critical focus on identifying associated neurovascular and skeletal injuries, which are present in approximately one-third of cases. Diagnostic evaluation relies on plain radiography and thin-slice CT to delineate fracture

Mesenchymal stromal cells in polytrauma: from immune dysregulation to therapeutic potential.

Polytrauma, defined as life-threatening injuries to multiple organ systems with an Injury Severity Score greater than 16, presents significant challenges for fracture healing, with nonunion rates substantially higher than in isolated fractures. The systemic inflammatory response syndrome (SIRS) and subsequent immune dysregulation characteristic of polytrauma create a hostile microenvironment that impairs normal bone regeneration. Mesenchymal stromal cells (MSCs; also termed mesenchymal stem/stromal cells) play critical roles in fracture healing through immunomodulation, osteogenic differentiat

Retrospective Cohort Study Comparing Rates of Symptomatic Implants Requiring Removal between Standard versus Low-Profile Tibial Nail Interlocking Screws.

This study aimed to compare the rates of symptomatic implants requiring removal between intramedullary tibia nails using standard (SD) versus low-profile (LP) screws. We hypothesize that LP screws will demonstrate lower rates of symptomatic screw removal compared with SD interlocking screws with no difference in secondary outcomes. We performed a retrospective cohort study of all patients 18-89 years old who underwent IMN of tibia fractures from November 2018 to February 2023 at a single, academic level one trauma center. The primary outcome analyzed was symptomatic screws requiring removal. S

Clinical Outcomes of Single Intramedullary K-Wire Fixation with Figure-of-Eight Suture Augmentation for Ulnar Styloid Base and Distal Ulnar Metaphyseal Fractures.

Background/Objectives: The optimal fixation method for ulnar styloid base fractures remains controversial. Conventional dual K-wire fixation may be technically challenging in certain fracture configurations, whereas hook plate fixation provides rigid fixation but requires additional implant placement and surgical exposure. The purpose of this study was to evaluate the clinical and radiographic outcomes of single intramedullary K-wire fixation combined with figure-of-eight suture augmentation for ulnar styloid base and distal ulnar metaphyseal fractures. Methods: We retrospectively reviewed 60

Ilizarov Circular Fixation for High-Velocity Firearm Injuries Causing High-Energy Tibial Fractures: A Prospective Cohort Study.

High-velocity gunshot tibial fractures present major reconstructive challenges owing to extensive contamination, soft-tissue devitalisation, and segmental bone loss that render early internal fixation hazardous. We aimed to document outcomes of a structured Ilizarov-based protocol for this injury type and describe a reproducible treatment framework for resource-constrained settings. Between January 2019 and December 2023, we prospectively enrolled 20 adult male patients with open tibial fractures from high-velocity firearm injuries at three Cairo trauma centres; all were treated definitively w

Biodegradable pins fixation for lateral condylar fracture of the humerus in children: A retrospective study of biodegradable pin fixation combined with versus without absorbable tension band wiring.

Biodegradable pins are increasingly used in the treatment of lateral condylar fractures of the humerus (LCFH) in children, but the strength of the biodegradable pin is weaker than that of K-wire. However, to enhance the stability of LCFH fixed by biodegradable pins, absorbable tension band wiring is often combined with the application, yet high-quality comparative evidence verifying the clinical value of this auxiliary reinforcement remains lacking. This study aimed to compare and analyze whether auxiliary absorbable tension band wiring has clinical significance for Weiss type III LCFH and to
