The Association Between Socioeconomic Factors and the Clinical Course of Nonoperatively Treated Pediatric Scaphoid Fractures.

BACKGROUND: There is a higher incidence of scaphoid fractures in children with lower socioeconomic status (SES), who may be more likely to underutilize health care resources. This study investigated the association between socioeconomic factors and the clinical course of nonoperatively treated pediatric scaphoid fractures. We hypothesized that lower SES would be associated with delayed and prolonged treatment. METHODS: This retrospective study included patients <21 years old who presented to a single institution with an acute scaphoid fracture between 2012 and 2024. Demographic and clinical i

Chaenomeles speciosa-derived extracellular vesicles accelerate fracture healing via orchestrating immuno-angiogenic coupling.

PURPOSE: Fracture nonunion remains a major orthopaedic challenge, and dysregulation of the bone immune microenvironment is increasingly recognized as a key pathological driver. Chaenomeles speciosa is a medicinal plant with documented anti-inflammatory properties, making it a promising candidate for immune-modulatory therapy. This study investigated whether extracellular vesicles derived from Chaenomeles speciosa could reshape the immune microenvironment to promote fracture healing. METHODS: Single-cell RNA sequencing was performed on human nonunion tissues to characterize cellular heterogene

Isolated Subtalar Arthrodesis.

Isolated subtalar arthrodesis is a common surgical procedure used to treat subtalar joint pathologies including primary and post-traumatic arthritis, calcaneal fractures, tarsal coalitions, and posterior tibial tendon dysfunction. Its goals are pain relief, rearfoot realignment, and improved stability while preserving more motion than triple arthrodesis. This review covers the relevant anatomy of the subtalar joint and its facets, clinical examination findings, and imaging modalities. It details surgical techniques for native fusions, neglected calcaneal fractures, and rigid valgus deformities

Burosumab as bridging therapy in autosomal dominant hypophosphatemic rickets with femoral fracture nonunion and iron deficiency.

Autosomal dominant hypophosphatemic rickets (ADHR) is a rare genetic disorder, characterized by fibroblast growth factor 23 (FGF23)-mediated renal phosphate wasting and impaired bone mineralization. Iron deficiency in ADHR increases FGF23 production and can worsen hypophosphatemia. Evidence to guide optimal management in severe adult cases remains limited. We report the use of burosumab as bridging therapy in a 19-yr-old woman with genetically confirmed ADHR (FGF23 p.Arg176Trp), severe osteomalacia, iron-deficiency anemia, and a low-energy femoral diaphyseal fracture complicated by nonunion de

Autologous Bone Peg Grafting for Distal Phalanx Nonunion: A Report of Two Cases.

Distal phalangeal nonunion is a rare but challenging condition that may result from crush injuries or inadequate stabilization. Surgical fixation is often difficult because of the small size of the fragment and surrounding soft tissue damage. We report two cases of symptomatic distal phalanx nonunion successfully treated with autologous bone peg grafting. The first case involved a 13-year-old boy who developed nonunion following a bicycle injury. The second case involved a 31-year-old man who sustained an open fracture in a workplace accident. In both cases, a cortical bone peg harvested from

Is arthroscopic-assisted treatment applicable for the treatment of pediatric scaphoid nonunion?

Pediatric scaphoid nonunion is uncommon. Immobilization is initially attempted; however, it causes delays in both intervention and healing. Therefore, surgical intervention has gained broader acceptance and has been implemented in recent years. Among these, arthroscopic management has emerged as a contemporary technique. This study evaluated the clinical outcomes of pediatric scaphoid nonunion treated with arthroscopic-assisted bone grafting and Kirschner (K) wire fixation. Six pediatric patients with scaphoid nonunion treated with arthroscopic bone grafting and K-wire fixation between Novembe

Outcomes of volar locking plates for scaphoid fractures- A single surgeon case series.

BACKGROUND: Surgical management of scaphoid fractures has traditionally been with headless compression screws. AIMS: Our primary aim was to establish the non-union rate in our series of patients whose scaphoid fractures were managed with volar locking plating. Our secondary aims were to assess; post-operative range of motion, post-operative grip strength, functional outcome and the removal of metal rate. METHODS: We underwent a retrospective review of 35 continuous patients who underwent surgery between June 2018 and August 2023 under a single surgeon for a scaphoid fracture using a Medartis

Challenging Removal of Intramedullary Nails: Description of 2 Techniques With Proposal to Classify Nail Extraction Methods.

CASE: We present 2 techniques for removing intramedullary nails. In the first case, an intact, incarcerated short proximal femoral nail was removed without making an additional bone window and underwent hip arthroplasty in the same sitting. The other case had a small distal fragment of broken femoral nail removed through the nonunion site, and exchange nailing was performed. A classification system for various techniques is also proposed, which may help orthopedic surgeons best prepare in the event of failure to remove the nail using the standard technique. CONCLUSION: Removal of an intramedu

Fluoroscopy-assisted percutaneous management of posterior pelvic ring injuries via multiple transsacral and iliosacral screws in the supine position.

INTRODUCTION: When patients are positioned supine for percutaneous screw insertion to achieve posterior pelvic ring fixation, the edge of the operating table can impede screw insertion trajectories, thereby increasing the risk of cortical perforation, especially during iliosacral screw (ISS) placement. Multiple screw insertion might also potentially increase the risk of cortical perforation. The aim of this study was to evaluate the feasibility, safety, and efficacy of inserting multiple (≥2) transsacral screw (TSS) and ISS in the supine position via the application of specific surgical strate

Persistent Infected Nonunion of the Proximal Ulna after a Free Vascularized Fibular Graft.

This article presents our experience in managing a case of a persistent infected nonunion of the proximal ulna that failed to heal after a free vascularized fibular graft. Free vascularized bone grafting is one of the best treatments for infected nonunion, but multiple factors must be addressed to achieve bone union. It is crucial to target septic, biological, and mechanical factors systematically using a step-by-step approach, even when faced with a worst-case scenario after a challenging surgery such as a free vascularized fibular graft.

Salvage of the Distal Radio-Ulnar Joint Following Failure of Fixation.

Distal radio-ulnar joint arthritis can result in significant impairment of upper limb function. This case report demonstrates a complex course of treatment in a patient with this pathology. Multiple procedures including fixation, matched ulnar resection, and ulnar excision with soft tissue stabilization failed. Ultimately, a Scheker prosthesis was able to achieve long-term functional recovery. This case highlights the challenges in managing distal ulna pathology, implant choices, and the impact of osteoporosis and smoking on bone quality and outcomes.

Inflammasome expression patterns during bone regeneration: predictive biomarkers and potential therapeutic targets in non-union formation.

BACKGROUND: Non-union formation is one of the major complications in trauma and orthopedic surgery. The inflammatory response at the fracture site is crucial for determining the healing outcome. However, there is limited information on the expression profiles of inflammasomes during both successful fracture repair and non-union formation. METHODS: Hence, we analyzed the cellular inflammatory response as well as inflammasome expression during successful bone regeneration and fracture healing failure in a standardized murine fracture and non-union model over 70 days. Bone healing was analyzed b

Humeral Fractures in Metabolically Compromised Bone: Outcomes of Structural Strut Onlay Allograft with Open Reduction and Internal Fixation - A Case Series of Three Patients.

INTRODUCTION: The "terrible tetrad" of the elbow - simultaneous posterior ulno-humeral dislocation, comminuted radial head fracture with dislocation, coronoid process fracture, and proximal ulnar fracture - is an injury pattern that simultaneously fulfills criteria for both the terrible triad and a Monteggia-type fracture-dislocation. Fewer than fifteen cases have been documented worldwide. We present two cases managed by different surgeons at different institutions and propose the formal adoption of the term "terrible tetrad." CASE REPORT: Case 1: A 50-year-old male painter sustained a right

Clinical Outcomes and Complications of Surgical and Conservative Treatment for Jones Fractures: A Systematic Review.

BACKGROUND: Forefoot fractures are among some of the most common orthopaedic injuries. Treatment of zone 2 fifth metatarsal or Jones fractures is controversial and many times dependent on surgeon preference. Management of these injuries remains without clear guidelines. Accordingly, the goal of the current systematic review is to compare the clinical outcomes and complications for both surgical fixation and conservative treatment. METHODS: Two independent authors completed a systematic review using the PubMed, EMBASE, and Cochrane Library databases. The Preferred Reporting Items for Systemati

Higher Rate of Treatment Success of Infected Femoral Shaft Non-unions with Ultrathin Silver-Polysiloxane-Coated Implants.

BACKGROUND: Infected non-union represents a severe complication after femoral shaft fractures, often associated with high recurrence rates and the need for multiple surgical revisions. Antimicrobial implant coatings may improve treatment success; however, evidence on the use of silver as an implant coating remains limited. This study aimed to (1) report short-term outcomes following treatment of infected femoral shaft non-union and (2) investigate if silver-polysiloxane-coating of implants leads to a significantly higher rate of treatment success at one-year follow-up. METHODS: This prospecti

What is the relative importance of anterior ring intramedullary implant diameter and cortical purchase on outcomes following percutaneous fixation of lateral compression pelvic ring injuries?

OBJECTIVE: Fixation of the anterior pelvic ring improves outcomes in operatively treated lateral compression (LC) fractures (AO/OTA 61-B1/B2). Despite a high variability in the choice of intramedullary implants used for anterior ring fixation, the relative importance of implant diameter and cortical purchase (length) remains unclear. This study compared reoperation rates and radiographic outcomes between implants of various diameters and lengths in LC pelvic ring injuries. METHODS: This retrospective multicenter study included patients from three international centers with acute (<3 weeks) LC
